  • There are a lot of rumors out there about Tourniquets and some could cause a lot of problems for a person suffering from a life threatening bleed. Most of these rumors come from the old way of doing things.
    If you haven't been to first aid training in a few years, you might be thinking you might be operation under the wrong information.

    Really appreciate the info. Growing up in Boy Scouts in the 90s, tourniquets were shunned or an absolute last resort with low chance of success and high risk of limb loss. I’m upgrading our vehicle first aid kits to include CATs and feel better having this info.

    Grew up thinking you could only apply for so long before toxins formed, therefore you had to release the pressure after so many minutes and then reapply! Thanks 👍

      You can do that, sooo release every 2 hours? Even if it's for a few seconds it would help save the limb

    In the military we had successful limb restoration up to 18 hours after. With shrapnel and gunshot wounds people can easily bleed out by the time you realize applying pressure wont work. We did things like applying a tourniquet immediately then the medic would assess if the wound and applying an Israeli bandage before removing the tourniquet. We would also keep one attached to each limb. If you loose your legs then you have two left on your arms to put on.

    A question for today's violent times with lock downs and clearing shooters. 4:24 You have packed wrapped the wound EMS has not shone up for whatever reason the time is over the 2 hour mark can the tourniquet be loosened and the wound be monitored? And if so does the 2 hour clock reset?

      Good question Will. This is a more complicated answer then I have room for here. Combat medics operating in austere environments are taught to reevaluate all TQs and, if indicated, convert to a pressure dressing before 2 hours have passed, or when the tactical environment allows. But this is easier said then done and it can be dangerous, especially if you don't know how.

    I had a full knee replacement surgery 3 years ago. They used a tourniquet on my leg and the surgery took about 90 minutes. After the surgery, my leg was completely bruised up from my foot up to my ass. My thigh muscles have never been as strong after that.

    A buddy had one on for 3 hours before getting it off, I started to get pretty nervous for his leg after I seen a picture and the timestamp was about 3 hours after application, thankfully he can move his toes and stuff, and should regain 100% mobility, though he is currently healing as that happened 7 days, 4 hours, and 46 minutes ago

    I like that sleeve tattoo I got one in Amsterdam at a place called Harley’s hash haven back in the 80’s (Wolf’s Head) I was thinking about putting a full body on my left arm, how much was that tattoo on your arm?

    Great Video! Question: What if the tourniquet is loosened even so often to allow blood flow and then retightened asap? Will this allow a longer "safe" time for use?

      Thanks Richard! The answer is no. A TQ should never be loosened after it's been applied. This is the old way of doing things and not recommended in modern medicine.
      There are a variety of reasons, but the most important one is that your Casualty needs all the blood in their body possible, and every time you loosen a TQ, the casualty is bleeding out even more.
      A TQ should never be removed unless under the direction of a qualified Doctor.

    What about speed of application? Would a tourniquet be preferable because you can use it faster? Should both hemostat gauze and a tourniquet be used in conjunction with one another just in case?

      I say that if you are faster with one TQ over another than you should go with what you like best.
      And should wound packing be used along with a TQ? The correct answer is no. Bleeding should be completely controlled with the TQ if it is applied correctly. However, I wouldn't mind a little extra precaution, so I might use both if I'm certain I'm not going to need it for another wound, or another casualty.
      It's important to remember resource management. If I have multiple casualties, I wouldn't use both gauze and a TQ, because I might need that for someone else.

    My 6 year old son had his blood drawn yesterday and the tourniquet was very tight and left on for approximately 3 minutes. My son was pushing his arm away until finally at 2 minute mark she was able to put the needle in. Than she had a hard time getting the tourniquet off, maybe it was too tight. Anything i should be worried about, any damage can be done in that time frame to muscles, veins, organs? Couldn’t sleep the whole night.

      No if your worried about your sons growth development even though you wrote this 4 months ago your son will be fine it’s different from an adult getting a tourniquet applied due to your son still being in development but based off of study it’s the same but if it’s not a serious injury release tension in the tourniquet and see if you can plug the blood with gauze if blood will not stop shooting out that’s when you retighten the tourniquet fully and then loosen it every 20-30 minutes and then retighten until you get to a medical facility I hope this helps your question if you ever run into that issue
