  • Lumbar Fusion Recovery Physical Therapy Rehabilitation Step by Step Guide

    Meeting with a physical therapist before and/or a few days after surgery is important so specific direction can be given on when to begin various types of exercise.
    Because of these variations, some exercises may not be appropriate for all patients. The surgeon's technique and the patient's individual diagnosis will influence what rehabilitation should be done.
    While the program will be different for each patient, here is a general guideline for post-operative spine fusion rehabilitation that should be customized for each patient.
    Week 1: Start Rehabilitation and Exercise
    A surgical team will typically advise a patient to take short walks and do gentle stretching during the first week after surgery.
    Day 1: Limit Exercise to Short Walks
    The patient must move frequently beginning the first day after surgery. The patient should walk as often as the surgeon allows, to the point of minor aching, but stop if there is any sharp pain.
    Days 1-7: Start Stretching
    For all stretches, patients should feel the stretch, but never to the point of pain, If a patient feels pain the stretch should be stopped.
    Stretching the hamstrings and quads
    A nerve stretch is achieved by lying on the back with legs on the ground, and slowly lifting one leg until a stretch is felt in the back of the thigh and through the hip. While supporting the raised leg with hands behind the knee, pump the ankle while holding the knee still.

    Patients can add more rigor and variety to their routines by using an exercise ball or resistance bands.
    The exact timing of when a surgeon will recommend adding dynamic exercises is dependent on both the quality of stability achieved at surgery and the surgeon's own personal preference.
    Guide to Dynamic Exercises after Spine Fusion: Weeks 6-12
    Because these exercises allow for motion of the trunk, many times they incorporate use of an exercise ball into the program.
    Do these exercises on a yoga or gym mat. If you don’t have a mat, try doing them on a rug or carpet.
    Weeks 1-9: Include Static Stabilization Exercises
    These movements are described as "static" because they are done without moving the trunk. They should be completed by moving arms and legs while avoiding any rocking or arching of the lower trunk.
    Stretchy elastic bands called resistance bands are commonly used for strength training and physical therapy. They are sold in sports equipment stores, big-box stores, and online.
    When using an exercise ball, maximizing the range of motion is not as important as staying in control, which takes practice if a patient is not familiar with using an exercise ball.

    I love the idea of doing bird dog with the ball. That's one of my big problems, I'm just not strong enough. (I'm post op 15 years) Strengthening my core has been super painful and not one physical therapist has suggested this. Thank you!

    Wow! I am on day 25 of my MIS TLIF of L4-L5-S1 and was wondering what I could do in the healing process as there wasn’t much information given to me outside of no BLT and walking some. Not sure how much I should or shouldn’t be doing as some of these exercises seems like it would help especially with the nerve regeneration.

    I had spinal decompression and fusion surgery with 3 titanium lmplants in February 2022. Got partial numbness in my rightfoot, and pains/cramps on same leg. Stiffness around the surgery area. Please what exercise would help me? Please.

    How to sleep with a S curvature...the C curve is towards right so the hollow is on left...and also straightening of cervical spine. Pls help as if my neck feels OK...my collarbone area doesn't feel OK and vice versa.. How do I sleep on my back? Can't sleep on sides right now as I have an injured left arm ..pls help?

    I am 5 months post spinal fushion, an extension to a previous fusion. I am waking every night sometimes many times with waves of sensation running through my pelvic area and down both legs, quite painfully. Any ideas what they are and can I exercise them away? Any advise please

      Nerve issues, see your neurologist promptly. And overtime you can exercise it away with medical guidance

      @@Si_mma I don’t have a neurologist. I do see my orthopaedic surgeon in the new year, but he already knows about these sensations as I woke up after surgery with these symptoms. Nobody seems particularly concerned about them

      Yes Champ, we stay fighting, go at them hard but respectfully and insist on getting to the root of it hence it’s complication from the surgery.

    I would definitely check with your surgeon and physical therapist before doing anything and then follow only their protocol exclusively. Some of these are more advanced than what has been indicated that I should do. I will be in a hard back brace for a minimum of 4 months and will not be getting down on the floor to do mat exercises for quite some time. Surely she meant to do these from a bed or chair, which is how they should be demonstrated. She does say to check with your doctors, but please don’t do any of these without getting specific clearance from your surgeon and your physical therapist who should be working in conjunction with your surgeon.

      @@drealomeli1020 I listened all the way through. If someone isn’t clear about their surgeon or PT after spinal surgery, it should come from the surgeon, not videos one finds on TH-cam. Most of these are contra-indicated for spinal surgery early on. And she acknowledged that as well, but many people who get spinal surgery don’t have adequate help or guidance, so my comment is not about her, but about the first and foremost need to check with your surgeon. Since TH-cam is a public space, I an entitled to my opinion on this topic and my intent is to add my thoughts on the clarity of the message. I’m sure you want that as well.

      Yes ! I regret mine 4months ago..I'm in worse shape than before...I can hardly walk and in excrutiation pain 😫 my MRI last week showed everything is good and xRay shows the hardware is good . So I don't know..I'm waiting for physio now.

    I was told not to start PT until after my 12th week to make sure the bone growth around my fusion, I can't even bend my legs enough to put on my own shoe's, my legs are very sore and what I find strange is that my heels hurt a ton, any advice on that? until I can actually see a PT, it makes it very difficult to even walk. would love to be able to go back to work sooner then later. said noone ever but I've been off work for 6 months. thank you for your video!
