    The last RUD was in 2016. Rockets from the Falcon 9 family have been launched 364 times over 14 years, resulting in 361 full successes (99.18%), two in-flight failures (SpaceX CRS-7 and Starlink Group 9-3), and one partial success. This tells me that overall the Falcon 9 is a very dependable vehicle. Does this anomaly affect its reputation in the business world? Any launch has risks associated with it, look at Starliner or Apollo 13, so I highly doubt that 1 RUD is going to have a significant effect, although a team will fact find to figure out what went sideways.

    After retiring from 48 plus years of civil and military aviation experience, two things are universally true in technology of any kind as well as life. #1 Sometimes shit just breaks, it just does. #2 Shit happens. Usually when you least expect it. Greasy TexMex food and cheap beer/tequila as an example.

    It's my understanding that the satellites are in orbit, albeit an eccentric orbit.... with SpaceX determining if they can pull them up into nominal orbits, or if they'll have to deorbit some, or all. From Spacex... "Although the stage survived and still deployed the satellites, it did not successfully circularize its orbit, but it did passivate itself as normally performed at the end of each mission. This left the satellites in an eccentric orbit with a very low perigee of 135 km, which is less than half the expected perigee altitude."

      They're dipping down into atmosphere that is too thick at their perigee. Their station keeping thrusters are intentionally very week to preserve resources. SpaceX updated their firmware to allow them to raise/circularize their orbit faster by thrusting harder but it will waste precious resources and presumably impact station keeping ability / lifespan. They also utilize some limited station keeping solar sailing capability using their solar panels.

    We live nearby the base and we watched the launch. It started out normal enough, a very bright flamy end rising up into the sky. As it passed though the cloud layer it left a pretty exhaust trail for about a minute. It looked like the first stage cutoff was normal, but at the point we expected to see the light of the second stage, it never occurred. We normally can see the second stage engine for a minute or two, before it is too dim to see. This is the first time we never saw the flame of the second stage. After a moment we just gave up and went back in the house. We live in Santa Maria which is nearby Vandenberg SFB.

    *CORRECTION; The 2nd stage didnt fail around 8 minute, that was when it reached the initial orbit. Normally when the 2nd stage gets into orbit it coasts for a while before relighting the engine to raise the vehicle into the final orbit. The failure happened at the 2nd relight when the engine exploded about 2 hours after. They had no choice other than to release the satellites at the lower obit. I believe it was a LOX leak

      That makes the most sense from what we know now, the booster landed just fine, it was only the 2nd stage that had trouble and like you said (which is what Elon said) it had trouble on the re-light

      Thanks so much. Normally when telemetry stops all forward backward motion, altitude and speed no longer register it’s gone as there are two or even three means of receiving telemetry data. Same with the IFT. The moment telemetry stop, the craft exploded. But maybe I’m wrong. Appreciate you taking the time.

    Ultimately it won't matter that it went poof. They probably got some really good data that will improve things in the future and they'll get to put those ion thrusters to the test to see what they can do even despite the fact they're likely to be lost.

    They weren't lost. They were put in a lower than expected orbit. And are attempting to raise that orbit. So far they have contacted 5 out the total number of satellites..

    Actually, this was a good thing. Okay, there was the loss of the second stage and probably all the satellites. What they learn from this RUD will give them the insight to re-engineer whatever went wrong. This will improve the safety and reliability of the system.

      Good point. We need to note that the 2nd stage is not designed to be reusable / recoverable. SpaceX did try but found that the technology needed was (for now) beyond what even the whizz kids there could master. But as you point out, the information they get back will help them to improve those 2nd stages and other, similar systems.

      Great Comment ! I will go with Option three (3), the Crew Dream Chaser ! The one to avoid is the current Russian Capsule ! One reason is comfort (1) Yiou sit with your knees in your face, (2) you de-orbit at at a high 6 to 9 gee loading, (3) designed for two Crew and flies with three Crew The Capsules and Space Planes are the First step for the Return to the moon to stay ! The Capsules and Space Planes is Vision 2 of 5 for the Vision for Space Exploration (VSE Presidential speech January 2004) ! tjl

    Unlike government run programs spacex will use all available information to very quickly take action to either correct any design flaws or design sensors detect errors that may have occurred. Where as government run programs always saw this as a way to spend more taxpayers money.

    Success always generates complacency. The better we are in any arena, the less attention we pay to the details that brought us to the plateau of success. That's when we assume nothing can go wrong any more. Then something - Great or Minor - does go wrong. A human at SpaceX overlooked something obvious and BOOM. It was destiny - as sucess ALWAYS generates complacency.

    There was Tea-Teb on board, necessary to relight the engine in a vacuum, which was needed to elevate to a circular orbit for deployment.
    It was LOX leaking. The 2nd stage reach primary orbit , SECO, in coast until ready to raise orbit. That's when the ka-ka hit the fan, the lox leak (my best quess) continued and accumulated around the engine bell since there was no forward thrust to bleed it off. Upon relight to elevate the orbit the Tea-teb and lox ignited including the accumulation which took out the engine. As you said, the sat's were deployed at 138km in an attempt to execute a hail mary.
    The Falcon 9 1st stage is still performing fantastically. However the second stage is a fresh build for each flight so the potential for issues is much higher as the 1st stage is evaluated and prepped each time for flight.

    To state the obvious, O2 is the oxidizer, TEA-TEB is pyrophoric fuel, spiced up hydrocarbons that need no excuse to spontaneously burn when introduced to O2. There is some small amount of TEA-TEB remaining as long as re-ignition is needed, for orbit circularization and de-orbit burn. So there should always be some available on a Merlin second stage until the deorbit burn ignites. However the quantities are presumably pretty small.

