The Strange Syndrome Caused by Cannabis Use | Cannabis Hyperemesis Syndrome | Dr Rege Explains

  • Dr Sanil Rege, Consultant Psychiatrist, discusses a clinical case of an 18-year-old female who presented to the emergency department on multiple occasions for severe episodes of vomiting.
As no medical causes were identified, she was referred to psychiatry to evaluate anxiety that was thought to cause vomiting.
    #cannabisuse #cannabis #cannabishyperemesissyndrome
Applying a diagnostic hierarchy, the cause was due to chronic cannabis use.
Cannabis Hyperemesis Syndrome is characterised by:
    1. Chronic cannabis use
    2. Cyclic episodes of nausea and vomiting
    3. Frequent hot bathing
The gastrointestinal actions of cannabinoids are mediated chiefly by CB1 receptors.
Activation of CB1 receptors results in inhibition of gastric acid secretion, lower oesophageal sphincter relaxation, altered intestinal motility, visceral pain, and inflammation.
The clinical course of Cannabinoid Hyperemesis Syndrome is divided into three phases:
Phase 1: The prodromal phase can last for months or years, with patients developing early morning nausea, a fear of vomiting, and abdominal discomfort.
    Phase 2 :
The hyperemetic phase is characterised by intense and persistent nausea and vomiting episodes.
    Phase 3:
Recovery phase. This is associated with the cessation of cannabis use.
Despite the well-established anti-emetic properties of marijuana, there is increasing evidence of its paradoxical effects on the gastrointestinal tract and CNS.

I started NA yesterday. Over 15 years as an extremely heavy smoker and at age 28 I'm making the decision to ween down. I've been feeling sick all the time, pot doesn't even get me high anymore so why even smoke it? Simple. I'm psychologically addicted to the substance.
Cheers to anyone making a change. I'm ready to start saving about half a grand each month.

Hello all, I would like to share my experience and also receive some answers. So yesterday I ended up going to the ER due to stomach cramps and a bit of nausea. I had to write an exam and those feelings got worse by the time I reached my university. I regretted not staying home because I was feeling something before I left but it settled a bit so I thought I'd be okay. Turns out not. I had to make a decision to go to the closest hospital. When I finally reached to the hospital, I saw the waiting room full of people. That's when I realized that I won't be seeing a doctor anytime soon and I have to endure through my suffering. After 4-5 hours of wait I got to see the doctor. I was in the hospital for 10 hours total. They took my blood test, did a CT scan and everything came back normal. When I spoke to the doctor again, I told him more about my symptoms and he asked me "have you been using marijuana?" This curious question has been raised by all the doctors I have seen related to my symptoms whether to walking clinics or ER doctors. My symptoms started last year in March and since then I have had to go to ER every 2-3 months. I was not able to quit weed though since the doctors first brought it to my attention last year. However, seeing how randomly these episodes come and interfering in my life and once again how I had to just be at the mercy of the hospital with their waiting times and also for the doctor to give me the right thing to make me feel better. I am so tired of it. My family also gets worried when I go to the ER for this. After yesterday, I have made a decision to just stop smoking, because nothing is more important than my health. The high wears off in what like 2 hours? But when these symptoms come, I have to deal with it for hours and have my life halted until I start to feel better. I just don't want to be worried about what I should eat, my appetite, indigestion, stomach burns, not being able to focus on work due to these symptoms. I enjoyed weed a lot and still have weed left but I will just give it away to someone. After yesterday's episode I have learned that these episodes won't go away and I have to do the one thing that will make it stop- quit weed. Even if it gets difficult and I feel like smoking it, I will just have to try and avoid it. There is no good feeling then to be able to write here feeling like myself. Anyone going through their symptoms would not be able to focus and write and talk to people. So this is very important everyone. We have to think about our lives 50 years from now and how we would like to feel. Can't just satisfy our impulses and fall into the trap of instant gratification. It's hard but this a journey I can be proud of and share my positive experience in a few years time. As much as I love weed, enjoyed it, it's time to let it go. Nothing stays forever. Gotta live and enjoy this life. And the hot showers/ bath are very much true. How many times have I had to get inside a hot bath to feel somewhat better. So don't be in denial and don't think your life will be incomplete without it. Build yourself, make yourself stronger because that's where the strength comes from, doing something we thought we could not do. Hope everyone finds their comfort soon! Now that I am on this journey, it would be great if anyone who has been through this and healed- can give me suggestions on how I can continue to heal my stomach and what I should be doing on a daily basis in terms of diet or vitamins in order to cleanse myself. Would appreciate it much!

