    Thank you for your help. These parameters are right on and I am blessed to have #1 A great surgeon, I'm an active 56 year old, and I have been studying and following every protocol months before hand so I'm ready and on board. And mine was for a massive tear, per my doctor, though the measurement of retraction was 3.5cm. He did this in order to allow the proper time for healing back to the bone with Dermal on Demand patches. 8 weeks in the sling, PT to start only after 4 weeks. The progress is slow, as expected, but steady...My goal is the return to work, where I was injured.

    Just had supraspinatus, subscapularis and infraspinatus repaired (all totally severed) Mon April 24/23. Great information and very educational to understand all the steps from typical surgery techniques and now best recovery methods! Thank you!

    Thanks. Just had complete retracted Supra-infra tear done yesterday. Definitely helpful. Still waiting for block to wear off. 😂. Didn’t realize the restrictions I’m feeling with breathing with this sling apparatus .

    Going in next week for repair of fully torn supraspinatus. Been scouring TH-cam for information and I really like your breakdown and explanation. By surgeon says 6 to 8 months recovery time, if I’m lucky. Thanks a lot for your informative videos!

      @@thenote2367 about a month after the first surgery I developed an infection in the shoulder. Had another surgery to clean it out. This push back my rehab another three weeks during the recovery. Have experienced a lot of tightness and popping in my shoulder and just had an MRI done this week. The explanation was bursitis with a buildup of fluid in my shoulder. Don’t really understand the medical jargon, so I will be speaking to my surgeon as soon as possible. I was hoping it would go better.

    Had my rotator cuff repaired Thursday. Only need one pain pill Thursday night. Other than that ibuprofen. Pain scale 1 to 2 at highest. Started my pendulums. That you for all the help

    One week until my shoulder repair (one complete torn tendon, two partial). Got a premo guy to do it. Your information has been very helpful. I'm also a runner and exercise so hopefully that will help with recovery. I have a marathon this fall so need to get this done.

    Great advice... I'm 20 days in on my second reconstruction in 2 years on my other shoulder ... 2 separate accidents 😭... My second surgeon was younger and far superior in 3D technology and communication skills explaining what he was going to do and after showing me what he did do including the actual footage (fascinating) ... Thank you I wish I'd seen your advice 2 years ago.... Great points .... Some of that advice applies for physio's too... They vary hugely... My first one was crap and set me back a couple of months until I went for a second opinion.... Again thank you... Cheers from Australia

    Thanks for all the informative videos. Surgery for me is Feb 10th tear unsure how bad and Tendonomy and bursa. At 66 I pray for a smooth recovery. Oh it is my dominant arm (right) with no pre existing conditions. Excellent health. Thanks again.

      UPDATE: first, thank God for the videos. Day# PO. No pain. Surgery not an extensive as Dr. thought. ONLY torn rotator cuff. Started passive therapy. 6 weeks in sling. No shower for 5 days. Health great!! Slept sitting up 7 days. In bed now, sleeping on back with pillows. Great surgeon. But boy non surgical arm hurts. Sore muscles. Lol getting plenty of workout. Again thank you for all your videos

    I had surgery for a full supraspinatus tear, this is my experience: day 1 and 2 post-op are painful, but starting day 3 I had no need for meds anymore, none whatsoever. I wore and slept with the sling for 2 weeks, then ditched it altogether. Of course, I am extremely cautious with my movements. Besides, I am doing daily: 2x very conservative mobility exercices, 2x very light EMS for the deltoid and 2x icing. I am a former athlete and stepped into surgery just 12 days after the tear, when my muscular trophism was still very evident. So far so good. Again, since day 3 I never ever needed painkillers. Many told me shoulders are more painful than ACLs, but I got two of those and, honestly, there's no comparison. Anyway, thank you Doc for these videos.

    I appreciate these videos... I had a total Shoulder Arthroplasty without the stem, and a rotator cuff repair. 2and half weeks post op. My doc said only wear the sling when I'm sleeping or outdoors. So far so good.... Again doctor, thank you.

    the doc is spot on. i just went through a rotator cuff surgery and i sort of overestimated myself and stretch more than i can and bruised it again..patient compliance is extremely important . do not be complacent.

    Very helpful and thorough, thank you. 4th Week after Tendondesis and shoulder Arthroscopy and can highly recommend the use of a good Physio, post op, preferably one recommended by your surgeon. Pain was intense after nerve block wore off, but manageable with pain medication, very little pain one week post op and getting stronger by the day. As stated , be sure to choose the right surgeon!

    infraspinatus and subscapularis repair
    Open lower trapezius tendon harvest and transfer with Achilles allograft augmentation
    Biceps tendenesis
    Extensive debridement of labrum interval release of glenohumeral ligament
    bursectomy
    subacromial decompression
    bony acromialplasty

    One thing I’ve done is to wear bright Orange Velcro strap on my wrist to remind me not to use arm unnecessarily when I am out of sling.. (e.g. doing home therapy). I had two anchors into two full tears and cleanup. Thanks Doc

