      THANK YOU DR. KARAM for FINALLY addressing this issue I’ve repeatedly posted on your videos. I will say after becoming post-menopausal after a hysterectomy from cervical cancer, HRT and topical estrodial cream has helped me topically on my face and other areas. However, I am under doctor’s care regarding HRT and estradiol cream for my post-menopausal symptoms. It is very important to have things monitored, but for me it is worth it. Nevertheless, as you have repeatedly said proper skin care including sun protection , diet and exercise are paramount. All have helped me tremendously to keep me youthful. Thank you for this video!❤🙏🏼

    it all depends on the oestrogen cream used, Estriol 1mg for the vagina is the weakest form of oestrogen and has been proven not to alter levels in the body, please address this we women have had far to much scare mongering about our hormones

      Because the consequences of being wrong are to serious and too high. So if someone gets a breast or ovarian cancer down the road and they were prescribed these creams it’s hard to disprove their potential impact on the cancer occurrence. More study is needed and that’s why most physicians don’t prescribe them if those history exists. Safety first.

      Estrogen does not cause cancer metabolic dysfunction does. It’s complete bs without MD estrogen is protective- women’s estrogen sky rockets in pregnancy why on earth would the body do this grow a baby if it were harmful - move on use common sense

      I understand that any application needs to be monitored but it seems strange to me that women produce huge amounts of oestrogen until peri menopause and then post menopause where we have next to nothing. I can't see how a small amount of a low dose oestrogen cream could be anything but beneficial once the individuals risk has been assessed.

      ​@vickimouat8092 I think when our bodies stop producing estrogen we still need it. It helps with our bones, brains, skin, and so much more. This is where a hormone specialist can help, not a facial plastic surgeon.

    The womens health initiative (WHI) study screwed up a generation of women. It was a horrible study. Topical and vaginal estrogen DOES NOT significantly increase the risk of blood clots because it is not metabolized through the liver. Oral estrogen is the culprit for this. There have also been several studies which have failed to show a significant increase in serum estrogen levels when a small amount is used on the face. If estrogen was such a bad actor...ie causes cancer, why isnt breast cancer more prevalent in 20 year olds when we are at the peak of estrogen production. If one is so worried about estrogen and causing endometrial stimulation then take prometrium which protects the lining of the uterus. I think estrogen can be one factor in a multi-faceted approach to skin care.

      Thanks for commenting. When it comes to skincare, there are very few studies in general on estrogen and molecular biological changes with production of collagen etc, let along dosing studies against clinical effectiveness etc. The choice you make about what route you take can be very rewarding or a mistake. In general, I don’t see reason to chose estrogen creams over tried and true active ingredients. With them at least you know what you are getting and you know there is no systemic absorption. I offer my opinion based on what I think and how I view the subject. I am generally conservative and prefer tried and true approaches that are predictable and safe.

    Dr. Karam, I listen to all your videos and take your advice, but I really think you should speak to specialised menopause doctors before attempting another video related to HRT. It would actually be a nice idea to have a podcast with Dr. Mary Claire Haver or Dr. Heather Hirsch and you can discuss all things HRT! Until then, there are several issues with the info you provide here. First of all, transdermal estradiol, even for systemic use, has NOT been shown to increase blood clot risk, which is why most doctors recommend the transdermal route these days. Secondly, only a very small precentage of breast cancers is due to a gene mutation, I believe it is somewhere between 10-15%. Most breast cancers are infulenced by factors such as lifestyle, breast density, age, parity, and so on. In fact, systemic HRT is recommended and proven to be safe for BRCA 1 and BRCA2 carriers who have had a oopherectomy. Furthermore, familial breast cancer is not considered a contraindication for systemic HRT. Now, why didn't anyone talk about this for the last 20 years? Well, because of the WHI and the panic it caused, which is only recently being resolved and more and more people are turning to HRT once again. Also, let's not forget estriol, why go with estradiol? Sure, estriol is a weaker form, and it will not have such potent effects as estradiol, but the risk of systemic absorption is a lot smaller.There are many estriol face creams on the market, and here (Denmark) vaginal estriol 0,1% (brand name Ovestin) is over the counter. And lastly, why choose between estrogen cream and all the other anti-aging ingredients anyway when I can just use all of them?
    Now, to be clear, I am not saying people should be bathing in buckets of estrogen without a doctor's supervision and advice, but let's stop villifying estrogen, please.

