May I say, sir, that you are on Godly tier. I have struggled not only to beat this boss battle on Extreme for around 3 years (in vain), but I am currently trying to beat it with no kills on hard. So I know how crazy-insane hard it is to beat this guy, and not only do you beat him on Extreme by not killing him, but you do it while taking NO DAMAGE. I'd be lying if I said I wasn't a little jealous... You are an inspiration!!!

Digging all these dudes saying "I tried this and got it on the first try!~" in the comments. Took me a solid dozen tries with this strat before I quite figured out how it works, then it as easy as they seem to think it was. Vamp's still easier than that Harrier fight, with or without this method. One thing I can't seem to figure out- Does hitting him with, say, the AK deal more damage than punching him with the pistol, like it does with the guards? I can't seem to tell if there's a difference.

It reminds me of when I used to rush this battle. I just used the Stinger as soon as the battle starts and if you aim perfectly each time he gets hit, you can hit him again and again until he dies. The downside of that is you actually kill him (no good if you do a no kill run) bit it would also the game slow as hell because of all the explosion effects. Vamp could be kill in like 6 seconds xd
tap tap - wait - tap tap - wait. i know it seems like i'm being a dick, but that's it... very similar to the fight with Liquid in MGS. two punches and a pause to reset the "fighting" stance. it's fidgety but it's doable. also, don't forget that hanging anywhere does the same "invincible frames" so don't rely on that one spot. you can use the railings dotted around too. also also, if you're struggling to take health away (this part is for anybody) aim for the head. one of the few games way back when, which allowed headshots to be effective.
It may be a while until you see/answer this, but how do you get him to not throw knives when he comes out of the water? Almost every time I grappled down to the ledge as you did he threw knives. Great job though!
Calling BS for two reasons. 1. Shooting Vamp with the stinger missle on hard mode doesn't even deplete his oxygen with one stinger. 2. After you bamboozle him by hanging on the ledge and melee attacking him, he jumps off immediately back into the water again.
1: It takes at least 2, and that's clearly demonstrated here. 2: You have to spam punches at him. If you spin kick him, which you can only achieve by doing a combo, then yes - he does jump back into the water. He'll also jump back in if you're too slow to switching sides and attacking after he begins to swipe his blade. The whole purpose of spamming punches is to keep him prone. Spin kicking him knocks him off his feet and thus takes him out of his prone phase. This isn't really that complicated.
Fearless ML Vamp doesn't just stay there and repeatedly attack me back, after a few hits he jumps back into the water. It's like you locked him into his attack animation.
The catch with attacking him directly is to NOT spin kick him...ever. You'll notice when I did he jumped back in. Never kick him. Punch until you see him readying his blade swipe, run through him, repeat.

Tried this multiple times and he jumps right back in the water. Exactly how it shows. He walks over I hop out get one punch in and he’s gone.
He needs to slice you. You have to time it to where you come out as he slices you.

I used the stinger missel, hanging trick. I cant get the two lunch pattern to work flawlessly.

That doesn't help if you're intending for non lethal playthrough, specifically for the Big Boss rank.
I did Vamp non-lethal, and glitched him, I got no damage on my 15th attempt, he got what he fricking deserved, using that Movement freezing BS.