@jordanm4853 I had chs and I did not know I had it. I smoked weed for 30 years ever day. My chs came about 3 years ago. The first thing that happened was I was smoking a spliff, and halfway through smoking the spliff, I had very bad nausea, stomach pains and stomach cramps and I then vomited, i didn't understand why i vomited so I continued to smoke. 3 months before this happened. My marriage broke down and I was missing my children so much I could not cope, so I was already in a bad state of depression and not sleeping or eating much. I continue to smoke after this first bout of vomiting. After this first bout of vomiting, i could not eat food, and my insomnia kicked in very badly. Maybe I'm just eating two spoons of my dinner per day, and maybe 2 to 3 hours of sleep a night. I continue to smoke weed. I worked through this whole period doing manual labour, and somehow my body was still functioning, and I felt strong even with lack of sleep and hardly eating food. I was functioning on Coca-Cola, Lucozade, and spliffs. Basically I was ill for 6 months and continue to smoke without knowing the reason for my sickness. 6 months later, I hit a brick wall, and my body started to shut down. I went from 17 and a half stone down to 12 stone in 6 months. I started getting stomach cramps, stomach pain, vomiting from time to time, gastro problems, hypnic jerks when trying to sleep, Tinnitus in the ears. once I did fall to sleep then I woke up with tremors. Muscle spasms. Muscle stiffness. Muscle pain. Bone pain. Muscle spasms in my back . Found it hard to walk. Numbness down my legs. Vomiting, nausea.Waking up with numbness in my arms and hands. Anxiety, Muscle spasms in my throat, throat tightening every time I lie down to try to sleep. Constipation, Haemorrhoids. I could not urinate properly. The Muscle spasms in my throat when I try to lie down were causing my throat to close. All this time, i was going to the doctor's and getting nothing back from the doctors with regards to why I was getting all these symptoms. The doctor was offering me muscle relaxant tablets but I could not take them because they made a lot of my symptoms worse. the doctor was telling me to sleep sitting up due to my throat closing when lying down. They sent me for every check-up you could imagine and found nothing about the causes of my sickness. They did find I had developed a hiatus hernia. As more time went past, the muscle spasms in my throat got so bad that I could not smoke because my throat started to close when standing or sitting. This obviously stopped me from smoking. My anxiety and depression were so bad from missing my kids and all the symptoms from the chs and I was still not diagnosed. I felt like I was dying. My nervous system was in a very bad way. I was researching everywhere and came across CHS. I then took this to the doctor, and they diagnosed me with CHS. I continue to work through this whole period. Looking back, I do not know how I got through. But I think the continuous moving of the body was helping me somehow. I started to eat a little better and slowly slowly I started to put my weight back on. After 1 year of me sleeping sitting up I was finally able to lie down without my throat going into spasms so I could lay down I started to sleep a little better slowly slowly. It's been 2 and a 1/2 years since stopping smoking. After 1 year, I was still in a very bad way, with many symptoms still remaining. After about a year and a half they found a 7 mm polyp which they cut out of my stomach. Through this whole period of me being sick with chs i had found having hot baths with Epsom salts was helping me a great deal. Putting warm olive oil all over my muscles was also helping. Drinking bone broth also helped a lot. After 2 years, my haemorrhoids started to calm down. I was able to go to the toilet better. Both urinating and pooing. It's been 2 and 1/2 years, and I still have tinnitus in the ears and spasms in my back and back pain, but it's nothing like the spasms and back pain I had before. I'm still suffering a little bit with insomnia. And when I wake from sleep, my arms and hands still go numb and numbness down the outside of my right leg mainly my knee and thigh. The biggest help I have found in my healing period was through cranial therapy. When I start to feel quite good, sometime I would try to drink alcohol and smoke cigarettes. Drinking alcohol and smoking cigarettes seems to bring back some symptoms. I tried to drink sometimes to test my body. But then I realised I could not drink. Same with smoking cigarettes. Some symptoms start to come back. I have come to the realisation that I can not drink alcohol or smoke cigarettes. And obviously cannot smoke cannabis for the rest of my life. Even standing next to someone who is smoking a spliff affects me. I was always a very fit and strong individual and felt invincible. But this whole experience has humbled me. Please look after yourself and your health mentally and physically. The one good thing I can see about CHS is that if you stop smoking weed and live a healthy life it can go away. As I said I still have some symptoms. But I feel like if I look after myself hopefully I pray to God that all my symptoms will eventually go away. it is possible that there is damage done that cannot be repaired from but I am very thankful and go forward with a positive mindframe. It is very good to see that there is information out there for people to find. It did not occur to me to go on TH-cam and type in chs back then. But even the videos on TH-cam here do not talk about half my symptoms. I obviously took chs to the Brim. And maybe I'm very lucky. But it is very good to see there is information out there and people are learning more and more everyday about CHS. If you feel you have CHS the beginner of CHS or you feel you have had it for a while please stop smoking now. Go and seek help. As I said the best thing that I found helped me was cranial therapy and God. God bless you all out there, peace 🙏

