I got Guardin' Gnome by accident. In a public match, a random dude carried the Gnome up until the concert. He died in the concert, and i didn't have a Defib, so i finished what he started, and everyone in the lobby got the achievement
I'm a bit disappointed that you undersold the Guardin' Gnome achievement... you don't just have to take the Gnome to the end of the level, you have to take him with you to the end of the EPISODE, of which the Carnival is the first level. oh, and the entire process of trying to win him as a prize in the first place requires you to play a very loud carnival game that attracts hordes of undead while you do it. my girlfriend might not have made it out of the Finale level, but, goddamn it, that fucking gnome sure did.

You get it by playing an online battle against either one of the games devs or someone who already has it. It's fittingly called 'Spreading like Wildfire'.
Todesnuss Red Dead Redemption had a similar one for their online mode called Rockstar, I think, where you had to kill a dev or someone with the trophy.
There are quite a few achievements like that. One achievement I remember was called "Friend of a Friend" and required that you Play a game of Co-op with a Dev/Play with a Trophy(or achievement)-holder. I wanna say it was for a borderlands game, but I'm not certain...

I actually got the nome achievement by accident. I thought it was nice to have a mini game i could actually win a prize in left 4 dead 2. My friends laughed at me but it was i who was laughing when i got the achievement.

The Platinum trophies in all 4 Souls and Bloodborne are almost the same though. They aren't really hard, just time consuming because you have to play each game at least 2-3 times to collect everything.

Then realised there was the achievement for not playing it for 5 years, and now every day is a torturous endeavour to what will at most amount to several minutes of happiness.... Just delete it and forget you own it. go play a MMORPG or something. grinding dailies will steal your soul and leave you unable to play video games after doing the dailies.

But for me, that purple unicorn is the Meat Wall achievement in Rainbow Six Siege. Over 100 hours, and that's the only achievement I don't have in the game. It takes a bloody amount of luck to pull off, and it's virtually impossible to boost it.

I got the Go Outside achievement pretty recently. Legitimately, too. And after finding out that the new update for the Stanley Parable was actually an entirely new game, and that they increased the amount of time you need for the equivalent achievement in that version.
Ahh, I remember getting that Gnome. Took us 4 attempts. Once gnome has fallen through level geometry in the open section before tunnel of love, and on another attempt gnome simply disappeared in the safe room. On the final and successful attempt one of us was hugging the gnome ALWAYS. So since there were only 3 of us left playing it took some REAL strategy planning how to beat screaming oak and especially make it to the stadium. But it's easy compared to Styx: Master of Shadows achievement Full Potential (or what was it name) - to learn every skill available - because to get it you need to complete every secondary objective, make a play through without being noticed at all, don't kill anybody, find every token (hidden thingy) and make a speed run of every level. Rayman achievement was easy because it comes naturally if you play the game a lot. TF2 was easy - as long as you own a community server =)))) PS. Damn it oxbox, today list contains 3 of my favorite games=).
If there's to be a threequel......the TF2 achievement reminded me of "Abstergo Employee of the Month" from Assassin's Creed Brotherhood. That achievement/trophy required you to get all multiplayer bonuses at least once. Doesn't sound too bad, except that that included the hellaciously difficult "Greater Variety" bonus, which itself required you to get 15 distinct bonuses in one (10-minute) match. Bonuses ranged from things like scoring the first kill of the match, to evading pursuit successfully, to killing with specific weapons or "powers." It required you to have nearly everything in MP mode already unlocked, but the real pits of it was that your "powers" -- things like the Hidden Blade pistol -- had monstrously long cooldown periods, upwards of 90 seconds. In, again, a 10-minute match. You also needed to use several of these "powers" and you weren't able to change them on the fly -- you could only switch between three premade entire builds either upon dying or upon going into your menus, but either way you wasted precious seconds waiting for your character to respawn in the match. "Boosting sessions" (private matches filled with people working together to achieve the bonus) were commonly organised on the game's forums and while I never took part in one (it just didn't interest me enough, I was satisfied not 100%'ing this game even though I had with AC2), reportedly even with a full lobby of people working together to get one of them the Greater Variety bonus, the 10-minute length of the match left very little room for error. So I have a hard time imagining anyone getting this in a public match.
The much harder gnome achievement was Little Rocket Man from Half-Life 2: Episode 2. You have to carry that little guy all the way from the first chapter of the game to the final chapter and shove him in a rocket.

