  • Chronic SI joint pain can be debilitating. Many doctors and physiotherapists do not look to the feet as a cause of unrelenting SI joint pain, back pain, hip pain, and sciatica pain.

When you are trying to determine why your back pain or SI joint pain is not going away there are many elements to consider. A often overlooked cause of stubborn SI joint pain that seems to fail other treatments can be found in the feet.

Not everyone has problems with their feet that are putting stress on their hips and sacroiliac joints. However, in this video we will teach you how to do some simple tests at home (remember: always confirm any diagnosis with your doctor) to help determine if your feet, and specifically pronation at the arches of your feet, are stressing your SI joints.
When the arches of the feet drop during walking or running that is called pronation. If a person's arches drop too much that is called overpronation or "Pronation Distortion Syndrome." This pronation can lead to abnormal wear patterns on your shoes. But it can also lead to aberrant biomechanics that can cause foot and ankle pain, medial knee pain, lateral hip pain, SI joint pain, and pain into the back.
There are some simple solutions that you can use to remedy overpronation if it is determined that it is causing your pain. We go over how putting in simple shoe inserts to help with pronation can bring SI joint pain relief. We discuss different types of orthotics, from over-the-counter to custom shoe orthotics. Those discussed in the video were (affiliate links below):
    * Aline Orthotics: amzn.to/3yXn8H3
    * SuperFeet Orthotics: amzn.to/3Pghrte
    * FootLevelers Custom Orthotics: www.footlevelers.com/
When the cause of your SI joint pain is determined and addressed, that is when your body can get rid of inflammation caused by joint and muscle stress. Looking to the feet as a cause of sacroiliac joint pain and learning how to fix SI joint pain through shoe inserts (when appropriate) is a game changer. Indeed, there are so many different areas of pain (back, hip, knee, foot, ankle, and SI joint pain) that can be coming from the feet that it is critical to check if your feet could be causing your pain.

Hello, I just wanted to say that you are the most intelligent chiropractor that I have ever seen. All my life chiropractors would not touch me.
I was born with a SI problem and should have been put in a body cast, the doctor told my mother that I would probably outgrow the problem, never did. It has gotten worse with life. When I was a grade schooler I always wore down my right shoe, my father would yell at me. At age 18 I was falling all the time and hospitalizes for a month at a time, terrible testing.. Now at 68 years old the doctors want to do surgery, both legs and feet have neuropathy and continue to fall. I will not have surgery now. I think that if You had been around when I was younger I may not have had all the problems that I have now. I am hoping that someone else can find you and get the help I could have had. Thank you.

Medicine was so different back then. Culture too was differnt. Being yelled at for wearing out your shoes...too bad no one exlained to your parents the car tire analogy. That ciuld have helped but instead our culture back then blamed the child for anything it seems. With that said, I beleive the Bible when it says the sins are visitrd upon the children 4 generations. Even before I knew that it the Bible I knew children pick up the problems of the parents. It takes true love to have a healthy life. I'm sorry for your sufferings. I hope you have peace these days.

If your feet are the problem, but they are causing pain in your knees, hips, and upper back, which doctor do we see to help alleviate the problem? Because the knee, hip, and the back doctors will not address the feet and the feet doctor will not address those other areas. They only look at what you claim to be hurting.

YOU’RE SO RIGHT! I had serious life-long sacroiliac pain, especially when I went to bed every night. I was also born with my left lower leg turned in at 25-30°. I was flat footed. At the age 52 I had surgery to correct the rotational deformity at Hospital for Special Surgery in Manhattan, Robert Rozbruch, MD. Both “shin bones” had to be surgically broken, and a metal rod from the knee to the ankle was installed. It cured my horrible sacral pain. It pulled my arch into place, and it corrected the knocked knee. Very painful surgery but worth it!

This is the best diagnosis of my SI issue! I've had problems for 6+ years. I've been to 4 different chiropractors, 3 orthopedics and a pediatrist. Everyone said it's a knee issue since the majority of the pain ran into the knee... Resulting in knee surgery making matters worse. Just recently now broke my femur since (just as you explained) my pronation caused my body to compensate to the lateral side of the foot. Well, that went too far and snapped. Yes, you are absolutely right! It's the feet! I'm subscribed to your channel. Looking forward to other videos, exercises, etc. ❤️😇

Thank you for this very informative video. I developed SI joint issues this year after trying to do straddles/side splits and performing deep hip stretches daily. I perfected my regular splits and became more flexible than ever. The more pain I felt, the more I stretched, and unwittingly, exacerbated my problem. I did learn a long time ago that an imbalance in one part of the body can create imbalances throughout since the body tries to correct itself; so it makes sense that I have scapula problems on my left side and left knee issues and now my hip problem is on my right side. In my search to fix this, I’ve learned a lot more about anatomy thanks to you and others. Your exercises have helped me a lot and I’m working on creating hip stability to eliminate piriformis syndrome.
And you might know this already since you had a left ankle injury, but I noticed your left shoulder slopes slightly lower than your right shoulder. I consciously try to keep my posture straight and need a visual cue sometimes because what feels “straight” to me sometimes isn’t. I practice lying straight and it feels crooked, so I’m trying to recognize this new feeling of being aligned.

You are a very astute listener and observer. Quite impressive! My right shoulder is elevated due to a severe AC separation injury I had in my late teens. I have hypermobility myself (with or without Ehlers danlos, not had a gentic test) so always have been very limber.
Stretching for hypermobility is a double edge sword, we injure easily but it feels sooo good to stretch, but can exacerbate joint problems.
It can cause widespread pain, fatigue and general anxiety disorder without a known trigger.
Orthopedics do have hypermobility knowledge but nothing like they should know as if we do too much damage thats who's going to operate on it, if not them, then the neurologist.

