Garmin ECG Expands to Fenix, Epix, and More!

  • Garmin has just rolled out ECG access to their Garmin Epix Pro, Fenix 7 Pro, Venu 3/3S, and Tactix 7 AMOLED series. Here's a look at how it works, from start to finish.
    Garmin has just announced the availability of ECG (Electrocardiogram) functionality for the Garmin Fenix 7 Pro, Garmin Epix Pro, Garmin Tactix 7 AMOLED, and Garmin Venu 3/3S units/series. These four different sets of watches all share the same new Garmin ELEVATE V5 optical heart rate sensor package, which is hardware-capable of ECG. The only previous Garmin watch to have ECG functionality was the Garmin Venu 2 Plus.
    Garmin’s ECG feature will determine whether you have a normal sinus rhythm, or an Afib condition. It does this on-demand (when you request it), versus a 24×7 background process like some other smartwatches. Upon the conclusion of the 30-second test, it’ll give you the result as well as let you export the full report as a PDF to give to a doctor. This feature does not detect heart attacks.
    The functionality remains the same as the Venu 2 Plus, but given that Garmin changed the optical HR sensor package, they had to redo/re-fill portions of their FDA certification related to the ECG feature. Remember that Garmin, like Apple/Samsung/Fitbit/Google/etc, have all gone through the formal FDA certification process, which classifies the ECG app as a medical device. This includes doing a full study of 600+ participants and everything else that comes with proper medical device certification

    I wish they would add this ability to the HRM Pro+, it would be a nice way to give owners of older devices a new feature, and it’d probably be more accurate like the Polar H10 offers.

      You are not able to get an accurate ECG measure just by measuring through the wrist with a watch. When measuring ECG you normally use at least 4 measuring points on the body. Most of the part you measure what we call 12 or 15 point ECG. It shows diferrent areas of the heart on a ECG. To read and understand ECG, it takes a lot of practice and understanding on how ECG actually works. Ok you can see that you have a normal Sinus rythm, thats good.. But i think that if you dont have that, you will feel it somehow ;) So to me, ECG on a watch is a bit useless.

      @@Jako6400 This is all wrong. You can get an accurate 1 lead ECG using just 2 points of contact (e.g. the wrist and the opposite hand). Yes, it is not a 12 lead ECG, but it does have diagnostic value for certain conditions (e.g. atrial fibrillation--the condition for which the FDA has approved this). Users do not need to read their own ECGs; they can either forward a copy of the ECG tracing from the watch to their doctors or they can rely on the FDA-approved software if their condition is one of the ones approved for diagnosis. And, no, not all persons with atrial fibrillation will have symptoms such as palpitations or shortness of breath.

      Agree. I use the Polar H10 with Garmin devices and platform because of its higher accuracy (sampling rate, etc.). I'm prone to a-fib and have been waiting for Garmin to catch up with Apple - in fact I had a touch of tachycardia yesterday and wish I'd have already installed the ECG app on my new Fenix 7X Pro. If you have an H10 and Android phone, there is a free ECG app that a doctor in Europe wrote; unfortunately my cardiologist is not able to interpret the file format it saves traces in, but it's a good indicator for when I get back from long rides.

    Just wish Garmin would include passive monitoring for irregular heartbeats as well - competition like the GW6 offers this functionality in addition to ECG, and they compliment each other well.

    I bought a Venu2+ specifically for the ECG function. More than 50% of my tests were "inconclusive" so I decided to return it. One side note that no one talks about with this ECG function is that if your heart rate is below 50bpm (or above 120bpm) the watch will not render a decision either way on your heart rhythm. That happened twice to me as well.

      Interesting! I just got a Fenix 7X Pro. I can get into both bradychardia and tachycardia arrhythmia below 50 and above 120 so will watch for this (pardon the pun).

    That’s why I ditched my FR965. Is just incredible that lower tier watches have a lot of different features, but the top of the line triathlon watch, just doesn’t.

      @@Therevolution7 You are welcome. And yes, the 965 is a very very good watch, but it’s just a 955 with oled, and as you can see, Garmin really don’t care too much about it.

