      also if you can give alternatives to L- glutamine please, my gut is so messed up that whenever i take L- glutamine( EVEN A small dose, it gives me a migraine or anxiety) please help @@ByronHerbalist

    I have SIBO and just had an endoscopy done which showed that I had ZERO digestive enzymes in my small intestine. The doctor decided it had to be a "sampling error" and told me not to worry about it. But now I'm worried, lol.

      Worry about getting another doctor ASAP! I can't believe these idiots, l'm sorry but he's putting your health in peril..l had malabsorption forever undiagnosed l believe caused me to get 2 (needing 3) joint replacements...worse "idiopathic " OSTEONECROSIS misdiagnosed until l was in a wheelchair at 52. Great diet, never smoked or drank, no autoimmune n TG today l walk no cane! New York City's best hospitals BTW! Get better Friend 😊y

    Informative video! However, I would like to point out something that many health professionals overlook. Pancreatic issues can also be caused by Mast Cell Activation syndrome. It sounds scarier than it really is, but it affected me. Mast cells are in virtually every organ (including the pancreas) and when mast cells flare, inflammation ensues. I've dealt with EPI on and off for the last few years. My flare-ups usually lasted 2-3 months at a time. The Enzymes worked to a point, but what helped me more than anything was mega doses of quercetin. A good bromelain-quercetin supplement helped to calm down the inflammation. Again, I realize, I may be in the minority when it comes to the causes of EPI, but it is definitely something to look into.

      Great input! I'm quite obsessed with pancreatic elastase and see huge benefit to tracking and correcting this nexus point of digestion and the immune system. So glad you're finding support. Quercetin is hugely beneficial for many and I recommend it all the time when indicated. Love it!

    My name is Juan. I currently reside in the states. I understand you don’t see patients outside of Australia, however I was wondering if for this once you could consider making an exception.
    About a year ago my life changed. I was a healthy 29 year old when suddenly I got very ill. I lost 65 pounds in 6 months and doctors couldn’t find out what was the reasoning for it.
    After experiencing symptoms for nearly a year the doctors finally had an answer. I had sibo and Epi which was most likely triggered by antibiotics I took in the past.
    My doctors here in the states haven’t been able to treat my condition correctly yet. They lack understanding on the topic and as the time goes by I’m getting weaker and weaker. My elactse level is

    Thank you thank you thank you. This is the burning question I have at this point in my journey, and hopefully in a year I will have the answer. I strongly suspect it's a chicken or the egg scenario, but It's likely a risidual sibo case for me...
    For anyone reading, I had Sibo in 2018 (breath test confirmed, but wasn't told which type) and went through a round of general antibiotics (flagyl) from a well-meaning but clueless Gastro doc. It wiped out teh sibo but I had to learn everything about gut health myself and do the work of rebuilding, because the doc I saw literally told me "If it comes back I always got the flagyl up my sleeve"... literally cringe thinking about that guy. Doesn't know what he's talking about.
    Elastase was 115 about 6 months ago but had a abdominal CT scan to check for things like cancer etc - all was clear. I can seriously relate to the anxiety your patients feel.... It just recently tested again and it was 64. Which of course is scary to see.
    I use the foodmarble breath tester and still suspect lingering Sibo (methane elevated on tests, constipation symptoms etc.), and I've been doing a DIY approach over the last year + on and off to address the sibo (oregano, allicin, dysbiocide and FC-Cidal, even the Sibo yogurt from Dr. Davis' Super Gut book which actually helped my a lot) and I have pretty normal/regular BMs but can fall into constipation from stress. But my transit time is still 36hrs+ last I checked, so everything is very slow moving unfortunately.
    I take Betaine HCL before high protein meals, and Enzymes with every meal and I swear the enzymes are pretty night and day usually. They significantly help me. But I'm not taking a prescription dose, just a normal dose for an over the counter one (currently use ones from Bioptimizers and Silver Fern, tried another called Pancreas 8x that I got from Fullscript). But even with enzyme supplementation my elastase isn't improving, despite having a decrease in symptoms if anything.
    Anyway, I'm finally seeking out a new gastro doc again to get a workup to see what's actually going on - it's tough because at this point I feel like when I'm not in a phase of slowed motility or constipation I feel pretty good. But to me this looks like a moderate case of sibo that's prevalent enough to cause other issues downstream (upstream?) to the pancreas and the LAST thing I want is to be more at-risk for a sibo relapse...
    Buy thank you @bryon for making this video because I think we need more awareness about this topic in the sibo community.
    it's scary to see low elastase, we all instantly think the worst I'm sure. which doesn't help our situation either!
    But just knowing it's common in your patients is good to know. I think the pancreas Enzyme Replacement Therapy (PERT) is still very helpful and should be the first thing we do (however, the prescription one Creon in the states is ABSURDLY expensive. With insurance it was $1500 a month, so I opted to get stronger OTC brands) but it sounds like addressing the Sibo and sealing the gut is the key here.

