    When your skincare doesn’t work for you, it can be the most frustrating thing. Last year, my acne went from manageable to severe seemingly out of nowhere, and everything I previously used no longer worked. I went from a .5% salicylic cleanser and bp spot treatment to using Differin, 2% salicylic cleanser, 10% bp wash, high frequency, chemical peels, etc. Despite giving each product 2-4 months at a time, my acne continued to get worse!
    My Dermatologist saved my skin! Not only did I get on a much simpler routine, but I was also able to figure what internal factors and lifestyle habits were exasperating my breakouts. My skin showed significant improvement in the first two months.
    Consulting a derm when I was at my wits end was the best decision I made for my skin❤️ I only regret not doing it sooner.

    I found that the cleanser is the most important step and the step you should spend the most time on (even a double cleanse). Spending about 2 mins actually washing my face (not scrubbing!) really brought a difference in my skin, in addition to consistency. I think a lot of people might not realize they aren’t cleansing properly and putting actives/moisturizer on top of half cleansed skin-which is a recipe for disaster. (At least for my acne prone skin it made a big difference)

      Yessss 👏 as an esthetician, I spend a really good amount of time cleansing my client’s skin and prepping them for the next steps. A lot of people are not cleansing their skin long enough. Thank you for bringing this up!

      @@ThereSheIsMakeup oh yes, I think it’s the most important step and the one people generally slack on. And the rinse, for residue. I probably spend the same amount of time rinsing my face off as I do cleansing 😅 it’s made a BIG difference

    These are wonderful tips! I think another reason why skincare may not be working is lack of using UV protection!
    I have been using Adapalene for a couple of years now and it’s been the single best ingredient I incorporated. I did “buffer” it at first over moisturizer and do find that when I get a little sensitive, it’s usually because I used too much product or was out in the sun too long without reapply spf.

    5 laser treatments in, and I’m working on getting rid of a brown spot on my cheek and broken capillaries next to my mouth. The treatment is doing wonders. I regret not visiting a dermatologist sooner. I would’ve saved an enormous amount of money and effort😅 All those products I’ve used did nothing for that brown spot. All it took were some laser treatments to fade it out of existence.

    There were points while watching this video that I felt you were talking directly to me! I've just restarted adapalene after being disappointed by retinaldehyde to control my acne, and I am definitely an ance sufferer who has always felt left out of the vitamin C game. Great video, its confirmed my intention to get in to see a derm as soon as I can get an appointment!

    strip everything back to the basic was the best decision i've ever made. with a huge boom of products launches especially from k-beauty brands, i used to feel the need to get the new products whenever a skincare influencer reviewed it. as an acne sufferer, none of those healed my skin. why? because i'm using too much. i have been in a phase where i use 3 toners, 1 essence, 1 serum, 1 ampoule, a lotion moisturiser then seal it off with a balm moisturiser in a single routine. it took me a good half an hour to finish my skincare routine. after 4 years, deep down i felt tired. i invested so much effort, so much money, so much time in my skincare routine but nothing really works. until one time i stare at the mirror after shower. i was thinking... what if i use those money to go to a derm and get a proper consultation and a proper acne medication instead of trying more products back then, i won't be feeling so dissapointed. i did see a derm and was prescribed a tretinoin to treat my acne. i was instructed to keep my skincare to the very basic and i did just that. i keep my cleansers because those are just basic hydrating oil based and water based cleanser, cicaplast baume as moisturiser, a sunscreen and ofc my acne medication. my skin shows a lot of improvement after the first 3 months mark. after that, i add a salicylic acid solution from PC and a vitamin c serum in the routine and that's it! my skin continue to show improvement for the upcoming months. i didn't change my skincare routine ever since. it is simple, straightforward, quick and effective for my skin and i'm really grateful for that decision i made back in march 2022.

    I tried your recommendation for moisturizers for oily skin. I am 55+, oily and I now know I need to use something to repair my barrier. I will most likely always be oily skin type and I'm fine with that, more important was taking care of it! Thank you, always great informative info!**

    I implore you all who are watching to listen carefully and take this advice to heart. In my humble opinion, this is one of the most important videos ever made in regards to good skincare practice and realistic expectations. I'm at least a decade older than Dr. Ellis with none of her medical degrees, but I've had over 25 years of trial and error and dermatological consultation and all of this information is right on the money. If you're struggling with acne, be encouraged. With proper treatment, the acne-prone skin of your youth that you hate, will be the beautiful, dewy, wrinkle-free skin you'll be grateful for with age. The backdoor benefit of acne medications are the anti-aging effects.

