As far as the 18s or 20s I'm guessing you'd have some clearance issues to the fender lip, those are quite a bit taller than the tiny 16s on this truck.

This was stock.leaf pack with the 4" block. Wheels are 16s x7 I think, tires are a 205/40R16

Nope, still lots of frame clearance.

Is there supposed to be weight on suspension components when doing suspension parts? Or is that excluding u bolts?

For the rubber bushings yes there needs to be weight on them when you torque them to spec, U-bolts just hold the axle to the leaf pack so you can simply torque those.