      Totally agree. I would never want advice from a person who isn't a gynecologist when it comes to taking hormones just like I wouldn't go to an orthopedic surgeon for advice on having heart surgery. Mind you there are many 'drs' on youtube giving advice about topics that are their specialty just like there are many I fluencers giving advice on subjects they have no knowledge about.

      I’ve used all the medicines you talked about for years- retin A - vitamin C - peptides and nothing has helped to the same degree as estradiol- there are companies like Alloy that compound it for you after taking your history, dr Karam if you would speak to an obgyn specialist you who specializes in HRT you will hear and hopefully understand a whole new perspective to using estradiol on the face

    I’m a big fan of yours Dr Karam, but I think you may have some outdated info here regarding HRT risks.
    The WHI study that created so much fear around cancer in HRT has largely been debunked.
    In fact, many OBGYN’s go so far as to call it medical malpractice.
    I’ve seen significant improvements to my skin with estradiol cream applied to both my face and hands.
    As a disclaimer, I also have regular blood work done as I’m an HRT patient in my early 50’s. There’s been no additional fluctuations by adding a topical estrogen cream.
    Just a friendly suggestion, try listening to a few clips with Dr Mary Claire Haver about HRT. She’s one of many practicing OB/GYN’s trying to educate on the pros and cons of HRT.

      I am a big fan of Dr. Mary Claire Haver, as well as many others dispelling the myth behind those studies that resulted in the fear mongering around HRT. However, my biggest frustration is the information that isn't being discussed, which he spoke about around the five minute mark, & that is if someone has a genetic propensity for getting certain types of cancers due to mutations/variation in the BRCA1/BRCA2 genes. That drastically increases the risk of getting hormone dependent cancers- namely breast cancer that is estrogen dependant. That is a very real thing. I had two aunts who died from estrogen dependant breast cancer & I recently met a woman with estrogen dependant breast cancer who spoke of being disappointed because she wasn't a candidate for HRT. I am getting tested for variations/mutations in those genes & if they come back positive, I will be scheduling a double mastectomy ASAP. That won't stop me from revisiting the possibility of taking HRT. I'm praying that I don't have any issues & can get started immediately with HRT & all the wonderful health benefits as a result. 🙏🏼

      Yep. And, re cancers... my sister CURRENTLY HAS breast cancer and her MD has her on a topical vaginal estrogen to prevent "leaks". My sister asked about putting it on her face (since it is in the news) and the MD said it was fine since the topical is very low dose and doesn't go much further than the skin.

    The research done on HRT replacement is extensive, and I think this video is going to scare women unnecessarily. I wonder if ur wife uses HRT replacement systemically as many educated women do, one of the benefits being keeping the skin supple. The amount of HRT absorbed when used topically is minimal at the most, far far less then any systemic (pill, patch) and my plastic surgeon and dermatologist both recommended highly. No one is saying not to use it to replace retinol etc.(or your cream), but in conjunction with it. I doubt anyone is bathing in it 😂. I usually find your information very on point, but I feel this video is misleading.

      This isn’t an HRT video. This is a pros and cons of topical estrogen for wrinkles. This is a personal decision as is HRT and when one starts that they do have a proper history taken and discussed with their physician. I think the point you are missing is that this is a medical prescription treatment and needs to be treated as such. Data on topical is very scant

      @@UterineBlackmail I just sometimes take a tiny bit of my body creme and mix it with my moisturiser for face, but actually I find if your hormones are balanced in your body with biodentical you do not need to put it on your face.

    I obtained estrogen cream through the online prescriber (Ally). On my medical history, I disclosed that I’d had ER+ breast cancer with consequential bilateral mastectomies. The prescribing doctor said that this is not a contraindication and estrogen cream is ok for me to use on my skin. Sooooo… I will halt this treatment after seeing your video and do more research. Thank you!!!

      If in post menopause, estrogen cream is a danger, and can create brest cancer, than why we don't develop brest cancer in our 20' s..30's? Does women search for the main cause of brest cancer? From Dr Ryke Geerd Hamer and form Dr Gilbert Renaud you got all the answers about cancer and more.

