Methane SIBO Treatment: Case Study

    I commonly see Methane SIBO, also known as constipation SIBO, in my patient population when using the SIBO breath testing. This condition often presents with bloating and distention, burping and excessive flatulence and the most common or predictable symptom of constipation and slow gut transit time (time that food takes to travel from the mouth to the toilet bowel).
    Today I wanted to share a case study from a patient of mine who had been trying to clear methane SIBO for some time. They had used common frontline therapies but after each breath test found that the methane gases were still quite high.
    We implemented some common treatments that most of my methane SIBO patients get and we layered in some advanced treatments to try to help shift the environment in the gut and reduce the excessive methane gas.

      @@ByronHerbalist Todd, I have to add, it would be very invaluable if you could do complex cases like Methane + Hydrogen Sulf because those are some of the hardest things to treat and perhaps even share diet approaches that those patients undertook to see better success!

    So helpful! I found it interesting that you use high dose antimicrobials as a last step, when my naturopath prescribed those right away. I forget my exact breath test results, but they were quite elevated at baseline for methane SIBO. I’m done with that treatment and still have slow transit time. I plan to try your suggestions. You have given me invaluable information, thank you!

    I've been having a tough time getting treated, decided to be my own doctor and start testing. Found methane archaea not in the normal range, found Enterobacteriaceae and Psuedomonas overgrowth and, found gram negative bacteria like Salmonella(typhi) Enterica though unsure of active infection, all this from one stool test.
    My gut test showed abnormal inflammation and high risk for dysbiosis, worse still not enough good bacteria to counter this.
    So now I'm starting to self treat with Allicin and Atrantil, I definitely wouldn't mind advice but I'm overwhelmed by how expensive and ineffective the traditional medicine of this has been alone.

    I have been suffering with severe Sibo many many years. I have tried so many solutions, holistically none worked. I finally did research and have switched my whole diet from low FODMAP to carnivore. Within two weeks I noticed a significant difference after a month I am feeling 100% better.

    Thank you so much Todd. This is really helpful. I would love to hear about more case studies with Methane SIBO or Lyme Disease. It's true that every case is different but it's very interesting to see the different combinations of things that help people.     Thank you so much for sharing. It's great to see alternative treatment options. In regards to the turkey rhubarb is it possible to take in powder form as the tinctures contain alcohol. If so could you please give the dosages. Please continue sharing as it is very useful info.

    Struggling with super high methane levels. Naturopath has me on Allicin and Neem, but the strong herbs have caused me to have urgent diarrhea 1.5 weeks into treatment. This is frustrating because the first week felt amazing. SIBO was first treated with antibiotics and relapsed after a month. Using triphala as a motility activator but it causes some uncomfortable bubbling. Unsure what to do next

      Are you noticing anything with the Triphala? I've been on it for months, my 6th bottle I think. I don't think it's doing much.. still slow transit time. And that's taking HCl, TUDCA and cascara sagrada too.

    Hi Byron
    I’m in UK had an extremely stressful last 2.5 years and started developing some really nasty symptoms that you would think was LPR acid reflux
    The symptoms exponentially got worse after taking high doses of HCL and Pepsin for 8 weeks and I was sent to a gastroenterologist they carried out all the tests and came to conclusion I had severe GERD and after a SIBO test they said I had extreme methane SIBO (started at 30 and peaked at 87)
    The gastro thought the hiatus hernia was causing all the problems and programmed me in for Linx surgery on 20th July 2023 next Thursday
    In mean time in April the things that were stressing me were concluded and unbelievably I gradually started to feel better still have a lot of symptoms (blocked ears, reactions when eating tremors and some palpitations but nothing like before
    Had a last consultation and doctor noticed I looked and acted a lot better and asked what had happened and postponed the operation stating that it looks like it may have been the severe SIBO and decided to treat the SIBO to see if that might save having a operation
    He has prescribed Rifaximin and neomycin would you think this is correct or is there another way

