    I have been on a 30 year crusade to get rid of the sebaceous filaments on my nose. My constant search to find a product/routine that will get rid of them is what got me into skincare. NOTHING has helped remove them and I have tried it ALL. I have accepted having them to a degree but they really are the bane of my existence.

      Same here. NOTHING works. Products or removing them either cause irritation, flakiness, bruises, peeling, redness…everything except smooth! If I dont touch them for weeks, they get so full you can definitely see them w/out getting close. I hate them, I want them to die lol.

    Me: consistently cleanses skin, uses moisturizer with niacinamide, uses clay masks, uses salicylic acid, uses retinoids.
    Sebaceous filaments: 👁👄👁

      Ditto! I find that oil helps to remove them. Once a week or so, I use coconut oil or mineral oil on a cotton ball to wipe them away. It's all I've found to help.

    I hate sebaceous filaments. I use cleansing balm and a gentle foaming cleanser and they never go away. I would use acids but my skin is really sensitive. I usually gently pluck them out with a tweezer and then it just leaves large pore holes

      I like benzoyl peroxide cleanser. Just leave on a minute and rinse off.

      I thought my skin was too sensitive for anything but using SA and BP washes for a short time and washing them off has been awesome. I can now leave on for longer and occasionally not wash it off. Just can’t do multiple days in a row.

    I’m plagued by the sebaceous filament on my chin. Niacinamide, Tretinoin and CeraVe SA cleanser in the AM. Helps.

    I use the Vanicream liquid cleanser as my hand wash which works great as a facial cleanser for me so I just give my face a rinse pretty much every time I wash my hands. Also a good reminder to reapply sunscreen.

    The only thing that helped me with sebaceous filaments is massaging my oil cleanser in like five minutes! I do that pretty much everyday

    I have to shut my eyes in the car wash. The motion of the brushes makes me car sick. It's the only time I ever have that feeling in the car. 🤷 Thanks for the tips for treating sebaceous filaments. I always have a hard time understanding the difference between those and whiteheads.       @@laurenpetro4807 pick one and see how your skin likes it. I’d recommend the 2.5% benzoyl peroxide cleanser from acne free. Use it one time a week for one week, then 2 times a week for 2 week, 3x/week for 3 weeks, etc., until you feel like your skin isn’t reacting to it. I personally never had any reaction and think you’ll be fine but everyone is different

    You should try the facial marula oil at Trader Joes. I use it only as a hair oil after I wash my hair. It's great and not greasy. I only use 1-2 drops.

    I use Aztec Indian Healing Clay, 100% natural calcium bentonite clay. Mix with apple cider ginger to make a paste. Leaves my skin clear and smooth. Kroger carries it, $10/1 lb. It’s been around a long long time. Hope you try it and like it. Beautiful week to everyone!

      Yes, she's talked about the aztec clay and I'm pretty sure she and a number of dermatologists aren't fans of acv on the face.

      @@db-gb5xi ACV is nothing more than apple scraps, sugar and water. It is a recommended additive to this powder, it is not an acid. I was a Nurse at MD Anderson Cancer Center in Houston and would never use anything nor suggest anything harmful. You can make your own choice but educate yourself.

      ​@@vra735The pH of ACV can be between 2-4, so yes, it is technically an acid, is acidic, and contains acetic acid. Apple cider vinegar is actually made from apple cider and fermented into the vinegar. Something made with apple scraps sugar and water is not ACV. I'm surprised you told people to educate themselves but I can tell you mean well and are just trying to defend yourself. I personally did not do well with acv and bentonite clay. It's way less of an irritant using plain water.

