What else would require so many people? In my area, it is 2 ALS units, 1 engine company, police, and possibly an EMS supervisor all to work the cardiac arrest.
I don't about your als trucks, but my mine are 1 emt + 1 medic, so for two als trucks, you get 2 emt's, and 2 medics. Plus, you get the supervisor (usually a medic), and 4 people from the engine to do CPR/lifting. A cardiac arrest requires a lot of people, because it is very exhausting for a couple of people.
+Expert Fire/EMS Productions. four people is more than suffice to to work a code prehospital. one on airway, one compressions (who will switch as needed) a lead medic pushing drugs and interpreting the rhythm and the fourth drives.

The first medic unit was already transporting a patient from a previous run. The hospital is just outside the frame of the shot.

We run codes with 4 people and a Lucas. 2 Medics and 2 Basics. Plenty.

not everyone has a lucas, but 4 people is cutting is tight. However, that is certainly doable.
