    Anyone else notice that he only talks about the pros of pellets and completely jumps over the pros for creams/gels and also injections, while focusing on the negatives?

      Ryen, thanks for your comment. Yes, there are several modalities of Testosterone supplementation in men. The most widely utilized are Creams, Pellets and Injections. In my training, which includes an advanced Fellowship with the American Academy of Anti-aging and Aesthetic Medicine, A Master's Degree in Nutritional and Metabolic Medicine from the University of South Florida, and a certificate in Advance Bio-identical Hormone Therapy, and a certificate in Advanced Endocrinology from the University of South Florida, I have been taught in significant detail the pros and cons and all of the ins and outs of the various modalities of administration. I am expertly skilled and, in fact utilize, all modalities in my office. I am sorry that you think I am biased toward one modality over the other. However, there are clearly advantages and disadvantages of each methodology along with significant medical literature on this topic. In my office, I pride myself on developing an individualized plan of treatment for each person who comes through. So, if a person has particular needs or desires as to which modality they might prefer, I am able to meet their needs and properly recommend a solid treatment plan.
      I have treated thousands of patients with all modalities of hormone administration in both men and women. While I get excellent results regardless of the modality, I absolutely see a significant qualitative advantage of pellets over the other modalities. Does this mean that pellets are the answer for everyone? Absolutely not. There are many reasons to use the other modalities and each prescription is made on a case by case basis. So, while I do prefer pellets and I have the experience to say exactly why, I do know how to effectively use all modalities and the final decision on which modality is used is made with the patient after a full consultation and thorough discussion of the various options.

    This is good information. I am a 55 year old woman on HRT. I use a combination. I have had the pellets twice (just made an appt to re-insert) and what I can say is that for one, you don't have to remember. So many people just forget to use the daily creams. I always say that is why so many babies are born, women can't remember to take a pill every day. I went on a vacation and instead of packing my HRT creams, patches and pills, I just had the pellets recently put in and it was great. My pellets are testosterone and estrogen. Then I have T cream and E patch & pills that I use as I start to feel the pellets beginning to "fade" (if that is a good word). I can't always get in to my provider (long distance to her office) when I feel like they need replaced. And I use extra T when I feel like I need it. I am surprised about all the negativity on this video. No one HAS to have HRT, if you think it is a racket, don't do it. Simple. Thank you doctor, for this information!

    If injections were the most expensive , Most all docs would be saying the injections are the best. Took me year to get my doc to agree to give me prescription for injections. Not saying all docs not saying this doctor, but cant deny there is a lot of motivation in kick backs

      I agree. Pellets are great for the market, and they suck after you drop. Same here, my urologist first put me on them but I didn't like how my mood changed when I was empty. He opted to try injections and I'm doing better and feeling great. He can increase the dose but not the frequency (a second opinion I got from another urologist) so you can reach your optimal range.

    Doctor, thank you for the insightful video. I have some experience with pellets, at the end of 4 months and a half I dropped significantly while under them. So it was hard for me to remain in the optimal level (for me) steady. When my urologist changed it to injections though, I was able to stay in the optimal window longer. We increased the dose but not the frequency. Again, eveyone is different and I wanted to share my humble experience. Great video though. Thanks again.

      So, any technique can be applied improperly and get less than satisfactory results. Many men need their inserts between 3 and 4 months. An experienced pellet doc will learn, over time, the dose and interval that works best for a patient. The goal is ultimately to have very little "wearing off" at the end. While there are people who have a more difficult time, most of my patients have very smooth courses with little in the way of up and down, once I learn their body's physiology and adjust the dosing and interval to fit their needs. The key is to listen to the patient and adjust to THEIR needs and NOT to apply an artificial dose and interval strategy that does not take each particular patient into consideration

