Polytone. Your question kept me busy today! It has that very distinctive headstock. In 1983, he states he endorsed a newly designed solidbody Polytone and a Polytone amplifier for about a year and half, right around the time of this video. ('84)

Polytone Contempo which is exactly the same guitar as an El Maya EM1200.

A #11 on an ending is not a wrong note. It's quite typical, actually. It implies a lydian or #4 mixolydian sound.
I thought he showed John Hart the right note as he played twice the wrong one.
