    Microneedling: What you need to know, form dermatologist | Dr Davin Lim
    In 99% of cases acne scarring will have a unique pattern that will consist of many different scar types. This includes box car scars, ice pick scars, rolling scars, tethered scars, hypertrophic scars, pick scars, pebble stone scarring, and atrophic scars. The amount of each will vary between each individual. Additionally the skin type of the patient will need to be taken in to consideration as well as the severity of these scars. Will microneedling address all these scars? Short answer is NO.
    It’s an easy concept to understand, - treat the scar type with an appropriate method and you WILL get a response. So, what does microneedling do? Well it’s a form of CIT or collagen induction therapy and provides collateral damage to abnormal collagen. This treats surface scars such as mild rolling scars, tethered scars attached very superficially and also mild box car scars. The process itself can release cytokines that can benefit the upper layer of the skin and stimulate a controlled environment for normal wound healing, - essentially new collagen to replace old scars.
    Microneedling packages are sold throughout Asia, in Australia and probably World wide. Why? Because it this treatment cost very little to perform. The microneedling device cost between $70 all the way to $2000 - a fraction of the cost of lasers, and much less than RF microneedling.
    Skin tearing or ripping is a marketing ploy, sure it is true with poor technique and long needles on a derma roller if one uses a 0.25 to .5 mm needle, it does not apply. These needles are way too short to cause skin damage. If your epidermis is 100 microns, 0.25 mm needles penetrate almost 2.5 times the depth of the upper epidermis. Hence why you will see in my videos that I can cause lots of bleeding with 0.25 mm needles, obviously with the correct use of a dermaroller or skin needling.
    The use of energy based stamping devices like INFINI micro-needling radiofrequency or Intensif is far safer than devices that provide high hertz vertical mechanical needling. Why? Because needle delivery is vertically delivered without sliding. If one is proficient at microneedling and with the correct lubrication gel or Hyaluronic acid, this creates minimal problems. However if one miss-times the movement, it results in skin ripping that is WORSE than skin needling. So, at the end of the day, you need a good microneedling device and someone proficient at conducting the procedure. Most companies ship the device, ask you to read a booklet and watch a TH-cam video - and you are fully trained to really Fu*K up someone’s face. Sorry about the profanity, however I would like to spend my time treating pathological scarring, and not iatrogenic scars- scars caused by trauma to the skin.
    In summary, treat the scar type, this requires skill sets that most dermatologist or plastic surgeons do not possess- this means you HAVE TO BE proficient in all aspects of scar revision from scar excision, scar subcision, scar elevation, NOKOR subcsion, TCA CROSS, paint and peels. One should be proficient in testing dermal thickness prior to the use of high powered lasers including CO2 and erbium ablative as well as non-ablative lasers like Fraxel, and know when to apply these devices. PIE requires you to master all forms of vascular lasers, and atrophic scars need fillers or fat grafting. If you cannot fill proficiently you are not a scar revision specialist as most scars will have an element of atrophy- in this case fat atrophy not collagen atrophy. This is easy to test with a cannula. Do I employ microneedling- absolutely, in the correct context. I use manual microneedling with a tiny 30 g needle, and go vertical or obliquely into the scar to feel for the crunch. This way I cause as little collateral damage as possible to the surrounding tissue. Additionally with this technique I control the depth of the needles, and vary it according to feel.
    In conclusion, microneedling can be useful in the management of mild non- tethered scars especially if treated early. I prefer manual treatment - a cross over with mesotherapy to achieve the best results. This treatment is only one skill set in the many that is need to treat acne scars.

    I don’t know where you are finding all this pricing for the Microneedling pen. As the one I pay for are not cheap. Cartridges for a case of 12 are around $294. The pen it’s self was around $5600.
    Unless I am just getting ripped off but I do believe it is all in the quality of your equipment and something that cost $5 does not seem safe to use on someone face.

    When I got microneedling done, the lady pushed down hard on the pen on multiple spots multiple times, used like 3-5 treatments immediately after the procedure (when my skin was still bleeding), and told me not to take anything anti-inflammatory for two days after because she said the inflammation "helped the body heal". That was honestly the worst waste of money of my life and I'm still pissed about it. It was like $900 🙃

    This is my view on microneedling. In summary I do use this on a daily basis, however I prefer manual microneedling over devices. The BIGGEST mistake I see is that people go too deep with this procedure. Stay safe. Dr Davin Lim. Brisbane. Australia.

