Guys, Remember that Haven sits at the top of one of the mountains that make up the city of Mistral. And the Vault of the spring maiden is just a cavern inside of that Mountain which was VIOLENTLY shaken and quaked by the fight between Cinder and Raven. It is VERY possible that that quaking is what opened a fissure in the side that lead to a the lower levels of the city below.
My theorie about the first of the three Questions is that Raven used one of them. As a Maiden she had the chance to do so and it might explain the way Cinder got out, Raven Cut herself into the Chamber to get to the Vault.

To be fair to the Vault chamber, it WAS just subject to a rather substantial collapse, courtesy of two Maidens duking it out. That'll compromise the structural integrity of just about any building you've got set up.
Not to mention the difficult part was never getting into the vaults chamber. If you know where it is anyone could tunnel through a mountain and climb to the vault given time.

I found it a bit annoying,. An unnecessarily blatant call out to the shipping community. Ruby should have called out to her sister first, then others. Yang (Miss "all I care about is my sister"), should have called out for Ruby first. She did the same thing in 3xll when she told Weiss to look for Ruby while she looks for Blake. Not to mention there are at least two instances where Yang just brushes off Ruby because she is upset. Kind of a hypocritical love for her sister IMHO.

Raven didn't gain the maiden powers until long after she left Ozpin, and there is no indication she went to Haven to look into the vault. (In the previous volume, it looked like she was not sure how the vault door would open after she put her hand on it, and was surprised when it did.) It was likely Ozpin that asked the first question, or possibly the previous Spring Maiden (the one Raven killed) asked it, which may help explain why she ran away in the first place.
You do raise a fair point, however if we say that while Lionheart wasn't the first, maybe it was Raven that cemented it as him not being the last. Perhaps Raven raising questions (similar to how Yang is recently) and in a play to secure loyalty revealed the truth to her, but then obviously backfired. [Although I will concede that my memories of the earlier volumes is a bit hazy, so I might be miss-remembering something in Raven's backstory that would better confirm/deny my theory.] Not only that but bringing in the previous spring maiden would raise the question of how a young and untrained girl (assuming Raven was being truthful) reached the relic, why ask only one question, and then leave without Oz/any security knowing without some lead on her; identity/motivation/ or otherwise.

I’d assume that the knowledge relic was locked away in a past life when a different spring maiden was allied with him and he just lost track of the next successor after she died(?). I just realized something watching you ... The cave is completely opened for the villagers to find where Cinder got out which means people can find the cave and if they find a way to get to the tree they'll find the portal to the other dimension since they couldn't close it xD
The crack in the cave was opened by the monumental battle between Maidens. It shook the Academy so badly it almost knocked Ruby off her feet up above, and it made huge rock chunks fall from the ceiling.

Raven wouldn't care if she DID hear a 'sploosh', and I honestly don't think she would have because that drop was more than three hundred feet and I didn't hear my dog jump into a swimming pool while I was on the other side of the house. There are real things that can be critiqued if you absolutely need your fiction to align with reality in every way, but this is not one of them.