    I had 2 large bone Spurs removed from clavicle…decompression… and a whole in my tendon repaired.
    I was a body builder for 11 years when younger. I am 58. A hairdresser for 40 years.
    Going to PT was frustrating to me because I felt rushed. I did not feel as though I was having enough time to do a thorough therapy session. So I do PT at home one hour and half a day. I go to PT once a week for new motions.
    At 7 weeks the pain was resolving and I was able to lift my arm all the way up above head. So I got excited and body builder brain hit and I withstood the pain as I slowly lowered my arm . In my head… building my strength.
    The next day was excruciating. So I did nothing for a day… went to PT … told her what I did so pls no strength training.
    This has not been a week yet … however I cannot lift my arm pass shoulder height now and ROM is not near as good. Very stiff. My deltoid burns and my tendon in elbow hurts. I am curious if you think I may have injured the repair or if maybe I strained my muscle or maybe even torn my muscle. I am devastated as well as feeling stupid. In 2 days will be my 8 week mark. Going for an orthopedic message to loosen the area. And I have an appointment with surgeon this week. Wish me luck 😳🥴
    Your thoughts? Would it take more than something like this to ruin my repair. I do have one anchor .

    Just finishing up my second week after rotator cuff.. I had two tears and also my bicep muscle was torn... started physical therapy .. the ice machine is a game changer... my only Chief complaint is I'm experienced a little bit of numbness on my wrist just below my hand... is this normal... and obviously not sleeping well which I know is normal .... but of course having a recliner is a game changer... enjoy your videos and my surgeon seem to be experienced in very confident.... I had no bruising or anyting.... but like I said just that little numbness some experience and a small area of my wrist

    Thx so much for you comments. I’m 11 days out from my rotator cuff surgery (Feb 4).
    I slipped on unseen ice which resulted in two side tendon tears and a severe full depth “top” tendon tear/retraction .
    So I’m recovering now. The MD required 6 anchor pts. 😖.
    The four general pts you shared are great.
    I’m having my first return visit tomorrow. Expecting to review the surgery pics and plan out rehab.
    I’m a male, age 65. I have my personal heath specs/habits/BMI etc in good order but I’m concerned about the existing tendon quality question. To an uneducated eye… the tendon pics looked tough, like hamburger.
    I will be replaying your videos. Any other questions you my think of send them over.

      Wow Randy, this is a huge tear(s) if the surgeon needed 6 anchors. The key is to go slow. Make sure you understand exactly what you restrictions are, so you can have the best chance for recovery. Good luck and let us know how you are progressing!

    Very good info, I’m on day three of my second rotator repair, this one is less painful. I attribute that to the ice machine theropy .I recommend watching-all your video’s .

    Thank you for sharing this.
    What advice with you give to a 50 year old who has been diagnosed with posterior labrum tear after an acute traumatic event following shoulder instability. I've had PT for the last 6 months where ROM and strength has improved to 80% but the instability and pain still remains.
    I'm active surfing, snowboarding and skateboarding and have seen two surgeons ; One in the UK and one in the US both recommending that the shoulder instability would need repair. And that it is a higher chance of shoulder dislocation if I do not have it repaired.
    But my PT is recommending me to do my activities despite of pain and shoulder instability and wait with surgery until I have had a shoulder dislocation because surgery at 50 has a higher risk of developing shoulder arthritis, stiff shoulder and chronic pain after surgery.

      That is a tough one. Usually an anterior labral tear (bankart) causes instability not a posterior tear. Did your shoulder dislocate ? It is a very unusual to dislocate posteriorly 95+% of dislocations are anterior with an anterior tear. I would need to see you and your MRI images as well as perform a physical exam before I could give you my opinion. I'm sorry that I can't give you more of an opinion.     My horse is home from training and I can barely lift my saddle to put it on her . This sucks. I'll have the surgery soon and.hopefully be riding by next summer

      The only draw back is that I'm experiencing a rather expected complications related to Biceps tendon as it keeps popping and clicking and sometimes causing me pain
      I'm back in the gym but strength is at 50 % of the past

    Love your videos. I’m 4 days away from my rotator cuff surgery. Can a nerve block be used with my ICD on the same side of my body (left) as my shoulder that requires surgery?

      Interestingly My nerve block prior to surgery was well done in fact as the doctor injected medicine I could feel it travel down the exact path of my pain from back of shoulder to mid arm, which actually felt reassuring and was painless in my case!

    I don’t have a tear but I need surgery for my distal clavicle a distal clavicle excision my surgeon will clean up the bursitis and remove the fatty tumor in my arm

    Just had open reduction internal fixation shoulder surgery, 24 hours ago, really wish I would of checked these videos out 1st. Day 1 coming to an end, Horrible!