There are several achievements in AC4 that can work, but I think the worst case is All Rounder: where in multiplayer, you have to play every game mode at least once, there aren't that many game modes, but you also have to use every ability and ranged weapon at least once, there are very many of those to unlock through levelling up, saving up the money to buy them, and keeping track of what ones you already used. Not to mention that you need to pay for Xbox Live Gold/PlayStation Network to access multiplayer if you got the game for Xbox or PlayStation(don't quote me on that if I'm wrong)
Normally I want to get every achievement in a game but in the AC games I left the multiplayer achievements out cause I didn't really like playing multiplayer and it would definetly take too long. But I did collect every figurine in my Abstergo office and for that I had to spend a lot of time in MP.
Please include the little big planet achievement! It's called "who needs breakfast?" I think you have to play it for 12 hours straight I found out the hard way

PvZ: Garden Warfare 2 tells you to go outside when you promote a character 5 times (max their rank), but that's not as hard I suppose with it being "babies first battlefield". Little Rocket Man was a tough one, playing all the way through HL: Episode 2 without loosing the garden gnome and send him into space. The One Free Bullet, Completing HL: Episode 1 by only firing one bullet and having to rely on gravity gun, crowbar, grenade and rocket kills.
don't forget Half-Life 2: Episode 2's Neighborhood Watch achievement for taking out every Strider without losing a single building...you really have to memorize when & where every one of them spawns, how fast every one moves and the exact path each one takes...my brain isn't capable of that level of planning & memorization
The underwear in Dead Rising is a reference back to the knight Arthur from the "Ghosts n' Goblins"/"Ghouls n' Ghosts" games also by Capcom. It's like how they hid Ryu's hadouken in "Mega Man X 2"...

Dixie_Normus I was lucky and had to do only 2 attempts for it. The first attempt was our first playthrough of the game. I accidentally threw the gnome under the bleachers and it got stuck. The second time through we got it.
What about Destiny: Flawless raider. You need to have 6 people not die through an entire multiple hour long mission or learn to somehow clear the whole thing solo
That got a lot easier after the expansions though. Vault of glass was still level 23 or whatever and at max light light level you could breeze through it
What about: What a Long, Strange Trip it's Been from WoW. to get this you have to complete practically EVERY holiday achievement and considering there is roughly one holiday per month and at least 10 achievements per holiday it takes years. That purple proto drake reward though... so pretty...

I would have made it in one of it wasn't for one little thing: School of Hard Knocks - the achievement that _both_ the PVP:ers and PVE:ers hate. To this day I still don't feel that the drake was quite worth it. The free master riding I got with it was pretty nice though. ^^ (They should have kept that one. It was the only thing that made it worth it.)
holy crap you must have really been on your grind... I started the achievement as a noob (had just joined right before LK dropped) so it took me a LOT longer. I almost exculusively flew that drake from when i got it (Cata) through the end of WoD when i got the Clutch of Ji-Kun.
wow, yeah just looked up the comp cape and you are right: it sounds like a nightmare to get. no one should ever have to play runescape that long; it's inhumane. Outsidexbox could create an entire video on the most ridiculous feats of strength you can do in MMO's like What a Long, Strange Trip and the Comp Cape and such. In WoW alone i can think of at least 4 achievements that, when explained, would probably scare you way from the game forever because of their scope.
Speaking of team fortress 2, there's also the achievements on the rocks and running with scissors where you had to lose 3 games of rock paper scissors in a row with rock, and win 3 games of rock paper scissors in a row with scissors. Whichever one you throw out and whether you win or lose is random, and one person has to pay for the Taunt, it's a lot more difficult than that sounds
A new addition to this is the 'Trolling' achievement in Shadow of War. You have to weaken a captain to 'almost dead', then find a nearby Olog-hai (making sure not to get too far from the captain or he'll vanish), lure the captain and the olog-hai to each other, avoid killing the captain while you try to mount the Olg-hai and then hope he does enough damage to kill the weakened captain. This is after making sure it's not a captain that can heal himself
My plan, once I can pull it off, is to weaken a captain near an olog-hai captain, and then use the olog-hai captain, that does more damage than your average olog-hai, to kill the other captain. Simple, right!?

I actually helped my older brother get the Vidmaster achievements with some friends of his back when I was a kid. It was brutal, but awesome. Back then, the recon helmet was truly legendary. You saw somebody wearing that thing, and you knew fucking with them would be the last thing you ever did in that game.

