And in the very beginning, while you are saving Jack rider (right before penguin attacks you) azrael appears on top of a building in the background

"From the ashes of Arkham the fire will rise" could mean a threat coming from Arkham? I believe the Arkham Knight is an ancient inmate or guard of the Azylum so it would be a reference to him. "Gotham will burn" I think it's a reference to the chaos that consumes Gotham in Arkham Knight. "You will burn too" it either mean the death of Batman or the end of his legend.
Edit: Coming back to this comment after 6 years… I think I was pretty close yeah, keeping in mind that I guessed those only by watching the cinematic trailer of Arkham Knight when it was released.

For the first two lines, he's actually referring to the events of Arkham City
"You are the warrior who will close the gates of Hell" refers to preventing the plans of Strange and Joker from succeeding
"You are the warrior who will save this day" refers to him accomplishing his goal in stopping Strange and Joker
"In doing so, events will occur which you cannot stop" Joker's death, Harley Quinn's Revenge, the following years of peace in between City and Knight, and ultimately the villains banding together for the events of Knight to happen

I think it would be interesting to play as Azrael in Arkham Knight. I picture his playing style to be a bit more hard hitting than Batman (Azrael does have some measure of superhuman strength), but not as agile or fast. I think his combat would also involve his blade, and his takedowns would be a little more brutal than Batman's.

In a way, yes. Azrael made it sound like some divine intervention would set the fires of hell loose on Gotham. But really it was a one night war on Gotham with Scarecrow and the Arkham Knight leading the charge against Batman with most of the villains following behind. Azrael approaches Batman on a side mission in the game requesting that he become batman's successor should he die at the hands of the arkham knight. Batman goes through training exercises for Azrael, during which he discovers that the Order of St. Dumas has brainwashed him into betraying Batman. The player is given control of Azrael and then given the choice of trying to kill Batman, or leave Gotham to bring the Order to justice. It was a fun side mission chain, but it ended up kind of anticlimactic with the foreshadowing in this mission.

when I was doing this guide the one where azreal was on that thing I grapple on to it and he came out of nowhere and jumpscared me
Can I just say that Rocksteady wasted part of this mission and Azrael would have been an excellent boss?!!
Anyway this was one of the best games ever created
Not really since city already had its fair share of bosses - grundy, ra's, freeze and clayface - plus the timing was not yet right. However this opportunity was definitely missed big time in arkham knight

I’m glad they made Azrael a playable character, but they should’ve gave him a story DLC. Hopefully in the future that’s fulfilled.
He's not appearing at the third site for some reason.
He is there, but you have to be vigilant, he does stay hidden until you visit certain areas:
1st appearance, After saving Catwoman across from the courthouse
2nd appearance, After visiting the Steel Mill for the 1st time on top of the ferris wheel
3rd appearance, After interrogating Mayor Quincy Sharpe, next building over
4th appearance, After saving Vicky Vale from Joker's snipers/helicopter crash look up after talking to Joker and scan the roof tops.

Yes you can. There are certain missions like defeating Two Face which follow right after the main story is complete. Also the Riddler Conundrum continues even after the main story is over allowing you to find all the Riddler secrets at your own pace

No. In the comics, Jean-Paul Valley Jr. was the first, he appeared as Azrael in Batman: The Sword of Azrael #1: Oct. 1992, Michael Washington Lane appeared as the second Azrael in the mini-series during the Battle of the Cowl arc in 2009.
Azreal's outfit looked cooler in this Batman.
Azrael is giving away an easter egg. From what i know city of Terror, or arkham world wont just have scarecrow, but i have a feeling that Scarecrow will let Trigon loose on Gothem and batman will stop Trigon, but lose Gothem in the prossess. that will be so cool for a third game.
