      thankyou; my name is john, flight and critical care medic- the medics on the ground are actually my heroes- they have to answer a tone not knowing if it4 is a "bogus 911" or life and death; all thru shift- here in US, I did a 48 hour shift-only medic in HUGE area- 21 REAL 911s in 48 hours- all survived, but it is taxing on the medics

    Geez Louise, I broke 2 ribs falling down the stairs a couple years ago and I still whine about it. What an incredible woman...14 ribs!! I'm bowing to her!!

      Deemaree Dubois I fell off my son's bed not long ago, probly bruised my ribs, and it hurt so bad, for so long. I feel like a whiney baby too. but it did hurt, can't deny that

    Though semi retired now, what I do and when and where, means so much to me. Please support air ambulance charity in your area. We are a voluntary service. ....That older patient was one tough woman, bless her. ...This vid may be 6 years old, but is still valid today. Our training is what enables us, and I'd rather be out during bank holidays than sitting around at home. Thank you for your support. ❤

    Sick of seeing comments saying ‘I’m glad I live in America they would do this or that or the other’. In America if you can’t afford treatment you die. The treatment in the uk is not only free it’s very very good. They save lives everyday. They don’t treat patients on a financial basis. I would much rather live in the UK than America!

      Jeez-louis, why don't the authorities make all driver's get professional training.?
      If I sneeze wrong my job is at risk... I have to pass a physical and my license is checked frequently. People would be less likely to be so careless or irresponsible.....

    I admire those doctors , high compassion level and so darn smart! I remember when my oldest daughter fall out of the window on a concrete...Doctors were putting her bones together without enough pain medication/ somewhat sedated; there were several staff on a top of her holding her body and her screaming as they were trying to put her broken arm bones together(she was only 8 and they needed so many people to hold her down...I was explained that it is ok; I was crying in a hallway as I heard her screaming when they were putting bones together ,I couldn't do anything about it as I was not allowed in her room. I do remember begging outside the room for her to have more pain medication. I really like how doctors in this show take care of the pain!!!

      Yulia Orme : I’ve been around plenty of doctors who don’t treat pain properly. One of the worst experiences was having to assist during a bone marrow biopsy on a toddler. Poor baby was screaming for so long and I’m sure it wasn’t the first or last time it’s happened. I saw that a lot with adults but the toddler really sticks with me. I hated to see my daughter in the smaller amounts of pain kids go through normally so I definitely feel for you & your girl. The terrible thing is that there’s nothing we can do about these lousy doctors! And it’s only getting worse with the CDC guidelines for pain management.

      At my hospital we give morphine and ketimine to kinda sedate em dumb docs at that hospital 🤬

      Man this is horrible! I had to have bones put back together too at 7 yo and they put me under general anesthesia to do it since I was so young... But kid or adult, I think we have enough efficient ways to control to not have someone scream at the top of their lungs in agony...

      I broke my leg. They were going to aline my bones and I asked them to give me Versed,midazolam, so I might scream my head off, but I won't remember. They did and I woke up with my bone aligned and don't remember anything. I needed surgery about 50 miles away to another hospital. Woke up post surgery. Don't remember the ride to the hospital or being prepped for surgery.

      @Dalestar 1979 I.ve never had broken ribs, but wow, i can only imagine. Im sure that chest tube is no picnic either. One thing I didn't care for was the Dr's attitude.He had her dead and buried. I think Dr's should be hopeful and speak more positive and state the facts also

      She was one tough old cookie and while I hate to say it the dr was completely correct.Had she gone on a ventilator chances of her recovering would've been slim to none.I honestly was amazed that she lived as I knew of a healthy 30 year old that died from less.

    Vivianna is tough as a box of rocks! I've had broken ribs from many years of football and motorcycle wrecks. They hurt badly. She is tougher than I am!

      She really is. I once had a bad chest infection and coughed violently resulting in three broken ribs. It was bloody agony. For Viviana to be so stoical with eleven broken ribs and flail segment shows she's very tough. Respect.