Oh and about that sainthood, i'd pronounce you as an Apostel of my Belief if you're able to proovenly do 1 Miracles and 39 Grand Deeds for The Old Ones, Crimson Clan and 2 Heroic Deeds for the Mechanoid Void Republic. In other Words its much easier to be pronouced saint by the Christian Pope 2 Times in a Row with 100 Years inbetween each Sainthoodingness.
See Stanley Parable has been sat in my Steam library for years, with me not thinking about it, slowly moving towards the achievement. But now you've mentioned it, I want to play it...
that hitman achievement didn't take that much planning. and no mention of the half life 2 episode 2 gnome, just the left 4 dead one, which was easy mode compared to half lfie?
When it was still possible to get them, "Scarab Lord" and "Veteran of the Shifting Sands" in World of Warcraft required the combined efforts of an entire guild worth of players, several months of time, extreme good luck, and heretofore unknown levels of planning. The Blue Scepter Shard segment by itself required at least four trips into 40-player raid dungeons, thousands of gold worth of rare crafting supplies, an assault on one of the opposing faction's capital cities, slaying hundreds of brutally difficult elite enemies in several far-flung places around the world, and fights against two of the most difficult world bosses that existed at that time.
To go with that Guardin' Gnome achievement, Dark Carnival is arguably the 2nd hardest campaign in the game, with the 2nd to last chapters ending being a particular pain in the ass when a seemingly endless stream of infected come out of one room while you and your team are sprinting down a very narrow corridor.

Me and my best friend got it our first time playing Dark Carnival together. It took fucking ages but goddammit that gnome made it to the chopper.

Demons souls is harder, I farmed for 200 hours for a pure sharpstone chunk and still never got it.

I got the Go Outside achievement pretty recently. Legitimately, too. And after finding out that the new update for the Stanley Parable was actually an entirely new game, and that they increased the amount of time you need for the equivalent achievement in that version.
Ahh, I remember getting that Gnome. Took us 4 attempts. Once gnome has fallen through level geometry in the open section before tunnel of love, and on another attempt gnome simply disappeared in the safe room. On the final and successful attempt one of us was hugging the gnome ALWAYS. So since there were only 3 of us left playing it took some REAL strategy planning how to beat screaming oak and especially make it to the stadium.
But it's easy compared to Styx: Master of Shadows achievement Full Potential (or what was it name) - to learn every skill available - because to get it you need to complete every secondary objective, make a play through without being noticed at all, don't kill anybody, find every token (hidden thingy) and make a speed run of every level.
Rayman achievement was easy because it comes naturally if you play the game a lot.
TF2 was easy - as long as you own a community server =))))

If there's to be a threequel......the TF2 achievement reminded me of "Abstergo Employee of the Month" from Assassin's Creed Brotherhood. That achievement/trophy required you to get all multiplayer bonuses at least once. Doesn't sound too bad, except that that included the hellaciously difficult "Greater Variety" bonus, which itself required you to get 15 distinct bonuses in one (10-minute) match. Bonuses ranged from things like scoring the first kill of the match, to evading pursuit successfully, to killing with specific weapons or "powers." It required you to have nearly everything in MP mode already unlocked, but the real pits of it was that your "powers" -- things like the Hidden Blade pistol -- had monstrously long cooldown periods, upwards of 90 seconds. In, again, a 10-minute match. You also needed to use several of these "powers" and you weren't able to change them on the fly -- you could only switch between three premade entire builds either upon dying or upon going into your menus, but either way you wasted precious seconds waiting for your character to respawn in the match. "Boosting sessions" (private matches filled with people working together to achieve the bonus) were commonly organised on the game's forums and while I never took part in one (it just didn't interest me enough, I was satisfied not 100%'ing this game even though I had with AC2), reportedly even with a full lobby of people working together to get one of them the Greater Variety bonus, the 10-minute length of the match left very little room for error. So I have a hard time imagining anyone getting this in a public match.
The much harder gnome achievement was Little Rocket Man from Half-Life 2: Episode 2. You have to carry that little guy all the way from the first chapter of the game to the final chapter and shove him in a rocket.

There are several achievements in AC4 that can work, but I think the worst case is All Rounder: where in multiplayer, you have to play every game mode at least once, there aren't that many game modes, but you also have to use every ability and ranged weapon at least once, there are very many of those to unlock through levelling up, saving up the money to buy them, and keeping track of what ones you already used. Not to mention that you need to pay for Xbox Live Gold/PlayStation Network to access multiplayer if you got the game for Xbox or PlayStation(don't quote me on that if I'm wrong)
Normally I want to get every achievement in a game but in the AC games I left the multiplayer achievements out cause I didn't really like playing multiplayer and it would definetly take too long. But I did collect every figurine in my Abstergo office and for that I had to spend a lot of time in MP.

