  • Post Op Nasal / Sinus Surgery - overview, instructions, what we typically recommend to do and not to do
    Note: these instructions do not apply for nose jobs (rhinoplasty)
    these are our general recommendations - these may not apply in all cases, and other surgeons may have different recommendations.
    Check with your surgeon regarding their specific instructions
    Pain may be managed with Tylenol and/or Motrin
    These can be taken together every 6 hours
    or can be taken alternating every 3 hours
    Narcotics may be prescribed for more severe pain
    If so, follow the instructions
    Many narcotics contain Tylenol - if so, don't take them with over-the-counter Tylenol at the same time
    Afrin (oxymetazoline) over-the-counter nasal spray can be used to slow, or potentially even stop, bleeding
    Neil Med Sinus Rinse should be used starting the day after surgery
    use it at least 3-4 times per day...and there is no upper limit
    Rinses should be high volume, and low pressure
    Avoid strenuous activity post op. Anything that gets your blood pressure up can increase the risk of bleeding (exercising, sex, etc)
    But don't just lounge around. Light activity like waking prevents blood clots in your legs.
    Most people only miss a few days of work unless it requires strenuous activity.
    Don't fly for about 7-10 days after surgery. The altitude and pressure changes can be uncomfortable and dangerous.
    Don't swim or get water in your nose - the only liquid going into the nose should be the Sinus Rinses.
    Don't take Sudafed or other oral decongestant as they can increase the blood pressure and risk of bleeding. Elevating the head and using ice over the face/nose and in the mouth can help decrease swelling and congestion.
    Don't aggressively blow your nose. Light blows and snorting /sniffing is ok.
    A little bleeding afterwards is normal. A "moustache dressing" can be used.
    If the bleeding is brisk, using Afrin and pinching your nose for 10 minutes may stop the bleeding. Ice can also help decrease bleeding.
    If you experience any facial/eye swelling or bruising, and especially if there are any vision changes, call your doctor immediately.

      I know they probably won’t see this, but I have a question. I just had sinus surgery and it went great. I can breathe pretty much perfectly. But I haven’t used a sinus rinse. I don’t like using them. Do I need to do that? What’s the risk if I don’t

    I've just had Septoplasty surgery a week ago and going back after 1 to 3 days is impossible! The meds and morphine was impairing my ability to focus and I felt very very tired for several days. Simple things like chewing and speaking caused discomfort and at times much pain. I would also NOT recommend going back too soon as this will put you into contact with germs, flus and other viruses which is not what you need so soon after surgery. Today is day 8 and I'm only now beginning to feel normal again.

      @@925sterling3 Hi there. The best way to put it is that I must have gone from 20% airflow on a good day to 100% nearly all the time. It's amazing.
      My right nostril was completely blocked at least 70% of the day. Hot days and warm nights were the worst.
      Yes I snored as well.
      If you have a deviated septum which is causing discomfort and breathing impairment then I'd highly recommend it.
      The results were immediately noticeable and the day after the surgery I was overwhelmed that I was able to breath properly and take deep breaths for the first time in 15 years.
      Its really nowhere as bad as I could have imagined and nowhere as bad as I've read about. Just take the necessary precautions and have enough rest after the surgery.
      Time has flown by! I'm approaching month 3 and only have a negligible amount of swelling (10%) which no one else can notice except me. If I had to do it again I would have the surgery tomorrow again without a doubt.

      @@Brent.1844 Thank you for putting your comment. I have stuffed left side for over 15 years niw and on topbof that my dentist screwed up and sent the implant into my sinus. Now that it needs to be taken I am worried. The doc mentioned about the risk of eye damages. I have time till midnight to think over and do or cancel my surgery tomorrow morning. It is strange that they told me about possible complications this late into surgery

    I had sinuplasty and turbinate reduction three weeks ago. First two weeks I was very stuffy and only the sinus rinses offered some relief. Into the second week I was blowing out huge long chunks of hardened mucous that once expelled gave much relief. This I was told is normal shredding of scars from the turbinate reduction. Now as I start my fourth week I have very little stuffiness and a much clearer nose. I had little need for pain killers. So it's a gradual healing process but very much worth it.

