It’s true that pain from trauma or overuse often has clearer solutions, usually with some relief from tools. But what about the other cases related to anatomical variations? I’d love to hear some effective methods for those! 🤔💡
Great question! Anatomical variations are much harder to diagnose. Even after an MRI, anatomical variations are not normally noted. I typically don’t see people not respond Well to treatment focused on improving mobility of the hips and lower back to address rooted piriformis issues. Even in those with anatomical variations, they typically don’t have pain their entire life. Something usually changes to cause them discomfort, whether it be an increased activity or hypertrophy. They typically figure out ways to manage themselves overtime.

I'm still confused about my problem. I was trying to pick up a heavy dumbbell on the ground and with a bad form, i instantly felt pain radiating at my lower back. That's was almost two weeks ago and my back hasn't really gotten better, I'm not in any pain when i walk or lay down, just when i sit and bend over. There are some days where I feel better and some where is much worse than the days before. I also feel tightness on my left hip and it runs down the side of my glute but nothing pass that. Also my lower back hurts and i title my head forward to look down. Any ideas on what i have and how to fix it doc?

I don’t have any pain in my lower body or hips, but I have the achy throbbing pain you were describing in my whole back. But I have a sharp pain in the middle of my upper back. The pain is mainly in my upper middle back. It hurts to sit and a little bit when I am laying down. I’ve been in this pain for about 1.5 weeks now what do u think?
Oh great question. Middle back pain can be tricky. If it feels better to move that is a good thing. You can also have referred pain to the middle back. If it doesn't feel better soon, best to have a physio take a look

Well it depends on worse. Honestly. I usually notice that when people have pain it’s because they go too far with the stretch too fast. Try it again but lower range and less intense. But if pain persists then you can hold off
@ i have annular tear and i noticed that the pain is getting worse everytime I stretched my hamstrings. 🥺
Is it bad to stretch hamstrings when u have annular tear and disc protrusion? Or should i try it again? Thank u for answering.
@@dirotora It's not "bad" but if you stretch the hamstring you are also getting the nerve at the same time. Try with a little knee bend during the stretch and see how that goes.
I have annular tear and mild disc protrusion L5-S1. The pain is radiating on my glutes. Then some slight tingling sensation on my left leg. The more I move or stretch my back it gets worst again. So i am afraid now to stretch or move. Will it get better? For how many weeks? Should I always take pain meds everyday so I can stretch? 😢
Hi there! It will get better but it could take some time. The disc has limited blood flow which impacts healing. When I work my patients and they say that I usually tell them to reduce the intensity of the stretch. I would not take medication just to stretch.
@ yeah coz i noticed the more i try, the more i injured then back from the start instead of healing. But its been months on bed and i am feeling weak 🥺 should i take pain meds so i can stretch? 🥺
Injured my back during squats is it a herniated disc or something worse? Ok so on Aug 03. I was doing squats at 135lbs x6 and I felt a pop on my right side and weakness of the right knee. I thought I herniated disc l performed a straight leg test after a few days and I felt no pain most of the pain was concentrated on my lumbar area and inner right hips. 7/10 when standing for long periods of time. It was unbearable walking long distance triggered it also. Tingling in my right legs and muscle weakness in my right calves And numbness in my right foot Also i noticed that the side lower back is slightly higher than the right I can feel it when I rotate so I took 2 mos off the gym And rest. Now I returned to training aside from squats I can train 90% capacity. I can do deadlift with minimal pain. I can leg press and do most lower Body movement easily. - the pain went from a 7 to a 2 - can walk and stand for Long periods of time without pain can bend down pick this ups -can do ab crunches - the numbness in my right leg went Away aside from one specific Spot on calves -incresed flexibility and strength in lower Body But there's still issues - I feel pain in my groin area in my cock and lower balls mostly numbness and tingling occassionally -there is still that specific Spot on my calves that is numb I can still feel sensation in it and everything it's not like completely numb -shocks tingling in both legs -back Still slightly uneven -back still feels abit painful weird -feeling weird sensation in the abdomen -pain overall went from 7 to 2. Am I going to be paralyzed and end up in a wheelchair? This happened to me a few years back 8 years ago almost the same symptoms aside fromt back slightly being higher and it went Away. I injured myself doing squats.
I do the bridge and straight leg raise almost every day together with all the other stretch exercises shown in earlier videos. I am pain free now for almost a month or so. I do feel my lower back after a day of hard work but we all feel that every now and then. No more sciatica for me which I could not have believed 4 months ago when I could hardly walk 60 feet without excruciating pain.

It’s hard to describe my pain but it definitely goes down my leg, and pretty much stays within my lower back around my tail bone. It’s hard to sit and it’s constantly uncomfortable pain. Even if I’m laying down and taking like a deep breath I can feel the pain.

I watched your video disc or muscle I feel pain when bending to the side less forward. What can it be?? Background: I had a severe lower back and right leg pain episode when I stayed in bed for over a day. I had to climb into a control shaft underground when exiting using the right leg. Maybe laying down in the bathtub for long in a bad position. Overall my work is sedentary. Now I feel I need to take care of things. Another thing to mention is the pain episode started a day after climbing.
Yes you can have similar symptoms related to a disc or sciatica when bending to the side as well. You are correct, if you take some time to improve the nerve health and overall strength of your core and hips, then you will feel better as well. What can I give you to help you get started?