      I would definitely check they out with your oncologist. The ones I talked to before making this video universally said it was a hard no if there is a history of breast say especially Estrogen receptor positive. Again it’s risk vs benefit.. and that’s something each person has to assess for themselves

    I use it on my face, the top of my hands, and my chest. I had Leiomyosarcoma 21 years ago and it was hormone-receptive. A Radical Hysterectomy sent me into immediate Menopause. My hair thinned out quickly and I had all of the classic menopause issues. I use Estradiol and have for about three years. My bloodwork is fine, and I feel better. That is what matters to me. :)

      Did you get all of your hair back? All of my hair started falling out in the last year and a half my periods stopped in July and then it just went completely off the rails last summer and I got on estrogen patches and progesterone pills in Sept and I've been using estrogen on my face for the last maybe 9 months and my hair finally stopped falling out about a month after I stated the actual patches. The cream didn't change anything for my hair but my face totally improved. My hair was thinning and I swear if the wind blew it felt like it would just start falling out. I'm using Rogaine as I have been for a few years and I'm hoping it's going to come back because I've lost 70% of my volume and I only turned 56 last week so I'm hoping so. My blood work is perfect and my primary doc just wrote prescriptions for all of the HRT I was paying cash for with an out of network Meno doc and approved that I'm using the cream on my face also.

      @@StayGold_MrsE My hair is a little better because I rinse it and soak it in protein water. My husband died almost two years ago. Got married last year and he died after a few months. I am to the point now that I do not care much about my looks, because I am so broken down. (My first love, my mom, and my son all died around the same time as my husbands. 2023 and 2024 were not nice to me.)

    Je n'ai pas le courage de faire mon commentaire en anglais car le sujet est un peu compliqué.
    Pour ma part, j'ai 57 ans et je n'ai pas de traitement pour la ménopause, je suis passée au travers sans problème majeur. Le seul problème, c'est la sécheresse vaginale, j'ai une crème pour cela et j'ai demandé récemment à ma gynécologue si je pouvais mettre un petit pois sur mon visage, comme je le mets ailleurs, donc 1 jour sur 2. Elle m'a dit que c'était vraiment sans aucun souci! Aucun risque. J'ai de l'hypertension légère mais sous traitement et un Hashimoto donc le traitement aux hormones n'était pas très bon pour mon hypertension m'avait elle dit, sauf si j'avais commencé de suite, dès la ménopause. Mais je ne l'ai pas fait car mes symptômes étaient très légers. La sécheresse est venue 7 ans plus tard et j'ai commencé uniquement la crème localement. Ma maman et la maman de cette doctoresse ont pris un traitement pour la ménopause, ma mère pratiquement 25 ans, jusqu'au décès de mon père. Elle était tellement jeune grâce à cela et aucunes douleurs musculaires ou articulaires. Maintenant, elle a 87 ans et prend toujours son vélo pour faire ses emplettes. Les hormones,avec un bon suivi, nous gardent jeunes ! Il faut juste suivre les recommandations,les doses et consulter dès que quelque chose d'anormal se produit. 😊

    This is a very very important topic that you’re talking about today. I am 44 and going into my perimenopause just started getting hot flashes so one of my nurses suggested me these topical estrogen and progesterone. I felt the difference the very first night more than a week into it and then I’m using it twice a day and instead of using one full pump I’m using halfthe morning at night my hot flashes are almost gone. I don’t want to be on hormonal therapy, but you’re saying this can be a replacement confused if I don’t use the topical one that I have to do with my hot flashes. Also that Nurse has been using topical estrogen for over 10 years now and I love her skin.

      I believe you are making my point. Your improvement of hot flashes (indicating absorption in your blood) while using estrogen cream is what I was referring to as a potential undesirable effect of a skincare product. Not everyone wants or should have the hormone in their bloodstream is the point I was making in the video. I am glad it’s working for you. Helping your skin while helping your symptoms of perimenopause.

