You Are Probably Anaemic: The Science of Iron

  • A sizeable proportion of the Indian population is anaemic, and it largely has to do with the typical Indian diet that is high on grains and legumes but low on animal protein. While it is slightly better among meat eaters, most non-veg folks in India don't eat meat regularly enough.
    To make things worse, most supplements are also largely ineffective unless people do smart things like consume them with a bit of lime juice and eat them before meals.
    In this video, I've collaborated with ‪@NutritionistAmita‬ to explain the science of Iron and how to make sure you are not lacking in this important mineral.

    Also worth mentioning the role of copper in iron absorption. You can't absorb iron without copper. While iron is found in meat, copper is mostly found in organ meats. It's also important to preserve the iron copper and zinc copper ratios which is of almost 10:1

    Hi! Thanks for this video. One thing I would like to note is that there a difference between being iron deficient and being anaemic. One can be iron deficient (low ferritin, low iron) without necessarily being anaemic (low Hb) and vice versa (that there are other types of anaemia not related to iron deficiency). I think it's important to note this because there isn't enough awareness of what tests are needed to diagnose Iron Deficiency Anaemia. Very often, diagnoses are made by just looking at CBC which will not include ferritin and other levels leading to misunderstanding of results and deficiencies, incorrect use of supplements etc.

      Right, there is a confusion in this. My blood CBC shows RBC low but Hb is fine. I wonder if either of them have to do with iron deficiency.

    Thank you for doing this video. I wish you could also elaborate on low ferritin. A lot of doctors and nutritionists feel it’s ok to have low ferritin - however this could be farther for my the truth. Low ferritin cause deep fatigue, hair loss, paleness among other things. Absorption is key. And so gut health plays a crucial role.
    Our beings are complex, I wish doctors and nutritionists understood that and dealt with it with more curiosity.

      I think everyone has a certain level of depth they prefer to form their niche around. This is mine. If I make it simpler, then it will just be prescriptive advice (that I am not qualified to give) rather than inciting a curiosity for people to learn more science (which is what I am keen on). But yes, the balance is hard and not easy to figure out.

    Very confusing. Dr Pal says eat meat once or twice a week. Now another doctor saying eat meat in larger quantities 4 times a week. Doctors, please sit down together and decide what to suggest for a healthy normal diet.

      From what I understand meat is a big category just like vegetables are, so making blanket statements about how much meat to consume, irrespective of the type of meat and method of preparation is quite reductive. For example, generally white meat consumption is healthier than red meat consumption, where while one can consume the latter, it must be done in lower quantities and less frequently. Additionally the method of preparation matters, one can eat the same chicken either deep fried like KFC or more balanced like a chicken soup, one can understand which is clearly healthier. In fact a similar thing regarding method of preparation can be found in vegetables as well, the same cabbage can be consumed as a deep fried cabbage Vada or as a healthier stir fried thoran, clearly one method of preparation is healthier than the other. And finally about Dr Pal, while a decent amount of his advice is quite good, I do feel when it comes to matters of meat consumption he does tend to subtly pander to the vegetarian sentiment among a certain loud section of our population.

      Amita is saying "IF you are anaemic, you need to increase your meat consumption". She is not saying everyone must consume X amount of meat. Dietary advice is not universal. Dr Pal's advice also needs to be seen through the context of his focus on gut health. If your gut is not healthy, absolutely, you must up your intake of fiber and probiotics and reduce animal protein/fat. All advice is contextual. Not universal.

    This question has haunted me for a long time. Where the hell is the iron in green leafy vegetables. As far as I know chlorophyll has magnesium in it instead of iron. So where then is the iron in all green full of chlorophyll vegetables. Let's get the science right.

    Ashok, I think you missed a nugget here... Actually in the Indian villages, any one with anemia is recommended "Goat blood" thrice over 3 weeks. So the goat blood req is told in advance and blood collected and given as a cake(since it coagulates) wrapped in a banana leaf. It is prepared with curry leaves and fried. People vouch for having their haemoglobin levels up! 😊

    I like most of your videos, but you need to make sure that you’re stating the fundamental facts correctly. I usually don’t comment on videos, but couldn’t resist on this one as someone who has done a lot of post doctoral work on hemoglobins.
    Hemoglobin contains iron in the ferrous (Fe2+) state, which allows it to reversibly bind oxygen and transport it in the blood. However, when the iron is oxidized to the ferric (Fe3+) state, it forms methemoglobin, which lacks the ability to bind oxygen. It’s only the iron in cytochrome C, associated with the respiratory chain, that goes through an oxidation reduction reaction.

