    Triglycerides can still be high on a low carbohydrate diet if you are
    eating more fat than you can burn. Excess dietary fat will go to the fat
    cells to be stored. Once the fat cells are full and if there are still
    triglycerides in the blood that are not used for energy, they will
    linger in the serum and cause damage to the heart.
    This is why caloric restriction is still a must on a low carbohydrate
    diet. Dr. Nadir Ali says that those with insulin resistance and obesity
    should be on a low fat low carbohydrate diet until triglycerides fall
    below 100 mg/dL and HDL-C rises to 50 - 60 mg/dL.
    Eating a lot of fat can also raise your postprandial triglycerides like
    how eating a lot of carbohydrates will raise postprandial glucose. High
    triglyceride spikes are inflammatory like how high glucose spikes are.

      How to lower triglycerides? Mines is 2700 as of now. Weird because 500 couple months ago.

    My doctor called to say that i needed to go on Statins (not happening!) and was alarmed at my triglyceride level. I wasn't alarmed but a bit confused as I've been on keto for over a year. Watching this video, I had a 'eureka' moment when Dr Berg mentioned fasting 12-14 hours before a blood test. I've never heard that before but it makes perfect sense - and I have no doubt that is the problem in my case - I'd probably just had a delicious lunch! So grateful for this information. Many thanks

      Also the MCT oil in the coffee, with the whipping cream these raised my triglyceride levels after a couple of years of daily consumption!

    Dear Dr. Berg
    My case, I am a 48 year old male, athlete, I do intermittent fasting 18:6 every day of the week (with some exceptions) and my diet is low carb, eating mostly nuts, fish (salmon), avocados, salads, chicken, eggs and dairy (cheese). My BMI is 23.5, my body fat is 13% and my visceral fat is excellent according to impedecia meters.
    In my last blood test (the previous one was 2 years ago and I was not fasting), my total cholesterol was 290 mg/dL (they did not differentiate between HDL, LDL and VLDL) and triglycerides at 180 mg/dL. I feel better than ever, energetically. I play beach volleyball and I have a lot of energy.
    In the analysis, I observed that the Acetone in urine was 10mg/dL so I was in ketosis since I had been fasting for 20 hours when I did the analysis. Also, the Urea was slightly high at 54 mg/dL.
    I believe that my triglycerides are elevated because my body is in ketogenic mode and uses them as its main source of energy and that is why they are elevated. The liver will be exporting VLDL and my adipocytes will be releasing triglycerides to the liver.
    I would like to know your opinion about this situation (High Total Cholesterol, TG, and Urea), but the truth is that my condition is better than ever.

    THANK YOU!!! I think I finally know what to do to bring down my triglycerides. I've been doing low/zero carb for 4 months, and I was surprised when my triglycerides were 269 mg/dl (total chol 337, HDL 79, Calc LDL 210). I will be reducing the amount of fat intake and increasing my activity level. I will also make sure to fast 12-14 hrs prior to the test. This was never mentioned before the last test. I'm so thankful for a plan of action! 🎉🎉

      Right there w/ you! Strict carni for 4 mo's, losing wt., feeling MUCH better. But, had equivalent of bullet-proof coffee(s) b4 lipids & I look like a walking heart attack...Doc wanting me on statins! A1C great, fatty liver corrected! Now I understand...thank u!