    I really feel for the son of the 88yo. My nana died @ 94, I was very close with her. It was like losing a best friend. It doesn’t matter how old someone is when they pass, they’re still going to be missed terribly.

    I always get a giggle when a patient receives ketamine and their eyes get wide. It must be quite a trip - but thank heavens they use it.

      Ann E - it’s not a nice feeling!!! I hallucinated one time that I had Xmas tress dancing across my hospital room (my surgery was Jan 14- so Xmas still in the air!!)..... and then chickens came in my room..... I’m a nurse so I knew of the possible side effects and I tried so hard not to “let” the hallucinating happen....but you have no control!!!

      Which is an animal tranquilizer. Hard pass for me. I guess I feel so strongly about it as my (then) 4 week son was accidently given 30x the amount he should have been given....Dr realized mistake immediately, but still......

      My mother had it or another drug that caused hallucinations after a major surgery and trauma. My dad went to go visit her while she was hallucinating about tiny angry gnomes on the floor. She flipped out because she thought my dad was stepping on them and making them even angrier. She still tells us about it sometimes.

      When my daughter was 5 she broke her elbow and was given a shot of ketamine. She asked me why there were so many dinosaurs without tails walking on the walls? When the nurse came back into the room to see if my daughter needed more medication for her pain, I said no I think she's good. Lol

    A few years ago, I went to a hospital here in eastern Pennsylvania for an MRI. This hospital is also a trauma center. Upon leaving, I saw one life flight helicopter lifting off and another one landed right behind it. Talk about busy! Great job paramedics, nurses and doctors!!

      I hope you're using "Praise GOD!" in a colloquial sense, because the surgical team and a long history of scientific experimentation and research went into saving her life. Also, she's 88, and will die soon enough. Please leave God out of this. (P.S. I am so happy to see that she recovered; what a beautiful person she appears to be. And how the son suffers for his mama, too).

    My mom's boyfriend died after having a stroke last year.
    He was only 54 years old, and he just started acting really weird that morning, spilling his soda all over himself and then taking off, on foot, until the police picked him up and brought him to the hospital.
    By the time my mom figured out where he was, he was already on life support/oxygen, and had passed away within the next 72 hours or so it was extremely fast and extremely devastating.
    How fast your life can change, all in a moment.

    As someone who's been in 2 serious accidents one on a push bike other on an e bike I can say breaking even one rib is the most painfull thing 5 years after the last accident I still feels the pains

    The patient that fell off a ladder while he was cutting trees had his bone snap through the skin and he was lucky enough that the doctor realigned the bone and he was taking to the hospital for surgery by ambulance.

    In my experience, the older people get the more they want to survive. They want to get the most out of life and when they are in Viviana's situation, she will fight the good fight to get through it. But elderly people are quite vulnerable. They often can't survive the intervention. Its almost like their body chucks it in. I am 68 next week. I still have parenting to do. I am scared that something will happen to me that threatens that. My grandfather died just before I was born. My dad said it was a huge disappointment that he passed away.

    My daughter received Ketamine because she broke her femur and the bone was sticking out of her leg. She said she hated it and it gave her horrible hallucinations.

      If I had to choose between horrible hallucinations or the most extreme pain one can encounter Id rather choose ketamine.

    In the US they dont give a person anything besides morphine to set a fracture like this mans. Nothing! They could learn a lesson from these medics. You need to give an anesthetic agent while setting and realigning a fracture but they dont give it in the US. These medics did a wonderful job of keeping the man comfortable and safe. I understand there are risks in using anesthetic agents but there is also a risk in not using them.

      sorry bud, gotta throw the BS flag. It depends on where you are a medic. I carry morphine, dilaudid, fentanyl, versed, valium & ativan. I don't believe in unnecessary pain. Sorry you had a bad experience or whatever, but don't blame us all.

      Kristie Taulton it is an illegal drug and is only allowed for medical use in the UK so I wouldn't be surprised if it wasn't used in the US

    They don't give you no pain killers here when your in a accident. You have to wait and wait while your passing out- crying,groaning- you have to wait until diagnosed.