    I did micro needling in a dermatological clinic two days ago. I've been doing it since 2017. My skin is younger and prettier. The pores on my skin are smaller, the wrinkles around my mouth are gone. The dark circles under my eyes are clearer, too. I love micro needling 😀

      @@dimplepandey9570 Yes, microneedling will absolutely help. I am speaking from first hand experience. I had rolling & boxscars that I successfully resurfaced and improved on my own with at home microneedling. But you must use tretinoin or retin-A in conjunction with that regularly for maximum optimal results. I recommend you use those nightly. Make sure to moisturize afterwards and night, and always use sunscreen, that's extremely important.

    This is the best explanation about scars I have ever received. The comparison with the wire on the ceiling makes it so clear. Now I understand how a scars form. Here in Belgium you get little attention from a dermatologist if you consult them in connection with blocked pores or scars. I am now 75 years old and sometimes still get outbreaks with skin rash accompanied by blisters. Through a treatment with Roacutane (years ago) and repeated antibiotic cures, my skin is now oversensitive and extremely dry and thin. Unfortunately the distance to Australia is so far .....
    Thank you for all your good information Dr. Lim, I learn a lot from it.

    I am so thankful for finding your channel. I have been going to different dermatologists for years and they have all been disappointing because they seem like they dont care or its all about the money and start treating me with just looking at my face from a far... You are a breathe of fresh air and so honest. I hope to one day find a good Dermatologist as good as you. I have hormonal acne that started in my late 20s and have scaring and it sucks.     I've had 5 Dermapen treatments with my dermatologist. She has gone progressively deeper. Started with 25 and my last visit she used a 2.5. It has done wonders for acne scars, fine lines and wrinkles. I've also had some fillers and I'm sooo happy with the results of both treatments. Love your videos.

    You should spend some time to chat with older tattoo artists. We have been doing scar relaxation, and dry needling for at least sixty years. Our needle groupings in my experience having used both micro needling wheels and tattoo machines, offer superior results for very specific issues. Such as relaxing burn scars, helping to plump up deep groves and wrinkles. As well as treat very thin skin. The way our machines work, they do not penetrate as deeply. And the variety of needle rigs make a real difference as well. The needles are grouped differently, closer together, woven in rows or arranged into rounds. This allows precise usage, and the added benefit of being able to use pigment when treating scars to help blend them in as well as relax them.

    Hi Dr. Lim, I very much understand why using a disposable dermaroller is the way to go (for at home microneedling) and understand the importance of using 0.25-0.5mm needles. I remember you mentioning that getting a cheap disposable dermaroller off of ebay is perfectly fine but am having a hard time finding one, as there are so many options and have heard horror stories of some with fake needles i.e. just pointed pieces of metal vs. actual titanium needles. Would you happen to have a brand or link you recommend that I can feel confident in buying? Thanks so much

    Hi doctor Lim, I have a video idea!! As a child I was very fair skinned but as a result of playing outside I got very tan and been maybe sunburned once. Is the sun damage permanently? Because my complexion never been the same since.

    My clients and I love Microneedling i combine different vitamins depending on the skin i never tell them how many sessions they need or the improvement, because everyone is different 🤔 i do tell them that 60% is what they do at home (skin regimen) how do u know the amount of sessions needed or measure the improvement 🤔

      accutane is a miracle drug in my own opinion, I didn't see any side effects and after severe cystic acne for over 5 years it cleared my skin within a year (usually 4-6 months for other people but mine was extremely severe so I had to take 2 full courses)

    Questions:
    1. What depth for best collagen production & how often.
    2. What depth for product like bit C absorption and how often?
    3. Is it true that Vit C should not be used after or with micro-needling because it causes granulomas?
    4. What to use with and after deeper micro-needling 0,5 and above ?
    5. Are NOT genuine needle derma-rollers safe? ( the ones that are made of needle uni blades where needles get wider, where the needles are bet 400-600 needles per roller.
    VS the genuine dermatollers where needles installed within a wheel , where the needles are the same width on the bottom and middle and are not as many max 200 needles per roller)

    I don't have acne.. I just want a good treatment for anti aging.. To smooth fine lines a tighten my skin a bit.. I'm so confused and sick of wasting my money.. I'm sorry but this video has just confused me more.. I thought micro needling was the way to go... Scared to trust anyone..

      No there is micro needling called Endymed Intensif that reduces pore size and tightens skin and reduces hyperpigmentation (if you have any).
      You might have to do 10 treatments to see drastic results if you have deep wrinkles and when you’re done your skin still gets better in time (that’s including your aftercare)

      He clearly mentioned that the method can help with fine lines anf wrinkles. The idea of the video is that it's not a universal solution, a holy grail for everyone, and you should be careful when doing it at home. But a dermatologist can assess your individual case and pursue the best course for you. That's the idea that I got from what he said.