    I have been doing strict Keto for 11 months. I have not eaten any potatoes, pasta, rice, bread, beans, grains, crackers, chips, cookies, fruit (except limited berries), no processed foods of any kind. My diet consists of Whole Foods steak, beef, chicken, fish, eggs, bacon and sausage, cheese, broccoli, cauliflower, Brussel sprouts, leafy green salads, hard nuts. The only treat I have is full fat yogurt with whip cream and a few berries. My total carbs are generally under 20 a day. Sometimes zero. Im a type 2 dietetic since diagnosed at 50. I was pre-dietetic at 46, I’m now 58 yrs old, 5’5” and have went from 192 to 168 lbs. I drink very little alcohol except for a very occasional 1-2 low carb beer, but nothing for 2 months. I stopped taking insulin at all 6 months ago. Morning BG is now 125-160 and evening before diner is 110-130 generally. I also do intermittent fasting and monitor my BK’s. I am on OMAD most of the time, occasionally I have a noon lunch can of sardines, Vienna sausage, or salmon, and 1/4 cup of nuts, diner at 6pm. I do not snack. I have done many 48 and 72 hour fast, and a couple 96 hour water fasts. I drink one 20 oz black coffee per day, and water only. I thought I was doing very well, but yesterday I got my blood results. I was troubled and scared that my A1C was still 6.2, and my Triglycerides were 285, improved from 297, but not at all good. The blood was tested 40 hours of fasting. Previous to that I did OMAD all week. Of course my LDL is way high, VLDL is 58 and also High, and My HDL is 33 and way too low. While my triglycerides were down slightly, all of my cholesterols were higher. How could my trigs and VLDLs be so high? The Dr wants me to come back tomorrow to talk about the abnormalities in some of my results. I don’t understand why I’m not doing better than I am. Even after a 48 -72 hour fasts I will still see BG of 130. I don’t know what to do or how much better I can execute The keto diet. I don’t want to take statins, insulin, or other drugs. I figure that I can loose another 20 lbs and maybe that will help me, but maybe I need something to break my genetics and the cycle my body is in. I keep studying with Dr Berry, Dr Berg and others, and I can do this, it’s not hard for me, but I am so disappointed in these results. My plan now is to cut out nuts and berries totally, reduce my proportions at my mealtime, which I have been eating all I want until satisfied. I never feel stuffed anymore, but I think if I cut my portions I will loose another 20 lbs. If anyone can help me I would be so grateful.

      Dr wasn’t concerned at all with the high triglycerides, but he was concerned about the LDL and basically said I could have a stroke any minute and he recommends statin drugs. He was happy with the 6.2 A1C and said that was great for not being on insulin for 6 months. I told him I was going removing more carbs out of my diet, reducing meal portions, exercise more, and let’s see what things look like in 3 months. I asked about testing to rule out thyroid, kidney, pancreas, bile, issues. He said he didn’t want to waste time investigating when we already have statins that are proven to work and the benefits out way any side effects. I said no to statins.

    This is hysterical. And I mean by the timing of this video. Which was an issue of when I did keto two years ago of what my doctor told me why to be careful with cholesterol. I started keto again back in mid-august of this year. I just went to the doctor last Friday. I had a blood test done. In July I was 279 lb my A1C back in May was 10.4. let's just say I blew my doctor out of the water with results and I blew myself out of the water as well. I lost 21 lb I now weigh 257 lb. My A1C dropped to 6.9 and when I asked him about my triglycerides and cholesterol, which was my biggest concern this time that I wasn't expecting the weight loss and the a1c to drop as low as it was, but 3 for 3 they were normal. 😱😲 I started tearing up over everything. But the most interesting thing that Dr Berg you've mentioned was when you talked about fasting before getting a blood test for triglycerides. I didn't eat until the night before I didn't get the blood test until like 12:30 in the afternoon which I didn't eat at all not breakfast either so that was a large window of fasting time so that is why my triglycerides and cholesterol are normal. So what you said is true . I'm glad you passed this information along thank you. ❤️🙂

    People, among other things, often suffer from an excessive number of pathogenic lipoproteins in the bloodstream. Those molecules have a lot of triglycerides and cholesterol in their composition, which the laboratories classify as LDL or 'bad' cholesterol. Pathogenic lipoproteins are the problem and triglycerides in the blood, cholesterol is always good.

      Mine keep going up. I do have a thyroid problem, though. Been keto for a year and carnivore for 6 months and triglycerides keep going up. Last lipid panel my triglycerides were 150

    Mine went down, and my HDL went up. LDL did, too, particularly while fasting. Please consider fasting exactly 12 hours before your lipid blood draws.

    I had this question. I don't remmeber if I ever asked you or not.
    I got a home cholesterol kit because I want to be prepared to defend keto if my cholesterol is up (it went up when I did atkins a decade or more ago). It was up a little, not too bad. But my triglycerides were extremely high. I repeated the test a few days later and triglycerides were normal and cholesterol was up more.
    So thinking I just need to go in to the lab for the real professional test and see what the numbers really are. I don't care that much if cholesterol is high becaue I have learned a lot about cholesterol lately. But I would be very concerned if triglycerides were up on keto. that said, you gave me some things to think about