    You are perfectly correct. That is why estrogen cream can be dangerous in early age, or with hormon related cancers in the patient's life or in the family history. In perimenopause, if the patient has syndromes related with perimenopause, low estrogen level can be detected by blood test, or in menopause, the HRT is used on the skin, patch is more safe (for dosaging reasons), but cream is acceptable also. And it has a dosage limitation. And estrogen level have to be checked regularly. And the safe zone is not even the top normal range. HRT has a lot of positive effect if it is used correctly, not just on the skin. HRT is used for decades now. What is new in this topic, if a patient want to use the cream on her face. It is not a moisturizer... A lot of dermatologist use it and prescribe it. Vaginal estrogen cream on a face- it is not a good idea. But after a consultation with a dermatologist, prescribed in a right dose by a pharmacy formulated cream, and a continous control by a gynecologist and endocrinologist (in my country a gynecologist often have a specification for endocrinology as well), it can be relatively safe to use. The effect for the skin, if a skin is estrogen depleted, can not even compare the use of actives like retinol or vitamin C or etc. It is shocking. The actives do not make miracle with the skin, plastic surgery does, but most of the people have not and will not have enough many for a good quality facelift. To treat an estrogen depletion caused skin damege, skin aging is affordable. But it can not be safe, just if it is in the right hands. Dematologist, gynecologist and endocrinolgist is necessary for the safe use.

      The OTC bioidentical estrogen creams have no medicinal properties,. Creams given under prescription as HRT by compounding pharmacies do. OTC are like a placebo that might make you feel and look a little better but do nothing for your estrogen levels. I actually went through two tubs of the exact one flashed at the beginning of this video before starting REAL HRT. Only doctor prescribed HRT impacts hormones. In the right doses as per test results and symptoms, prescription HRT is effective for overall health.

      Thank you for a balanced overview of a very complex topic.
      At age 43, I was using a very low dose estriol cream prescribed by my gynecologist for some perimenopause UTI issues. I was fit, metabolically healthy with no family history of any associated potential risks and was told by my doctor that overall “I was the perfect candidate” for a problem free experience using this mild treatment modality.
      Well 2 months in I had a stroke…
      Thankfully I fully recovered.
      Even doing your due diligence is not full proof. Things are always a risk vs. benefits and sometimes there are unknown factors at play.
      Many, maybe most won’t have this happen to them.
      But it’s good to really evaluate if it’s worth the risk if there are effective alternatives.
      Things could have gone very differently…

Additionally, studies on estrogen’s impact on collagen and dosing for clinical effectiveness remain limited.
      While everyone has their own preferences, I personally favor tried-and-true skincare approaches with well-documented benefits and no systemic absorption concerns. Some prefer skincare to stay in the realm of skincare, and that’s a valid choice.     I would love to discuss this directly with you - I have been a subscriber for years so no agenda here. I am a British Menopause Society Advanced Specialist and Consultant Gynaecologist with more than 30 years experience in the specialty and run a very busy women’s health and menopause clinic in the UK - I like to think I know what I’m talking about!!
    I genuinely believe that local E2 facial creams will be the next stage in facial/skin age maintenance. Personal experience backs this up. There is a huge synergy in women already on HRT - and we know form many scientific studies that the risks of modern systemic gold standard body identical HRT regimes are virtually negligible. There has also been off licence use of oestriol vaginal cream safely applied facially for many years. This is safe when used vaginally even in breast cancer patients as there is no systemic absorption. I’m advocating proper scrutiny and review by physicians exactly as you would offer for Tretinoin and other POMs and not the “Wild West” which some propose. I believe nothing will enhance collagen like oestrogen! Happy to have a frank discussion which I’m sure your audience would welcome!

    Yes 👏 thank you for letting ppl know what could happen. I was on HRT the pellets and at 52 I had a heart attack. Healthy 52 who works out, eats healthy…. Too many influencers are pushing estrogen/HRT without explaining that it’s not for everyone and what could happen! Menopause stinks, I’m going through it ….. but I’d much rather go through that than another heart attack!!!

    Always helpful and informative - thank you for your input. I started topical estrogen (im also on HRT) about 6 months ago. Jury is still out on the efficacy of it on my skin. I will keep using it and see how things go for another 6 months 🤷‍♀️

I haven’t seen any studies that exam efficacy of estrogen creams against a comprehensive skincare routine using well received actives like retinol, vit c, niacinamide, HA, lipids, botanical lighteners etc. I don’t see reason to choose estrogen creams over the above actives so I hope you're also using a comprehensive skincare system! With them at least you know what you are getting and you know there is no systemic absorption. I am generally conservative and prefer tried and true approaches that are predictable and safe.

