    I understand why he says you get better results pulling slightly down and outward with facemask, because there is much less resistance from the bone that way. However, I can't help but ask myself why someone would want to do this unless they have a significant vertical deficiency. But more of the people I see with poor facial development tend to have a vertical excess if anything due to downwards growth. Makes me wonder what the option is for someone with vertical excess but front-to-back deficiency.

      Maxilla which is too up and forward (deficient growth), causes the nose to bulge and eyes to be pressed against the brow. This is craniofacial dystrophy.
      Ideal maxilla is down and forward because it allows the lower third to project more.
      Compare the Mew's twin studies, where one of the twins was a mouthbreather and the other a lip-sealed nosebreather. The better face (lip-sealed) has a maxilla that is down and forward.

    The last part is especially interesting; pulling the upper jaw slightly downward.
    It’s definitely correct in my case, the upper jaw is already quite short.
    Even though I do not have an underbite it seems the development of the upper jaw might’ve been compromised by either the teeth extractions or the retainer or both.
    Good to know.

    I have upper airway collapse. Thank you Dr. Lipkind for answering my question. I'm currently working with Dr. Newaz to advance my soft palate with a face mask because my airway is very narrow in that specific region of my face. I also plan on getting mandibular advancement surgery to correct the overbite following face mask treatment.

      @@highsay9769 So I have rates from last year. $8,700 is the total. The initial first payment is 2,747.25, which is made on the day the MSE is installed. The remaining balance can be paid in monthly payments for 18 months. You can also ask that they be spread out to 24 months which is what I'll be doing. So 24 monthly payments of 248

    so it's impossible if someone has a recessed face.. to bring the maxilla forward... the only way is to facemask down and get some forward growth but... with some vertical growth as well... Isn't it possible to achieve counterclockwise rotation?

    What about class 3 cases with a longer mid-face? What angle should they pull? What does he mean by "slightly downward," -15 degrees? -30 degrees?

      You should pull at the angle of your occlusal plane, if you pull upward you will get an open bite.

    wondering why lipkin is against the DOME. my doctor recommended it alongside mse treatment to ensure splitting but i think I'm going to decline.

    Wow this is huge info. Too bad the arms of my MSE were cut off and I can't use the facemask anymore 😂 you know I had it pulling slightly upwards too lol

    Do you need face mask if upper and lower jaws are deficient? But they're in a class 1, like both jaws are retracted back causing airway problems

    Can you ask if a person after getting a split, expand at a semi rapid rate according to Dr John mew . Maybe that removes the chances of uneven or asymmetric expansion maybe .

    Ron can a tooth borne expander split the suture and disrupt all the sutures like an mse can? When I had an expander on the teeth attached to the wings were expanded. Is that indicative that bone borne expansion did not take place?

      You need to take a cbct scan to check but sometimes it has mixed results in adults

    I really don’t understand this video. The jaw forward? back? The reason why people would want a face mask? I’m not following when you say that people want to bite “this way” but when they smile the bite returns to its regular position. I’m lost…

      somebudy thinks their bite is in incorrect position,they feel more comfortablr with their bite when their lower teeth are in a knipped state ,probably because their maxilla is underdeveloped

      That would be considered a complication of surgery, not a typical outcome.

      a surgeon told me that 30% of patient get more tmj problem
      30% improve and 30% dont change
      is there any way to check the health of the tmj before doing anything? i think one of my joint is luxated for 8 years now wich gives me asymetry only when i push my jaw forward not at rest

    How long does the diastema between the two front teeth last in most cases?
    I'm getting MSE soon but I'm worried that I'll have a big ass gap between my teeth for like 6 months.

      It all depends on how long you expand and how much. It all depends on how soon you get into braces. The gap really isn’t that apparent

      Attaching a facemask to a TAD-expander is bone borne.

      Bollard plates does not get as deep into the maxilla as MSE TADs. Plus there is zero documented successful cases in adults with bollard plates.

      @Oba Instinct I mean you just google it. It's bone anchored protraction with plates that have to be installed surgically. There are even other ways like using tads drilled into the sides of maxilla instead of plates which is done by guys like Juan Carlos Pérez Varela.. The protraction itself is then done again with class 3 elastics but the anchoring is bone borne.