    Two data points for me: I'm 5-40g carb Keto for 6+yrs. Last two blood lipid tests are early identical, 1 yr apart:
    First number is 02/2024; second is Nov 2022; third number (normal range)
    Total Cholesterol: 376 / 369 (100-199) >>
    Triglycerides: 156 / 155 (0-149)
    HDL: 30 /30 (62)
    VLDL: 30 / 30 (5-40)
    LDL: 284 / 277 (0-99) >>
    LDL/HDL: 4.6 / 4.5 (0-3.6) >
    Glucose: 105 / ?? (70-99) >
    The two marked with an > are elevated, >> are WAY over the "normal" for people.
    My PCP is freaking out over the two Total cholesterol and LDL values.
    I do have MS (treated/stable), so I am super concerned about messing with my lipids with statins.
    I am generally healthy otherwise. I'm one of those MS people who you would not know has MS from looking at me.
    My weight is fine, I'm between 139-144lbs, 5'6". I eat 5-40g of carbs at most / day; zero sugar, and usually in the

    Thank you! I just did a spontaneous blood test and tryglycerides are 507mg/dl. Sure I had some meatballs and a butter coffee right before the test. Now I know what's on 😄
    Will do another test after fasting to double check.

    Things to help me are magnesium bisglycinate, 1000 mg a day, high dosage of epa fish oil 2000 mg a day, l am still taking even though my triglycerides is within the acceptable range. Fish oil is prescribed by my cardiologist and magnesium bisglycinate by my naturopathic doctor.

    I'm on keto for 3-4 months already. Already lost 11kgs. So we just had our annual medical exam. My triglycerides doubled than normal. Our company physician prescribed high dosage of lipitor. LOL

    Just had labs. I fasted. All cholesterol was in normal range but triglycerides were a little high at 166. I’ve been on low carb for 3 months. Curious about the triglyceride level.

    I was concerned. All other “cholesterol” and thyroid levels are wonderful exception triglycerides which is 289. The doctor says I need to reduce carbs even more, I’m like what the hell. Thanks 🙏

      @@Drberg It answered the high Triglyceride concern! Do you have a video for those who refuse to do veggies and what alternatives there would be to be successful doing keto. He refuses to do veggies and wont drink green drinks. But he has seen that one meal a day with lower carbs and the fasting has made him feel better but hes still eating carbs and sugar just not like he was. He has numerous health issues but Im thankful he has reduced and tried doing OMAD. Appreciate you Dr. Berg! Thank you.

    Even when I was doing Paleo (with berries, raw goats milk and the occasional potato) my triglycerides were 48. Cannot imagine how low they are now.

    I've been on intermittent fasting for 3 months & so far I've lost 27lbs & 4 inches on my waist size. Recently I got the results on a fasting blood lipid panel & my Triglycerides jumped from 139 to 232 after 3 months of IMF. Everything else on the lipid panel was haywire too: Cholesterol 219, HDL 38, LDL 135, Non HDL Cholesterol 181 & Cholesterol HDL Ratio 5.8. Last year at this same time everything on my lipid panel was normal.

      @The Complaining Chavnel I think the logic is that stored body fat is almost 50% cholesterol & when we loose a lot of weight over a short period of time that stored cholesterol is released back into the bloodstream which accounts for our abnormal levels.
      The only way to process all that stored cholesterol is through a diet that will help our liver & digestive tract remove it out in out stools. Green leafy vegetables, oatmeal & others are supposed to be good for that but you'd be better off looking it up yourself because I probably would mess it up if I tried to write it all down here.
      I'm due for a follow-up recheck in October so I can't tell you if it's worked for me or not yet.
      I tried going back to a regular diet & shifting from 20/4 IMF to a more lax 17/7 IMF after I reached my target weight but in a matter of 14 days I gained 8 to 10 pound right back again - lol.
      Keep trying & keep doing research, hopefully you'll find something or a combination of things that work for you. Good luck!

    I did the test without fasting and I got mmol/l 6.48, then and I repeat it next day with fasting and got total different result : 1.04 !!

    I been working on blood sugar for about a year. Eating more fat helped. When i added fiber(chia seeds) my triglycerides were cut in half in 3 months. i wasn’t concerned about triglycerides really beforehand.

