Resident Evil Village The Battle against Salvatore, if his form the first time you meet him wasn't disturbing, wait until the Boss Battle when he constantly Vomiting Acid & your flesh starts to melt off the bone.
I personally was expecting the House Benevinento sequence from RE Village to make an appearance on this list. One of the best, if not the best, psychological horror scenes in gaming. Especially when you know what shows up.
As an honorable mention, I'd like to include Josh being "sawed in half" in Until Dawn. Reason being is because at first you think he's dead but it was fake. I won't give spoilers to those who haven't played the game yet.
i was just thinking Until Dawn when there are 2 people tied up and the buzz saw thing goes towards them and you have to pick only 1 to save while the other is slowly cut in half and you see the aftermath with all their innards dangling out. Shit legit freaked me out and I'm a grown ass man lol I STILL think about it.

But still, great moment, viceral action, tension going through the roof, all make up an amazing and horrifying scene. Ugh! I borrowed that off one of my dad’s mates not long after it came out; I was about 13 or 14 at the time and loads of things in that game gave me the fucking creeps. The monster in the school with a giant mouth that opened sideways. Every single time the environment changed into that nightmarish alternate world; especially the hospital. Probably every scene with Lisa; she just downright gave me chills from the moment she came into the game; she only existed in the alternate nightmare world so I thought she was a ghost or something. Where that policewoman gets possessed by that thing and she draws her gun and tries to kill you in that amusement park. That eerie place with normal rooms and nightmare dimension rooms mixed together near the end where you aren’t anywhere in particular (I think it’s where the bit you mentioned happens). That crazy old woman who goes on about God all the time. Even those creepy symbols etched on the doors you had to find the corresponding keys for - those with strange names that sounded like biblical names for demons or something - made me feel dread when I opened them. And much more besides… I’m surprised I actually got through the whole game to be honest haha
Infamous Second Son, the pretty early scene where Delsin wakes up in the hospital and you get hints and glimpses of just what the big bad did to everyone else in your tribe… by using her Conduit power to grow concrete shards and boulders inside them as a form of torture! Plus several of the scenes when you copy a new ability and see the original user’s backstory are pretty brutal, even the big bad’s (she chose to turn on a little Conduit girl in order to save herself, and it’s implied that the little girl went on to be the Origami/Paper Conduit from the mystery side quest, she never meets Delsin, but he promises to tell her story.)
The very first zombie in RESIDENT EVIL...we were in a dark basement. There had never been anything like that before. We screamed like little girls. I have no shame.

Most disturbing for me was probably Drakengard. There is a part when giant flesh-eating babies come floating out from the sky and start devouring someone. After that, you have to fly around with your dragon and shoot the babies out of the sky...
I haven't even played it but I've seen bits of the Outlast Whistle-blower DLC with The Groom... I thought I liked scary games but that took it to an entirely different level. That made me realize I'm getting too old for the horror games haha

Given how messed up MK's fatalities and brutalities have becomes should be number 1. They showed among the tamest lol but I guess they'd also get demonitized too 😅

In Grand Theft Auto 4 when niko is talking to mikhail faustin’s wife about the Yugoslavic wars. She asks him if he worries about his soul, niko goes onto explain that “After you walk into a village and you see 50 children, all sitting neatly in a row, against a church wall, each with their throats cut and their hands chopped off, you realize that the creature that could do this doesn't have a soul.” This scene was especially disturbing because it describes with detail the horrors that niko faced during his time serving in the military but it left a lot of people wondering and asking if niko witnessed these children being brutally executed, or if he had actually taken part in executing some of the children himself.
Another moment in Silent Hill 2 that rivals the Pyramid Head scene is the boss battle against Abstract Daddy, where we learn some really horrific truths about Angela's past. Other disturbing moments I can think of: Harverster - The scene where some cannibal babies devour their mother's body. Blackout (rather obscure game from Denmark) - A scene where you actually partake in the shooting of a snuff film. No joke. Postal 1 - The original ending (so twisted, the developers changed it for the Redux version) Super Columbine Massacre RPG - The whole first half of the game, to be perfectly honest (though I guess I can single out the moment when the game actually shows pictures of the shooter's corpses) Manhunt series - Some of the executions are legit disturbing, in great part because of how realistic they can be. Max Payne - The first level, at least for me (Michelle's cries for help are too realistic for its own good; plus the bad guys killed a baby)
There are plenty disturbing scenes in the underrated Little Nightmares games. But the scene in the second one where you get betrayed and it’s revealed you are the thin man, stuck with me. Gave chills

If you're going for a level of disturbing relative to ESRB ratings, Lusamine's fusion with Nihilego is up there. Also add the frozen Pokemon to that one. When you add Ardos wanting to blow up Citadark Isle with you and all of the Cipher subordinates on it, Pokemon has had it's share of dark moments.
Clementine having to sew her arm shut by herself and her painful reaction to it on The Walking Dead was pretty awful to watch. Should've made this list.