    I just had the InSpace balloon after two unsuccessful RCR's, second with allograft. Completely torn supraspinatus and partially torn infaspinatus 4+yrs ago. My surgeon had wanted to do (even) a partial repair but said given the poor quality of remaining tissue it wasn't in the cards. Questions to you are : In theory is there ANY possibility to repair even partially this tear now? Does the spacer placement damage/weaken what tissue I have left?

      I think if your surgeon didn't do any repair it is because there was not a good chance that it would heal. If that is the case it won't get any easier to repair in the future. The spacer does not damage or weaken the remaining tissue. I just put one in the other day. The only reason to use the InSpace is if you can't repair the rotator cuff.

    My first shoulder surgery the surgeon did not repair my supraspinatus and my infaspinatus. He left it like that in my shoulder only did an impingement, a decompression and a clean up of frays. I had to go to another surgeon after my first surgeon lied to me that he didn’t see the tears even though I had an MRI of January 2023, and 10 months later and MRI in October 2023. Now I have to go through this whole surgery again and I feel this is negligence. I’m not looking forward to this. He left a thick large tear in my supraspinatus tendon in the shoulder. He also lied to me and said that he did not see it when he went in with the camera. How do two MRIs show large thick tear but yet he didn’t see it somethings not right here.

    Dr. Orcutt, thanks for the great info and all of the videos. I have watched many of them. I am just past the 4 week mark of rotator cuff surgery. I had three complete tears (supra and infraspinatus and sub scapulars) and a partial tear of the long head of the bicep. My orthopedic surgeon told me it was a "mess in there" after surgery. This was both a chronic and then acute (bicycle accident) injury so I am sure it would have been a challenge for anyone. My surgeon used an Regeneten membrane in an effort to augment the surgical results. He told me that if I am a "good boy" post-surgically I will likely get complete recovery. In my younger years I was extremely physically active and the wear and tear is catching up with me at this point. This was not my first orthopedic procedure so I have some knowledge of recovery and what to expect. I have found the recovery from this procedure more difficult than my previous (non shoulder) surgeries. It is more debilitating and still wakes me up from sleep at a month out. I am being careful and I do not think this is to unusual considering the extent of my procedure. One suggestion I would make (if you do not already do this) is to inform your patients that having discomfort considerably out past the surgery is definitely possible. I have not taken any narcotic since the day after surgery. I typically take 800mg of Ibuprofen before bed and manage some rest that way.

    I'm 66, disabled, and a torn rotator. I'm going to pt first, but if I need surgery should I have a repair or reverse shoulder replacement?

    To determine a good experienced surgeon, how many full tear surgeries are considered enough to perform? Comparing to non experienced surgeon.

      Elena, In general a study came out several years ago in regards to replacement surgery that suggested that a surgeon should do at least one a month of a specific surgery to be familiar enough to do that surgery. That being said, I think that is the bear minimum. Good luck, are you having surgery soon?

    I've had 2 surgeries within 10 months and I have been through therapy each time. I don't yet have the range of motion that I so much desire. My patience is running thin. BTW I am 61 now and high ambitions, but I am very limited. I was forced to retire because of what has transpired. I'm not able to raise my right arm beyond my shoulder pinching inward. I am rehabbing at home and I am losing hope of ever getting range of motion. Any advice is welcome.

      I'm in same boat two surgeries in about a year. Second one was total shoulder replacement. 6 weeks out, pain is suicidal. I need to take Valium just to go to therapy.

    Are those just fibers of tendon torn or the entire tendon is torn? If fibers of tendon are uniformly chipped away how can one know by MRI?
    So recent graduated surgeons are better for natural repairs?
    What do you think about superior capsule reconstruction?

      Sorry Khan, I am not sure I understand the question. You can have a partial rotator cuff tear and it can be seen on MRI.
      I think you need a very well trained surgeon regardless of when they graduated.

      What happens with disuse to the tendon, does it just become thinner. Any way to find out through MRI?

      Most surgeons in the states that do a lot of shoulder surgery will do SCR. Surgeons who do few RCR will certainly NOT do SCR.

    My surgeon tells me to stop smoking pre and post op, for the surgery. I am a heavy smoker, is he bluffing me ?

    im 3 week out from rotator surgery. i had 30 hyperbaric oxygen treatments before going in. ( actually for dental surgery) and im going in for 10 more. Do you believe this gives my healing an advantage?

    Why are we put through steroid injections and PT before even considering surgery? My rotator cuff was, "A mess". There were many delays in treatment.

      Itcajh, thanks for the comment. I think we try to not just jump to a surgical solution. That being said, in the right circumstances this may be a very short period without significant delays. Did you recently have surgery?

      10 days ago. I'm a retired nurse. Many delays. 2016 MRI showed a full-thickness tear. I didn't push surgery because I needed to work. Then while swinging up a belt sander there was immense pain. VA refused to do a MRI due to shotgun pellets in me, despite being lead, then getting referrals and u/s delayed treatment. @@rotatorcuffexpert