    Thank you for your video! I would also like to see more case studies if possible. I think it's very important and an amazing way to learn. I have SIBO/IMO for 8 years, I'm desperate to get better and I've been trying everything, but nothing seams to work. Also I have endometriosis and severe pain every month, especially wen my intestines move are having a hard time, so if you have any cases of women with this kind of mixed pathologies that with be excellent!       Hi Byron
      I have had a terrible time for last year with Acid reflux in the throat and ears eventually moving to chest middle of night and daytime attacks trouble breathing etc
      Went on omeprazol a PPI and this caused bloating gas and generally felt terrible
      went to GI and he had tests carried out including SIBO test turned out to be very high methane results no Hydrogen
      Results started with base breath of 50 then after taking the sample growing to top of 90 plus around 90 minutes GI doctor has not contacted me but I’m quite constipated do small movements but can’t clear bowel and have intolerance to a few foods and alcohol and soft drinks it’s been a KNIGHMARE
      Are these crazy high resting at 50 and can it be cured or managed
      Currently taking
      Ginger and Artichoke x2
      Nutri Advance Candex SIBO x2 each meal
      Digestive enzymes x1 each meal
      Is there anything I could add like Turkey Rhubarb

    Hi Todd, my recent Genova SIBO breath test is high methane at 24. The levels are 21, 15, 11, 9, 9, 15, 22, 24 in the time columns. Hydrogen time columns are under 10 apart from last 2 columns 26 & 35. Looks like I have SIBO and LIBO.
    My main symptom is constant weight loss, went gradually down to 123lbs/56kgs from 167lbs/74kgs in June 2019 despite eating well. Scans/tests have been okay. Very underweight at 5'9" struggling to walk. Was positive for methane SIBO in Jan 2021 tried GI Microb-X, low fodmap, S Boulardii, PHGG, Caprylic Acid, Candidefense and many more supplements since but still lost weight. Have constipation, mild bloating, stomach is hard after eating, slow motility, lots of belching, intermittent gallbladder/bile flow pain esp when eating fats.
    Hoping my new practitioner can treat methane SIBO properly? Can I ask do you see many patients with weight loss issues? I really need to stop the weight loss and restart absorbing food. Thanks Kevin

      This is a less common symptom I see. A good strong enzyme can help some here!

      @@ByronHerbalist Many thanks Byron Herbalist Todd. I'm in London UK btw, have lived &worked in Sydney and Melbourne previously good times! Yes, I've had Pure Encaps pancreatin in 2021, as was diagnosed with minimal pancreatitis in Jan 2020, and this helped reduce pancreas pain when eating. Then in 2022 I found I don't really need pancreatin as pain was greatly reduced & my elastase improved from 208 to 378, and sometimes over 500 w/o supplementing. I do have Terranova digestive enzymes sometimes, I find tudca helpful for reducing gallbladder/bile flow pain (my bilirubin is always slightly elevated at 26). But nothing helps stop the weight loss or regain weight. Scans/tests have been okay. Recent Metabolomix/organic test shows low B6, B12, very low B9, slightly low digestive enzymes, very low taurine, low good bacteria etc.
      I've read methane SIBO can cause weight loss as the bacteria can "eat" the food and stop bile/enzymes from working. It also blocks absorption. I think previous SIBO teatments have not worked, as supplements made me more constipated. I should have 2 bm's a day which is my healthy normal, but only have 1 or less. I will start going to go to saunas and will hopefully get a good treatment plan from my practitioner including an effective prokinetic &biofilm disruptor.
      As awful NHS GPs prescribed PPIs/ranitidine hs2 historically (2018-19 on &off) and had antibiotics for HPylori (2015), SIBO could be a root cause. I understand this congestion can lead to pancreatitis and gallbladder issues.

      Tests and scans have been fine so was wondering if I too have had some kind of SIBO. My consultant has basically fobbed me off with IBS!

I did have leaky gut (elevated zonulin), pathogens, high secretory IgA, high Geotrichum candidum, high e coli, low bifidobacteria/Akkermansia etc in my gut test in 2021. Gut test in 2022 with another lab showed no leaky gut, normal secretory IgA, no pathogens but high e coli, low bifidobacteria/Akkermansia remain. The latest test does not cover Geotrichum candidum so may need to check this. Recently, as I have intermittent gallbladder/bile flow pain, I've looked into the Moritz liver flush, I've completed 2 flushes and I've passed a lot of stones so appears I need to keep going to fully cleanse. Also following my practitioners methane SIBO treatment &biphasic diet no sweet potatoes, grains, fruit etc. Hoping my body can stop losing & start regaining weight soon! ☀️

    Hi Byron!. Does shortness of breath and brain fog common symptoms in methane sibo patients? I have an abdominal distention(bloating) that doesn't settle down, it's like a permanent symptom of distention and with that comes shortness of breath( slight but still noticeable) did you ever find these symptoms in your patients? I tried raw garlic but doesn't seem to help, i start to have odor problems, same when i have eggs( probably sulfur metabolism problems).. could it be the H2S sibo type?

