  • Shoulder impingement is the first step toward worse problems like a rotator cuff tear that might need surgery.
    But no need to worry about surgery at this point if all you have is shoulder impingement! It’s nearly always curable in this stage, as long as you take care of your shoulder pain properly.

    Good Morning Dr David, thanks for taking the time to explain this and the other conditions associated with shoulder impingement, this is THE best explanation I have seen and heard! I injured my left shoulder just by sitting up using my left arm more than the right while reading at the time and 'crack' something went and instant pain! I saw your channel, followed your guidance re sling etc and I do have full rotation now, but there is one exact spot seems it's just behind and on the edge of the Acromion you described and my shoulder is sore in that spot, so I don't lie on it and starts to ache if I am on my mobile/ holding a book reading for a while, so try not to do that. I read about AC joint injury, looked at some pictures and it looks like what I could be experiencing. Been fit all my life, love reformer pilates, and just bought an Areopilates reformer, can you recommend any specific excercise I can do to heal this? Thank you so much for all your time, you are fabulous, please keep up the great work you are helping many people with, even me in England 🏴󠁧󠁢󠁥󠁮󠁧󠁿 UK! Kind regards, Mary. ❤️

    thank you for this video. i read that book. i've been dealing w a bad impingement for several months. pain is at the rear delt. ever since i started dead hanging my range of motion has increased but am still experiencing pain when my arm is internally rotated. i will try the other exercises you demonstrated in another video.

    Doing the shoulder shrug with arms on the wall makes both shoulders and my neck feel better. It's very effective.

    Dr.David thankyou! This clip & the one where I managed to work out I have nerve pain & it's coming from my neck have crystallised that I've been misdiagnosed & prescribed the wrong exercises! I have scalene issues, neck issues & shoulder impingement (although ultasound guided cortisone showed subcromial bursitis so it's progressed obviously). Thankyou! I have maxxed out my health insurance for the year but your exercises have eliminated my pain in days

    You talk sense about the shoulders, I have been swimming for 10 years now, lifting a heavy objects or carry any heavy shopping bags give me pain on my shoulder ( right). I was diagnosed with RCRSP, I shall follow-up your exercises that you recommend will let you know the results afterwards.

    I feel that chronic shoulder impingement never goes away. If it is not so severe, I think these exercises work. Doctors never had me get an MRI done and went to many in El Paso. Luckily was able to get my hands on some medication from Mexico. Hoping it relieves the inflammation in my forearm. Don't have the best health insurance so we shall see how this medication works.

    Hi Dr. David. Your videos are very helpful. I’m about 62-year-old male and fell while jogging on my elbow. That was three months ago. I have mild pain occasionally during the day, but I sleep well at night as long as I lay the right way.
    I’m hesitant to go to my doctor, because I think he’s going to immediately say that I need an MRI and all these expensive tests. What are your thoughts?

    Heading to surgery on the 15th of April for the rt. shoulder. The orthopedic surgeon said he wants to cut a good sized incision into my shoulder on the end of the rt.collarbone, trim 1/4" off and clean up the bone spurs. I had a shoulder surgery on my left shoulder which was done laparoscopically. They cleaned it up less aggressively and I had good results with three very small cuts to accommodate the scope..20 years since then. I am 68 and the arthritis has set in on the rt. shoulder, had two steroid injections with little results. I do weight training and cardio classes 3-4 times a week but wondering if I really should be doing such an aggressive surgery. They called it clavicle resection to free up the joint. I don't know the surgeon but he seems nice. I felt the laparoscopic approach may be better so it isn't such a long recovery. Probably less chance of infection. The MRI report also said there is a slight tear to the rotator cuff also said there's bone edema. I asked him if he would smooth out the slight tear laparoscopically and he said he won't do that. hmmmm...It's hard when you are listening to a professional hoping for the best. Everyone has their own opinions but I certainly am feeling anxious about my dominant arm and surgery. I don't know anyone else in California to go to. Thanks for your explanation..I am searching.

    Question... if a person has a arthritic condition... such as PMR, does that condition magnify the impingement? Or maybe even help the bursitis or tendinitis to form quicker?

    David, I have marked subacromial bursal thickening with bursal calcification consistent with calcific bursitis and extensive supraspinatus calcific tendinosis. Can I heal without surgery? I am female, 40s.

    I’m not sure if I have something co-occurring going on or what but… I had what started as just a pinched nerve feeling in my shoulder or arm. It was just uncomfortable.
    I rested and it went away. Then one night I slept longer than usual and woke up & the pain was unbearable. I did tons of stretching and rested my arm and neck w heat for about a week or so and it got better. Walking helped also.
    Now it’s been over 4 weeks since that first day. My whole right side of my body gets tight, my neck tmj area feels swollen or numb, it goes down my neck to where it meets my shoulder and is tender there, then I can find pain below or behind that and over a little. Kind of where the hump is. Sometimes it feels like a knot. Crazy pressure and pan on shoulder blade or next to it. Like I can’t reach it. Now it’s going past it, and pain and pressure below that in the mid back. It’s going down to my hip on the same side and wraps around to the front.
    When I lie down I’ll get spasms in the front of my shoulder on my upper chest, I can feel my back shaking/spasm sometimes. When I close my eye on this side of my body and try to focus with the other eye, my face muscles around the eye that’s closed twitch. Same with smiling.
    That’s when it’s bad. It’s like the whole right side of my body is effected. The pain in my arm and shoulder back is the worst. Is this related?
    I also have had a dent in that side of the neck before it reaches the arm for years. I’m not sure if it’s from when I broke my clavicle or what but I always thought it was a little odd & could be related. It’s a pronounced indentation and you can see it. My neck cracks very easily and grinds when I do rotation stretches.
    Sorry for the long comment I just had to verbalize since no one has seemed to be able to help.