      @@sailuvijl6621 I am using Silver Ferns digestive enzyme for every meal, and some bigger meals I also add in 1-2 capsules of MassZymes from Bioptimizers. That one is more expensive but I think it’s the better enzyme. If having a lot of protein in the meal I also always add 1-3 caps of Betaine HCL + Pepsin at the beginning of the meal.
      Enzyme timing is also at the beginning of the meal, or if it’s multiple I space out and take one at beginning, and the rest in the middle.

    Don't forget that if chyme is not acidic when it enters the small intestine, the pancreas will not be stimulated to release pancreatic enzymes. I have EPI ( elastase 70) and none of my symptoms fit with any specific disease. I took a Heidelburg test which confirmed very low stomach acid.

    I had covid 19 BAD, march 2020. 104 fever for a week! Double pneumonia. They think this had something to do with my epi.
    I also have elevated bilirubin.
    Like a point over. It fluctuates.
    Had EUS. Mri. mrcp. Ct. Blood tests. Showed nothing.
    Had a colonoscopy and endoscopy. Nothing.

    Just received my comprehensive stool test report and SIBO breath test. Both Elastase and Secretory IgA are very low. Waiting for an appointment with my Naturopathic doctor and will give the report to the medical doctor as well.

      Yes I have pancreas elastase as well. Did the stool test. I was prescribed pills but it’s not working. I go for another appointment on 8/8/24

    This was a huge lightbulb moment for me! I had a GI Map and basically had no elastase-1. My son is also having similar problems to me. Do you know if there is something external that could cause people in the same family to experience this? We did just find black mold in our house and I'm wondering if that could be related. Or something viral maybe? Thanks for your amazing and super helpful content. It's really so helpful.

    Hello, first and foremost, thank you for this video and your knowledge/advices. I live in France, therefore cannot order your product. Is the oregano essential oil potent enough to fight SIBO ? What posology would you suggest please ? Didn't do the test for SIBO yet but i have most of the symptoms since 2 months : constipation, heavy bloating, gas (sometimes hundred of times a day), reflux, feeling full after eating, burping a bit aftet eating (never happened before). I'm 33 years old, took a lot of IPP and got some food poising during a travel in Asia. Thanks in advance.

    Trying to solve my own issues, I found that if the food chime leaving the stomach isn't acidic enough due to low stomach acid. Then the CCK won't trigger the pancreas to release its enzymes or it only tells it to release a small amount. Not sure if that sheds light on the connection or not. Just a thought.

    I have read that supplementing enzymes causes the body to further dial back its own production and creates a dependency on the supplements.

      I haven't seen this in any study but definitely floating around the interwebs. I would use and recommend them if helpful and wean off when possible tracking elastase-1 if it was below 200 to guide the weaning off process

    I have both and my dr says just take the enzymes don’t need the xifaxin anymore. Should I question that ? My symptoms have all but disappeared on the enzymes .

    I have low elastase and I think I have sibo but my gastro doesn’t do anything about it. Doesn’t want to give me a breath test.

      I agree this doctor is brilliant. There is a Sibo Dr, she’s actually called the Sibo Dr. on the internet in Australia. She works with people in the US. There’s actually quite a few Australian functional medicine practitioners that will work with people in the US as well.If this can happen it would be awesome!

      could be poor bile flow, could be bacterial or fungal imbalances too among others to explore. Finding the root cause is half the battle!