    Very interesting! I’m on HRT and I use estriol (half a pea size) on my face. My hormone panel hasn’t changed since I started the estriol, so I guess for me it’s safe (or doesn’t affect my bloodwork/hormone levels). As I understand, the other two forms of estrogen, if used topically, can affect me systemically?

    you are right considering the risk of estrogen. Not every women is a candidate for HRT and not every women can use it over a long period. Estriol is very good for the vagina and bladder, as you have mentioned and has to be used forever . But the higher concentraded estrogen can be much more dangerous . I also would prefer your TRIFECTA etc.

    Such a timely video, my story, I am post menopausal and on systemic menopause hormones, estradiol and progesterone. I have had no issues. I went to one of the well known tele health sites and for the estrogen face cream. They say it's not systemically absorbed. I was using it for 4 months and suddenly got breast tenderness and some minor spotting, it literally was feeling like I had increased my HRT but the only difference was the added face cream. I have obviously stopped the face cream. I am curious if this is happening to other women.

      It’s not outdated at all. The data doesn’t exist to support its use in people who have a history of these cancers. In addition to a thorough reviews of topical estrogen and skin. I have literally asked several practice gynecologists, cancer gynecologists, and medical dermatologists and the hands down consensus they would never prescribe exogenous estrogen in any form in patients with this history. So I have to go with that. I am here to share. You can’t get this without a prescription anyway so hopefully a good medical history is taken and ultimately the choice is yours like everything else right? It’s just a personal decision if the benefits outweighs the risks.

      Similar to my experience; prescribed for gyn problems, breast got tender, spotting, weight gain ,sluggish. Felt better when I discontinued it. Was to use the estrodiol vaginally. Not for me anymore.

    I do agree that you can’t be haphazard with Estrogen levels and need them them checked regularly. There is a lot of new research that shows the benefits of estrogen and debunks the 1990 study -which was done poorly and showed a .025% increase of cancer in the test group, which is statistically insignificant. The bloodclot study showed an increased chance of bloodclots from 19 out of 100K to 26 out if 100k some say it’s a almost 50 % increase, I still say 7 out of 100k is insignificant. HOWEVER, you do need to be treated with HRT under the care of a doctor who is up to date on how to do it. Smearing it on your face or taking huge amounts of Phytoestrogens like soy and flaxseed cause issues.

    Thank you for the video and the information and your concern for this use. Anecdotally, I am a bio identical hormone user, this is my choice and for my health over 15 years, this is what has helped my health (so opinions on that are not welcome as I am dealing with my own health and my results). Aside from that I discovered through curiosity that the estrogen I was using on parts of my body - when I used on places like my lips eyes - seemed to rejuvenate my skin. Now I get quarterly bold work and have for these 15 years. I think it’s worth women to consider that the levels of estrogen we have when young could never be matched by a topical cream. I also use your skin care and gotta be honest A there is no way I could run into my face the amount to put me at risk. That said - I use your skin care but I’d love to see more long term research to the interesting results I’ve seen in myself.

    There is financial interest in supporting the estrogen creams or other anti-aging treatments. I hope the other doctors that are mentioned in the comments supporting estrogen creams are not selling them or are affiliate marketers. For me cancer is a risk I don’t want to take. Wrinkles are not killing anyone. Prefer to save my money for surgery.

    Hello Doctor, I have a question: I am on HRT anyway, monitored by my gynecologist. I use systemic estrogel on my arms. Would it do any harm if I use some of it on my face, without increasing the total amount? I understand your point that there is a danger of overusing.     In the Whi stydy the increasing Rick of Brest cancer was related to the combination œstrogènes and progesteron. Œstrogen only may be safe . Espacially in topic the absorption is minimal.

    I have been closely following developments in approaches to menopause- Mary Claire Haver, Kelly Caspersin, Louise Newsome etc etc and am always learning. You present a more cautious viewpoint, a very important set of questions.
    I would love to use your Trifecta because i have developed a trust in your guidance, but the monthly cost is prohibitive for me.