      Kadir, thanks for the question. Nebido is a longer acting form of Injectable Testosterone called Testosterone Undecanoate and theoretically it should last longer between injunctions (2-4 weeks depending on the patient and the physiology). However, in practical usage it is not as effective and as reliably steady in its release as we would prefer it to be. If I prescribe Testosterone Injection Therapy (which is not my favorite, I prefer Pellets and then creams as the second option), I am no moving toward a shorter acting form such as Testosterone Cypionate divided into smaller doses, yet on a daily basis. In this manner, the level of Testosterone available to the body can be kept more evenly at the blood level that is determined to be best for any particular patient. The exact level of Testosterone in the blood will vary from individual to individual on the basis of a number of physiological reasons. So, as in most of Hormone Therapy, the exact treatment is individualized to every patient's initial blood levels, physiological make up and specific hormonal deficits

    As far as I know testosterone produce in the morning,, - I wake up every day 6 o clock in the morning and have two questions
    1- is my body missing the morning sleep to produce testosterone till 10 am ??
    2 - I do exercise on empty stomach in the morning before I have breakfast - as cortisol rise in the morning am I doing any harm to me for my testosterone production?

    My test level was 39 one week and 79 the next week and im 38 and have had low t for 6 yrs and its hard to find a doc to put my on a gel

      Barry, I really feel for you. I understand, I see so many patients who have been either given mis-information or mis-treated due to the poor understanding of exactly what proper Testosterone management should be in patients. There is no question that you SYMPTOMS are far more important than a level (though your levels do seem quite bad). If you feel poorly, you have the absolute right for a properly trained doctor to take the way you feel into consideration.
      I remember the first day in Medical School. They taught us: "If you listen to the patient, he will tell you what is wrong". Far too much today, that has been turned right on its head. More and more today I hear "IF your lab test is abnormal, THEN you can be treated". Medicine has really gotten into a bad way. There are a lot of reasons why, just know that I still value the patient first. Don't get me wrong, lab testing is very important, but it is not the primary determinant of when and how to treat.
      Your situation definitely deserves attention, you seem to have suffered needlessly for way too long. I know that I can outline a treatment plan that will very likely get you feeling and performing better.     I’m a 48 year old male and weigh 294lbs. I have just had blood work and my t level is at 468 . I have hyper tension and diabetic would you recommend I try test therapy for weight loss and increased physical and mental energy

      I can't recommend T for weight loss, however, I can say that without your T being properly balanced (and from what you are saying, it appears you are DEFINITELY NOT balanced) there will be little chance you could lose weight. Being hormone balanced allows better sugar metabolism and maintains muscle mass, both of which are crucial for proper metabolism and ultimately weight loss. In my experience, the overwhelming majority of men are absolutely NOT happy with T levels of 465

    Natural production will go down a lot due to TRT ?
    How long will it take to achieve maximum natural level approximately??
    Or it won’t go back to it’s natural production ??

      Of course, if on exogenous TRT, the natural production will go down. Once you stop, most authorities believe that natural production will return to baseline within about 6 months.

      Yes, Clomiphene and HCG along with a nutritional/supplemental program have been effectively used to raise Testosterone levels from within and without the fertility issues that exogenous T has.

    Is there any chance to prevent Hair loss while on trt ??

      Kadir, Thanks for your comment. My response is that Hair Loss is not as big an issue with Testosterone Therapy as many make it out to be. Indeed, it does happen on occasion, however, it is more age and genetic determined and hair loss can be corrected in the majority of cases with proper metabolic management. First, anyone with Hair Loss should change their nutrition. They should move toward a Gluten Free, anti-inflammatory nutritional program. Second, they should make sure that they are using a chemical free, organic shampoo. Next, a medical professional should make sure that the Thyroid is optimized, that Iron levels are appropriate and that the patient is on a good solid nutritional supplement that is formulated specifically for hair support. Most of that time, that is good enough to restore hair to the patient's satisfaction. On occasion, there are times that a DHT blocker can be used in men and women to help. However, those medication should be used with extreme caution as they have been associated with some disastrous and sometimes fatal complications, especially when used in men who do not get Testosterone supplementation. I hope this helps you understand hair loss and its potential association with Testosterone

    My wife is getting ready to go on the pellets, and we are talking about me getting the pellets for T also.
    She has certain concerns like, not wanting to grow whiskers and any other weird side effects. I have certain fears about cardiac health, developing breast tissue, baldness, etc. Can you please speak to this a little bit? Especially as to pellets and any difference it provides?