    Thanks guys a big help, had surgery today and have a slow trickle of blood and fluid and a very sore throat, surgeon recommends irrigation after 48 hrs. Definitely hard to sleep with a sore throat and plugged solid.

    Very informative. Recently I got my nose endoscopy done and found my right nasal passage narrowed down because of some bone deformation or bend. Doctor further recommended a CT scan and found minor infection on left sinus ( cheek) along with obvious bone bend on the right side (but no infection on right cheek sinus). He prescribed a saline spray two puffs twice a day in both nostrils (Day & night -before bed) and suggested a minor surgery to address the root cause of bone deformation leaving the surgery decision on me, depending how I feel and evaluate my issue.
    he didnt prescribe any antibiotics. According to him the use of antibiotics would be a temporary solution and infection would return back anytime once the antibiotic course is completed unless the core issue has not been addressed so in other words putting me on antibiotics was simply futile.
    If kindly allowed, i desperately would like to share my CT scan results with you for your evaluation and advise. I will be very grateful.

      Don’t do it I have deviated septum and I use spray … one of my friends have the surgery she ended up worst and even suicider

    I’m 2 days out of surgery. No big deal. Groggy for about 4 hours after surgery (have someone hang out with you at home). Took 3 pain pills through the first night, mostly for ‘just in case I need them’ but since that first night I’ve needed no pain pills or Tylenol. You’ll have a scratchy throat for a few days from the tube they use during surgery (not helped by mouth breathing). Mostly it’s a nuisance and very similar to having a cold: you have to rinse your nose with saline every few hours, which causes it to drip (and unlike a cold you can’t blow it). You have to sleep upright for a week and breathe through your mouth. The antibiotics may or may not give you diarrhea. I wish I hadn’t watched so many TH-cam videos before surgery- they had me SO stressed out, and really it’s nothing much.

    Had frontal sinuse surgery and the removal of a septum spur almost 3 weeks ago. Found out that I have had a sealed frontal sinuse cavity for at least 6 years and it was a fungal infection. I have been dizzy at times and I am constantly draining. I have had a constant headache for years but now it's not like before. I'm happy that the headache is gone but I found I really don't like anything inside my nose at all

      @@sandhya431 always a small risk of that tbh. This is why you have to be very conservative in treating things first. and then staged surgery (of the good methods), instead of just praying for the best.

      @Kiwi_4321 I still have sinus problems occasionally. My sinuses on the left side of my face feel like they pop often, and I have to rinse with a neti pot. Other than having a sealed off sinus cavity again, I'm scared of growing more bone spurs that will aggravate my sinus cavities again. For the most part, I'm ok, but when the weather changes quickly, the left side of my face will start hurting.

    I had all three of the procedures you rattled off that your office does. I will say you were correct, I certainly have “lots of blood.” Lol I also have the plastic splints in there that are stitched in, but I can’t even feel them to be completely honest. My septum was so jacked up it was completely collapsed on the left side. My nose is actually straight now!

    Hello Doctor and community, hope you and your loved ones are healthy and safe! My old new problem has brought me back to this reliable channel once again. I had got my fess and septo done last year in March.
    Everything went well for a few months after the surgery but since August last year, I have been having regular episodes of sinus infections with one thing in common, congestion/breathing issues on the right side/nostril.
    Have been showing to the surgeon regularly and every time I have been prescribed antibiotics and anti-allergies for few days. I showed to some other doctor and says that there has been some overcorrection because of which am getting dry and it is super-hot & dry here in Delhi which is why am getting these issues persistently. Am using Vaseline, oils, rinsing, steam but getting only temporary relief.
    Writing in the hope to get some advice that could help me get rid of this problem.
    Obstruction is constant on the right side only, sometimes I whistle while inhaling/exhaling. Please suggest.

      @@turiya9526 Same anti alergic on a regular basis and corticosteroid spray once in a while may be 3-4 times a month.I am no Doctor but septoplasty is not something one cannot live without.

