Your videos are very informative and honest. I can tell by how detailed you are that you probably are a great surgeon who is meticulous and pays attention to detail. I just had my all on 4 surgery a week ago on both top and bottom at a great place in downtown Chicago. Had my remaining teeth removed and the implants done. The palette bulk of the temporary set annoys me a lot but I know it’ll be much better once I get my permanent zirconia ones in a few months. I think a great video idea (if you haven’t done this yet) would be on what to eat after the surgery, both right after in the first couple weeks and then until healing takes place and you’re ready for the final set. I see some places say nothing but a liquid diet and then a soft diet of food that can’t be chewed but only super smooth foods like pudding, yogurt, etc and then others say more than that is OK as long as it’s relatively soft. The info out there is pretty vague and contradicting aside from everyone saying nothing hard or crunchy. Also another idea is how to brush the teeth right after surgery for the first few weeks. My instructions just say to brush teeth with a soft toothbrush gently but I was so scared and it is super painful right after. Plus with stitches and wounds I didn’t know if I should avoid the gums, avoid the stitches, etc. I did call and ask but before that I searched online because it was during hours my dentist wasn’t open when I first needed to know. People have so many questions and when you spend this much money on something you want to make sure you do everything right.

My concern with the all on four option for my upper arch are the micro gaps that are created with angled abutments. They will create an entirely new host of issues, especially with the bone loss and over time inflammation. Decisions,. decisions. For the upper I'm leaning towards an implant bridge.
Not out to set doctors against each other, but I would love to know your opinion on the 3 on 6 treatment from Utah? Seems that implants are usually placed day one with a very attractive temporary. Price seems comparable to all on four but you retain your natural gums. Hard for me to see how that's not beneficial. But I'm also speaking as a layman
Isn’t it much better do do the all on 4, but spread out: first extraction, then bone graft(if necessary), let it heal 4 to 5 months, then place the implants and let it heal again for 4 to 5 months and then the final denture? All good implant surgeons/prosthodontists advise to do it that way instead of doing all in one procedure.
very good point. back in residency in 2008 - 2011 that's the way i always did it. it was tried and true. when i came out i did my first all on 4. a patient asked me for it and i warned of the risks. it was a lot of eggs in one basket. it went great but i still never pushed all on 4. over the years more and more people asked me to do it for them so i was like ok, here are the risks and as long as you're ok with it, we can do it. but after success after success and not over 11 years having done tons of all on 4, i think its a waste of time to do it the old way. i would feel comfortable doing all on 4 in my dad's mouth
About to undergo a dental bridge with the Veterans administration. They decided that due to my young age pulling seemingly good teeth right now would not be good and the best for me. I have already had the large bone graft and have had the time to heal. As this is provided for me at no cost, I think it might be best to do the bridge and have viable teeth if ever the bridge were to fail to have anchor points for an all 4 after I age. Thoughts? Also, they stated that they need to reshape my bottom teeth to get my bite to work. My bottom teeth have moved into the area where my top teeth reside. Can you make a video on reshaping teeth?
I have had an unusual set of circumstances with my teeth. I went to my dentist to have my teeth cleaned (it had been 13 or 14 months since my last cleaning). My teeth had been fine and rather suddenly because of a dry mouth problem they think was caused by medication, almost every tooth in my mouth had started decaying at the gum line. It is a devastating blow to hear that . I had not noticed because it was in the back side of almost all my teeth I don't notice the back side of my teeth very often. I have to have a meeting with the dentist to discuss my options. He could not recommend dentures due to dry mouth and dentures would be difficult to keep in and feel comfortable. At this point he is trying to decide what to do and says he's never seen anything like this in all his years of dentistry. So I am blown away and anxiety ridden about what he will suggest or consider. It's basically almost like a doctor telling you they have to take all your fingers off and give you artificial ones to replace them. I have considered what they call "permanent" dentures or all on four. But I am drowning in anxiety at the thought of having all my teeth pulled. I've never had anything like this to hit me as badly and make me feel like i have no control over my "health". because I have always had pretty good teeth. I need some help and reassurance. I know you are in a different state. I am in Tennessee, but not sure of my next step until I have this sit down conversation with the dentist. He talks like I have no choice in the matter and need to have most all my teeth pulled. As i said before, it's extreme anxiety that I am experiencing thinking I will never be the same again. Hi Dr., thank you for your informative videos. I have level 3 advanced furcation defect with tooth mobility level (1) on my tooth 14 (maxillary1st molar) ONLY. The bone loss (furcation defect) was caused by periodontal disease which has been treated with irrigation. My gum is responding very well to irrigation treatment and it has become healthy again. There has not been calculus and tartar on my teeth as I have had regular tooth cleaning with hygienist. I brush and floss my teeth daily. My teeth have always been quite clean. Now, there are 2 options for treatment: 1) extraction and dental implant 2) root canal treatment and bone grafting with flap surgery and, finally after 3 months, putting a crown on the tooth if RCT and grafting surgery become successful. Do you think the 2nd option can be viable and successful? I'm now choosing the 2nd option and having the root canal treatment. What graft materials and tissue stimulating protein do you suggest for bone grafting?
I have an old Implant that is sticking up 5cm. I am 58 and have significant bone loss. I have lost the top ridge where my teeth go dentures are hard to wear. Three dentist are involved and it will be 100 thousand dollars when I am finished. All on four on top Implants singularly on bottom. What is wrong with this picture? Why did one back out?
This may not be the right video but I had a question my wisdom teeth removed over a week ago the bottom ones were horizontal I am young when I smile or open wide anything where I shift the left side of my face I feel like a knot on my left side below my teeth numb maybe idk what the word would be all of my gums teeth tongue lips feel fine my voice sounds normal but how I speak feels off. I also noticed it comes and goes when I’m laying down I can’t seem to find anything on it do you have anything? That could ease my anxiety? This is such a loaded question anything could help
sorry i've never heard symptoms like that. what i'd recommend is giving it at least a month and see how you're doing. try to not think about it too much because it definitely does add to the anxiety

no such thing as zirconia coated titanium. there is definitely zirconia implants. there seems to be less cases of peri implantitis with them than titanium but its still possible
I just got the surgery. All on 4 is a permanent option unlike dentures that you glue in but can take out each day, or snap in dentures which sort of look similar to all on 4 in a way if you don’t know much about the differences. But anyway the All on 4 implants can only be removed by your dentist once they are placed. The person who has them can’t remove them. It’s a great option and the closest you can get to natural teeth. Plus it prevents bone loss unlike dentures and you won’t have your mouth caving in. Hope this helps answer your question.

I just watched a video of a Doctor Who said he prefers the implant bridge so much more. And he mentioned the germs getting in there.

I think more dentists push all on four as a good alternative just because is easy money, after all, is the less complicated system to put in.