      Tim, thank you for the note. Appreciate you stopping by and taking time to review the videos and alert a little bit about hormone therapy. Testosterone is such a wonderful hormone., Obviously everyone knows that it is the "male hormone". However, few people actually realize that testosterone is 1 of most prevalent hormones in women as well. The concentration of testosterone in women far outpaced is that of most of the other sex hormones. The number "looks smaller" because of the different units of measurement, testosterone is present in women in the literature is very strong that it is a very beneficial hormone properly restored in women that have insufficient levels.
      The good news is that testosterone in women is crucial for energy, focus, brain health, bone health, pelvic health leading to less incontinence with coughing and sneezing, muscle health leading to less falls after the age of 60, reduction of visceral and subcutaneous fat, improvement of sugar and cholesterol metabolism, breast cancer protection, improved sex drive and improve sexual responsiveness. There are many other benefits to testosterone as well. These are all well-documented medical literature. Sadly, the FDA fails to recognize testosterone as an essential hormone for women's health and vitality is the age. Conventional medicine also misses the point. The problem, however, with testosterone is that is indeed an "androgen". As such testosterone may be responsible for oily skin, facial and body hair, acne, change in the voice, change in the hair, and irritability and aggression. These are inherent excess effects of testosterone regardless of the motor administration (pellet, cream, injection, sublingual) the expression of excess effects is largely related to a woman's genetics, the amount of sex hormone binding globulin and other very individual factors. And in every single application of testosterone to a woman there is a balancing effective getting the best benefits noted above and balancing them against any potential excess effects. While it is commonly believed that pellets may provide a greater incidence of excess effects, this is more likely due to ineffective and inappropriate dosing by the provider and not by the modality of administration itself.
      I have been providing pellet based therapy for nearly 15 years. I have done almost 12,000 inserts, about 60% of those in women. It is always about properly balancing the dose to get an effect against those of excess effects. And the incidence of these problems really is unrelated to the mode of administration and more related to the ultimate dose combined with other genetic and physiological factors. Testosterone pellets happen to be the easiest mode of administration, give the smoothest course of action and provide very high patient's satisfaction. Administering and fine-tuning the other modes of administration can take a great deal of time longer and is subject to absorption across the gut or across the skin which can change from day-to-day.

      Hi Jamie, thanks for the question. Bio-identical Testosterone is exactly the same as the Testosterone in your body. As such, it causes no serious side effects. While this may seem to be a controversial statement, the reality is that there is nearly 100 years of study on Testosterone with several hundred studies on Testosterone and its effects. Every well done study shows the remarkable positive health enhancing effects of appropriate Testosterone supplementation. There are however, a relatively few poorly done studies that try to make a claim that Testosterone is in some way dangerous. Those studies for the most part are poorly done, have a a particular agenda behind them, are financed by those that benefit from men with more medical problems or are just down right wrong.
      I can assure you that Testosterone therapy, properly done by a trained professional is heath and health enhancing. While too much Testosterone can cause symptoms of Testosterone excess (agitation, irritability, aggressiveness and the like), it cannot do long term serious harm
      Testosterone does not cause "spots"
      Testosterone can be used in diabetics. In fact, it is well documented that Testosterone enhances sugar metabolism and improves insulin resistance. so I would argue that EVERY type 2 diabetic should be on Testosterone for long term health benefit.

You know, no one died of low testosterone.       David, I do know that BHRT therapy is widely available in Europe. However, I cannot say exactly what forms. My preference is sub-cutaneous pellets. Creams next and then injections. I am sorry, but I just cannot know what is available/Legal in England. I do have a couple of patients who come to Orlando for their care from England, since they are here, they can take their Testosterone or other hormones home with them.