      If your abdomina distended then your lungs get pressure.
      You will have shortness of breadth.

      If your root cause of SIBO is mycotoxin exposure, this can definitely cause shortness of breath. Low iron also will do it.

    Thank you so much for sharing, this is a fascinating case study. Did the patient stop the herbs before re-testing? And, when using herbs containing anthraquinones for chronic constipation associated with methane production do you have concerns about reliance? So far, I’ve resisted stimulant laxatives for that reason but am I being over cautious? 🙏🦋

    I’m almost certain i have sibo, i have slow gut function, bloating… all my problems started after taking medication for over a year for heartburn/indigestion, then i had my gallbladder removed which made my digestive system even worse!. I now have quite a rare condition called sphincter of oddi dysfunction which is extremely painful before i had my ERCP to cut the sphincter but it can still come back and cause more pain in the future. My bile flow seems poor but taking things that stimulate bile can actually irritate my sphincter of oddi. I’ve started taking pure extra virgin olive oil each morning and will take garlic daily now too. 😀

    Wonderful video. The case studies are very helpful. I’ve got a case that has been really tricking me. Constipation dominant symptoms with foul smelling gas but lactulose breath test showed >2ppm methane and very little >8ppm hydrogen. Flaxseed, hydration, movement, magnesium, and probiotics are not getting things moving. Patient used Prucalopride and noticed a significant improvement in bowel movements and symptoms. Without the rise from hydrogen and methane in the breath test, what should we be looking for in this constipation dominant case? Thank you Doc!

    I have been strugling wirh methane sibo for so many years now. I cant get the stuff you are recommending in my country. The only ting that ease the symptoms is oregano leaf. Terrible disease. Keep up the good work and make more videos with methane dominant sibo cases.

    The problem in California is finding Doctors that test for SIBO. There are no Doctors within three counties around where I presently preside. Even then , I don’t know if there are any Doctors past these counties ? .

    I like the case study - thank you for sharing. It’s helpful to see a real world experience with testing to support it. Have you come across any herbs that minimally disrupt the good bacteria? I have SIBO, IMO, and Candida overgrowth, but also low good bacteria which makes it difficult to incorporate herbs which can affect both the bad and good bacteria.

      Yes this is very important! I like to start with herbs that balance first and if needed moving towards stronger antimicrobials. Sometimes we can get away with probiotics and probiotics as an effective treatment. More on that soon!

    My breath test showed normal hydrogen and hydrogen sulfide, but methane at 40. Fighting constipation with mag oxide and gum arabic (soluble fiber). I would like to try some of the herbs you recommend to speed up transit time (turkey rhubarb, ginger, etc.). Which one is the best to start with and where is a good place to find them online?     Todd, Thank you so much for your videos! I just found your videos by searching archea and sibo on TH-cam. I have been watching and taking notes on several and lightbulbs are turning on for me. It is helping me to understand how previous treatments have failed and how 1 treatment helped me feel amazing for 6 months but didn’t last. What you said - all about the constipation and gut transit. Question: What is your opinion on NAC as a biofilm buster and how does it compare to rock rose? I’ve been taking NAC but wondering if it effective as a biofilm buster. Thank you.

      Thanks for the kind words! I don't frequently use NAC but have colleagues that rely on it as a biofilm disruptor. I find when we really need to disrupt biofilms we need the strong stuff. Bismuth can be tricky to work with and is a prescription here but when it is combined with a few extra bits it can really disrupt and shift treatment resistant bugs. H. pylori is a big one here!

    Good video. I myself have been struggling with SIBO for over 10 years and have not found the right combination of herbs to finally get rid of my Methane SIBO for good. Was wondering if your liquid herb combinations are sold and shipped internationally. Or if you can recommend a protocol. I have used Biotics ADP, FC Cidal, Allimax, biome balance with limited success, but was never able to fully eliminate my SIBO even when combined with Rifaxime and Flagyl for 5 weeks.