    I've been using phytoestrogen cream (Pueraria mirifica, Dioscorea villosa, Foeniculum vulgare) which also contains hyaluronic acid, vitamin E and citric acid. It helps me with hot flashes in premenopause. I put the small amount on my inner thighs and elbows, neck and stomach. I didn't know that it could be used as an anti aging cream. I am on retinol, vitC, hyaluronic acid and niacinamide that I bought in separate packages and use as you instructed in your previous videos.

I would suggest you stick with that and only use your prescribed cream for it's intended use.

    Sadly - many of these comments are simply wrong. I’m a UK based menopause specialist and gynaecologist with years of experience and can confirm that topical oestrogen performs way beyond other skin care - without significant risk. It’s all the bad and inaccurate information about HRT which is so disappointing and unnecessary. This has HIGH reward and LOW risk…..

In fact, there is very few studies in general on estrogen and molecular biological changes with production of collagen etc, let along dosing studies against clinical effectiveness etc. The choice you make about what route you take can be very rewarding or a mistake. Personally, I don’t see reason to chose estrogen creams over the above actives. With them at least you know what you are getting and you know there is no systemic absorption. Not everyone is ok with that possibility. Some just want there skincare to be skincare. If you question that there is absorption.. you should have a look at the studies. Again… I think the information in this video is clear enough to allow the person to think for themselves. I offer my opinion based on what I think and how I view the subject. I am generally conservative and prefer tried and true approaches that are predictable and safe. We are talking about making skin look more youthful after all.

    Great patient teaching Doc. 70 yo RN here. I’ve dabbled in BOTOX, a few fillers and laser when making the big nursing bucks in Cali. 10.5 years post-menopausal now. Used BHRT for 1.5 years during “the change.” The time of life that was difficult for me regarding the emotional and physical changes related to aging was mid to late 50s when I would have considered some targeted plastic surgery. I am a skin care freak who is often mistaken for 60 which I enjoy. Lots of breast CA scattered throughout the fam. My decision: There comes a time when many of us lucky ones see aging as a privilege. I am at that beautiful place. Therefore, I am a hard NO to topical cosmetic estrogen for skin care. I will stick to my actives and an and a couple of home devices that have science behind them. I do not have FOMO.

    The estriol creams that is sold through a lot of online pharmacies, etc in my opinion is hype. The other ingredients in such creams contain Vitamin C and hyaluronic acid that induces changes and plumpness in the skin that wasn't seen before due to the lack of. Estriol in my opinion and many other highly noted hrt/obgyn specialists states estriol is the wrong form of estrogen to produce youthfulness to the skin on the face. It's a dream sold in a bottle with many desperate women. Hrt and proper skincare, sleep, diet, and excercise with minimum stress will keep you glowing.

    I dont have an problems with wrinkles, but a bit with slight jowling...not too much.
    I would prefer something uninvasive, can you recommend?

      If your jowling is very mild, radiofrequency microneedling + collagen-stimulating skincare like the Trifecta are a great start. If you want dramatic, lasting results surgery might be needed, especially if you’re noticing laxity beyond just mild jowling.

    A great video about the pros and cons. You ask the question why hasn’t this been used over the past 30 years then.. well, we haven’t been receiving the gender specific research we deserve over that time. It’s taking women demanding better for this to be considered seriously really in the past year. Your point about more research being needed is exactly what we want! More information to understand the role estrogen plays in all our female organs is needed so our doctors can be educated on it and not use outdated or incorrect studies to guide our healthcare. I agree that we should be cautious about this on our skin given the points you’ve made but a large percentage of us peri/menopausal women are literally tired and unwell from the lack of medical research being done and are taking matters into our own hands and becoming our own test subjects. We’ve been forced into the private hrt clinics and jump at any new products that might help us. This is probably not the best approach as you point out but this is the frustration and desperation level. Thanks for providing the pros and cons. We’d love conclusive research to back it up.

      I am not sure why this is a HRT.. I deal with the manifestation of aging and most of them happen around menopause of course. Surgery addresses the laxity and the skincare I discuss addresses the skin. When people are using solid skincare and practicing sun protection menopause doesn’t impact the skin. It just doesn’t. There are 70 year olds who have better skin than some 40 year olds because they have been doing retinol, vitamin c etc etc for most of their adult lives. That’s what inspired me to create a comprehensive skincare product (Trifecta) that had all those components in there and was easy to use. I can’t speak to anything other than that… my specialty is rejuvenation of the face and skin. HRT isn’t my thing and this video wasn’t about that. It’s about choosing the best approach for you. My research and opinion is expressed in the video.