    Why does this have to happen? How do we go from having perfect skin as children to having disgusting gross repulsive nauseating skin like mine??? I hate my face skin
    Probably the only way I can fix this thing is to go to a dermatologist and get lots of treatment but I can't afford none of that stuff I used to have low self-esteem about my skin
    Now I have low self-esteem plus hatred towards my skin and I honestly don't know how to fix it but I'm trying. 😢😣

      I promise you that you can see improvement with 1) retin a 2) Vaseline 3) sunscreen and hats 4) drink enough water. You can get retin a online. Search around for good deals. Even if you could afford a dermatologist I would tell you to do these things first. You don't need to spend money on fancy products. I got depressed and neglected my skin for over a year. And then I got better and started to care about skin again. It took about a year but my skin is back baby! Don't think you can get there without these things. Tough love but you have to care enough about yourself to not cheat. Care every day. Don't give up! We need accountability buddies and that's probably why you watch this channel. ❤

      @@jackiegerarde9938 but the thing is I do wear sunscreen everyday for 20 years I've been protecting my skin from the Sun I wear a big hat two different at the same time and I still have so much ugly discoloration ugly sunspots I can't afford prescription retinol I don't know about the over-the-counter stuff because which one will cause me breakouts and some of them are too expensive I can't put anything greasy and oily like vaseline on my face because immediately I will break out
      I should be treating wrinkles but instead I'm still treating acne my skin won't let me use products to treat wrinkles because immediately it'll break out I've wasted so much money on anti-wrinkle creams that all they did was make me break out because my skin just won't let me
      During the summer time it won't let me moisturize because I'll get pimples everywhere I know the more expensive the product of better quality but sometimes I just can't get the really expensive stuff and even if I do there's no guarantee that it won't make me break out. ☹️ I just feel like I have bully mean skin like my skin is a schoolyard bully that just won't leave me alone and I wonder how many other people feel the same way I do about their skin 😢😭 but I appreciate so much the advice you gave me it sounds like wonderful advice but the problem is my skin won't let me especially the Vaseline

      Get a gentle cleanser such as Calm & Restore by aveeno, I use this and has helped my face lots, a gentle moisturiser, sunscreen. Iam not sure if you have acne, but Differin is a good one can get that at Target. Self care is the best care, take care of yourself, do your own spa day and pamper your face. I promise things will get better😊

I take care of myself I eat healthy I exercise I live a healthy lifestyle but my skin doesn't show it in fact alcoholics have nicer skin than me how is that fair???????     Hey Dr. Dray, is it safe to shave / ‘dermaplane’ the face when sebaceous filaments are present? I know it’s not recommended over active acne but does it pose the same / other risks when it comes to sebaceous filaments?

      I've tried oilplaning instead of dermaplaning, just because I don't like the feeling of dermaplaning on dry skin.
      I noticed that they looked less obvious but didn't go away / get irritated, which is good.

      I just had my dermatologist zap 23 sebaceous hyperplasia spots on my face, several of them needed a second treatment. Have a hard time with sebaceous filaments on my chin, I’ll try adding more moisturizer to my routine.

    I’ve had these on my whole nose my entire life and have never been able to even minimize them ever 😭 I think I’m cursed to having them permanently, people think my nose is covered in tiny blackheads

      Same for me. Nothing ever clears them. They can be pushed out but they never fully come out like blackheads where you can see the empty pore. I’ve been using Paula’s choice Salicylic acid and a toner. Which I don’t think most toners really do much, but it’s more than a toner, it has the niacinamide and other good things in it like most “serums” have. It’s managing them, thankfully they can be hidden very easily with the concealer I use (Age Rewind). But they’ve made me self conscious my whole life! I’m paranoid that people look at me and see what they think as blackheads and think I’m dirty. ☹️😢 They take up my ENTIRE nose, T-zone, chin, and forehead!!! So literally my whole face. I know I’ll never be able to have the clear, clean looking skin almost everyone else has…

    I've used the pink clay mask on both my son's t-zone. I just use a tiny bit and it did a great job I rolled it up to save for a later time.

    This might be a silly question, but how do you know (with non tinted sunscreen) that you cleansed your face properly from the sunscreen?

      Probably by how your skin feels. You can probably also tell by your hairline.