      @@turiya9526 I am better now. My passages are much more clear now. What is the need of it, I had additional condition, nasal polyps, (25 years of history) that is why I went for it but I regret, operation should be the last resort. Have you taken second/third opinion. I got it done in Appollo Spectra, Dr. Lalit Parashar, he is fine, very experienced. But again operation should be the last resort, find some more organic ways. I am happy to share more info, if you want to call me.

      Had turbinate reduction surgery 2 days ago. Doing the nasal irrigations several times a day. Been driving me crazy not being able to blow my nose. And now my eye won't stop watering. Did you have this issue

    For more than a year I've had a non-stop headache in the middle of my forehead. The Doctors finally figured out it was my frontal sinuses and I had surgery a few days ago. My headache was gone immediately after I had the surgery and I felt much better. But the very next day my headache returned exactly as it was. Now I don't know what to do. Is there a reason why this could happen? Can another surgery possibly fix this? I will try anything to get rid of this. Thanks in advance for any suggestions or help.

      A few days is too soon to make any determination. In the early healing process, it is completely normal to have more swelling and pressure than previously. Hopefully it's improving again at this point.

    i had my septoplasty with fess with b/l partial inferior turbinectomy surgery but even after 2 weeks of surgery i am unable to breathe properly in one of my nostrils can you tell how long does it takes to breathe properly

      @@RBLong0928 Without knowing what was done, I can't say for sure. The tearducts drain into the nose. So if anything is swollen, blocked, inflamed, etc... in the nose, things can overflow up the "pipes" and come out the tear duct openings

      The Neil Med Sinuses rinse can cause the rinse to drain from the eyes if the nose is very congested, while using it. I usually stop rinsing in that nose if it occurs, until the nostril becomes less congested. I've never had the surgery though.

    Great vid Doctors, thanks for helping us. I need to do my sinus surgery because I had polyps and my nose cartilage not straight this cost me lost smelling and breathing 30 to 40% only from nous. Doctor said it's easy surgery, not cost a lot of pain and bleeding only first days and you can go back to work after 2days from the surgery. Does this true? and can I ask you about sleeping can I sleep first days and showering and eat? And can I breathing first days? Thank you so much

      You can definitely eat and sleep, but there will be some discomfort. When you can go back to work depends on what was done and the nature of your work. You really should just go by the recommendations of your doctor.

    was just finishing up my two weeks post surgery feeling great and i hemmoraged. bled for five hours in the emergency room cause they couldn't stop the bleeding - not even with packing. my surgeon finally arrived and coderized the two spots in my nose thta were bleeding. seriously the closest i've felt to dying. that blood would just run down my throat and i was coughing up clots every five minutes for FIVE HOURS

    What is the best sleeping position for someone that had a sinus and oral surgery and has facial swelling? I read that lying down on your back should be avoided and that the patient should be upright. But how can anyone sleep upright comfortably?

    I’m currently entering day 3 of recovery from sinus surgery and am grateful for videos like yours. While my condition appears to be improving i plan on taking the next two days off from work. Will I be able to go to work Friday morning after I go in to get the splints/nasal packing removed? How invasive and time consuming (generally) is the post op follow up appointment?

      Completely depends on the type of job you have, your pain tolerance, whether you're still on pain meds, etc... As for the follow up appointment, depends on the amount of debridement (if any) that needs to be done. Sinus surgery varies significantly in how much was done.

    I had a deviated septoplasty surgery 6 days ago. My forehead up to the top of my head is real sensitive to touch on my right side. Also when I get up in the morning my forehead hurts so bad like congestion. Is there anything wrong? My doctor said it doesn't have anything to do with what he did.

    i was told i need some pathways in my sinus cleaned out so i am less prone to bacteria infections... currently is hard to kill an infection do to narrow path ways... I never had an infection before... i am 63 now... what is the procedure i need called?? thanks

    POD 2 and still having double vision post BESS. What could be the best intervention. We called surgeon and he stated area needs healing.

    i'm day 1.... i've been using neil med and its derivatives for a while, i just went to town the day of surgery instead of the the day after surgery.... oooooooooh... okay. so i'll stop now.
    i'm curious if you're given a narcotic+tylenol, and doc said don't overlay with more OTC tylenol, how about switching between the two? is that okay?