      @@mrnicbst that’s what I meant. They had to update the sensor going from the 955 to the 965. What they did, was launching a new device at mid cycle, putting an oled screen, charging more, but it’s the same overall.

    Just one small additional point, you must be in a compatible region AND your watch must be getting an active GPS signal… if you are sitting in your basement, this won’t work, it will tell you you are not in a compatible region, so go outside, put your watch into an activity mode that activates the GPS. Once you get a GPS signal, try the setup again and you wil be in business.

      You don’t actually need GPS at that moment. Though, your phone does. I believe at some point your watch needed to have GPS in the US. But in my case I did all my setup without any GPS signal, deep inside hotels, airports…and a Chipotle. 🤣

    I use my watch to track sports activities and appreciate all the data, the maps and the navigation I can get right there on my arm, but I really have no interest in all the other gadgetry at all. I understand that Garmin wants to be at the front with the newest technology and such, but I hope they continue to make sports watches that are mainly for sports, built from the best materials, but without all the 'bling' and thereby hopefully at affordable prices.

      I actually like this because it's very useful for people like me who are out in the wilderness a lot and if your not feeling well you can almost give yourself a baby wellness check. High altitude sickness is no 🤣 joke

      @@fastm3980that’s true, but afib ist not a hallmark of altitude sickness. The only thing all people experience with increasing altitude is tachycardia (the heart is beating faster in order to meet the body’s oxygen demands while the partial pressure decreases). It might be more useful to track your oxygen saturation as long as you have an idea of your hemoglobin level. That said, all of this does not help you in that immediate situation. It might even give you a false sense of security. In doubt: descend.

      A 12 lead ECG isn’t even a gold standard to detect a heart attack. Cardiac troponin levels could show a possible heart attack, manifesting itself as something called an NSTEMI on a ECG. Always trust how you feel, learn the signs and symptoms, carry aspirin in the home.

      I highly doubt they would ever do this, but, if they could find out a way to enable the ECG to produce an on the spot HRV reading it would be incredibly helpful. Coros paired with HRV4Training to do this for a morning spot measurement with his app and it’s very cool.

    I have a new Epix Pro and can get all the way to where you place your thumb and index finger on the bezel. From there nothing happens. I have tried resetting my phone, my watch, the app, etc. Any pointers to troubleshoot?

    Very frustrating feature… it stops running if your heart rate is under 50. I would think that a lot of people buying a high wnd fitness watch have heart rates under 50 when sitting around … i get frequent premature ventricular contractions that i would like to monitor with this, but it keeps quitting on me because my heart rate is regularly under 50… so i have to get up and do sone jumping jacks or something to get my heart rate up in order to get an ECG readout

    Just got an Epix Pro gen2 Sapphire, and now I think I should get a ticket from Garmin to enter the US and activate this feature... Or maybe they can set something up in the US embassy in The Hague, which would prevent unnecessary flights. Or, maybe... they can just unlock it in Europe too... I'll settle for that too...

    I own the Garmin D2 Air X10. Same hardware and sensors as Venu 2 Plus which has ECG functionality. Did Garmin forget to make this update available to D2 owners? I have tried to unlock ECG when in the States without success.

    I don’t really understand the use case for manual ECG, think I used 3 or 4 times on AW. Do you need to have a heart condition for it to be useful?

      @@fukzen I disagree. Yesterday was a prime example of a ride I pushed a little harder than I thought and I went into mild arrhythmia. Sometimes I can correct it with really cold showers (like yesterday). If it's bad I need to take a blood thinner and "pill in the pocket" from my cardiologist immediately. I have been cardioverted five times. This feature is VERY useful for people like me who can't just feel it, and I trust its accuracy because the FDA wouldn't have (finally) cleared it without data proving it works as advertised. I have two friends with Apple watches that detected their heart attacks, and both doctors agreed the outcome would have been worse without immediate response (but I use Garmin's platforms and don't like Apple).