      They cut to the stage one landing and never cut back to stage one. I’m guessing it was gone. Someone else said it was fine for another hour or two but then why did they never cut back to stage one if it was fine?

    An hour ago they were trying to save some of the sats by using their ion thrusters. Did it all fail that fast? Or is your report of death premature?

      Yes, it is premature, but the outlook is bleak. It does not appear that the ion thrusters will be enough to overcome the atmospheric drag of the low perigee passes with what I'm guessing are initial eccentric orbits of about a 135 km x 300 km. Even if they did succeed in raising their orbits, the operational life would be significantly shortened by the reduced fuel remaining.
      From SpaceX: "The team worked overnight to make contact with the satellites in order to send early burn commands, but the satellites were left in an enormously high-drag environment only 135 km above the Earth (each pass through perigee removed 5+ km of altitude from the orbit’s apogee, or the highest point in the satellite orbit). At this level of drag, our maximum available thrust is unlikely to be enough to successfully raise the satellites. As such, the satellites will re-enter Earth’s atmosphere and fully demise. They do not pose a threat to other satellites in orbit or to public safety."

    There was NO RUD while Starlinks Satilights were attached stated by Elon Musk.     6 responded well enough early this AM to begin a (Muskisum ) we put the Ion eng in its mode of Warp 9, slow for Star Trek.

    I think you are using the wrong term. The oxidizer is oxygen / O² The tea tab is the reducing agent.

      Indeed the oxidiser is LOX or liquid oxygen. It combines with the kerosine in a chemical reaction: combustion! Kerosine is likely to freeze at the temperatures to which it is exposed in the shadow in outer space. LOX will probably stay liquid. TEA-TAB is an igniter. It will immediately ignite when it comes in contact with LOX. Of these three substances, it has the highest melting point and thus is the most likely to freeze under these conditions.

    I think that once they released the starlink satelites, they attempted a relight of the second stage engine, and it exploded on relight. or they either used the FDS system.

    Need some failures to learn from. And we have not had alot lately.
    Are the fairing recoverable? Good chunk of money there.
    This will generate some great data on the ion drives and maybe enable calculations to determine minimum altitude to release from.

    ..I like to watch all of the launches from Vandenberg from my front yard in O.C. This launch wasn't typical of what I normally see. The Rockets usually put on quite the show. But not last night/evening. There really wasn't a separation from booster to second stage visible. The trail was odd too. I wasn't surprised to hear that it was a failed launch. Although, I must say that SpaceX launches usually go off on time without problems. I also didn't see the return either. But sometimes they return to Texas. Maybe next time........

    I think it hurts the reputation of SpaceX only because of the inherent bias of our media.
    I think it hurts SpaceX because of the contempt many people have for Elon Musk .
    I don’t think it should smear the image of SpaceX because of the outstanding performance of their aerospace technology.

      No matter what, it’s unacceptable. Especially when it comes to human lives and negligence. I don’t know the statistics on the risks for using SpaceX or others but it will often come down to acceptable risk VS zero risk and who is willing in risking it at a great price or tremendous cost. Demonizing accidents by blaming SpaceX can be a way of making things worse and contribute to a culture of hiding problems instead of fixing them. I would bet the numbers are better for SpaceX than anybody else. These are learning opportunities for everyone.

    The FAA and NASA will be concerned if any of the hardware is also used for Manned flight! THE FAA is always want a corrective action report. The upper stages are all brand new, so this could be a component quality issue, a revision that lead to a leak or either Oxygen or RP1, either of which will lead to a RUD, due to an uncontrolled fuel / oxidizer mix..

    It's weird it was the second stage, which would have been new, and not the booster. As you said, there was probably a bad seal or gasket. If it was, the question now becomes how many are made in a batch. If ten are made per batch I would assume the other nine would be compromised. Concerning the satellites being released prematurely, I would assume SpaceX knew something was wrong and was trying to save the satellites. Seeing how the second stage was able to 'get away' from the satellites before it exploded I would assume it would be the FTS which caused the explosion. I think SpaceX was thinking they got the satellites that far so they might as well try and get them to orbit, after all what's the worst that could happen, they fail and burn up in the atmosphere instead of blowing up with the second stage?

    I’m wondering if sabotage is a possibility. My wife brought up some interesting points regarding the several incidents of Boeing, SpaceX, and I think there was another recent failure. Too many coincidences close together. Otherwise, I’ve been wondering how long we could keep going without something happening anyway. We’ve gotten used to launches seeming routine when space is nothing like routine, it’s very dangerous and we can’t become complacent about it.

    In your description, you mentioned that shortly after the fairings were jettisoned, they noticed ice beginning to form on/around the MVAC engine. Everyone has jumped to the conclusion that something went wrong with the engine. But by the timeline, I'm wondering if something on one or more of the payload satellites began to leak, and it was an issue with one of the Starlink birds. Could be that one of them caused the MVAC engine to collect the ice and/or other leaking material, thus initiating the loss of the second stage? I did hear in the commentary, well before the end of video, that the second stage FTS system had been "safed". So I'm not sure that was initiated, unless SpaceX could turn it back on for use (it could be that just the auto-portion was safed). I just think that when all the issues began to appear at or just after fairing jettison is a suspicious coincidence . . .

    Too much ignorance in this video. RP-1 freezes at -60C, Oxygen freezes at -218C. Oxygen is an oxidizer, not a fuel. Please google before putting out ignorance.

      Thanks rat. This is why I didn’t think the kerosene would freeze under those conditions on the sun side of earth, and at the altitude indicated. It had to be liquid oxygen.

      As liquid oxygen escapes its inclosed container into a vacuum it will expand and it will get very cold, kerosene will probably just freeze solid if it gets cold enough and I don’t think the fuel kerosene will look like a foam as it escapes because of its surface tension, maybe not an expert
