      I’m a bit confused. None of the links to the orthotics show and insert with the little bump you show in the video. Please provide a new link to those exact inserts. Thank you.

      Your videos are the best! My podiatrist in Nebraska only gave me a shot and when it helped reduce pain, said it was Morton’s. Didn’t do ultrasound or anything. I learn so much from your videos and really appreciate it. Wish I lived closer to your clinic. 😁

    Hi foot doctor, I am 24 yr old with no history of diabetes and it's been 2 days since the skin at the top of my 3rd, 4th and 5th(pinky) toes roots feel numb and for 4th finger the numbness or insensitivity goes till the root of the the nail. If I follow from the roots of the toes towards the ankle I feel the numbness of skin above the veins web. Can you please give your opinion on this, whether it's something serious or treatable at home? I have tried massaging but nothing happened. Thanks👍

    Hi there thank you for your video! Very informative! I was going to ask for further clarification on the last point you made about the pad. Are you saying to buy a pad, trim it down to size and stick it in the same area or to merge the orthotic and the extra pad (placed at the bottom of the orthotic).
    In my experience i have had an ultrasound and have Morton's neuroma between the 3td and 4th as well as bursitis. Additionally my right foot is also slightly larger with an os trigonum in between the talus and calcaneus. I wear toe spacers, have orthotics, and mindfully wear wide toe box shoes to maintain function.
    I do a lot of work on my feet. As a student currently almost finished my schooling for RMT I found this was really helpful to view the over pronation of the foot as a causative factor and will be looking at similar patients in practise to analyze their gait with this in mind.
    I have been prescribed cortisol steroid shots for the neuroma but I am not sure if that is the best idea as I have been told a variety of things for various physicians for and against shots. It isnt painful all the time but i have altered as much as I am willing to.
    What are your thoughts?

    Hi ya. Been suffering from this for over a year in both feet now looking at solutions and stopping any exercise on my feet apart from the walking I have to do at work. It was caused by doing ballroom and Latin dancing wearing cuban heels which I think were to narrow for my feet. Any time frame of when this clears up as its getting very dull waiting for it to go away?

    Thanks for pointing out that over pronation can exacerbate the issue. Snowboarding can make my MN unbearable. Snowboarding stances are generally toes pointed slightly out (duck stance). I plan to adjust that inward, wear good wide boots, and insert a metatarsal arch pad underneath my insoles. Hopefully this will do it and I can ride hard regularly with minimal pain.

    Custom orthotics were super useful at first and metatarsal pads. I also switched into zero drop shoes (Altras) with plenty of toe room. I also continue to use toe separators (correct toes). Calf stretching helped a great deal too. Went from barely being able to walk to doing most activities again. I can still have flare ups but many days I forget I have it.

    Great info! Much appreciated! Im a woman who for yrs worked in law office mgmt for huge law firms running around in high heels and Im now paying the price. I will try these tips and I'm sure I will get some relief esp with the orthotics and pad underneath. Another thing I do is use toe spacers. I have silicone toe spacers and also I use toe spacer socks in the winter. Also sometimes while I'm watching tv or youtube videos, I simply lace my fingers between my toes and gently spread them apart and with my other hand I massage the entire plantar fascia, focusing on the balls, arch and entire heel. Thanks again for the great video. Im a new subbie now too!

    I think I have capsulitis. I've had Morton's neuroma before and I notice that the pain was building under the second toe this time. I would notice that my foot felt good when I started my fast pace walk but would be hurting by the time I finished my five miles. It would do the same two days later when I would walk again. I quit walking six days ago and started to ice my foot. What I believe happened was I bough a lower priced shoe (Saucony Cohesion) instead of the higher priced shoes (Saucony Ride, Brooks Ghost) which I usually purchase. Would you think that the Cohesions with less padding in the insole could have caused my problem? If so would it be alright to buy a thicker insole for this shoe until I wear it out? Thank you.

    What if two neuromas appeared on an MRI in 2016 but do not appear on an MRI in 2021? I thought they never go away? I now have metatarsalgia (says new doc) and use a different orthotic (not custom!) to address this issue plus PT....You are ever so right, all of the things my first doc did (for early issues) were temporary fixes, as pain has remained since 2016. Recent injections did zero. Great exercise ideas...Re: shoes and having to use an orthotic: should one stick with a neutral sneaker?

    • @IknowGodIs
      @IknowGodIs 3 ปีที่แล้ว

      I have both of those issues going on at the same time-I’m sorry you’re having trouble, too. Who knew! I think my favorite “Bob’s” shoes along with a bad ankle sprain several years ago and a broken big toe joint that makes my foot look like it has a bunion (that took 7 years to fully heal,) seem to have caused these cascading issues.

    but not where you step on it and apply weight.
    It feels like for every step I take someone is pushing a har metal pen button into my heel to a click.
    Does anyone else feel like this, the pain occurs far away from the neuroma?
    It is not directly painful, but very uncomfortable and annoying, but not pain like a knife of burn or anything.