Whenever Nemesis chased me in the original Resident Evil scared me I stopped playing for awhile. The lickers were a close 2nd. But every time I saw Nemesis my heart stopped.
I think everyone forgot how terrifying it was playing max Payne and hearing the baby upstairs whine and his wife dead. the entire sequence was just terrifying but then again maybe I was just 10 lol
Nah WM, in Phasmagoria the part where the main gal gets her face ripped apart by a demon was probably one of the most disturbing things ever in vid game history. That was gruesome.
I have to agree. Not sure why but it seems like they picked one of the least horrible deaths in Phantasmagoria as their most disturbing moment...maybe because they couldn't even show some of the others? But some of the other things on this list they couldn't show either, so I'm not sure. For such an old game I didn't expect it to be as gross as it was.
There's a brief moment at the start of _Homefront_ where a little boy of about 4 or 5 has both his parents executed in front of him. It came out of nowhere and the way the boy cried hit me so hard as a father I paused the game and cried. I was so angry at the game for pulling what felt like a sick stunt and had no stomach to play more than about half an hour in before I quit and never went back to it. By all accounts it isn't that great a game.
I strongly agree with MW2's fourth mission being ranked at the top. It's such a morbid mission to slaughter innocent people in the game as it would be in reality, with of course these massacres ongoing today.

Honestly I think most scenes with Mephilis from Sonic 06 could make it onto a list like this, but if I had to pick one, it’d be the scene when Mephilis kills Sonic. I got so many nightmares as a child from that.

Persona 3: the bad ending Persona 4 golden: Izanami’s reveal Persona 5: all bad endings and the depths of Mementos Twisted Metal 2012: Sweet Tooth’s story PlayStation all stars battle Royale: the corrupted versions of the characters during the polygon man boss fight Spider-Man PS4: Sea of Poison and Yuri becoming a serial killer. Final Fantasy Xlll-3: bad ending Batman Arkham Knight: Professor Pyg side mission.
The area's the moment in video games for me was horizon 0 dawn. It was like my 2nd or 3rd time going down a zip line and I messed up and jumped a second time and she fell to her death. The animation of her silent scream sent chills down my spine.
Regarding I Have No Mouth And I Must Scream, I remember reading something about a game developer who had a son with cancer, and used his experiences in the hospital to shape Ellen's character when "that bombshell" was dropped.
No 20 was definitely the freakiest thing I've seen in game, if I didn't know better I'd swear link took a bunch of lsd before entering the shrine at lake hylia.
Special mention to the rooftop scene in Life Is Strange. You're forced to try and talk down a character from jumping to her death. Since your actions affect the narrative, there's a very real chance you can fail and she's dead for the rest of the game.
Halo the first appearance of the Flood. Those creatures are basically space zombies Batman Arkham Knight Jokers cremation. It was so nerve wrecking to burn Joker’s body while your in his face. It’s even worse when playing new game plus and the Joker comes alive to scream and laugh at the gamer. Final Fantasy XV episode 13. So difficult and anxiety inducing playing as Noctis alone fighting troopers armed only with his ring and sneaking around. GTA V torture scene. Do I need to say more? Injustice 2 Superman ending. So scary seeing Superman have Batman under mind control. COD Black Ops. Russian Roulette. It’s so disturbing not just watching Mason and Woods play the game, but also that it may have happened to real American troopers during the Vietnam war.
I should give an honorable mention to Splatterhouse 3 (1993) with the scene of Rick’s wife, Jennifer, turning into a mindless beast. The scenes of her right before the moment and when she became a mindless beast were terrifying.
Zero Time Dilemma deserves a mention. Because the game has timeline-jumping shenanigans, you are forced to kill every single character, repeatedly, often in horrific ways. (The acid showers--where six of the nine characters vote to kill off the other three, and because we have multiple timelines all nine get a "wash"--stands out, but there are plenty of others, as well.)
If we’re going with a Mass Effect moment, I would have gone with finding David Archer in the Overlord DLC. Seeing him honestly fucked me up when I first saw it.
INSIDE is definitely one of the best game out there, definitely better than most of the AAA games nowadays. Glad it made it on this list cause that scene was wild.
Even though it’s about 25 years ago when I played the first original Tomb Raider game on PlayStation, I still remember it because I always thought the last two levels were cursed af. That giant, half formed head and torso, er- _thing_ that breaks out of that egg and drags itself around on it’s arms you fight as one of the last bosses; the mummified cat things and whatever those other creatures were that hatched out of them orb things, and most of the red and pink walls appeared to have pulsing, moving _veins_ in them; as though they were _alive_ like you were inside the body of something… I don’t actually know if they _were_ veins or the old graphics of the time just made it look like that but still fkin creepy though.
Wasnt there a scene in , I think in a doom or wolfenstein game, where you were placed on a cart and then your limbs were sawed off to be rebuild or so? It takes quit long and its one by one you go from room to room and one limb at a time was removed. That was pretty disturbing.
Honestly nier automatas one of the best games I've ever played. It's a fucking masterpiece for what it did imo. The bit when you have to either choose to wipe pascals memory, destroy him or just walk away is crazy. No matter what decision you choose you choose wrong basically. That and the soundtrack really is phenomenal on that game.
Another disturbing moment is from AI the somnium files, that moment is the annihilation route where 4 of the main characters (Iris, Oda, So Sejima, and Boss) are killed and Date’s mind is swapped with Seito Sejima and Boss’ bodies
I'm surprised Martha is Dead didn't make this list. First off you literally cut your dead sisters stomach open and take out a bloody two headed fetus and then take a picture of your dead sister and her fetus together in some kind of morbid family photo. Then there's the flashback scene where the main characters mother literally cooks the family dog alive and force feeds it to a younger version of the main character.