This sucks so bad I’ve been smoking weed since I was 12 off and on I’ve been smoking weed now, I’m 21 now and never really felt the syndrome till I was 20 this syndrome messed up my life from something I enjoyed every day now, I am here in pain while my stomach literally feels like it’s eating itself and I keep having constant urges to throw up without nausea no warning at all, it’s terrible, definitely not worse then what I’ve been through but it definitely sucks immensely, I wouldn’t wish this upon anyone, I smoked last night and tbh im addicted to marijuana and looked up all kinds of foods not to eat with CHS, I wanted to prepare myself a meal before I got high maybe it’ll change how I feel in the morning, so I smoked a really juicy infused blunt, felt pretty good for a while then I just felt this pressure slowly creep up and I felt a really bloated feeling after, so I made sure I stayed hydrated, the next morning it was no different I felt like crap, extreme waves of nausea and anxiety, which only makes you feel worse, I have no other choice rn but to go cold turkey, I hope anyone feeling this syndrome can fight the feeling to want to get high, marijuana isn’t worth your life, stay safe out there everyone

Would have periods of vomiting that I chalked up to a sensitive stomach or food poisoning. This past weekend had 3 trips to the ER and the first question they asked me was if I smoked. I was thinking BFF to myself “here we go, blame it on the weed.” Went home and researched and realized how spot on my symptoms are. I’ve lost about 26 pounds in 3 months, currently on day 4 not smoking. If y’all really refuse to quit after feeling like this all the time, it proves how addictive weed can be.

I think there needs to be a lot more knowledge about how extracts/concentrates cause this very specific and very horrific syndrome.
For me, I didn’t need to cease smoking cannabis - just had to cease smoking extracts and hash and stuff. As long as I stick exclusively to smoking flower with a normal and balanced amount of THC/cannabinoids, I don’t get any CHS syndrome.
He’s absolutely right that hot showers are the one and only way to treat them. Luckily it’s VERY effective at treating CHS, at least in terms of relieving the symptoms.

I thought my future held only dying from some rare stomach cancer/disease after suffering endlessly and practically living in the shower. I have been to every stomach doctor/ specialist within driving distance, they have diagnosed me with one thing after another, from ulcerative colitis, gastritis, gerd, IBS, h.pylori (which i was treated for 4x) to the latest crohn's/krohn's (sp?) disease, yet my symptoms never matched up in the end and the meds they prescribed made me extremely sick. I've smoked since i was 17 except a few years here and there, i never got sick until i quit for 2 years and started back, and i couldn't figure out why, i stopped drinking anything carbonated or alcoholic, no red meat, no pork, no lettuce, pizza, spaghetti, the list of what i couldn't eat slowly became bigger then the one i could and i lost 120lb. The thought that i was doing this to myself all these years, about 20 years in fact, is as depressing as it is liberating. 😢😮😊

I have CVS, and every doc I met wanted to scope me and then tell me to stop smoking weed (I don’t smoke). It took so long to get a diagnosis. These attacks are similar to what I go through, except CVS makes you kinda comatose with migraine symptoms (light and noise sensitivity), you have nonstop vertigo, clothes soaked in sweat, it’s horrible. My heart goes out to my fellow suffers- to vomit every 5-10 minutes for 6-8 hours straight is hell on earth.

