Cihangar had a severe spinal deformity, and ailments related to it, his entire life. That is why he stayed in the palace to be near the best doctors. It most likely caused damage to his internal organs, such as his kidneys and heart. His condition worsened when he rode horseback, and he rode to campaign with his father, and afterwards fell ill.
Mustafa and cihangir is the most pure relation in this entire series.