    Consistency is very important, I can attest to that from my own acne journey. But it also backfires. Whenever I brought up concerns about dryness and irritation after long term treatment, the many doctors and estheticians more or less dismissed me, telling me to keep it up. They had me use benzoyl peroxide, salicylic acid, and AHA’s daily. My skin was clear, but literally cracking in places. I really wish more professionals in the industry actually listened to their patients. I had to give up on doctors and heal my skin alone, and that’s very tough. I got my acne under control with gentler options on my own, but my skin barrier hasn’t been the same since.

      Consistency is the key in the consumption of acne treatment, but it's also vital in knowing when to stop the acne treatment. It's similar to any drug use, overdosage is never good. When your acne has cleared uo, it's advisable to get back to basics, gentler products, not the harsh acne treatment.

      You can better your skin barrier! Use fresh aloe vera directly from the plant. Clean the stem with water and wipe it down with a paper towel!'cut the plant in chucks then slice off only one side of the skin only when you take it out of the freezer.. hold on I will explain further ahead. Since you are going to use the otherside with the skin as your holder. Put the chucks in a tupperware then into the freezer. At night before bedtime take a chuck out of the freezer! Slice off one side of the skin let it sit out for about 5 minutes then grab the side that has the skin with your hand! then the side that has the aloe smooth it with some pressure on your skin all over! Cheeks moving upward! , then apply chin in circular motion ! Upwards forehead, nose following the shape of the nose! , neck upward! even chest if you like. Then stop! wait for a few minutes and repeat until you finish the juice.
      Leave it on and go to bed. You need to monitor your skin. Do this every night. You may also want to
      incorporate I would say after 1 week of using the aloe purchase hydrocortisone 1% apply to cleanse skin wait till it drys ( air dry) apply the hydrocortisone then apply the Vanicream moisturizing cream on top of it. While doing these treatments you must monitor your skin. I understand very well what you said! I would like the Dr. To give us names of good dermotologist we can trust ! Like for instance I live in Chicago. Fyi I bettered my skin barrier by doing these treatments! Wash your face with a gentle cleanser like Vanicream! Also i recommend a Argan oil from a brand called Skin Deva very affordable and authentic! It's one of the best argan oils I have tried. It's real fyi. You can tell by the color & scent. I know how it is to have a skin barrier that is out of whack. I am going to purchase a strawberry seed oil from a brand called Bybi They are from the UK look it up if you like! I think that's a good option for you too. God bless!🙏🤲

    Definitely give products time to actually work.... I swear by some cosrx products that AT FIRST I was like "this does nothing for my oily skin" 2 months later I'm like "wow these things are amazing"

    Can I ask a question? I recently experienced the lovely condition of perioral dermatitis and had to put my usual spa-grade skincare on pause for a few months. I swapped them for drug store derm-approved products (LRP and Cera Ve) while I was rehabbing my skin. I found that I really like these products! My sensitive skin is better served by the simple glycerin-based formulas and, thanks probably to the protective power of dimethicone, I haven’t had a single pimple in weeks (it used to be normal for me to have 1 or 2 at all times). I might be a convert to this simpler regimen, but I have all this expensive skincare sitting around not being used, including all my actives and exfoliants, and I don’t want to throw it away. A lot of my old items are from Eminence Organics. They are natural products and are very aloe, plant/fruit acid and oil-heavy, whereas my drug store products are more clinical and very glycerin and dimethicone-heavy. It’s like I have two different approaches to skincare in my bathroom and I don’t know how to re-build my routine. Can these two approaches be used in the same routine? Will they contradict each other? I’m most worried about moisturizing with both oil and silicones products (creating congestion) and my new favorite LRP Toleraine cleanser not being strong enough for the heavier oils in my “clean” products.

    I am 41. I used tretinoin .25 every other days for over year and saw no difference. A few months ago something started to irritate my skin, so I stopped tret. My skin looks the same with or without tret 🤷 I use it for anti-aging. I will resume using it, but I would love to see more results.

      That's likely because .25 is not a high enough strength to have an anti aging effect. That strength is meant more for acne or for getting used to tret. Once your skin has acclimated to .25 most people move to .05 then ultimately 0.1% which is the optimal strength for anti aging 😊

      There are other alternatives available within the retinoid family (I don’t consider retinol effective); some are geared to more sensitive skin types, but still as effective.
      0.025, 0.05, and 0.1% tretinoin will all give you the same result in the end, and one is not better than the other (the only difference is higher strengths tend to work quicker, the end result will be the same after a year on any concentration).

      If you go to Paula's Choice site, they sell sample sizes. I bought a travel kit size and realized I only like 1 or 2 of the items, and didn't order the other products.

      @@jmwonderland2601 Thanks! Years ago, the drug stores used to have smaller sizes, I think they were meant mostly for people traveling, but I used to pick them up, to try them out.
      I really wish they would have larger sizes with sunscreen, these 1-4 oz sizes is really too small. I think these small size were originally based on being so they would fit into a beach bag or a purse for work. But since its part of skin care now, I would like some 16 oz sizes, to try to keep costs down.