      The reason patients are asking for estrogen cream is because women are being lead to believe that our peri/menopausal skin will respond to the use of this product because we are estrogen deficient. I mentioned HRT because many of us who are in this phase of life are looking for help as all our organs seemingly need estrogen to stay healthy. You’re saying that’s not true yet you also say you don’t know (much) about menopause. That’s problematic in terms of the education you receive and many doctors and nurses I’ve talked to acknowledge they have received little to no education about it. What if the information you’ve learned from has been biased towards men’s skin and anatomy and maybe there are differences for females that haven’t yet been studied much? I hope that explains why women see a connection between this cream’s use (to reduce the effect menopause has on our skin) and HRT research and usage. I agree after using good skincare and sunscreen for many years along with occasional lasers that skin can look younger and behave younger but we are being lead to believe this will also help. You’ve shown its risks well.

    If estrogen cream is so amazing for skin, why isn’t is more known/popular? FOLLOW THE MONEY! Estrogen cream is relatively inexpensive. If it works as well or better than many Dr. ‘s famous ultra expensive creams, skin treatment companies’ a multi -hundred dollar creams, then those companies will lose$$$$!

    I do it, 1/2 pea size of estriol cream at night, no more. just 4 months in, so no verdict yet as I believe real change in skin is noticable only after 6-12 months of use.

I personally don't believe estrogen creams offer the same low risk, high yield results as tried and true active ingredients like Retinol, Vitamin C, Niacinaminde etc so you might consider incorporating those into your skincare routine if you aren't already.

    Face tape can address sagging, but what I would think would eventually result from face taping, would be saggier skin, from all the stretching. Do you think this could result Dr?

    Although using good skin products helps your skin and safer then estrogen cream. Estrogen does more and their is a big difference. But for people who cant use estrogen. You can use good products 😊

    Dr Karam, I am a huge fan. However please stop this utter misinformation that topical estrogen can cause endometrial hypertrophy. In fact topical estrogen is used IN PLACE OF systemic estrogen in patients who have a history of BrCa and endometriosis. Some patients may also choose estriol instead of estradiol based on their risk factors. Please move on from the original Women’s Health Initiative (WHI) study results that were published in the early 2000s, when I was a Research Associate at the American College of Obstetrician-Gynecologists. Subsequently there have been numerous publications clarifying those results. Unfortunately the way those results were mistakenly extrapolated set Women’s health back for 20 years. I would never have expected you to be promoting misinformation about women’s health. So please stop this until you catch up with current science and more recent publications from the WHI.

      @@dramirkaram I get that you're discussing this in the context of skincare, but your commentary on topical estrogen goes beyond that and into endocrinology and dermatology, where accuracy is key. Some of the risks you mentioned-like endometrial hypertrophy and DVT-are associated with systemic hormone therapy, not properly dosed topical applications. Stating that there’s little research beyond the 90s is also misleading, as more recent studies continue to explore estrogen’s role in skin aging.
      Additionally, while retinol and vitamin C are excellent, they don’t replace estrogen’s unique effects on collagen loss due to hormonal changes. If the concern is about safety, that’s a fair discussion, but presenting it as a ‘no-brainer’ dismisses its potential benefits for those who might be suitable candidates under medical supervision.

      I think that’s the point of this video… something that is meant to be skincare probably shouldn’t require such a careful medical history evaluation. But if you are going to use estrogen creams… be smart and make sure it’s safe for you. Respectfully, this has nothing to do with functional medicine but has everything to do with sound advice from a physician.

      @ Many insurance companies refuse to cover hormonal therapies for perimenopause etc… on account of one study that was conducted and the findings were taken out of context. Its a problem for many women in the USA currently. I just want more physicians who are well educated on the subject to be an advocacy voice

      Respectfully, I think Dr. Karim needs to do more research on this topic as many other dermatologists and plastic surgeon professionals do recommend estrogen cream. More and more studies are coming out and have come out to show that it is not absorbed systemically.