    I read about encapsulation of pellets within the body. The testosterone would then be unevenly delivered, which can even lead to punctual overdose and, in worst casse, to cardiologic emergency, as it was the case by an FTM in Germany.
    What can you say about this kind of side effect? Can you give some recommendation about any specific product with a lower known risk of encapsulation?

    Is it common for test levels to drop when switching from injections to creams? It happened in my case. I went from 1092 ng/dl on injections down to 420 ng/dl on cream.

      Hey, thanks for the question. Please understand that this is not a forum for medical advice, so please consult your doctor as it regards your specific dosing needs. However, my philosophy is to dose as much Testosterone as is needed to adequately control symptoms. There is no GOOD literature that shows serious long term negative effects from Testosterone levels that relieve symptoms. So I prescribe enough Testosterone to relieve SYMPTOMS and not to achieve certain blood levels.

    I got pellets. I"m not happy that I did. I inject HCG twice per week so I have no problem with injections. There's no way to get an even amount of testosterone with the pellets. It's just not controlled like an injection (or multiple injections) would be. Also, it's more money for the doctors who prescribe pellet therapy vs. teaching someone how to inject properly. Just my opinion, right or wrong.

      You know, that's actually the good thing about multiple modalities. If one doesn't work for you, there are others. Pellets are VERY popular and many men and women do quite well on them. Yes, it is a bit more expensive, but for those who chose the modality, the expense is worth the convenience of the "fire and forget" type of treatment. In addition, I have seen many who were not happy with pellets prior to being in a doctor office that is expert in their application. Many times it is more unhappiness with the management (either they do not like the doc or the doc is not a true expert in pellet therapy) and once they get proper pellet care, the patients become quite satisfied. The bottom line is that no matter the modality (cream, shots, oral or pellets) it is really a matter of how well the doctor practices the technique and the relationship between the doc and the patient

    As I am doing my home work to select a doctor for my treatment I found a very confusing fact is :
    Some source including doctor and internet saying “ low testosterone cause less sperm and infertility “ ..... as a normal man some should go for TRT as he needs fertility.
    Others saying “ TRT side affect makes less sperm count and infertility “ So according to this information some can say then I won’t go for TRT as I need to save my fertility ,
    Doctor can u pls Ans what is this ?
    Does TRT as a little side affect to make infertile or low testosterone make ppl infertile ??

      Kadir, both are true. A man with Low-T may have fertility issues and a man treated with Testosterone applied from the outside (that is creams, Pellets, or Injections)can suffer from infertility. And both may see an improvement in fertility: the "natural Low-T" by having the body increase it's production of T. And by discontinuing the externally applied T in the case of treated Low-T.
      However, there is an alternative. Men wishing to preserve fertility and still raise their testosterone levels, may respond to a woman's fertility drug called Clomiphene. Clomiphene may stimulate the body to increase its internal production of Testosterone and reduce the man's low testosterone symptoms. It is not as strong or effective as externally applied testosterone, but it dose give satisfactory improvement in many men who want a family. After they have their family, many men on Clomiphene will convert to externally applied testosterone.

    Can testosterone injection be taken like this??
    T- Injection after 6 weeks injection again then every 12 weeks?
    Is that normal & wouldn't testosterone in your body run out or low due to large amount of time between doses.

      When testosterone needs to be re-dosed really depends on the modality and the person. Injections can be done from daily up to twice a week for ideal results. Creams are applied daily, sometimes twice daily. And finally pellets are put into place every 10 - 20 weeks depending on the person and their metabolic rate.

    Gels are a more consistent level applying them each day. At least I hope so as i'm coming off injections to give it a go. Keep off alcohol because that ruins your levels. Easier said than done.