My husband has been smoking for 15+ years and for the past 5 years that I've been with him he has gone to the ER many many times for cyclic vomiting. In the beginning of our relationship we went to Colorado, and he got sick. We went to the ER, and after they ran tests on him they told him it was probably CHS. He basically was like "no way". Then he had convinced himself (and me) that he probably had chrons disease because at one ER visit a doctor had no answers for him and said "maybe see a GI doctor because it could be chrons or colitis". Yet, he still hasn't gone to see a GI doctor. He has eliminated red meat, fast food, soda, most dairy, now coffee.. all with no relief to the cyclic vomiting.
A couple months ago he was landed back in the ER and had CT scans done, as well as other tests and everything came back perfectly fine. No signs of anything. So again, a mystery. The doctor there mentioned it could possibly be CHS, but he dismissed it again.
I'm currently typing this as he is throwing up and refusing to go to the ER. I have no idea how to get him to stop smoking to see if the symptoms will go away. He is so stubborn and marijuana is a part of his identity at this point.

@@PsychiatrySimplified Thank you. So yes hot showers do help. I ended up doing more research on it since I posted this, and I think I found something in regards to treatment using capsaicin topical cream.
"The interesting, almost pathognomonic, characteristic of CHS is the relief of symptoms by hot showering or bathing. This effect was initially proposed to be motivated by impaired thermoregulation mediated by the stimulation of CB receptors in the preoptic area.13 However, the receptor that may actually deserve credit for this phenomenon in CHS is the transient receptor potential vanilloid-1 receptor (TRPV1), a nociceptor found in the peripheral nervous system that is involved in the pain relief we feel from endocannabinoid stimulation and which also regulates body heat.21 TRPV1 is activated by exposure to scalding heat (109°F).22,-24 Interestingly, capsaicin also stimulates the TRPV1 receptor.24 Lapoint25 identified the clinical advantage of the physiologic link between cannabis, heat, and capsaicin when they applied the chili pepper derivative (0.075% capsaicin) to the abdomens of several adult patients with CHS and observed relief of their nausea, vomiting, and abdominal pain. Additional case reports subsequently also described the treatment of CHS with application of 0.075% capsaicin to the abdomen and 0.025% capsaicin to the back, arms, and abdomen.26,27 "
My husband was throwing up for 24 hours straight when I came across this information. I immediately went to Walgreens and luckily they had a single tube left of this stuff.
I got home and put a pea sized drop on his abdomen and another pea sized drop on his back. Within 15 minutes the nausea had completely subsided. He is currently sleeping and has not thrown up since. The only negative side effect was a burning sensation that lasted for about an hour.

The article you posted to here I found years ago, and yes the capsaicin cream or basically chilli extract does work and so do drastic temperature changes as well, like hot to cold showers back and forth. I last went to the E.R. In 2019 and I would go 1-5 times a month since 2012!! Basically I found an article on R-CPD or “The No Burp Syndrome” weed or cannabis is not the cause of our nausea, and vomiting, in my opinion of course, it’s caused by not being able to release stomach or abdominal gas. Basically you can’t burp, and then you work yourself up into a panic not been able to release the gas, which then causes you to make yourself throw up for the relief of some gas escaping while you throw up. Back in 2019 I figured out never let myself hold in a burp or a fart and voila, all of my crazy nausea symptoms went away, while also never reducing the amount of cannabis consumed. I was throwing up for a week a couple years ago and made two visits to the ER, just for the symptoms to subside for a little and then I vomit again at home. I was getting multiple diagnoses from the nurse saying it was CHS, to the Dr just saying it was a stomach bug.
Well flash forward to this week and it’s happening again, although my family members who don’t smoke were also throwing up. So I am not sure if it’s CHS or just a stomach bug.
It’s been going on for four days and the second day I actually felt better ate, and smoked later. Like some other commentator said after my high wore off I got extremely nauseous again and threw up all my food.
Feels like I can’t keep any food down. I will say hot showers actually make my nausea worse
Edit: I had a nurse come to my house and give me a home IV therapy and it made me feel so much better. Only $200 compared to a hospital visit and waiting forever.