    I'm on day 5 post-op. I feel less painful in my nose right now. I still cannot breath through my nose tho. It's been bleeding and building up dried blood. I have to wash it every few hours. My worst nightmare was that the anesthesia dose seemed too strong for me and i was not allowed to eat or drink anything after the midnight before surgery. I ended up having urine retention. I went to the ER right after I had the surgery few hours later. Now I'm having a catheter put on. It's been 5 days though. I don't know when things will get better 🥺🥺🥺

Now I can breath 90% nicely ☺️ (my left nostril is fine but it still has something to do with the right side, not really bad though. It just couldn't be 100% clear and healthy compared to when i didn't have to suffer from nasal congestion and all that stuff). i had surgery in May and now it's October. Things started to brighten up in July I guess 😬 thank you for asking! ❤️

    Thanks a lot for the information.I underwent Navigation -aided sinus surgery 10 days back and my surgeon said I am recovering well.
    However I have asked to do Nasal irrigation twice daily for next 3 months.Is that ok? Will that cause any water retention
    (if not drained out properly)
    in all my newly opened up sinuses?

      I would recommend going with what your surgeon instructed. Every sinus surgery is different. Everyone's anatomy is unique.

    an inferior turbinate reduction can cause permanent empty nose syndrome suffering.ent experts like chhaabra and steven houser who see ens patients every week say that empty nose syndrome occurs in about 20%of patients after a conservative inferior turbinate reduction.i suffer terribly 24/7 from empty nose syndrome.both my inferior turbinates where reduced between 25 and 30%.ens is not rare.an ent would not have a turbinate reduction done on themselves knowing that it could cause them immense suffering from empty nose syndrome

      Mine changed slightly. The nostril doesnt look as big one one side. They look more even now. It’s a bit more straight, but for the most part it didn’t change drastically

    I had my concha bullosa excision today. lots of bleeding and mucus. I started rinsing as soon as they told me to take the cotton pads out of my nose. They didnt specifiy when its ok to blow my nose but I might try tomorrow as I am full of mucus,blood and congestion

      @@bongsalamat1969 so 4 month later. I almost never use nasal spray which I used maybe 5 times a day or more. Breathing so much better. When I Breath through my nose it doesn't feel like I'm suffocating or not able to breath. Bad breath I'm still struggling with though. Definitely worth the surgery

    I have bilateral concha bullosa. Right is so big that it squeezes between the nasal wall and septum. Left is still small. Right is giving me difficulty breathing, pain in right eye, pressure headache, tinnitus, dizziness. Will conchaplasty procedure solve most of my symptoms. It's been a year now. I want my life back.

      Concha bullosa can narrow the passages and cause symptoms. I can't say in your situation if they are the cause of all that you are experiencing. Only someone that has seen you, taken a thorough history, performed an exam, and scoped/scanned and reviewed the images can answer your question. And if you aren't sure after a 1st opinion, seek a 2nd and see how the answers match up.

      @@GeneLiuMD I saw a second doctor and made thorough tests and scan. When all meds and sprays didn't work her last option is surgery. It will be a reduction of the lateral part of the lamella but preserving the mucossa. She said she'll do enough just to make an adequate airways and restore ventilation on my sinuses. Do you think my doctor is on the right path?

      It varies depending on how much was done. Can be days. Can be weeks. Shouldn't be months.

    It will be a week after surgery tomorrow (correction of a deviated septum and sleep endoscopy) and I had constant dizziness for the first few days. Now I’m experiencing sinus congestion/pressure pretty bad, is that normal? After watching your video I will say I did stop the sinus rinses after day 3. Maybe I should continue

    Hi Doc and community, I am in the second week (9th day) of my recovery from fess and septo. Everytime I do rinses blood is coming out, sometimes looks fresh blood without any mucus. For how long is it normal to irrigate blood after surgery. My surgeon said 2-3 weeks. Is it normal? I have a lot congestion and without rinses it's not getting clear but when I do it I get nervous looking at my blood. Please let me know what I can do to heel up faster... Thanks!

      I'm assuming that you are not having continuous bleeding in between irrigations. Having some blood is normal. It all depends on how much bleeding you have, what your underlying medical conditions are, what medications you take, the exact nature of your surgery, etc... Best bet is to go with the advice, recommendations, and guidance of the surgeon that did your operation. Wishing you a quick and uneventful recovery.