Elizabeth's Lobotomy was very disturbing indeed. It was the first time I heard of the thing when I was 15. Crazy, but I love that game.
white phosphorous scene in Spec Ops: The Line should be minimum top 5. Easily one of the most disturbingly memorable moments in video game history.

Whenever Nemesis chased me in the original Resident Evil scared me I stopped playing for awhile. The lickers were a close 2nd. But every time I saw Nemesis my heart stopped.
I think everyone forgot how terrifying it was playing max Payne and hearing the baby upstairs whine and his wife dead. the entire sequence was just terrifying but then again maybe I was just 10 lol

Nah WM, in Phasmagoria the part where the main gal gets her face ripped apart by a demon was probably one of the most disturbing things ever in vid game history. That was gruesome.
I have to agree. Not sure why but it seems like they picked one of the least horrible deaths in Phantasmagoria as their most disturbing moment...maybe because they couldn't even show some of the others? But some of the other things on this list they couldn't show either, so I'm not sure. For such an old game I didn't expect it to be as gross as it was.

There's a brief moment at the start of _Homefront_ where a little boy of about 4 or 5 has both his parents executed in front of him. It came out of nowhere and the way the boy cried hit me so hard as a father I paused the game and cried. I was so angry at the game for pulling what felt like a sick stunt and had no stomach to play more than about half an hour in before I quit and never went back to it. By all accounts it isn't that great a game.
I strongly agree with MW2's fourth mission being ranked at the top. It's such a morbid mission to slaughter innocent people in the game as it would be in reality, with of course these massacres ongoing today.
So any playthrough of grand theft auto, saint row, mafia, the goodfather, state of emergency yeah sure killing inocents is sooooo disturbing

I think there's only 1 single "disturbing moment" in all of the Pokémon games, when Lusamine and Nihilligo merge in Sun and Moon.
The area's the moment in video games for me was horizon 0 dawn. It was like my 2nd or 3rd time going down a zip line and I messed up and jumped a second time and she fell to her death. The animation of her silent scream sent chills down my spine.

I should give an honorable mention to Splatterhouse 3 (1993) with the scene of Rick’s wife, Jennifer, turning into a mindless beast. The scenes of her right before the moment and when she became a mindless beast were terrifying.
Zero Time Dilemma deserves a mention. Because the game has timeline-jumping shenanigans, you are forced to kill every single character, repeatedly, often in horrific ways. (The acid showers--where six of the nine characters vote to kill off the other three, and because we have multiple timelines all nine get a "wash"--stands out, but there are plenty of others, as well.)

Even though it’s about 25 years ago when I played the first original Tomb Raider game on PlayStation, I still remember it because I always thought the last two levels were cursed af.
That giant, half formed head and torso, er- _thing_ that breaks out of that egg and drags itself around on it’s arms you fight as one of the last bosses; the mummified cat things and whatever those other creatures were that hatched out of them orb things, and most of the red and pink walls appeared to have pulsing, moving _veins_ in them; as though they were _alive_ like you were inside the body of something… I don’t actually know if they _were_ veins or the old graphics of the time just made it look like that but still fkin creepy though.

It takes quit long and its one by one you go from room to room and one limb at a time was removed. That was pretty disturbing.

Honestly nier automatas one of the best games I've ever played. It's a fucking masterpiece for what it did imo. The bit when you have to either choose to wipe pascals memory, destroy him or just walk away is crazy. No matter what decision you choose you choose wrong basically. That and the soundtrack really is phenomenal on that game.
Another disturbing moment is from AI the somnium files, that moment is the annihilation route where 4 of the main characters (Iris, Oda, So Sejima, and Boss) are killed and Date’s mind is swapped with Seito Sejima and Boss’ bodies

First off you literally cut your dead sisters stomach open and take out a bloody two headed fetus and then take a picture of your dead sister and her fetus together in some kind of morbid family photo. Then there's the flashback scene where the main characters mother literally cooks the family dog alive and force feeds it to a younger version of the main character.
