Here I describe the technique of subcision. This is a surgical method of releasing acne scars using a variety of devices, needles, and instruments. Subcision is best for anchored or tethered acne scars.
The best thing to do is to examine under tangential or angled lighting, as this will reveal all. Once this is done, scars can be subtyped and mapped, with high resolution photos. The second aspect, which is equally as important is to examine scars upon animanation, namely when you speak, smile and move your face. This will give me an idea of the amount of tethering and anchored acne scars. Time and time again I get request for ‘what is the treatment’ with static photos, an impossible task to answer correctly. Acne scar assessment has to be done live- with the patient in front of you, and lighting from all angles. Photos do not map scars as well a real time examination under magnification and lighting.
I then select the best method to address our acne scar type. Once again it is the method of acne scar removal that will give the biggest change. Taking note of your skin colour, I devise a plan that may consist of -
Ice pick scars are best treated with chemical peels such as TCA CROSS or punch excision
Rolling scars are best treated with fractional lasers Fraxel, INFINI Microneedling, Dermarollers, skin needling and subscision
Box car scars are best treated with laser resurfacing using a co2 or erbium laser such as Profractional
Atrophic scars are best treated with fillers such as Juvederm, Restylane, Bellafill, or even fat transfer
Most clinics will by default ‘prescribe’ a laser treatment such as Fraxel, fractional laser, or have an answer such as Derma Roller or microneedling. Whilst these treatments can give some results, they do not offer Gold Standard acne scar treatment solutions.
A bespoke or individualised treatment approach is best, because it is your scars that are being treated and not just a device that is being used for ‘acne scarring.’ If you look at my acne scar before and afters you will notice one trend- everyone will have a mixture of scars and very rarely will a person have only one scar type. Most patients will present with a variety of scars, some patients will have more atrophic acne scarring, whilst others will have deep ice pick scars. Some patients will present with acne -pimples- zits and acne scars as well. The combinations are endless.
So the very first step I do is to ask- what types of acne scars do you have? What gives me the most information is angled lighting. This is how you visualise scars, and it is the same method I use to find acne scars, map them out, and then treat them. Most patients notice their acne scars when lighting conditions changes, classic examples are when they are in the bathroom, or in a dimly lit room, or when light shines at a certain angle. It might be at night in the car, or the reflection of glass or a mirror. I use the same principles that patients see their scars to examine, identify and treat acne scarring.
Any specialist who does scar revision to a high level would use angled lighting to determine scar types and if appropriate ask you to animate. Shining direct and bright surgical lighting reveals very little, but by dimming lights and shining light at different angles reveals a whole dimention of acne scarring. In many of my upcoming videos you will see me using newer techniques such as INFINI PRP with PRP mask occlusion, and various subsciion filler combination techniques.

Hi Dr Lim, I just have to ask you although I know its not something you would not advise, if I were to numb my skin then use a 1 ml fine needle to try on myself to try and improve a raised scar on my lip do you imagine i could see improvement lets say after a couple of treatments? Or does this process work best on rolling or sunken scars as over in the UK Subsicion seems not to have caught on as of yet.

But just wondering if you can get to feature something about the benefits of Platelet Rich Plasma (PRP) for the skin? I will be undergoing this treatment sometime next month because of my herniated disc. The doctor mentioned to me that the excess of the PRP shot can be also used in the face. I’m so happy that I discovered this video, as well as some of your others. I have (what I believe to be) atrophic scarring on my cheek. I’m not sure if it’s a scar from my childhood, or possibly chicken pox as I had a severe case of it in my childhood. My body was left plastered in scars. It has left almost like an indentation on one side of my face. One side appears plump and the other indented. People always make comments on my physical appearance. “You look like you’re on drugs”, amongst other things and after years and years of it I am completely exhausted and spiritually drained. I had no idea if it was possibly genetic, scarring, or nerve damage etc. I had absolutely no idea what to even search for, despite trying dozens of times. Thanks so much for this information, I feel so hopeful that I can begin to fix some of the things that bother me. I could almost cry 😢 I’m excited. Now to find a good dermatologist that can help me!

Dr. Lim, why is it that you are the only dermatologist that understands about using angled lighting in order to see the severity of a skin depression and the shadows that the defects or scars cast on the skin, even if it’s not acne scarring?? Dermatologists here on Long Island, New York put in filler and treat your skin under very soft, diffuse overhead lighting so your results come irregular and look worse when you get home and look at yourself in in natural (side) lighting. When you go see the doctors initially, with the texture or contour issue, they think you’re crazy because the lighting in their offices is unusually diffuse and quite flattering and hardly shows what you are concerned about. But side angled lighting shows what’s really there. I just don’t understand why this is not a concept used by all dermatologists. It’s very frustrating and upsetting …..