    I have impingement/tendinitis in my left shoulder for years. Pulled a muscle in my shoulder at 18 doing bench press, this led to an “Anterior capsular repair” surgery at 25. Shoulder has always been weak since. Have had impingement/tendinitis ever since due to weakness and I’m 50 now. Which exercises would be best to help with this? Flared up recently and I’ve REALLY had enough now. Thanks.

    I workout at the gym plus I was using trekking poles while hiking then I did some trees removal and my shoulders started hurting so I backed off for 2 weeks and they're better. I wonder if the labrum gets inflamed? I can't figure it out. Stretching didn't help but it didn't hurt.

    the strange part is i got full strength and range of motion with my shoulder like raising up no issues, raising to the side, only when i internally rotate thumbs down will the pain trigger on the supraspinatus / acromion, the only range of motion i’ve lost is picking things heavy in-front of me (single handed) putting my arm behind me and as stated earlier twisting my arm internally thumbs down. is this normal for an impingement or can it be deeper than that?

    I have pain in my bicep. I also have popping, cracking and snapping. The ct arthrogram confirms a superior labral tear. One surgeon agrees and the second surgeon said he doesn’t know how to read a ct arthrogram and he was guessing as to if he was reading it correctly. He still says no, that I have impingement. My pain has been in the same place for 3 years I had a subacromial injection a week ago. It has made my shoulder worse. I was in p t and they had me doing hanging and it made my shoulder worse. That’s why I wonder if the first surgeon is right and I have a labral tear and not impingement

      CONCLUSION:
      Subacromial impingement with mild supraspinatus and
      infraspinatus tendinosis and overlying subacromial bursitis.

    Ugh... I'm a CNA and also have terrible posture and over the last few months my shoulder has really started to hurt I noticed it hurts when I raise my arm up over my head and I can only raise it up so far before it really hurts and I can't reach to scratch my back anymore like I used to and I'm pretty sure that I have this and I'm sure it's from my posture plus my job and the constant repetitive motion of pushing and pulling and lifting but I hope to find some exercises to help thanks to you!

    Is it okay to continue exercises that dont cause any pain? For me, deadlifts dont cause pain. Is that okay to do with an impingement or rest from all exercises that uses the arm at all?

    First question: So when would it be appropriate to do external and internal rotation exercises to strengthen the rotator cuff muscles?
    If there is no eccentric movement then the muscles can't get much stronger which is needed to keep from getting the muscle imbalance in the first place?
    I understand pushing like exercises initially because it's a more baseline stability exercises. You have one exercise (the windshield wiper) that's compressing the numerous into the socket and getting external and internal rotation in there.
    Second question: My thoughts are the exercises you mention, some hanging, then addressing the shoulder shrug, lats, teres major, and pecs by doing lat and front raises, face pulls, rows, lat pull downs, and pushups.
    Then do the accessory exercises of internal/external rotations. Is that correct?
    My third question: if you are doing pre hab work to ward off any shoulder impingement for a overuse over the head athlete (swimming). Do you do internal/external rotations as accessory exercises to big shoulder complex movements?
    Hopefully you are able to answer this for me!! I've watched a lot of your videos and your comments about stability makes sense but it's hard to let go of some of these core shoulder exercises every single PT I've known until I've come across you have done.

      I only mentioned external and internal rotation because they are most popular, but there are a lot of others to that I can think of that involve bringing the arm away from the body to put the rotator cuff muscles through light eccentric moment to build strength. Really looking forward to your response!!! I'm learning a lot.

    After two months of my shoulder starting to hurt (I thought it was shoulder impingement), Im starting to get aches and pains everywhere in my arm. Sometimes it feels like some of the bones in my hand are broken, sometimes it feels like my hand is about to separate from my wrist.

    I have a Rotated cuff, I've tried the exercises but IM in too much pain. Can you give me any advice?

      Hey Elaine, sorry to hear that! Maybe you are trying to do too much. It may be a good idea to start exercises at a lower intensity. The key is to start small and work your way up trying to avoid making the pain worse. Here is a playlist of videos that may be able to help you:

    I actually have impingement on one side and a small tear of supraspinatus on the other, and the impingement is more painful and bothersome

    my pain is at the middle of my left shoulder and biceps area. its been 3mos tried to rest it but i still feel pain specially when putting clothes.

    Hi Sir. i got my shoulder impingement from playing badminton. i followed your exercises and it was healed. but after playing again often my impingement came back. Can tell me what should i do to avoid this again. thank you!

      Hey @midnightmusic46, we are so sorry to hear your impingement came back! Don't worry, most of the time these things come back from overdoing it, but if you take your time going back through the exercises and rehab you'll recover just like you did last time. What's important here is, making sure that you integrate back to your sport slowly. Regressing is pretty normal, but the best way to ensure you'll be healthy and able to play for a long time is to make sure you start slow after taking your time strengthening your shoulder. Make sure you are doing movements that require you to press (push-up position type of exercises) and shrug/elevate your scapula (shoulder presses or shrugs). Also, hanging!     Can impingement in the shoulder cause pain when your shoulder hits a 90 degree mark? Thats where i feel pain in my shoulder

    Does sleeping on your side cause this?

      Great question! Sleeping on your side can cause some shoulder problems especially depending on how rotated the person is. However, if that is the most comfortable position to sleep in, then it is still okay to sleep on your side. Then it becomes important to perform exercises/stretches that can help reverse these changes.
