How to treat acne scars- NOT with Fraxel Laser

    How does the Fraxel laser treat acne scars?
    Fraxel lasers, like other fractional lasers produces tiny columns of laser energy that penetrate the skin. Depending on the type of skin you have the depth of scars, and type of acne scarring, your Specialist will determine the depth and density of treatment. Laser energy is concentrated in the deeper dermal layers of the skin, and this breaks down acne scars. The skin responds to this breakdown of old collagen, and stimulates new collagen. This is the basis behind acne scar revision. Lasers are best for certain types of acne scars, remember, not all acne scars are best treated with lasers, and methods such as TCA PEELS, fillers, surgical subscision, PRP energy based microneedling and RF devices are needed for a targeted approach.
    What is the success rate of Fraxel laser treatment for acne scars?
    Over and over again I stress that acne scar management should be done at a specialist level, and tailored according to the patients skin type, acne scar type and downtime. Most patients who have acne scars will have a variety of scars including deep ice pick, rolling, box-car and atrophic scars. Fraxel in theory will improve all forms of acne scarring, however a targeted approach will give the best results. Acne scars can be treated with two types of Fraxel laser- the 1550, or dual, or the Fraxel RePair or Co2 laser.
    How does Fraxel compare with other fractional laser machines?
    Fraxel is FDA approved for the treatment of acne scars, however it is almost impossible to compare all our lasers at the same level. Why is this so? Well, because every patient with acne scars is unique. Scar types, depths and extent will differ for all acne scar patients, as well as their skin type and downtime. Importantly, cost can be a factor as well. As a guide I rate Fraxel as a modest treatment for acne scars. There are many other better lasers for acne scars. CO2 CORE Fractional using higher energy settings can be more effective in deeper scars, but the trade off is longer healing times and it should be used in caution on darker skin types. Fraxel 1550 in this case is safer. CO2 lasers made by DEKA and Lumenis have higher density levels, and more power. ‘With power comes responsibility- spiderman’- however these lasers outperform Fraxel CO2 by far. The future however is not solely in lasers, for now it lies in insulated micro needling energy devices, like INFINI RF, in combination with PRP. Lasers are not redundant, they still play a political role for rebates, and clinically for shallow box car scars. Remember, it is never about the device, but the method of scar revision that gives optimal results. Everyone will have a unique pattern of acne scars, and find the correct method will give the best results.
    Can Fraxel be used in all skin types and skin colours?
    Yes. Fraxel treatment is excellent treatment for acne scarring in all skin types, including darker skin scar patients. Asians, Latino, Greek and Middle Eastern patients can be treated with an excellent safety profile. In this setting, the 1550 Fraxel is the safer one, however this treatment, in my opinion is outdated with the even of insulated RF based microneedling systems such as INFINI in combination with Platelet Rich Plasma.

      Well, ... I'm not saying the laser is bad, as with any company, some devices are outstanding, others... well, can be improved upon. Creating a consumable for a CO2 laser is not a smart move for the company, and that has to be passed on to the patient, hence why the price of Fraxel is so expensive in comparison to other fractional co2 lasers.     amazing review of lasers! As a dermatologist in the Philippines, buying a laser machine really costs a lot for a practicing dermatologist and every company tries to convince you to buy their brand. thank you so much Dr. Davin for sharing your experiences with these devices.

      You are welcome. Yes, every company will try to convince doctors, even Specialist who know their devices better than them (from a clinical point of view), that their device is the best (Thermage for example... same company as the one that makes the Fraxel RePair). Yes, some lasers within that company are stand out, like Fraxel 1927 and the Clear and Brilliant system, but the rest does not stack up as industry standard.

      Took awhile for someone to publicly announce that there are BETTER treatments for acne scars. I am saying it works, but hey.. Fraxel should have more power, revise their delivery system, and not have a consumable tip for the CO2... there is no need to ...except for the company to make $$$.. this means patients are disadvantaged.     Dr. David Lim, would you say Recosma laser that's quite popular in Europe, the future for treating acne scar as well? I personally suffered really bad scarring from high intensity fraxel treatment on my acne scars years ago and it really affected my personal life.     Fraxel TRANSFORMED my skin, got rid of fine wrinkles and made my pores less visable and did reduce some scarring. The best thing I ever did! However, at Dr Lee Clinic in Bristol, i was told that deep white pigmented scarring would not go away and they did not. I shall have a look for other things, but my scarring is minimal and I was reborn with younger looking skin!

    Love your vids, Dr Lim. But I have to say love this one the best for the honesty. I am Asian and suffer from melasma. My Dermatologist suggested Fraxel as treatment. I bought a package of 4 then additional extras. The downtime was more than the 3 days I was advised (it was more like 10 days) and my melasma has actually gotten worse on one side. I have now pretty much given up on trying to treat it, other than wearing sunscreen, staying out of the sun and cover up when outside of house. Would love to see you do a vid on treatment for melasma, especially for Asian skin.

    Hi, Dr Lim thank you for the very insightful video. I wanted to ask what are some of the options for people who have acne scars but also have darker skin complications (African decent). What are your favourite methods to treat ice pick scars, boxcar scars for these skin tones? Would it be Different for darker skin or the same but just less evasive? it would be interesting and really helpful to see before and afters of some of your patients with darker skin tones.     Dr. Lim is so candid and honest - why does he not have his own tv show yet?!! I'm going to have the Infini RF done here in the next few weeks with platelet plasma. My doctor had also suggested a combination of laser, Infini and platelet plasma for the first round. After viewing this video, I think I'll pass on the laser and will try the Infini and platelet treatment.