    HI Dr. Ellis! I'm using a 1% Clyndamycin/5% Benzoyl peroxide treatment for my acne, but also want to use my 0.05% tretinoin. Recently I've been alternating using them each night, but I'm wondering if I can layer them together instead? I've read conflicting things... Thank you!!

    I used Adapalene, Tretinoin(0.025 then 0.05), BP (5% , 2,5%) Salicylic acid even sulfur and nothing worked, which sealed the deal for me that my acne is "hormonal".

    My derm started me on 0.1% Tretinoin and the Rx was written for daily use. I knew that was to much, to fast even though I had previously used Differin gel nightly for a month and Vit C daily for 3 months. I started with once a week tret for the first week. I’ve been on every third day for last three weeks. I’ve gotten through the flakies, finally. Should I start every other day at this point? My derm is out of town for a month so no help there. Also, is it safe to incorporate Vit C back in to my regimen. I’m slowly getting back to using niacinamide at this point because it was irritating my skin something awful.

    Hi, Dr Ellis! Can you please clarify if skin tends to heal faster after subsequent microneedling sessions? My skin was horrendous after my first session (extreme redness, sandpaper texture, peeling) and it took 5 days to heal completely, but I just had my fourth treatment, and I noticed that my skin has bounced back a lot faster after each one, despite my aesthetician using deeper needle depths (2mm now) and making more passes across my face. My skin just looks a little tender after each treatment nowadays, and it looks back to normal after 3 days. I suspect that the skincare products I'm currently using have also contributed to my skin recovering faster than before, but I'm just curious if skin can get "accustomed" to microneedling?

    I just started using trentinoin (sp) and look every morning to see if the spots are gone. Actually I think they are a tiny bit lighter. Lol
    I did use Olay retinol for over a year.

    My derm told me to use my altreno only once a week because of my sensitive skin and rosacea, but I'm wondering if there is anything that could be weaker that I could use almost every day 🥺

      I think you just have to wait till ur skin adjust to increase frequency. Adapalene or differin is the weaker option but maybe the Altreno once a week is still better than using a weaker option daily? If you could even tolerate that daily. Let your skin adapt. It just takes time. I know I have super sensitive skin I can’t even tolerate adapalene everyday.

    But how can you work on one issue then pivot to another?? For me I did that for my acne and then when I got it under control and changed to next issue my acne came right back. Hormones through all this out of whack

      You’ll still want to use products to fight acne but it’s more about tackling one issue at a time so you’re not introducing a ton of new things to your skin all at once!

    How do you treat patients with severe eczema? Which steroid creams do you find most effective and what are your thoughts on dupixent? Do you have any tips for more natural healing that actually help? So sick of looking and feeling like a monster!

    i don’t use tretinoin every night because i alternate my tretinoin nights with chemical exfoliation nights. how can i move towards daily tretinoin when i know i need to chemically exfoliate? (esp bc tretinoin still makes my skin flake)

      You really don't need to chemically exfoliate more than 1-2 times per week (and some people even do it less depending on their skin needs/type). If you're still flaking with Tretinoin, try alternating with soothing skin barrier reparative products instead.

      @@kellyc3897 i see. my chemical exfoliants of choice are super gentle so i actually don’t notice much of a difference nor am i breaking out, but maybe that i means i need a lower dose of tretinoin or higher dose of chemical exfoliants

      @@zq5127 remember that Tretinoin is going to resurface your skin as well so even though your exfoliants are gentle, the combined impact of them with the retinoid could be greater. Ingredients like niacinamide or azelaic acid can be great complements to Tret. If you're committed to alternating with exfoliants then at least wait until your skin is fully adjusted to Tret and not flaking anymore.

      @@kellyc3897 i’ve been on tretinoin for 2 years 😭 but i’ll see if i can go to a lower dose of it, currently on 0.05% bc 0.025% didn’t do anything for me when i first started but maybe now it’ll work

    I am 20 years old. I started accutane last summer (1 year ago). I feel like I aged 10 years :( I am 3 months post treatment, will my skin ever go back to how it was ? I heard that isotretinoin, like retinoids, thins the outer later of the skin temporarily, but thickens the inner layer on the long run. Is this true ? I need hope 😭 I also heard that accutane continues to work up to 2 months after the treatment, should I still have hope that my skin will re thicken?. I've been using differin, vitamin C, hyaluronic acid since I stopped accutane and I reapply sunscreen every 2-3 hours. Please help?

    I usually see something positive after 1 week. That’s what gets me going and after 1 month everyone should be swing a big difference 😬
    Just saying and I’m 53 years (and maybe this is the reason why 🤷🏻‍♀️) 🤔