    I tried a few bottles of niacinamide from The Ordinary, didn't work at all.
    I can squeeze them out every single day. I have no acne at all, I am 58 and have them around and at the nose and on the chin only.
    The kind of mild retinols from The Ordinary didn't work either.
    Real retinols or tretinion you can't get without prescription in Germany.

      Try a Salicylic acid serum, in the morning and evening; Salicylic acid loves oil so it gets pulled into the pores filled with sebum and dries them out, clearing the oil.

    Had them ever since puberty. Never knew, you could even do anything about them, since the TH-cam (or systematic skincare, especially outside of official treatment)
    wasn't a thing yet, back in the 90s.
    I mean, I used to go to a derm as a teen because of acne. Got Benzoyl peroxide cream and that was that.
    No education about the importance of moisturizer or sunscreen, not even a hint, that stuff like Salicylic Acid might be helpful...yikes.
    I've been using the Cerave Retinol for a while and it has definitely cut down my still persistent oilyness and also improved the enlarged pores on my nose a little...Not sure, if it has an anti aging effect on me, but I had fairly smooth skin, to begin with. Never liked sunbathing, luckily.
    Still got (smaller) sebaceous filaments, so I have added an SA toner in the morning.
    Gotta be careful though, because if my skin is the slightest bit irritated, sunscreen will start stinging and burning me.

    How does one tell the difference between sebaceous filaments, tiny hairs, or blackheads on your nose? I have huge pores and they always look full of something 😩

      You can’t unless you’re a derm 😅 basically they’re treated the same way, so you just wing it. Benzoyl peroxide cleanser worked for me

    I’m on tretinoin every night. I use a salycilic acid cleanser at the moment as tolerated, just bought it. As I’ve been on tret for a year.
    I have some of these sebaceous hyperplasia (are they the same thing?) and hopefully they go away in time also blackheads/whiteheads around my chin jaw area
    I’m here looking for what you suggest we do and what we should use 😊 have a lovely day.

    Haven't watched the vid but simply using a glycolic cleanser every alternate day with moisturizer helps! Especially moisturizer, it really lifts up those dead skin cells and get in between the cells seperating them!

    I use Cerve SA cleanser and Paulas Choice SA serum with tinted sunscreen in AM.
    In eve I use Albolene and Cerve SA cleanser together and then put on adapalene.
    Any Thoughts??

    I have a white spot on my face. I only see it in up close selfies in certain light. I cant remember if its a sabecous filament or not. Zhe said a laser might work on it. But i havent done it yet.

    I have recently used Klairs cleansing oil on my neck (not the first time) and I don't know what it is!? Either allergy/ reaction or purging. There's lots of tiny bump with white dried filaments coming out. It's not itchy and also some small red pimples are also coming out. I went to a derma and she gave me anti-histamine and I took for 4 days now (6 more to go) and it's still there. Can anyone tell what this is? Have anyone of you experienced this.? And yes I have acne on my face especially now in chin area.

    As much as I love Dr. Dray, that is distracted driving right there. I know influencers do this all the time, but people have actually been injured and even died as a result of this.

      @@Danm2201 People used the same argument when mobile phones first came out. You drive...you must focus on the road. This is what you teach your kids. Accidents while livestreaming is a thing! It's called distracted driving for a reason. Silly we have to wait few more years to collect data to know what should have been obvious from the start.

    I have sebaceous hyperplasia. I've been using retin A for over a year and have only seen mild improvement. Is there any type of procedure you can recommend. They are mostly on my forehead. 😔

      I just had 23 sebaceous hyperplasia spots zapped by my dermatologist, he used a desiccation device to zap the trapped oil. A few spots needed a second treatment and I think they’re going away now.

      Accure 1726nm laser, it blasts the sebaceous glands that produce this shit into oblivion, search for a dermatologist that has it. Or take Roaccutane if you don't mind the side effects.

    I know this is way of topic, but the way you use your seat belt in the car is really dangerous if you get in an accident. The lower part of the belt needs to sit on your hip area. Because of the bag in your lap, out have the belt under your breast. In an accident that will cause some enormous trauma to the soft tissue inside your abdomen.