      Hi there spu3! thanks for your comment. I have had extensive experience with all delivery modalities for testosterone. I will say that commercially available gels and creams are among the LEAST effective methods of obtaining therapeutic levels of Testosterone. Typically, these preparations are limited to about 16 mg daily. In my experience, adequate levels in the blood are obtained only with dosing that provides 100 mg or more in daily dosing. For this, compounded Testosterone is actually the best (for transdermal delivery). In addition, compounded T is mixed with specially designed creams that are much more effective in promoting transdermal transport and are much less irritating. Alcohol based delivery products can be quite irritating and in fact burn uncomfortable, especially if applied to the scrotum. So, for best results, you should really work with someone who knows how to properly customize treatment so the proper levels of serum Testosterone are obtained (largely based on symptomatic relief as opposed to any specific "target" number). And with someone who knows what delivery mechanisms are the best for any particular individual
      For me, the three delivery types for Testosterone in men are injection, transdermal creams or gels and pellets. I have treated hundreds of men and overwhelmingly, pellet therapy provides the best and most predictable Testosterone levels with the smallest amount of administration headaches. IN addition, there is no question that qualitatively, men prefer pellets to any other delivery mechanism due to superior compliance, ease of administration, lack of risks (especially transference) and a host of other reasons.

      The first thing is to be skilled. It is extremely rare for me to give too much. Almost all of the time, if there is a discrepancy, it is because the guy has gotten too little. And it is exceptionally rare to have side effects within what every range a skilled MD will put the T at.
      The bottom line, even in the extremely rare instance of too much, it does really burn off quickly

      That is a very interesting question. In older men, above the age of 25, we recommend Testosterone Cream on the scrotum. (not gel as it has alcohol in it and it will burn the scrotum - but we do not use gel as the dose is too small). However, I really cannot comment on a 3 year old. I would most definitely defer to the recommendation of an endocrinologist. We generally treat adults over the age of 25.

      Ive been in the medical industry for over 10 years. I can tell you without a doubt, since 2008 doctors are scrambling for any new revenue source they can find. That being said, I do believe your good docs would still stick with pellets. The lack of the constant peak and trough is by far the biggest benefit. I’d say this is the “safest” method, yet not the best method for those wanting to get huge, or feel better instantly.

    You failed to mention that pellets raise your Hematocrit level as well as Hemoglobin to the point where is was experiencing stroke level blood pressure / horrendous headaches and shortness of breath to which I needed an emergency phlebotomy and now scheduled phlebotomies . You were not the prescribing Dr ./ However I came here seeking gel info - that looks like my only option as with low T comes low cholesterol in conjunction with a PSK9 inhibitor is causing myelin degradation - feet burning - NO one does the real research

      I am so sorry that you are have such medical issues. Goodness!! It looks like you have several issues here that need to be addressed. I want to be clear, that the issues you have may require the attention of a medical specialist. So, please make sure that your headaches and blood pressure issues are checked out. In this forum, I really cannot address any specific and personal medical needs.
      However, as it relates to some of the hormone specific questions you mentioned, I will endeavor to address those. First, continuous use of Testosterone may in fact lead to an elevation of the blood count. This is a benign condition known as “physiologic erythrocytosis”. It is very simply a normal healthy response of the body to an increased need for oxygen carrying capacity. No properly done prospective study has ever shown the harm in the increase of the blood count in response to Testosterone therapy. Far too often, physicians who are not experts in hormone therapy and who have not read the extensive body of literature on this topic will confuse this healthy response of the body (quite similar to the production of red blood cells when people live or work at high altitude elevations) with a dangerous medical condition known as Polycythemia Vera. In deed PV is a very dangerous blood cancer that leads to an increase of all of the elements of blood (Red Cells, White Cells and most importantly platelets). This increase, particularly in platelets is indeed responsible for an increase in blood clotting and stroke. Again, Physiologic Erythrocytosis in absolutely NOT a dangerous condition and has nor known connection to adverse outcomes.
      Physiologic Erythrocytosis does NOT require regular phlebotomy (drawing off excess blood)
      In addition, the proper prescription of Testosterone will lead to an improvement of cholesterol parameters. And this effect is linear, meaning that the higher the level of Testosterone, the more positive effect on the cholesterol/lipid profile. In fact the effect is so profound that testosterone will lower the risk of cardiovascular disease even in men with the most sever forms of heart disease. This is clearly demonstrated in one of my favorite studies of Testosterone on cardiac outcome in men with known severe cardiac disease: doi.org/10.1016/S0735-1097(16)32098-8
      Finally, PCSK9 Inhibitors are exceedingly expensive medications that have questionable value. While there are some studies in the US that show MODEST improvement, emerging literature shows that the modest benefits of these medications may be far outweighed by the downside: (doi.org/10.1080/17512433.2020.1787832 ). Of all of the medications studied for the reduction of cardiovascular disease, Testosterone, by far shows the greatest benefit.
      Chris, so yes, there is REAL research out there, you just need to find a provider who knows, understand and has experience with the current medical literature.