Great question. I have seen it work but the key is to stay consistent. You may not notice change for months, but just stay with it!
This morning I was stretching while bending down nd getting back up my lower back starting hurting right away. I was stuck nd couldn’t walk just happen this morning . Never felt so much pain sucks
It does suck! our body can make us stop in our tracks. It's going to be rough at first but you will get better!
I have a positive reverse slr, pain goes down the left leg if raise my right leg, the nerve compression is more on the left traversing nerve root should I do this?
Great observation. So, if severe I would slide it and not tension it but if you are doing generally okay, I would hold the stretch on the R first then the L. Lots of different ways to do it based on how you feel after.
Then what is it? I have pain in my right buttock, both sides of my thigh (but not the top or back) the front of my knee, more towards the bottom of the knee cap, splits into both sides of my shin, and goes to the inside of my ankle.
Adding that it is worse when sitting/getting up from sitting. Standing or laying flat with a pillow under my knees is the only position that my leg doesn’t hurt.
@@ahandmadehomestead great questions. That's a tough one. Pain from the buttock into the lower leg is often nerve but not always. You can have muscles and nerves refer into that area. If tingling and numb feeling then I would lean toward nerve, if not, then could be hip or upper leg (IT band, Quad)

If a physician I would go in with an open mind but also consider seeing a physical therapist before undergoing MRIs or other imaging. I don’t know your specific situation but most of the time it’s better to see a physical therapist before, testing or consideration of injections

I am 66 and 2 months ago had laminectomies on 5 levels, L2-S1. I also had 5 levels of bi-lateral facetomies, and 5 levels of bi-lateral foraminalostomies. What would you say is the recovery time for all of this. My surgeon says up to a year. I've been really struggling. I will also add I am HLA-B27 positive with family history of PsA, and also have Lupus.
Hi! Thank you commenting! Wow you have been through a lot. When I work with patients like yourself the recovery time is usually longer. The surgeries are meant to protect the nerve not improve the tissue quality or movement of the spine. That takes work before and after surgery. Sorry you have been struggling. I saw this video since I started to find out what is the reason behind my lower back pain, and I hope you can help me I’m still suffering for a lower back pain for 4 months, I don’t know what it is but it’s sharp at the middle of my lower back, also everytime I bend forward and backward I can feel it too, and every time I twist my right hip I can also feel it, but when I’m laying on the bed I don’t feel any pain at all To be honest I’m having a discomfort feeling everytime I’m standing long time period of my work because of my lower back
Great questions! If you are not having any calf or foot pain the chance of significant nerve involvement is low. It sounds mechanical and related to movement which is good. hey doc love the vids i’ve been having this lower back pain for like 3-4 months now and feel the most amount of pain when i bend or lean foward, especially on my lower left side, i can’t tell if it’s muscle or disc. some examples of basic things that hurt sometimes are getting out of my car, doing the action of putting on pants (if i am leaning foward). So i’ve been stretching it everyday, i do this stretch where i lay down and put my two feet against a wall and my legs are 90 degrees, and i do a slow pelvic thrust upwards and really feel discomfort and pain as much as i can. i just need some tips and recommendations because i do not want to go to a PT because they will have it on my medical record and i am thinking seriously of enlisting into the Air Force and do not want something like that to hinder me

That’s not true. But it’s about stretching the nerve and encouraging it to heal. It’s a very important part of the healing process.

thnx for the video. I've tried this about 3 months ago but I couldn't do it without a lot of pain. Now 5 months into my "sciatica journey" I can do it and it helps indeed. I first thought the tingling I felt while doing it was a bad sign but it's a normal sensation like you mentioned

A lot of talking but only answering 1 question. Can Sciatica cause calf pain only? Now what? You don't talk about any symptoms or treatment/medications.

When you bent over to describe throwing out your back I was like oh yeah that's me. Also feels like I have a thick book strapped to my lower back.
Yes, I’ve heard that before. Often times that’s what swelling feels like. The good news is it will get better!

Hi, thank you so much for the video, it gives me important knowledge. I am from Indonesia. I am diagnosed HNP L3-4, L4-5 and L5-S1. MRI result stated no deformation or DDD, no listesis, no spondilosis. I have no sciatica pain, my legs are all ok. It feels sometimes burning there at the lumbar but that's not the main that I'm suffering. I'm having this muscle spasm or muscle sore around waist and back area (sometimes left, or right or both at the same time). It's getting better gradually since I do exercise for LBP but not significantly dissappear yet, it's been some months since 26 July 2024 I had it at the first time (sudden waist cramp while I was sleeping). I was wondering is it related to my HNP or got nothing to do and it is separately my muscle problem. Thank you so much if you respond this...
Thanks for reaching out! It sound unrelated in my opinion. Or if it is, it's loosely related. The muscle spams and soreness respond very well to exercise. I would continue doing what you are doing and let your body heal.

Great question! So I typically recommend that you generate strength through that range. For example, doing some contract relax during the stretch or trying yoga poses.

@@katja4458 yes! You do a slight contraction then relax of the muscle. Repeat that process. It’s a gentle contraction not max effort
Hard for me to do the back exercises well as I am not that flexible. I am 68 years old now with arthritis in my hips and lower back.
Shoot! Just do your best and even with limited range you can still get the benefit! What exercise did you have the most difficulty with?

Thank you so much for this content. It gained you a new subscriber and me a new thing to do to try to alleviate this psiatica related calf tightness! I have a brother-in-law whose calf is in a perpetual tightened state of appearance from sciatica. I was worried this would be my fate too because it's not severe enough to me to warrant surgery, and PT has switched to strengthening rather than lengthening. Love this suggested movement and easy to do at my desk. Ty!!
