Hello, I actually have a question. Is root cause analysis used to improve the outcome when the process is already stable, or is it used to make the process stable, by removing the so called special causes?
Great question! So when a process is already stable, the only way to make improvements is to reduce the common cause variation for that process. Do that still requires investigation, measurement and analysis of your process to understand the root causes of that variation. So in a sense, your ability to understand the root cause of variation would also apply here.

Hey Mike, that's a great question, and a bit of a loaded question. Sooo, if your procedures require that you answer all 24 questions, then yes it's obligatory - because your SOP requires it. From a more holistic perspective in terms of identifying the root cause of a problem, and developing an effective countermeasure - No, it's not required to answer all 24 questions.
Hi.. While using 5W2H, how can we use 'WHY' as that's exactly the reason we are performing the root cause analysis. If the 'WHY' means something else from what I have expressed, please comment. Thank you.
Hey Narendra, when writing your problem statement (using Is/Is not) it's often possible to take your first guess at the "why" of the root cause. It doesn't have to be perfect, and we will use the entire root cause process to establish the true root cause of a problem.
