How can sinus tarsi syndrome cause FHL Tenosynovitis

  • This episode has a whole bunch of complicated-sounding terms that you may have never heard before.
    One of them is sinus tarsi syndrome, and the other one is FHL tenosynovitis or flexor hallucis longus tenosynovitis.
    Sinus tarsi syndrome is often confused with ankle pain.
    FHL tenosynovitis is often confused with plantar fasciitis.
    These conditions affect opposite sides of the same foot. Yet, one can lead to the other.
    Today on the Doc on the Run podcast, we’re talking about how sinus tarsi syndrome can cause flexor hallucis longus tenosynovitis.

    Hello, I was initially diagnosed-in thirty seconds- with peroneal tendinitis. Received some pt, felt marginally better and went back to work. Still in pain. Was then given a shot in the sinus tarsi area by another provider. Seemed to help a bit and the next day I cut the yard with a push mower. It then really began to bother me. Went on vacation right after that and pain was unbearable. Had to use a scooter and shorten my trip. Got home and saw dr # three! Diagnosed with sinus tarsi syndrome. Was given custom orthotics and bought very cushioned shoes and was told to take it easy for a few days. Never any real swelling. Started pt about three weeks ago. Many exercises. Had dr appt last week. She felt two areas on my foot for sinus tarsi and thinks I’m better. But now have completely new pain close to the tarsi but not the same!! It’s driving me nuts!!! Dr says it’s nerve and really didn’t elaborate at all! Prescription for a patch to change every 12 hours. With only three weeks of pt am I really better? I’m still doing pt for at least two weeks, I’d guess. What caused new pain? I originally had the issue start in January. How active should I be now? They’re saying short walks and bike rides are ok. What’s going on? Pt guy said being duck footed contributed as well as very tight calves. He wants me to walk straight. Not easy! Your thoughts are greatly appreciated. Dr appointments are hard to get when you need.

    I have seen an ankle/foot doctor, had a cortisone shot, had an MRI and I am 100% sure that I over pronated my ankle and that I have had this exact condition for 12 months but no one can tell me how to fix this issue. It’s so frustrating. I am going to see another ankle specialist next week so I can explain it again. How come you know Doc the issue and no one in central Florida east can figure it out. 😮

    Actually that makes total sense. Just diagnosed today with sinus tardi syndrome. Because of an ankle sprain/avulsion fracture on my cuboid several weeks ago, I’ve been putting more weight along my medial foot plane. Thanks for explaining the big picture!

    How are conservative outcomes? Can sinus tarsi go away on its own if you catch it early and use a knee scooter for a couple weeks around the house?

      Depends on the severity, circumstances, the patient health and what else is done to treat it.

    I have this problem due to an ankle sprain. I have pain in my extensor tendons and it hurts to walk. Does this problem ever require surgery? Could you recommend an excellent surgeon in Sacramento County?
    I’m not sure what exercises I should do, and looking on the internet seems dangerous. Thank you for this video

    I believe I’m dealing with this now. I need to fix it STAT because I’m starting compensate and alter my gate. I just started doing stability for sinus syndrome but should I get an ankle brace? Or do you recommends certain shoe besides just orthotics