      @@emilyandre3451 I don't know about England, but in America they don't. Not any at all until you're in the ER and AFTER they have done xrays, etc. It can be very, very excruciating.

    That poor woman with the multiple rib fractures and flail segment. They (the hospital/MD's) were considering just 'letting her go" when her condition was TOTALLY treatable/ manageable with intubation, mechanical ventilation and or a high Epidural block for the rib/ chest wall pain. Geez, at least give her a chance instead of just writing her off as she is elderly.

      I had a tracheotomy when I was 32 and weaning from it was HARD. Cannot imagine doing that at 88. What happens is they take it off for a short time but you get tired from breathing on your own so they put the vent back on. Having air forced into your lungs again is very painful. This goes on for a period of days because they go a little longer each time. I remember just wanting to keep it on to avoid all that, it was terrible. Glad she made it though. I want expecting she would...

      @Andrew Veitch - managing expectations is different to writing her off. Much better to lower expectations and have a good outcome then give false hope before a subsequent poor outcome.
      Also she most likely would not come off a ventilator had they gone down that route which is exactly what he explained.
      Also she was taken to HDU which is one step down from ITU (ICU) which are expensive beds per night and limited in number so I don’t think that’s being written off, the determine just tried to determine what is the best treatment for her best interest.

    I'm a paramedic in Colorado, I wonder how you treat the pain for a person on methadone treatmen? We have a hard time managing the pain but you have ketamine that works on a slightly different receptors along side the opiate receptors, we can only give midozelam and or Roc along with 150+ fentenil, 99% OC the time a MMT patient won't have any affect from the fentenil due to a very high opiate tolerance, I. My world over sedation is a concern so my hands are tied, heck I could give a MMT patient all the opiates in the truck and still it wouldn't be the amount they take every day so that's my dilemma, hope we get ketamine soon.

      I'm a paramedic in San Francisco, CA myself... so I can definitely relate to this. My department doesn't have that either, so we can only manage the pain in the ambulance so much until they get to the hospital where a substance like ketamine is more accessible. Hopefully it'll become accessible on the ambulance sometime so we can better help patients with pain management while in transport

      It's quite rare in England for your average paramedic to be able to administer ketamine. A doctor is often present. For opiate resistant patients you could provide analgesia with nitrous oxide or methoxyfluarane if you have access to those... Does your clinical director not put out guidelines for acute pain management for patients on methadone treatment?

      TAVOR VICEROY VALDOR here in nz the (sympathetic) doctors (in coordination with CADS - the MMT programme here) will carefully titrate the dosage of opioid agonists over their methadone and monitor for CNS depression. naloxone if theres overdose. other doctors wont offer pain relief at all for MMT patients
      . as you would be aware their usual maintence dose does not provide analgesia and these patients are often denied analgesia and the under treatment of their pain is common.
      ketamine and fentanyl are used here also.

    Iv seen a few accidents here with motor scootets.They are dangerous if you cant drive a car or motor bike you should not be driving a scooter.The trouble is the older people think they have right of way??i saw one old dear argue with a tram...who could not move.

      My guess is that it would have been a disability scooter.
      Many people are quite safe to operate them who are no longer eligible to hold a driving licence.

      Are you completely aware of ALL the circumstances surrounding the incident?
      What if the driver of the vehicle swerved to avoid a small child which ran out into the road?
      These things happen for all kinds of reasons - until you are aware of the facts, might be better to leave the charging and convicting to those in the know? ;)

    i broke two rings decorating and it killed I thought I would never breathe again, I couldn't cough or sneeze and she will have to to clear her chest, but I did have many broken ribs after dying 7 times and the using a defib on me and I got a chest infection I was only 49 then and I was in hospital 4 months learning to walk again, I heard my ribs crack while decorating and I remembered the pain from when I was 49 I am 66 now so an older body takes longer to heal