    Very interesting video. Thank you so much, DR Berg. What I understand on this video that the triglycerides might be high for Intermittent Fasting even doing correct way no snacks, low cards and high FAT. My concern is Is it just temporary? unless, It would be greatly appreciate any remedy/video for how to fix high triglycerides during Intermittent Fasting or prolong fasting?     Here is my report after 7 months on Keto.
    I will not name all the symptoms that have dissapeared. Just too many to list. I will just give the numbers.
    TG/HDL 50/75 ( from 220/40)
    LDL 350 ( from 104)
    Total cholesterol 500 (from I can't remember)
    A1C dropped from 5.7 to 5.3
    B;lood pressure from, 140/90 to 120/70.
    Weight. Went from 210 to 165 lbs. so far.
    I believe if I keep going like this, in another year my LDL will hit 1000.
    I will most likely drop dead feeling good. Did you read that? FEELING GOOD . That is how I actually feel

    I have a question. My triglycerides have always been high all my life above 200 and more. I am 54 male. And have been doing the keto diet for three months. Doing a lot of omega-3 fish. And 2000 mg of niacin per day. Very strict carb intake of 15 per day. I just got my test results back again and my triglyceride was at 160. When I had my test, I was actually at a 30 hour fast that day. Only thing I had was water. And I still had 160 on my triglycerides. And my HDL was at 35 still with the LDL at 95. What can I do to help continue to get my triglycerides down. Or what am I doing wrong?

    Been on keto 7 yrs. Just started carnivore 3 weeks. Lost 9 lbs blood sugar goes up and down (type 2) My arm and leg went numb went to hospital did all kinds of tests said not a stroke dont know why, sent me home. Blood test says my Tri test was 670 , LDL 84 and HDL 33??? What the heck

    I’m 6 months in on carnivore and my triglycerides went from 80 to 188. I wasn’t told to fast but it had been multiple hours. Not sure why after 6 months I still have elevated Trigs.

    Interesting. My last bood test was without fasting. Trigs were a little higher than other years, but still very low. Nothing to panic about, but odd. I have been low carb, about 50 grams per day, for around 10 years now.
    Before low carb they were getting up toward the upper safe limit and my doctor gave me the warning.

    On my last blood work test I fasted for like 21 hours and my triglycerides were higher than normal but still normal. My LDL went way up.

    I am 25 yr old , my diet is simple ground millet+milk+peanut+almond in breakfast, 1/2 cup cooked riced , 100 grams of lentils/soya chunk and 200 grams of vegetables at lunch and finally vegetable lentil soup for night. My BMI is 19. Blood sugar is 100 and total cholesterol is 116-130 but triglyceride is 200-300. I walk around 5 kms a day and used to do strength exercises but now i feel severe fatigue so stopped. My Protein intake is around 30-35 grams which is due to vegetarian diet. So why my TG is so high despite of low fat low carb but also low protein diet?

      Stop the lentils and soya. 200 grams of carbs in form of vegetables are too high. They still will rise your blood sugar. High blood sugar means your body will store it as fat. Fasting blood sugar of 100 is actually high. It should be around 80.

      Thanks for mentioning this. ‘Blood sugar high a few days after a night out’ That is a bummer and I do not think worth the night out. So many think that as long as you are good during the week you can indulge on the weekends - on carbs and alcohol. Not true!

    Alcohol has a particularly strong effect on triglycerides. Regular, excessive use of alcohol or even a one-time drinking binge can cause a significant increase in triglycerides. Binge drinking can cause a spike in your triglycerides that may trigger pancreatitis.👍

    Just got husbands blood work back after two months of keto and his triglycerides are 1382! Please help! He’s had zero sugar and very few carbs and has lost 20 pounds. His A-1 C dropped from 7.8 to 6.6 in four months everything looks good but the triglycerides

    I’m on an extremely low carb, zero sugar/ zero processed foods diet! 3 months now. Only drink water or coffee with half & half …tryglicerides were 214
    ( they were 480 before keto so genetics play a part)
    I’m 5’7 , 158 lbs.

    Interesting thanks, now I know how to reduce TGs, BUT do I need to reduce TGs, is it bad to have high TGs in every situation, for example a heavy coffee drinker, or can it be neutral or even good to have high TGs and they are not necessarily the enemy, if so please explain when the TGs are not the enemy?

    Judging by comments, this is one case where medication for 10% is likely needed. Likely in the genes. I'm a low low sugar person, nearly KETO, not overweight, and my number last week was way high at.......865. My CAC calcium score is 0, luckily. Researching now, going to the Dr next week to seek out something other than statin. I'm out of ideas after fighting this all these years.