PvZ: Garden Warfare 2 tells you to go outside when you promote a character 5 times (max their rank), but that's not as hard I suppose with it being "babies first battlefield".
Little Rocket Man was a tough one, playing all the way through HL: Episode 2 without loosing the garden gnome and send him into space.
The One Free Bullet, Completing HL: Episode 1 by only firing one bullet and having to rely on gravity gun, crowbar, grenade and rocket kills.
don't forget Half-Life 2: Episode 2's Neighborhood Watch achievement for taking out every Strider without losing a single building...you really have to memorize when & where every one of them spawns, how fast every one moves and the exact path each one takes...my brain isn't capable of that level of planning & memorization

Dixie_Normus I was lucky and had to do only 2 attempts for it. The first attempt was our first playthrough of the game. I accidentally threw the gnome under the bleachers and it got stuck. The second time through we got it.

What about Destiny: Flawless raider. You need to have 6 people not die through an entire multiple hour long mission or learn to somehow clear the whole thing solo
That got a lot easier after the expansions though. Vault of glass was still level 23 or whatever and at max light light level you could breeze through it

What about: What a Long, Strange Trip it's Been from WoW. to get this you have to complete practically EVERY holiday achievement and considering there is roughly one holiday per month and at least 10 achievements per holiday it takes years. That purple proto drake reward though... so pretty...

I would have made it in one of it wasn't for one little thing: School of Hard Knocks - the achievement that _both_ the PVP:ers and PVE:ers hate. To this day I still don't feel that the drake was quite worth it. The free master riding I got with it was pretty nice though. ^^ (They should have kept that one. It was the only thing that made it worth it.)

holy crap you must have really been on your grind... I started the achievement as a noob (had just joined right before LK dropped) so it took me a LOT longer. I almost exculusively flew that drake from when i got it (Cata) through the end of WoD when i got the Clutch of Ji-Kun.
wow, yeah just looked up the comp cape and you are right: it sounds like a nightmare to get. no one should ever have to play runescape that long; it's inhumane. Outsidexbox could create an entire video on the most ridiculous feats of strength you can do in MMO's like What a Long, Strange Trip and the Comp Cape and such. In WoW alone i can think of at least 4 achievements that, when explained, would probably scare you way from the game forever because of their scope.
Speaking of team fortress 2, there's also the achievements on the rocks and running with scissors where you had to lose 3 games of rock paper scissors in a row with rock, and win 3 games of rock paper scissors in a row with scissors. Whichever one you throw out and whether you win or lose is random, and one person has to pay for the Taunt, it's a lot more difficult than that sounds

A new addition to this is the 'Trolling' achievement in Shadow of War. You have to weaken a captain to 'almost dead', then find a nearby Olog-hai (making sure not to get too far from the captain or he'll vanish), lure the captain and the olog-hai to each other, avoid killing the captain while you try to mount the Olg-hai and then hope he does enough damage to kill the weakened captain. This is after making sure it's not a captain that can heal himself
My weakened captains tend to get killed by wild creatures, namely gruags
My plan, once I can pull it off, is to weaken a captain near an olog-hai captain, and then use the olog-hai captain, that does more damage than your average olog-hai, to kill the other captain. Simple, right!?
It doesn't help that the new Xbox One update keeps showing me this achievement as a 'Rare' acheivement

I actually helped my older brother get the Vidmaster achievements with some friends of his back when I was a kid. It was brutal, but awesome. Back then, the recon helmet was truly legendary. You saw somebody wearing that thing, and you knew fucking with them would be the last thing you ever did in that game.

I can't believe that Path of Exiles: Good listener (Listen to all optional NPC dialogue.[Which requires completion of normal and merciless and doing things in very very specific orders]). Since the newest update, this achievement is incredibly difficult!
Oh and about that sainthood, i'd pronounce you as an Apostel of my Belief if you're able to proovenly do 1 Miracles and 39 Grand Deeds for The Old Ones, Crimson Clan and 2 Heroic Deeds for the Mechanoid Void Republic.

When it was still possible to get them, "Scarab Lord" and "Veteran of the Shifting Sands" in World of Warcraft required the combined efforts of an entire guild worth of players, several months of time, extreme good luck, and heretofore unknown levels of planning. The Blue Scepter Shard segment by itself required at least four trips into 40-player raid dungeons, thousands of gold worth of rare crafting supplies, an assault on one of the opposing faction's capital cities, slaying hundreds of brutally difficult elite enemies in several far-flung places around the world, and fights against two of the most difficult world bosses that existed at that time.
the tf2 acheivement get 100,000 veiws on a youtube vedio and get a free ****** i forgot it was ether a hat taunt or all class melee wepon.
To go with that Guardin' Gnome achievement, Dark Carnival is arguably the 2nd hardest campaign in the game, with the 2nd to last chapters ending being a particular pain in the ass when a seemingly endless stream of infected come out of one room while you and your team are sprinting down a very narrow corridor.