    I got toe separators that are used for putting polish on toenails. I put these on my feet while I'm sitting in the evening. I do it any time I have the least hint of the pain. My goal is every night for prevention. No serious prob for 40 years now. Never wore heels, never wore shoes that hurt.

    I liked and learned from this video. One thing that disappointed me was how you talked about shoes which was a big flaw. Shoes stretchers don’t really work. There are shoe brands that have wide toe box that you completely missed. Also, socks can cause problem. So, Morton’s Neuroma is not only caused by how a person runs or the fact that they run, it can be caused by narrow shoes that cannot be stretched with shoe stretches.

      @DrTomBiernacki  3 ปีที่แล้ว

      Absolutely a lot is true, we tried to be very comprehensive. Some options work well for people in some situations and some don't. The vast majority of people we see with this are not young healthy people but older people with dress shoes and a lot of stiffness!

      @@DrTomBiernacki I started on a Speed Skating team at age 40 but I been skating/playing Ice Hockey since age 5. Anyway, my speed skating coach seems to think it might have been G-Force thrusting that we do with our left foot ( my Morton’s Neuroma foot). I was thinking possible injury from wall climbing and playing racquetball where I could feel the bones snapping on the nerve ending with inflammation. Maybe a combination of all three sports that I began at age 40 and now I am 59. Today I stretched the calf muscle the way it’s demonstrated in video and I could feel immediate improvement, therefore I now know a better way to stretch. Thank you

    • @DrTomBiernacki
      @DrTomBiernacki  2 ปีที่แล้ว +1

      Zero drop shoulda can be great for younger people, in generally this is because changes in gait(walking shorter steps). I do like the higher heel lift for older stiffer people (which are the vast majority of my patients.), A bigger heel drop usually offloads the ankle and hamstring tightness. This combined with the orthotic and metatarsal padding has been like 95% successful for me over 1000s of patients at this point

      My 4th toe clicks and hurts when I walk, too. Your comment is the first I've read of the 4th toe clicking. It seems like the problem is in the toe joint, but when I move and press the toe joint, it seems fine. Somehow, it must be the nerve under the toes that makes the toe click (the neuroma) but how does it do that?

      In those cases we would look at cross training in other areas, a possible injection and working on correcting any tightness/injury that might cause more pressure in that area. We would also evaluate why that particular foot is getting it the most.

    I have Morton's neuroma on the same foot as a bunion. Walking sideways to avoid the bunion has resulted in a foot that trails outward. The only thing that works for neuroma is wearing open-toed sandals. Now that the neuroma is better I have a bad callus under third metatarsal. No shoe has a wide enough forefoot with narrow heel. Tying shoe so it's tight over the arch makes it tight over the whole. (They used to make them.)

    • @DrTomBiernacki
      @DrTomBiernacki  2 ปีที่แล้ว +1

      Consider a supportive insole that prevents your foot from flattening out (becoming wider). If you hold the heel, then the front of the foot does not flatten as much.

    I developed plantar fasciitis in both of my feet in the summer of 2019... By January 2020 I was an excruciating pain and the doctor diagnosed me with plantar fasciitis. He was not able to do surgery due to covid patients overwhelming the hospitals so we waited until April 2021 is when he was able to do my left foot surgery.
    May 20 21 he did my right foot surgery....
    But now my right foot seems to have a whole different problem and it's excruciatingly painful and the foot therapist tell me that it was Morton's neuroma.
    I have gained a lot of weight over covid and with my planter fascia hurting so badly I haven't been able to do the things that I used to do..
    Like gardening and exercising and taking walks and some short runs...
    Are you telling us that if we get injections pain injections that I might be able to lose some weight that my pain would go enough away for me to be able to exercise a little bit?
    I've never been this heavy in my life...
    I way over 200 lb and I am 5'2...
    I only wear open sandals so that nothing is hurting my feet...
    Thank you for all the wonderful advice when it comes to pain you know how have any idea how much we appreciate it 🤔💖💖💖💖🥵

I finally figured I wasn't getting my b's and protein even though I took supplements. So I've introduced meat back in my diet and trying to stay off carbs and sugars. I'm feeling a whole lot better and my foot seems to be healing.

    Steroid provided some relief, but hasn't worked. it's been 9 days since my injection. I'm still having numbness and tingling/throbbing.
    What I've tried so far besides the steroid shot:
    Over the counter orthotics
    New shoes with the toe box stretched to make more room
    Toe spacers
    Iceing
    NSAID topical and oral
    staying off my feet as much as possible
    The neuroma on my right foot has improved a bit, but the neuroma on my left foot has actually worsened since this all began.
    I had significant swelling and inflammation of the foot tissues around the metatarsal heads which finally resolved. after the steroid shots.
    I'm not getting better. My left foot is getting worse and I'm afraid.

Great informative video thanks I will incorporate this in my daily routine , I have noticed my foot is definitely heavier whilst walking in my nb trainers suggested by my podiatrist.