      Bridgette Collum 80% of extrinsic skin aging is caused by sun exposure.... are you wearing a broad spectrum sunscreen that’s SPF 30-50 and applying enough as well as reapplying? As far as topical skincare, there’s a mountain of evidence supporting tretinoin for anti aging. AHA specifically Glycolic Acid, peptides, Vitamin C, niacinamide, and other antioxidants are also good ingredients to look for. Once you have deep lines though you’re looking at in office treatments. Botox and filler will of course take care of fine lines and volume. You can also try peels and laser.

    i wish id started microneedling first. i spent thousands on fraxel but tca cross and microneedling was what got finally gotten rid of my acne scars and it was a fraction of the cost too. my skin is not 100% perfect or flawless but it has definitely been a massive improvement.

      Your comment caught my attention. I have significant visible acne scars. I have done 3 treatments of microneedling and was actually thinking about fraxel hoping it would be a better option for me. Microneedling has improved the appearance of my scars but they are still visible. I feel like after the first treatment I haven’t seen much improvement. It’s only been 5 months since and I know it will take many months to see a final result

    I went to a dermatologist run clinic today (staffed by qualified dermatologists alongside dermal therapists) and they’ve suggested microneedling to me. I went in for concerns around rosacea, but I also have some mild (light/faint) melasma, and because of the melasma they said they would not recommend any laser treatment for the rosacea - as they believe this would definitely make the melasma worse. I don’t have any scarring on my skin.. my biggest complaint is just the rosacea.
    After seeing this video I am so confused.. It seems microneedling is best for people with scarring more than anything else. They told me the microneedling was to help rejuvenate my skin and improve it’s appearance - saying it should help stop the rosacea getting worse and calm it down, and also saying it would possibly improve my melasma spots.
    I’m at a loss, because even qualified dermatologists seem to have varied opinions etc. I’ve been to a few different ones and been recommended different things (although haven’t gone ahead with any treatment yet), so just can’t wrap my head around what the best path is.

    Hey Dr David Lim, thank you for amazing videos! Question - when can I apply Retinol 1% serum after microneedling? I researched online and the advice I found varies a lot in terms of days - some say 1 week or 2 weeks, some say 72 hours. microneedling done with 1 - 1,5 mm needle depth. Thanks in advance! :)

    I’ve done 3x out of 5x dermaroller package, performed by a GP in a beauty doctors clinic, and it was successfully ruined my scars even worse. I regret that decision until now, and never again I allow my face be touched by GP/aesthecian and esp by dermaroller.

    Who should be doing microneedling? I visited a derm and she barely looked at my acne scars and sent me over to the medi-spa for the aesthetician guide and perform the needles. I would expect her to look, touch and remove my mask!!! This is in the US. Does Australia do it differently? do we have to go to a plastic surgeon here to have someone qualified enough to perform scar revision?

    I used 0.5 mm microneedle to my cheeks, and it bleeds a lot. So I prefer to use 0.5 instead of 0.75 to 1mm, except for pain (I use numbing cream) it has less downtime than the 0.75 - 1 m. So it means I can perform microneedling 2x a month for 1 year and I am very happy with the results.

    I doctor, you are right microneedling can worse melasma specially doing it too deeply. In order to help one should use it at a depth of 0.25 with tirosinase inhibitors including peptides. I also like to use it alterned with chemical peels and an at home treatment using despigmentants and anti oxidantes

    I really need help with my acne scars, I’ve had acne scars for over 10 years now and I almost felt hopeless because it feels like I’m going to be stuck with it for the rest of my life until I watched your video
    I would like to ask you some questions if you don’t mind,
    1) do you have a long waiting list? If so roughly about how long
    2) do you charge for a consultation?
    3) For some one who has medium rolling and tethered scars, roughly how much do they pay?
    Reason why I’m asking is because I’m trying to find out how much money I’ll need before i decide to go to Brisbane

    Hi Dr Lim, I would like to hear more about needling RF from you. I’m taking the Einxel Plus treatment once a month now for my multi types of scarring. I found it seems work for me but with a slowly results I have done 4 times so far, shall I keep that 6 more times so will be 10 treatments in total? Or maybe go for any other treatments?

    Wish I could have had treatments with you! I recently did dermapen at a strength of 2mm on the whole cheek area and 1.5 on the nose and forehead. My scars do look improved on my cheeks and nose but I got a new line scar from the treatment and I don't know whether to continue with the treatments and ask to reduce the depth or to just cancel. They also offer vivace at the same clinic but it's pricey, I might be able to afford one such treatment, but no more. I'm not on Instagram, I might try to get on there.