      No.. follow your doctors advice .... I often use a combination of INFINI PRP and laser... laser has its uses for scarring and rejuvenation. Every skin type is different, as is every patient.     Hello! Please can you share if you have treated brown melanin skin? If you have did any of your devices cause hyper pigment to the skin? If so, how long or what would you have your patient do afterwards or what follow up procedure would you suggest.
    Many of these devise seem to work well for white and pink skin tones, but cause a real tragedy when applied to darker tones, even light cream complexion patients - would you agree.
    I am a person looking for a doctor who can help the recovery of skin tone and the removal of dark spots. Skin is not sensitive, but does darken quickly if rubbed to hard, heat, or picking.
    Any suggestions?

    this is a question about techniques in using the skinpen11 for acne scars(I'm a patient)I know it might not always be that great for this problem---But while having this treatment for scars (anyway)would it be protocol for the person giving the treatment to give equal attention to the entire face rather than focus on one large scar more than the other areas of the face? I'm just trying to figure out whether this practitioner knew what she was doing or whether she didn't.

    May I ask if any narrow tips are made for the RF Microneedling device? A single needle width might be good. Could the standard tip be customized?

      The insulation of the needles makes a HUGE difference. Infini RF is NOT perfect, the spring is too weak IMO, but the energy delivery is spot on. DL

    SO no lasers whatsoever? I've had 3 fraxels and 2 profractionals scions with minimal results with acne scarring. Please advise.

    you are doing a great job, doc. i have one question for you: how can i prevent acne? I do not consume eggs, nuts, almonds and try to eat healthy. But i often still have acne. I would be really grateful if you answered my question. Thank u in advance

    Dr Davin Lim I saw an answer from Dr. Weiner on realself . He said that the setting of Infini should be lower and longer pulse duration and higher TL would be counterproductive. What's your opinion about this?

      Yes, newer better things out there... I still use Fraxel, still love the 1927 ..but the 1550 for acne scars... well outdated. Davin Lim

    I'm getting fraxel on this Friday. I'm a bit nervous as I hear mixed reviews about using this treatment on dark skin (see my avatar) what would you recommend as you seem really knowledgble? I have s combination of ice pick/ with some box car scars on my cheeks and temples from acne in my 20s... mostly icepick . Am I safe with this treatment ?

    So unexpected video! Ty, Doctor. It is so sad we have to overpay for the not best treatment.
    Btw, who is the manufacturer fore "co2 core" laser? Don't hear about it.
    And what about Joule Sciton with all his modules for acne scars? Someone say it even "deeper" than co2 and get better results. I was done "cold erbium" ablation + Fractional skin resurfacing + smt else onto deeper areas of scarring from start and at the end of procedure ("all in one sitting" with 3 different modules)+PRP. Now I have a thick crust on the face.
    And dunno why in my country doctors don't like INFINI :< Almost all say it's not too strong to cure acne scars, lasers are much better for it...

    Fraxel not effective for deep acne scars cant recommend it but combination of pearl and co2 fractional laser worked for me i can tell.Have a nice day.

    @drdavinlim theres a device called FIXER by TENTREK LASERS in Colombia. They claimed is RF but I wonder if as good as Infini.

    Thank you so much for your honest sharing! I’ve had very bad acne scarring on my chin only and am very clear abs smooth elsewhere. If I don’t wanna fix what’s not broken, is it possible to do this on the affected area only?

      Hey, I think it should be possible. My cosmetologist hesitated to do only the sides of my face (where the scars are) so it appears to be an option. However, since I am only half European she went for the full face so that the skin tone blends better in case of discoloration.

    Hi Dr Lim, wish i'd seen this earlier. I had fraxel 1927 on an area which looked like a bunch of freckles but it was actually a few pimples that scarred many years ago. Unfortunately nearly 2 months later it is still red and darker. I'm thinking of making an appointment with you. Any suggestions what to do? Thank-you

      Hang tight for 2-3 months... should fade. Take baseline photo... compare.... if still there, I MAY help with other GENTLE Lasers. Davin Lim

    Wow. Now I want an appointment with you. Such I shame that I live in Spain. I have mainly rolling scars with some box and ice pick scars. Do you recommend laser type treatments over chemical peelings? I don't think here we have access to microneedling :(

      Sorry, Im at an European conference for Cosmetic and Medical Derm in Prague in 6 weeks on the so called ' expert panel' ( ME an expert LOL), anyways someone things I am, so will do my best to find out who is good in Europe.       Yes I am aware, its not as good as the micorneedling RF e.g.. INFINI or good ablative fractional lasers but can marginally improve acne scars. All the best, Dr Davin Lim

    Hello sir. I always watch your channel. There's no CO2 fractional laser nor any other acne scar treatment procedures that are available here in my city. (Kidapawan City, Philippines). I would like to ask if my acne scars (I believe it's rolling scars as described by how you define their appearance and characteristics) can somehow be healed by DIamond Peel. I'm on my 4th sessions and I'm trying to get atleast 10 sessions and the scars seems to be lighter now. However, with angle lighting I can still visibly see the shallow parts. Please give me an advise on what to do sir David.

      see someone that is a Specialist near you.... some patients need 1-2 treatment, some need 8-10..... I am only telling it as it is... I can NEVER give an accurate treatment plan unless I do 1. A psych profile 2. Examine under normal light 3. Examine under angled or tangental light 4. Touch scars, 5. Animate 6. Get a full detailed medical history 7. Understand your expectations. Anyone who does acne scar revision should follow the above rules... I am flattered by your comments however the aim of this video and channel is NOT to get more patients for me, but to educate YOU and others about acne scar revision.