      I am also nearly keto for 2 years now and I had 1200 TG. From things that Dr Berg mentioned I have only faty liver. I am in panic and on fibrates lowering TG

    I have never been told by anybody before that I have an eating disorder. However, after watching these types of videos, I now wonder if I do have one. Sometimes I will go without eating or drinking anything for 20+ days. On other days I have been known to eat 12 large pizzas, eat 2 whole carrot cakes, 3 pounds of Hamburger Helper, 5 or 6 coney dogs, a box of ice cream sandwiches, 4 cans of mandarin oranges, 6 slices of jelly toast, a bowl of Bush's baked beans and a half dozen creamsticks.
    On these days, which are getting more and more frequent, I usually drink 1 gallon of orange juice, 1 gallon of chocolate Nestle Quick milk, a quart of rootbeer and between 18-28 cans of Dr. Pepper. Even after watching these videos I'm still not sure if I have a true eating problem or if I'm just a hearty eater? I appreciate all constructive comments. Thank you all so much

    The more I fast, the higher my triglycerides. If fast for 4 days before a blood test, when they draw my blood, it’s so fatty milky blood. I just don’t fast before a blood test and then my triglycerides will be 50.

      Mine went up from 135 (normal) to 374!! after 7 months. Maybe we are being lied to? Maybe "normal" medicine is right? Frustrating. My liver values are fine. My blood fat is pure horror.

      I've been on IMF for 3 months & so far I've lost 27lbs & 4 inches on my waist size. I got the results on a fasting blood lipid panel this past week & everything was haywire: Cholesterol 219, Triglycerides 232, HDL 38, LDL 135, Non HDL Cholesterol 181 & Cholesterol HDL Ratio 5.8. Last year at this same time everything on my lipid panel was normal. My Triglycerides alone jumped from 139 to 232 after 3 months of IMF. I'm 5 lbs away from the high end of my normal height/weight range & once I reach that goal I'm going to change things up a bit - stick with the diet but spread my meals back out to what used to be their normal times. If I eat breakfast at 8, lunch at noon & dinner at 4 I'll still technically be on a 16hr fast but hopefully my fasting lipid panel will drop back down in the normal range. To be honest those high results should be expected because we're burning decades of accumulated body fat & releasing all that Cholesterol temporarily back into our bloodstream, hopefully our liver, pancreas, proper diet & exercise can flush that gunk out in our stools like it's supposed to & was doing fine before going on fasting diets.     In my last blood test my FG and A1c dropped a lot (IF and keto), but surprisingly my TG went even higher than before, and my HDL even lower. I recently found that coffee can spike your TG a lot, and maybe also liquid fat. I have my doubts about the liquid fat, but the days before the test I was drinking a lot more bulletproof coffee than usual,so coffee itself would likely suffice for this. I just quit coffee, so we'll see what happens

      I did the brilliant thing of fasting all night and throughout the morning for an 1130a Dr. Appt. But I needed my coffee with coconut milk in the morning. Dumb move on my part. My HDL was low, LDL & tri g’s were all elevated. Can’t tell if it’s a false reading due to the coffee intake. 🤦‍♂️

      I went through the exact same thing. Lost 6-7kilos in 6 months with better muscle mass and lower body fat. High protein low carb diet, but my triglycerides shot up to 350. Never been physically fitter, hb1ac in a normal range (was pre-diabetic earlier) so not sure how to read this

    Started low carb after my heart attacks. Exercising more... My weight is down 50lbs from ~220 to ~163... my A1C is down from 7.8 to 5.6... my Triglicerides are up from 180 to 195! Dissapointed to say the least. But it has only been 3 months so all in all I can't complain too much I guess!

    Also some people call lazy keto, keto. Some eat lunch meat, cheese of all kinds, unhealthy mayonnaise, unhealthy vegetable oil, corn products, pre-packaged foods labeled "keto", not organic meats, too much protein, ect.... So many ways to be unhealthy on a lazy keto diet and call it keto and a lot of TH-cam and Instagram videos promoting lazy keto recipes that are garbage. These are not people focusing on healthy vegetables, organic meat around 6 grams per serving and healthy fats (whole foods), only avocado or a good quality olive oil... So this could be the reason for high ldl or triglycerides, IMHO.