    Hi, I follow your videos, and they are really very helpful. But now i have a question about the Garmin Epix 2 Pro. Bought in Europe , but i am physically in USA / New York . I hoped to add thr ECG to my Garmin, but it doesnt give me the option. My Garmin connect app has Version 4.73.3. in the settings there are no indication for setup ECG , and there is no update for the Garmin Connect App. When i use wifi at the Garnin Epix and search for updates - also no success. Can you give some advice ?

      Try to set your Garmin account (in Garmin Connect) address to a US address temporarily (even just your hotel address).

      It’s not on the Fenix 7X, only the Fenix 7X Pro (the pro series had a different heart rate sensor and internal hardware).

      What worked for me:
      Steps taken:
      1. Clear cache and data of Garmin Connect app.
      2. Changed location in my Garmin Profile to US via the Garmin website (NOT GARMIN CONNECT)
      3. Enabled VPN and set location to USA.
      4. Enabled fake GPS and set it to USA as well.
      5. Opened Garmin Connect and logged in.
      6. Tapped the watch and added the ECG option.

    I couldn't take mine since my heart rate was too low :( mine is typically low 40s or upper 30s and I guess it needs to be a minimum of 50 to measure ECG

      People have done it successfully with Venu 2 Plus when it got this option. There are manuals online and they aren’t too complicated so I guess it should work in a similar way with these watches. I’m going to try it very soon.

    Hi ya, I'm using a 945LTE which through Garmin Connect gives basic Heart Rate data. By chance would you know what is the elapsed time frame during these recordings to be able to follow up on any irregular or unusual readings. Example: a low rate of 44bpm or a high rate of 170bpm recorded on an activity and how to know how long a period this reading was for and when?

      You cannot detect irregular heart beats by knowing timestamp and heart bpm rate.
      The only way to detect irregular heart beat is if it happens while you are taking the reading demonstrated in this vide. If it doesn't happen during the reading, it will not be detected.
      So, you might have irregular heart beats but you will still not be aware of that even if you have device capable of detecting it, if these irregular heart beats do not happen while you are trying to detect them.
      Do notice that common blood pressure monitors used at home, do not have ability of sensing irregular heart beats, unless that is specifically mentioned in its specifications.

      First, optical readings tend not to be as accurate as electrical (HR straps). Then, you need the accuracy from a device built specifically to trace an ECG waveform (higher sampling rate, better CPU and memory, etc.) to analyze the electrical rhythm of your heart.

      A 12-lead ECG is simply showing you it from 12 different directions, but as multiple medical bodies have shown, for Afib detection and basic ECG traces, these watches do so perfectly fine.

      You just need the rhythm, regular or irregular and if irregular is it afib. You’re not looking for fancy stuff like heart attack, bundle blocks, hypertrophy, etc.

      ECG doesn't actually have to do with sport HR readings. That said, I continue to see better accuracy with Elevate V5.

      Where I see the improvement is when I'm in Z3 and above - the Fenix 5 and 6 were really crap above Z2 for me (compared to Polar H10 on Edge 1030 as my baseline).

      It uses your GPS to make sure you are in US. So VPN does not work

    Note that using ECG on your watch causes Garmin to force MFA on your account forever. That can be a real pain if you have reasons to not want MFA. It should be your choice not theirs on the security of your data.

      @@DerekGreen123 I specifically called it out because last week someone got all upset that their friend was having a heart attack, and they used the ECG feature and it didn't say anything, and then they went to the hospital and they told him he was having a heart attack (and they were upset at the watch calling the feature wrong). The FDA has mandated and made it very clear it's not for heart attacks.

    Do not buy a Garmin Watch WITHOUT a metal case. I’ve bought two based on DC Rainmaker’s reviews & suggestions, and the band is always the weak point. The last watch broke where the band connects to the case. The cheap plastic cracked and now I can’t fit a new wristband on the watch it’s a $300 paperweight now collecting dust.

    Its kind of sad and cheating that garmin selling such expensive watch like the epix gen 2 that retailed for a lakh INR does not give an ecg sensor whereas samsungs watch 6 which retails for 20k INR has it 🤦🏻‍♂️🤷🏻‍♂️

      There IS a difference at the back of the watch (fenix 7 vs fenix 7 pro for instance). Look at the sensors. Different shape.