      Could very well be. In my experience with hundreds of men and Low Testosterone symptoms, most are very unsatisfied with a level of 430. This is a big problem. Most doctors say that this level is "normal". However, it is more important to treat symptoms than a lab level and In my office, men with symptoms and low T levels such as yours respond very nicely to Testosterone Therapy. I will typically raise the levels to 1,100 or more often with dramatic changes in the way men feel.
      Also, it is just not about feeling well. Many well done medical studies show that men with T levels less than 800 are far more prone to a significant number of diseases of aging. I would love to chat with you more about this. Of course this is limited information, however, you sound like the perfect candidate.       Nelson. I totally agree!! Prescription Gels can be virtually useless!! They mostly have woefully low dosing and thus do not achieve proper blood levels for symptomatic improvement in many men. Compounded Testosterone creams are far more effective as they can be individually created so as to deliver a dose that actually achieves a result. Shots can be effective, but current recommendations require daily dosing. My favorite is pellets that can be implanted once every 4 months and they deliver good blood levels that actually relieve symptoms and promote long term health and wellness.

      Yea, I hear that all the time. First, there are several different delivery modalities such as pellets, creams injections and others (mostly then for women). The main reason is that different people respond differently to different modalities. What is right for one person may not be right for another. In addition, providers may not be proficient with the use of all of them, so in that case, they only use one and only get good results with only one. And then they get less than satisfactory results with others if the ever use them at all. However, mostly, when I hear that creams are not effective it is usually because the provider is not using an effective carrier cream or the provider is not dosing properly. Transdermal creams and gels can be quite effective for many people. The problem is that the dosing is frequently inadequate. For example, commercial creams and gels rarely get adequate dosing and then that leads to inadequate levels (most transdermal applications in the US are less than 50 mg of Testosterone per application). Actually, many men actually require 200 mg per day of a transdermal application to get satisfactory results. This just might be the reason that they didn’t work for you.
      So, the bottom line is that most of the currently available modalities of administration do work. They just have to be administered by providers who know what they are doing. And then, they need to be the right application for the individual patient.

      John Carrozzella Thanks. I used Testosterone Atrevis. The standard dosage is 1/2 ml twice daily.
      The odd thing is that even when my numbers were decent, there was no corresponding alleviation of symptoms as there was with Cypionate.
      So T levels of “X” with cream made me feel like I would with T levels of “X-Y” on injections. Odd.

In addition, to know the exact mg dose you were getting, you would need to know the concentration of the cream in addition to the amount. Obviously, 1/2 a cc of 50 mg/cc will get a FAR less response than 1/2 a cc of 200 mg/cc. Ultimately, it is good that you are getting results with cypionate. In general though, I get great results with pellets, injections and with creams. It is a matter of getting the right dose (and I do not treat to levels, they do not mean much - results and symptom control is the measure of proper dosing) for the individual person.

    1- is my body missing the morning sleep to produce testosterone till 10 am ??
    2 - I do exercise on empty stomach in the morning before I have breakfast - as cortisol rise in the morning am I doing any harm to me for my testosterone production?