Acne scar revision is constantly evolving. The future is with no downtime procedures such as subcision together with PRP- filler techniques. New lasers, energy devices, and stem cells are the future. The best results are based not on technology, but technique. IMO acne scars are medical conditions and not cosmetic. If you reside in Australia Medicare recognises this as a medical treatment and a rebate applies only if Plastic Surgeons or Dermatologists revise scars.
Hi Dr. David Lim, I would like to know if how many times I can use TCA cross? Is it until I see a good result? Fyi, I' am using it evry 1 - 2 months and seeing a bit imorovement.
Thank you for your compassion. I have tried everything the Dermatologists have offered, which has been Retinol and Fraxel which I can see now is mot the optimal treatment for my atrophic scars! My scars have been holding me back for 15 years!!! I've seen multiple dermatologists and they all give me depressing information, one even said "those scars will never go away" and sent me home. And when I had severe acne the dermatologist couldn't even figure out how to treat it. Now I'm an MD myself and seriously it is not that difficult! Unfortunately they have been so incompetent and pessimistic. And worst of all, they made me feel shallow just for wanting better skin. I put on heavy makeup to cover up, but people around me think I do because I am vain. I've felt like a superficial person going in for "cosmetic surgery" when all I wanted was my scars gone. To me it was never cosmetic. And yet I live in a Scandinavian country where the healthcare system is supposed to be free and perfect. I used all my money as a student on treatments. They never worked, I feel they even made it worse. However if I can't find better treatment then yes I will visit you next time I'm in Australia lol. As you say it is about technique! X

I was able to get the techniques you use performed on my face last week here in Manila. I was treated in one session (lasted 3hours) with Subcision+TCA CROSS+Platelet Rich Fibrin Matrix injection (like PRP).
Your videos have helped me tremendously in making an informed and wise decision whether or not to follow my dermatologist's treatment plan. When I saw here that you're using exactly what my doctor has advised, I went and got the treatment.
I can say that it has been great so far. My next session will be Infini then Picosure then Fillers (only if needed and is the last step according to my dermatologist).

Yes, Fraxel .... I have 6 Fraxels but do not use them for severe scars. That company should be sued for saying it works in severe scarring.. mild mix scars, fair go...early scars 1550 Fraxel can do.. but severe scars waste of money. Sorry to hear...

I'm so close to pulling the trigger from flights to Perth. I can only travel a month before I'm going to head to Europe. My scars are mostly tethered, all over my cheeks. I do have one huge atrophic hole as well. I've seen a lot of people complaining about downtime from Infini and I don't want to have punchhole marks and other redness while in Europe. I believe I am a strong candidate for subcision.
Anyway my question to you is how much control does the patient get on what procedure they get? Obviously you know best, but if we have something like a holiday weeks after would you be more open to using procedures with less downtime?

Oh my goodness yes.. its most definitely possible to get rid of acne scars!!!!!! It takes a bit patience. Acne scars heal like how a flower or tree blooms if you know what I mean... im sure once its been taken care of you'll have better understanding and patience skills which is a win win... I know I have!! Microneedling is an amazing treatment for many..... Each session you see the scars get more and more shallow. I suggest you get a couple professional treatments first like I've done then you could feel more comfortable at home! First couple times I microneedled I was shaky Lol.. I bought my pen on overstock its called Dr pen the gold one.... You have to make sure and clean the needles with 70 percent Iso alcohal for five minutes before and after your session. Also let it sit in soapy water before the alcohal for a couple minutes before and after as well. Soap will get skin and oil type stuff off...Make sure and not let the needles scrape the side of the glass!! Make sure the needles are not bent, and make sure when you first get your needles in the mail they are not bent!!! Let the needles dry for 5 minutes. Wash your face and hands then swab your face with 70 percent Iso alcohal.. I actually have a bottle of the alcohal and little square pads of the 70 Iso alcohal that looks like like its just a moist towel handzanitizer that you get at chic fil a but its not LOL...Let dry, then put hyaloronic acid serum on your face for moisture... Needle your face(watch videos and learn as much as you can) try not to over do it... Then put hyaloronic acid on again after. Dont wash your face that night, stay at home and indoors that night. Then you put some good nutragena sunscreen on that week. Only do this every month. And if you want you can message me with any questions! I know a good bit about other acne scar treatments too! Take it easy... Be very kind to yourself:) like you would a friend. DONT be hard on yourself. And learn to ENJOY the journey... Love you self at all stages you deserve it all. Like I said microneedling works for a lot of people.. Give it a lot of time and treatments then try something later if you feel its literally doing nothing try another treatment you think would do good. Ive heard sometimes people stop after two or three sessions but it may take that fourth to kick start it basically... Make sure you can trust and feel absolutely comfy with your ethstatition... Theres wonderful rf microneedling, tca cross, fraxel, microneedling, and co2 lasers... Also subsicion and of corse filler but ya know..anyways. God bless. Anything is possible... Ive research further than anyone to know there are many people deep online that have gotten rid of acne scars. Dont let anyone tell you its impossible or a permanent situation. Im simply paying forward the comes who shared their story that helped me and encouraged me when I doubted what I could do..(i doubted I couldn't get rid of acne scars.... I doubted hoe strong we are and if we put our minds to something you best believe)

Because we have only BD Nokor 16G or 18G needle.

How banisher could repair deep acne scars with 0,5 size ? If banisher work, dr pen m8 work too, maybe the key is still using dr pen, dermaroller or banish for few years… 🙏
@@kzzaza1768 im not sure about that because we've only tried the Banisher 2.0. With consistent use, you'll see a huge difference after 8th use.