      @@GeneLiuMD Thanks so much for replying. Much appreciated. Yes, there is no constant bleeding. Maybe some times when I apply more pressure on the bottle then blood comes in with the mucus, I'll try doing it more gently and will observe . I'm following the regime/meds suggested by doctor. I've something up there inside my nose which doc said will dissolve on its own over a period of time. But it's making me little uncomfortable. My left side has become little insensitive, I'm breathing through both the nostrils but sometimes it feels only 1 is working. Doc said it is because they burnt some tissues of the one side and when the new layer of tissue will come back then I will be back to normal. Over there in US, do docs still burn some tissues? Here in India it seems a practice. They scraped, cut and burned some tissues.

      @@mackfrmbly We generally utilize tools and techniques that minimize the trauma to the skin (mucosa) of the sinonasal cavity. There are many devices that utilize lower heat delivery than traditional methods, and some that don't heat at all.

      OK, I get it. Thank you so much again. I have one last followup question, I'm still not able to smell properly, how much more time I can expect to get my sense of smell back. This was the main reason I went through the surgery but am nervous despite of removal of polyps smell has not come back. My doc said it be back soon but am on pins and the needles. Please let me know what I can do to get it back faster 🙏

    I just had sinus surgery, yesterday.
    Surprisingly: I’m not in too much pain (in itself) but my main complaint is that I can’t breathe through my nose, which is the worst feeling in the world.
    My nose keeps bleeding (which is a common symptom, and I had to change my gauze 4 times, since I got home) but thankfully, I have a follow-up appointment, next week.
    I pray they remove the splints in my nose, next week, but I watched your video, a few days prior to my surgery, and wanted to thank you for taking the time to share this information with us. It helped me learn what questions I should ask, and what to expect. I even asked for Afrin nasal spray, per your advice in this video, and they gave it to me! (*whispers* Thankkkkkk youuuuuuu 😆)
    Many people on the Internet love to exaggerate, scare people, and make things sound worse than what they really are.
    While everyone’s experience is different: you motivated me to proceed with having surgery, despite the large amount of anxiety that I had, beforehand, so I wanted to show my appreciation to you for that. :)

      Had all three surgeries that he listed (septoplasty, turbonates, and the sinus roto treatment). There’s definitely mucho bleed-o. I also have a metal splint in there, but I can’t even feel it to be honest.

    Please make sure you tell people to use DISTILLED WATER.. NOT JUST WARM WATER. Sink water can cause you to die of a bacterial infection.

    Perfect!!! I has sinus surgery today and you answered every question I wanted addressed. The one thing no one addressed was the Afrin spray and that’s exactly what I wanted to know as well as how soon it was ok to start rinsing. Simple and straightforward

      @@DavidHernandez-sn1kk … the first two weeks was rough. Lots of drainage. Honestly, pretty nasty drainage for 24 hours straight. That was about day 7. Then every day got better and better. By week three, barely anything at all. After a month, I was completely healed. That first couple days, Afrin will really help. I can’t emphasize enough to use the saline rinse .. it’s a lifesaver. I still everyday when I shower do the saline rinse.

    how do I sleep then, with my mouth open? it's really uncomfortable and its costing me beauty sleep. Any thoughts? (when you have the chance thanks)

      If your nose is congested, then mouth breathing pretty much is the only other option. If this happens in the post op period, then hopefully it's a temporary thing. Other than keeping the head elevated, applying ice/cold, and irrigating the nose, there isn't too much else to do to help minimize the issue. There is no way to prevent/avoid it entirely.

      Sorry that you have had this happen. There is no simple, straightforward answer to your question. Depending on the extent of the sinus surgery you had done, and how long ago, via which techniques...determines the likelihood of potential recovery, what might have caused it, and what the options are.

      My dad lost about 80-85% of his sense of smell after this surgery, but that's because he had to fix a deviated septum and remove a lot of scar tissue near his sinuses, he was on the table for 6 hours. I had the same deviated septum surgery (I didn't have any scar tissue) by the same doctor and I didn't lose my sense of smell.

