This video practically saved my life! (at least quality of life)
I live in an Asian country that doesn't have any podiatrists, let alone sports podiatrist. I stacked layers of craft foam to make metatarsal domes and the pad with a cutout. Have to wait for over 2 months to get an MRI to find out what's really going on in my MPJ. Thanks!

Jack Chang i am so glad that a video from the UK could help you so much. Anything else let us know

Thank you, really useful video. I have been diagnosed with a plantar plate injury and the NHS is not helping much, but thanks to great people like you on youtube, I have been able to start implementing conservative treatments myself. Could you do a detailed video on what insoles you recommend and how to find the right spot to place the metatarsal pads, cut-outs etc. ?

Cupid Alaska hi, yes we can look at doing a video on insoles and metatarsal dome placement, will get that up next week. Have a good evening. Nick

I love your Blue peter presentation style, especially the shoe over the shoulder move! :):):):)

    Thanks NIck,
I had a break on the 4th toe that was healing until it was accidentally stepped on, from then on it has never gotten better. Saw many doctors and they kept telling me to buddy tape.
Finally saw a podiatrist and he said my bones are well aligned after an xray but I told him I feel as if that toe doesnt quite touch the ground. He then suspected a plantar plate tear or injury and taught me how to tape it as you SAID. Would an MRI detect the extent of the damage? Also I dont know if I have a hammer toe because of the callus inside that developed while bone healing. I can manually straighten my toe but then it returns to position, unless grounded. Can a plantar injury cause a "temorary" hammer toe?
Thank you, you touched on many topics Ive been dealing with since the break last august.

Hi, thanks for the question. An MRI would most likely show is there is a plantar plate tear, however it is not always sensitive to it. Normally if the toe has hammered, it will not come back down if strengthened, however I have had a couple of cases where the toe did return to ground contact. I would get back in touch with your podiatrist, so they can help treat the injury and start you on some exercises to make things stronger again.

Thank you so much for your answer. I forgot to ask one thing. I am constantly wearing a post op shoe that was given to me when the toe was broken. It allows 0 flexing/movement of the toes and I strap it down rather firmly to keep the possible hammer toe flat. Is this fine to do? I am on week 1 of using the tape in the way you mentioned.

Hi@@sfkingalpha I didn't see this reply. I would keep the it taped as long as you were advised, then once out of the boot, you can tape it as in the video, however I would get in touch with your Podiatrist about a rehab plan

Thanks NIck, question, wouldn't the stiffer sole in the forefoot increase the likelihood of increased stiffness in the 1st MPJ ?,
and the use of thera-band, would sufficient resistance be attainable for the digital plantarflexors (only ask as thera-bands theoretically useless for achilles tendon rehab)

Hi Mark, thanks for your comment, the stiffness would increase through the 1st MTPJ, however the forces going through the 1st MTPJ are so vast and so much more than the lesser MTPJs, the 1st MTPJ still functions fine. From a clinical testing point, we do the Jacks or Hubsher test to look at Functional hallux limits, however this has no correlation in gait as the forces are just too large. Great question though.

@@NKActiveClinics Is this about windlass mechanism of the foot? May I ask, does sciatica nerve is involved in pain on metatarsal?

    • @NKActiveClinics

Hi @@ljudmillamihhalko3526 the Windlass is in relation to the 1st MTPJ and the reverse is in relation to the 2nd -5th MTPJs, the sciatic nerve can cause pain in the forefoot area yes.

@@NKActiveClinics Thank you for answering. Does it also involved in pain on the calf or only on the hamstring & the quadriceps muscles. Or it's depends of lactic acidosis. Thanks.