      I've been taking Biocidin drops with GI Detox + for three weeks now. My stomach cramps have nearly disappeared but I still have terrible constipation and slow evacuation. To eliminate the constipation, I've been taking 1,000 mg of magnesium citrate twice a day but that gives me diarrhea.
      I'm trying the low FODMAP diet but it's too early to tell if it's going to help. I am very confused as what to take and what to eat- it's very frustrating.

      @@markh2719 No, Mark I'm not positive. I self diagnosed myself with SIBO and IBS -M because my GI doctor was a waste of time. I found another doctor, via a friend, who suggested I get a MRI, a colonoscopy and an endoscopy to rule out Crohn's, colitis, celiac disease and diverticulitis. Cancer has been ruled out. So, hopefully, this doctor will diagnose me and I'll be able to get my life back again.

      It depends on what the underlying cause is.
      Carnivore drastically decreased my methane SIBO symptoms while on the carnivore diet, but because I had undiagnosed Hashimoto’s (hypothyroidism) the symptoms came right back when I discontinued carnivore.

      Carnivore. It keeps my Hashimotos in remission like nothing else. Meat from ruminant animals, fat, salt, water.
      I used to include black coffee but eventually cut that out and saw huge improvements.
      I supplement with lactoferrin (for iron levels) and selenium every day. I also take vitamin D3 (with K2) and a methylated B complex.
      I feel amazing. I hope you'll find what works for you. 🙏

    I have bad breath and sibo symptoms. I took antibiotics course and it's clear that halitosis is caused by bacteria. But unfortunately symptoms return in 3 days after completion.
    How can i solve this?

Maybe try low carb, definitely stay away from sugars breads and processed foods. Google foods to prevent sibo flair ups

      For mixed SIBO I'd treat the presenting symptoms and hope to clear them both. If not simply adjust the plan to target the treatment resistant form. Hope that helps :-)

    What if you have both hydrogen and methane sibo? I tested positive for both and they said I had more hydrogen than methane. I’ve had this 10 years now and no GI has helped me. I can only eat 10-15 foods now. My symptoms have been rough and I’ve developed another autoimmune disease. I’m losing hope.

      Excellent news! Garlic can generally be taken quite high dose if tolerated. Cloves too! On a weight basis for cloves you can go all the way up to 1 to 1.5 grams. Encapsulating the ground cloves helps, it’s potent!

    Hi man love your videos, i wish i was in Australia and work with you! My methane peak is only 16ppm in the large intestine, but I’m so symptomatic! Is that even possible? I’m taking rifaximin and Allicin and my constipation is way better but the bloating is still here! Also when I take allimax it seems I can’t digest the garlic, I keep burping for hours

    I think antibiotics screwd my stomach up early in the beginning of the year I think emeprazole screwed me up with constipation and I also took Ciprofloxcacin does antibiotics cause mathane sibo been like this for 4months lost alot of weight do to constipation is there anything I can do I heard that I need to repopulate my gut with good bacteria

    I reside in Canada, my son 19 was diagnosed with methane Sibo. After a year of tests, drs and specialists I did my own research and had him tested for Sibo. I would love your help with herbs…not a lot of help in Canada. But if you know someone pls refer them to me. My son is 10 pounds underweight and already tried the fodmap diet. It helped but didn’t cure him, he tried antibiotics twice and although symptoms improved slightly still not gone. I’m frustrated and so is he. Pls help!

    Can methane Sibo cause Gerd? I have had it every day for 18months and can’t tolerate PPIs, I took Biocidin for a few days, then stopped and ended up with very itchy skin for 6 weeks. Maybe I stopped the Biocidin too soon and maybe it was the Herxheiner reaction? My bowel movements are every other day….I have poor T4 to T3 conversion so I’m trying NDT hormone, I’m on levothyroxine at the moment. I’m not sure whether to treat thyroid first or whether to try say allicin first…..it sure is very tricky. 🤔😳 I have lpr too and my oesophagus isn’t always working as it should, I’m in the UK. I do have slow transit time, I’m bloated I think, but been this way for years so don’t know what normal is. Any advice would be appreciated. 🙏

      I am on NP thyroid and have methane SIBO. Here are some things I’ve found helpful:
      - Atrantil helps massively with bloating
      - Digestive enzymes help break down a heavy meal a LOT.
      - a capsule of ginger with a capsule of artichoke extract right before every meal.
      - a whole food, low fat, plant based diet 👍🏻