      There is no reason why you can't shower that day. I can imagine that some doctors might not be happy with hot showers as it increases blood flow

      Same here. They quickly told me a bunch of stuff at the hospital right before surgery & after when I was still under sedation. Little to nothing handed out, except when to go to emergency. 🙄 Random things pop into my head like, don't eat anything w Red dye. ??
      There should be an extensive clear list of discharge instructions & aftercare. With pictures!

    I had septoplasty with bilateral FESS with Turbenictomy with Ethmoidectomy under General Anaesthesia on 29.10.2020. I had no bleeding post op, just dripping. Had my first review on 5.11.2020 where the doctor also cleaned my nose. On the evening of 5.11.2020 around 7:54 pm my nose started bleeding profusely. I went to the ER, the doctor placed gauze in both my nostrils and packed it. The doctors examined my throat and informed me that there is no bleeding and everything looks fine. However the nose pack kept getting soaked. Is this normal?
    Also i have a flight on 11.11.2020. Will i be fit to fly by then?

    Great video. I am now 11 days post op but still unable to breathe through either nostril. My left is very clean but can only push out air, no breathe in and my right actually appears to have what I can only describe as a yellow cyst. My surgeon prescribed me antibiotics day 3 ad I also had sinitus. I am very worried this isn’t normally and considering going for a second opinion elsewhere

    Anyone here had trouble sleeping after sinus surgery? I've been having severe sleeping trouble 4th day after surgery (FESS, septoplasty and turbinoplasty)
    I am feeling very sleepy, as soon as I get from one state of sleep to the other my body is getting an intense jerk accompanied by a sinking feeling.
    I am very frustrated , exhausted and feeling defeated after an intense 18 months of long covid , I was hoping for things to get better after surgery and now I have this illness that I have no clue what it is or how to fix it.
    Side effect of general anesthetic?
    Side effect of taking tramadol? I was fine first 4 days and this started after tramadol.
    Are these hypnic jerks?
    The more I look it up, the negative content AI keeps presenting me is just taking ne to a very dark place. If someone has a similar experience, i would really appreciate some advice.

      Just had it done 2 days ago. Wondering if it got better for you?
      I have sleep apnea so I m constantly waking up. If you are breathing through your mouth & you stop during sleep to naturally try & breathe thru your nose you will likely wake up. Also sleeping upright doesn't help. You're definitely going to be tired throughout the day. I think tramadol makes you tired also? I was only given ibuprofen. But I am beyond sleepy.

      @maggienbob1304 it took 2-3 months for it to get better , I would suggest to try and manage the anxiety , it helps

    Thank you doctors. The video is very informative. I am a 69 yo man who just finished a 3-1/2 hrs surgery 4 days ago. I am in so much pain the 3rd day post-op when everything started to show. I have my left nostril enlarged by about 2x the regular opening. It's full of packing that started to come out with rinsing. My problem is with the Norco I was given. It gave me high blood pressure that wouldn't go away. My upper teeth gums are hurting. Tylenol and ibuprofen as you mentioned are much better choices. I wished I knew you guys before. Btw, I am still bleeding on the 4th day.

      @@GeneLiuMD Thank you doctor. I finally was able to sleep after 8 days of sleepless nights. Blood pressure went away without further treatment. Doctor said it was reaction from the anaesthesia. Upper gum and lip still sensitive after 10 days of surgey. For sure I will ask people to check with you if they plan for sinus surgery.

    I'm have my septum straitened in 3 days. I'm absolutely crapping it. I'm so fearful of waking up and not been able to breath although my nhs dr told me I'd be able to breath normally

    I had my adenoids removed (aged 34) and nasal turbinates shrunk, approx one month ago and still cannot breathe clearly through my nostrils. Still congested a lot 😞😞 can you provide me with any advice or possible reasons?

    What is a typical recovery for left maxillary antrostomy? Would you have congestion and difficulty breathing postop? How long is recovery?

      If you are only having a maxillary antrostomy and nothing else, recovery is usually really quick and relatively easy. Perhaps even just over the counter pain medications and and slight congestion and drip lasting a few days.