I was in denial for many years, had a few hyperemisis episodes with a year in between that lasted about 2 weeks each. A few months ago, after starting smoking again, I got slipped into another episode and it almost killed me. I couldn't hold anything down for 21 days. Vomiting immediately after I would run out of hot water. Eventually was sent by ambulance because started seizuring due to lack of potassium. I attempted to go to hospital several times during the 21 days, but because of the 5-6 hour long waiting times in the hospitals in Canada, I could never last in the waiting room. I couldn't spend that many hours away from the hor shower..it felt like torture. I will never smoke pot again. I am now certain that I have an intense allergy. CHS is deadly. I now feel great.

Constant contradiction in and of itself - My stomach is empty, but I'm not hungry. 😫I just force myself to eat things light and healthier like fruits, yogurts, soups and vegetables. CHS sucks. The frustrating part is how people say "oh you're not smoking the right strain" "your weed was contaminated" blah blah blah. I was smoking dispensary lab tested weed ONLY. That's not how CHS works. CHS doesn't care. Hot baths and capsaicin cream help a lot. Zofran can help with the nausea but won't wipe it out completely. The big part is keeping your electrolytes in balance. Get in a hot bath and drink pedialyte or other electrolyte beverages. When you get out you might become nauseous and throw it all up but at least your body had some time to absorb some of the vitamins and minerals. It's a miserable condition but it will pass! I promise! The biggest thing is abstaining from any cannabinoids. Your body doesn't agree with them and that's unfortunately the way it is. I've been through it 3 times because I'm stubborn and don't listen. One of the times I spent 5 days in the ICU while on vacation because they couldn't figure out what it was. Hydration is the biggest part. You can go without eating for a long time but hydration is key. Drink as much water as you can handle. It'll get better. I promise.

That's the closest thing to the CHS nausea that I can think of. Sound hurts, you are totally spaced out in the deep nausea. I couldn't even talk to my husband, cause talking and listening made it worse. Also, super hot water on the back of my neck was my only relief, would totally snap me out of the shock immediately. Part of the problem for me is that we have over 2000 years of documented marijuana use and only in the last 2 years is this a problem. They claim it's overuse but are they looking at everything? As the Dr mentions, marijuana actually helps with vomiting especially with cancer patients who can't keep food down SO how is it all of sudden causing people to throw up?? They have no real answer. What changed?
I have a major problem with the conclusion of these non-studies because they don't ask if something was wrong with the marijuana itself. I know since the legalization of marijuana, we've got an influx of trust fund babies who can afford the high barrier to entry and they don't give a damn about how they grow marijuana. There is NO regulation in terms of pesticides/insecticides. If you're a new grower, you don't pay attention to things like how quickly these organic and inorganic pesticides break down. They're using dangerous chemicals that even the FDA has prohibited for our food crops.
But, let's talk about these pesticides since some users don't know. Pesticides can be topical (outside of the plant) or systemic (inside the plant). They also have break down period. For example, if a grower has a spider mite infestation, they may use SEVINS. In dust form, it takes 3 days to break down. However, if you wet it, it can take up to 72 days to break down. We're working with plants that we're watering all the time. But, growers don't pay attention. I'm not blaming SEVINS for anything specific. We have neem oil, organic pesticide direct from the neem tree, breaks down in 5 days. You have SMC, depending on what you're using, it may take 3 months to break down.
But, I bring this up to point that we don't know what's really causing CHS and I highly doubt it's simply from people just using too much marijuana, ALL OF A SUDDEN. I am hoping that people start asking necessary questions and stop accepting these weird answers to a potentially made-up term. I think people are getting poisoned and it's not a real syndrome in the classic sense. But, how can we find anything out when we STOP at answers that don't make sense considering the long history humanity has with this plant. We need to know what's really going on.