    Vegetarian diet needs lots of coaxing from knowledgeable and responsible parents to the kids to eat the right kind of food..i remember as a kid i never liked eating vegetables but after years of coaxing from my mom i ate healthy food and now it has become a habit i lik to do..so next time ur kids don't eat healthy food dont giv up..cos u neither want a weak child nor a cruelty filled iron source!!

    A common comment I've seen in videos that try to logically explain healthy habits is along the lines of - but my grand uncle lived to 96 and he was a "strict vegetarian" all his life, and my grandmother's cousin twice removed ran a marathon when she was 85 years, and she only drank water for breakfast once she crossed 60. The nuance that genetic predisposition and demands from job or daily routines can affect health in different ways, and a science based diet can improve one's health irrespective of genetic predisposition and lifestyle is generally missed.

      A more basic fallacy is the law of large numbers + survivorship bias. Your grandparents are lucky survivors from a generation where many of their siblings died either in childhood or early adulthood. Most people are unable to grasp counterintuitive inferences from data and rely on personal anecdotes

    So the phytates, oxylates and polyphenols don't reduce the absorbtion of heme-iron? Does it mean, eating haleem won't reduce my iron in food?

    Informative video on Iron deficiency. Also you can include more info in some video about heme and non-heme iron side effects. There are many research which says heme iron is more risky due to its chances of increasing stroke, cancer and heart diseases. And since most research is done in western countries, we will less likely to know impact on Indian population.

    Great video Krish! Just a small doubt - heme iron intake is said to be strongly correlated with higher cancer risks, coronary disease, colorectal cancer...I tried looking into it deeper, but didn't find any good research clarifying heme vs non-heme iron benefits... Intuitively, I know 80% world is non-veg, so it can't be that non-heme is so cancerous! Please clarify if possible. Thank you❤

    9:11 Both Hindus and Muslims will have a problem with iron supplements from abroad, that reminds me of the Indian Rebellion of 1857. However, Muslims would specifically find heme iron supplements problematic, as blood is strictly prohibited in Halal dietary laws. The removal of blood is the main aim during slaughter, and it must be done in a specific manner.

    Interesting, I m vegetarian since childhood and do blood tests every year. I have never gotten a report where I m anemic or deficient in iron.
    What do you think do we Indians have some sort of adaptation for absorbing iron from vegetarian sources?
    Does the "famines in India make us more susceptible for diabetes" logic apply here?

    This is useful .. my ferritin level as well as hemoglobin level has been on the low.. all my life.. I constantly need iron rich food and haven’t been able to take supplements due to nausea but have to now every now and then have to take it

    It's a good video, but I wish you had spoken about how many women vs how many men have iron deficiency. Women have a greater need for iron, owing to menstruation, pregnancy etc. and women's daily requirement is more than twice that of men. I was a bit disappointed that this wasn't mentioned anywhere in the video, which even spoke about nuclear fusion in stars.

      Worldwide both men and women are iron deficient. Yes, women do need about 4-6mg iron more per day, but the iron requirement in women actually reduces post menopause. Hence, long term or casual IV infusion or supplementation of iron should be avoided.

    Super helpful. I realize that so many other factors such as dairy, caffeine, and now phytates prevent iron absorption. I have now resorted to taking the iron supplement, including slow release vit c when I am fasting or overnight, so I dont introduce competing factors. Thoughts?

    Thank you Krish for the great videos. I love the way you explain things. Can you please make a video on whether 1%, 2% reduced fat milk or whole milk is better to use? Here in USA whole milk I feel so heavy and adding more calories or fat. But I am hearing reduced fat milk has chemicals not better than whole milk. So I am confused what is safe and better. So I switch between whole milk and 2% milk alternatively.

      Ohh so sorry for you. High ferritin levels can be dangerous and in some cases, people can have high ferritin despite having hemoglobin problems. It's important to first reduce iron intake, throw away the cast iron pans and cookware and incorporate more copper to help with poor iron absorption.

      White Rice does not have much iron to begin with so I don’t see how it’s going to magically appear

      @@krishashok I drink green juice everyday with greens rotating every two days and have sprouts. I don’t know about anemia but my vit-D went low once. B12 was borderline. My doctor advised me to have a green juice to improve digestion there by increasing the vitamin absorption and gave me some supplements too. Now my levels are good. I quit those supplements 3 months back. But, continuing with green juice everyday. She recommended eggs but I think I have egg allergy, my stomach can’t tolerate it.
