Breaking kayfabe means breaking the 4th wall and behaving as you really are outside of the business. This means if your character is booked as a badass inexplicable force of nature it would be out of character for you to be sensitive and caring for someone. You can only put kayfabe away when you're absolutely sure no outsiders are observing. Undertaker held on to kayfabe and his character longer than anyone else, but this was only possible because he knew where to draw the line. This was why in the old days Heels and Faces were forbidden from being associated with each other because it broke the atmosphere of their rivalry.
The thing I've always loved about this is how you can see his heart still breaking when talking about how sick she was. He hates thinking about the fact he almost lost her. I've seen it a thousand times before, seeing her sick and knowing how that made him feel is what made him realize he loved her.
For as much as she poked fun at him the whole time with her cheeky retelling of the events, Taker actions speak volumes of his character: never took advantage of his status with the divas, coached her through his unbridled passion for this job, and looked after her in a time of real and grave need. Gotta respect that.