@@arrowbynum4784 this is why I promote people to grow their own. I remember in NYC, the dealers used to use roach spray on ghe reggie to kick it up a notch. THEN, there was "gettin wet" where they would dip marijuana in formaldehyde. You would know it was dipped because it was still wet and very brown.
Then, you have folks claiming the quality of the marijuana ("dispensary lab quality") they're getting but they can't tell us the strain, which dispensary, the grower, or when they picked it up. These are things we should know. That way we can get to the bottom of it.
But the doctors arenxt even asking important questions. They got their little pieces of paper hung up on the wall, stacked with a pile of arroagance, as if they know everything. They have half the story which is anecdotal, at best. Last time I checked, we needed double blind studies to prove anything. As they did in the past and continue to do today, they're literally just making up shit with nothing to back it up but their pieces of paper hanging on the wall.
F that. The BS they lied about yestersay with no evidence and no studies is being debunked today with evidence and studies. They said marijuana is dangerous for pregnant women. That shit was deuinked before they came up with fhe lie (re: Melanie Dreher). They claimed marijuana hurts the developing brain in children. That was debunked recently.
My favorite lie is the Tulane/Heath study that "proved" marijuana killed brain cells. Not only was the study debunked 3 years later after peer reviews realized the researchers sufficated rhesus monkeys with marijuana smoke. DEA ran with that. That was the last ttkme the govt approved funds for a marijuana study, something they could use to lie to the people. B.b,bbut waait.. there's more.. marijuana has been proven to aid in NEUROGENESIS.. huh!?.. yeah, marijuana helps develop brain cells, not destroy them.
So if they think someone as educated and experienced in marijuana, doctor arrogance, government lies will accept, it's just fhe marijuana... I aint even gotta say no more. They gotta come better than that.

Went to the ER twice in one week. Severe nausea and I couldn't eat or drink anything threw up for two days straight. The ER doctor pointed me to this. I'm sure this is it. It's been progressive and getting worse. Been a long term daily smoker for years and years. 2 years ago I started with infrequent nausea and then frequent and then sometimes with vomiting. All along with stomach issues. I had every test for liver and pancreas issues, they even put a camera into my stomach. They found no reason for this. I haven't smoked for almost 2 weeks and I am so much less nauseous in the AM and my stomach is starting to feel better. Sux I can't have THC anymore but that week long nausea was no joke and I never want to experience that again. The anti nausea medicine doesn't even work for CHS.

I was diagnosed with chs. The ER kept telling me I'd have to quit smoking for months before I saw improvement. Then I met another patient who had it and she said its caused by DAILY chronic smoking so just stop smoking daily and the symptoms stop. I did bathe frequently but not while sick because I didnt want to throw up in the tub. So instead I wore sweaters and blankets and it was 110 outside. I am also diabetic so THIS condition caused me to be Hospitalized in the ICU TWICE because the constant vomiting caused my glucose to skyrocket and I went into DKA twice. I just got out of the hospital a few days ago. They did a gastric emptying test to rule out diabetic gastroperesis.

I don’t get vomiting episodes anymore. But I feel that body has a dysregulation in temperature. Throughout the day I get hot flashes or feel hot/ cold. Could this be my hypothalamus trying to recover? Is there anything I can do to speed up recovery? I still crave hot showers and heat in general. This may sound strange, but I think it's worth talking about. Does anyone else feel like they could ball their eyes out crying but you're not able to? I've been watching movies throughout this experience, during the sad parts of the movies like the death of a beloved character, I would usually get a little teary-eyed and sniffle a bit, but there's nothing, I almost want to cry to let it out, but I can't. This is an under-researched syndrome, we're all patient zero to an extent, so I think anything we can share with each other could be beneficial. The anxiety has been the worst part for me, I haven't felt like myself since it started. I made an appointment with a psychiatrist and it's not too far out so that's some light at the end of the tunnel.
