  • Should someone focus on “neuroplasticity” or spasticity if you have damage to the brain? The answer is not so easy but It is SUPER important. In this video, I will discuss my rationale to this somewhat complicated question.

    ello, thank you very much.
    According to my understanding, the entire injured body is one unit. There is no separation. My method is circular movements thousands of times until the tension of the head, neck, arms, legs, back is released, all circular movements thousands of times.

    I am seven years out today from a right side ischemic stroke at age 35. I have left side hemiparesis and foot spasticity. I don't know if you're producing newer videoes or not, but I have to tell you how much I have enjoyed listening to your presentations on spasticity. I have cerebral palsy, so I deal with various levels of spasticity. Sure, I have always had doctors and therapists, who were interested in helping me with it, but until recently I didn't bother educating myself on it and why we need to be treating it. My point is we can't only recommend treatments without properly educating people. Once somebody understands, whatever goes further. I have a new neurologist, and he suggested doing Botox on various muscles. He is like you, he's very good at explaining and making somebody comfortable.

    In my experience, clonus and neuroplasticity were more connected. Whenever my brain learned something new, that night my clonus was at the highest intensity because spinal cord(dumb brain) didn't want to hand over control of that muscle back to the brain. Movement dependent spasticity was always present and still haven't gone away from certain muscles after six years even though I work out 5 out of 7 seven days. Still working on it. Godd video non the less.

    Because of my brother and sister in law, my mom's rehab stopped maybe 2 months after her stroke because at that point there wasn't any noted improvement. I fought for it to continue because after her stroke I fully educated myself on everything stroke related, especially neuroplasticity, but they weren't interested. It was a severe ischemic. It will be 3 years on Thanksgiving that I found her on her kitchen floor. She is still paralyzed on right side, her paralyzed leg/foot points down and veers outward. No improvement so she's confined to a wheelchair. She also has a severe case of Aphasia to the point it's really impossible to have a conversation and short term memory is pretty bad. I guess my question is, after this much time of essentially doing nothing, can anything be done to help at this point? I have read about bio markers that can show the potential for a person's recovery. I know in the beginning is the most important time for rehab for recovery, unfortunately my power for her to continue was removed. I know this is long and probably not the right place to be asking, I just came across you tonight. You are so fantastic and getting close to the anniversary of her stroke, and the fact I haven't been able to even see her because of the pandemic, just makes me really sad and in the back of my mind, I feel like even after a long while without any therapy, there still might be a small chance to improve her quality of life. I would take even a 5% increase at this point. Thank you for your time. You are truly a gift to those who are able to benefit from your knowledge and instruction.

    Thank you for this video! I am 6 months post craniotomy for a very rare MS TYPE.
    Dealing with right side weakness . I do everything myself, I have the Urias air splint and love it. How many times a day can I use it? My wrist is what is tight but my fingers are open and extend fully.
    Is the air splint considered weight bearing? I had an OT who said yes. I also follow the gentleman you spoke about. Yes hes inspiring! As are you!

    • @rehabhqofficial
      @rehabhqofficial  3 ปีที่แล้ว +1

      Thank you for watching and for sharing your story! It is not possible to do weightbearing in the long air splint however it is the one I prefer because it also keeps the wrist straight. The only weightbearing you can do is if you have the short arm splint. I hope that helps!! Also, I hope others will follow @the_road_to_recovery_2020...I think he lives in "beast mode" :)

    I had brain surgery 4 months ago because they removed the tumors. After the surgery I cannot move my right arm and fingers. After 4 months I can now moved my arm up and down. My wrist has a little movement and my fingers keep curling but it can feel hot and cold and also pain. What should I do? And when i feel pain at my shoulder my arms is involuntarily going down.

      @rehabhqofficial  3 ปีที่แล้ว

      Thanks for watching and for sharing your story! It sounds like you are making excellent progress! I strongly recommend working with a therapist in person. The shoulder "going down" can mean a variety of things and it would be best to work with a therapist in person to help you figure out the exact cause. Changes in skin temp and pain many times go "hand in hand". Here is a video that might help for your pain questions: th-cam.com/video/fbM6C49YBPI/w-d-xo.html

      Thanks again for your excellent questions Fred! Generally speaking, there is no "cure" for spasticity. However, again, in general, spasticity is lack of input from the brain. My conclusion is that the best way to "manage" spasticity is to keep working at the "neuroplasticity". I hope that helps!!

      I spoke to one of the leading researchers on stroke recovery in the world. According to him you have to allocate large part of your brain to the spastic muscle I.e. lymp. Means keep moving it. I had the same question.

      @@fredkotthaus1518 no problem, we all, help one another.to do the same, I do lungs walk, zigzag, obstacle and backward sideways lunge walk pronate feet walk. This is all in addition to normal walk outside which has different surfaces like concrete, asphalt, grass; with up and down grade. Next, I'm gonna take up little dancing. Btw, the different type of walk i do that in my basement hallway which is about 30 feet long. This is just for lower extremities. I'm quite stable on my feet now. Most important thing is not to sit at one place. I keep moving even if it is for 5 feet. Like dr T. Said spasticity is movement based. Body is protecting itself from future traumas. Our spinal cord has taken over the control because brain went off-line for those muscles. Finally, I dont take any medication bow. I was on muscle relaxants briefly. My stroke was massive caused by a surgery around carotid artery. TC

    It's hard to get ades in a nursing home to get people up that have PTSD and it's battle for the ades to get them up (not every day) so these people decline from laying in the bed. The drugs that are supposed to help with PTSD make the chronic constapation worse. It's a vicious cycle 😭😰 most people just give up. Is there anything that can help? I'm a Christian I believe in prayer but it is such a battle it's very discouraging. When to them the answer is more pills, pills then after they give more pills they still don't get them up 😒🤯. UGH!