I used to do tis work as a chiropractic assistant. I gradually specialized in antalgic posture. Often orthotics were the best option to support the posture to avoid the pain [antalgic] while the cause of the asymmetric motion was corrected and repaired. A common source of the asymmetry was rotational subluxation at T12/L1 interfering with the psoas minor placing the center of gravity over the weight bearing foot. It is the first thing I would look for with a low left shoulder as you manifested in the video.

I came across your videos after looking into Si pain. I have recently started going to a new chiropractor that does decompression therapy. I Ike your explanation on every subject. It’s very detailed and that’s why it’s so helpful.
I’m a tap dancer and had a nasty accident this February. I slipped on an icy surface, but didn’t fall bc I’m trained how not to , but it was so sudden that my body didn’t respond as it would in a dance setting I suppose. My spine went zig zag and I felt pain in two spots. In a couple of weeks I started to feel pain while dancing and it became more severe. I did an MRI and it came out with 3 bulged disks in lumbar part of my spine. Two of them were from 6 years ago injury, I believe. So, I stopped dancing for a month and started a PT, acupuncture and every time I felt some progress I’d re injured myself by making a tiny move in a twisting or bending over direction. Long story short- I stopped going to PT and acupuncture and started studying all I can by myself. I’m very grateful I found your videos.

My feet are super small with high arches but still wear evenly on both outside backs of the shoe. Always had great feet, not much heel use as too short, and a lot of dancing but recently sciatic and hip twist occuring so it is helpful to be aware of foot' eveness' and enhance for flex and strength at arch etc.

Thank you for this video. I’m a subscriber now. I have been in pain for nearly a year. It started at my inner knee and escalated to a severe attack in what I learned was my SI joint, in which I ended up in the hospital. I do have tightness and pain in the areas you said to check. I’ve been to multiple doctors, chiropractors, physiotherapists, acupuncturists, massage therapists and no one has been able to help. It’s been frustrating and depressing. For this entire time, I’ve been saying, “I feel like this hip problem is related to my knee.” Everyone says, “It probably is,” but not one person has said that we should look at my feet, and no one has been able to offer a solution. I got referred to a pain specialist who was supposed to give me a steroid shot in my hip, but I’ve been waiting for three months for them to all me back, and I see now that will be a short term solution anyway. But now I’m going to call the podiatrist as soon as they open today. Why can’t MD’s look at the body holistically to determine the root cause of a problem? They only look at the site of the pain, do an x-ray, CT, MRI, and say, “Nope. It not that. Bye!” Every part of my body, except my feet have been scanned or checked for this issue…Your video gives me a bit of hope that I might find some relief. I have been wearing OTC orthotics for years and do wear good quality shoes, but maybe I need a different type/ customization of orthotic.

I had this SI joint paint since 5 months on and off and I found out that by myself … docs all
Say it’s inflammation in the lower back and tension im the muscles … that’s the exact pain I had in my legs and still they said it’s from the back… today when I was wearing a little high Schoe then I felt that my feet are not straight they goes left and right …the pain is getting more especially since I stand at my new work

Y'all... I have been on a journey. For the last 11 months I've been in pain. Muscle tension like I didn't know was possible. My back and hip were so bad I couldn't sit, drive, stand for too long. As a last ditch effort before seeking surgery, I went to an osteopath. He has me doing postural exercises. While doing foot circles last night, my right foot popped and I gained a new range of motion. Suddenly, all of the tension in my leg, hip, and back was gone. 🤯 I've had two ankle sprains so that ankle is very weak. I can't believe my ankle caused ALL of my debilitating pain for the past year.

Another nugget of knowledge in my quest to resolve a 40-year old SI pain issue. I do have a small amount of uneven heel wear, but no pain in the legs/hips, nor hip rotation. HOWEVER...for almost exactly the same length of time, I have had problems with the muscles just inside/along the scapula. I had always assumed it was because I lifted heavy when I was young, and as I have a long torso I figured it was weak lower traps or rhomboids getting pulled. But I'm beginning to suspect it's not that simple. Fascinating stuff.

I invite you to consider start drinking distilled water.
All the minerals people consume from water causes the kidneys to be overworked (as the body cannot effectively absorb inorganic minerals, from water).
That being stated, so much of society’s arthritic, sciatic, even Alzheimer’s/dementia, and more, is from a build up of these minerals, in the body.

Hallelujah! Nice to see a DC discussing the importance of the feet on the rest of the body. As a therapeutic massage therapist I find compensations resulting from restricted ROM of the feet and ankles as a major cause of symptoms elsewhere in the body. Before turning to orthodeics I suggest the person lengthen shortened muscles first, then strengthen the weak areas, which usually means isolateral strengthening. The challenge is determining where the imbalances originate. Do you have recommendations on resources that might help with functional assessment?

Pinched nerve in hip caused complete paralysis in my leg, especially the foot.
Once I started to get feeling back, ended up with CRPS in foot
The only reason why my foot doesn't currently have a swollen neuroma? Choice of shoes.
Found things to help Mortons Neuroma, like separation of the toes or toe bars on base.
Birkenstocks work for me. Still cannot walk more than a few feet in fully enclosed shoes regardless of how well made they are, CRPS foot needs more foot room.
My foot doesn't roll in shoes with deep heel cups, my legs are less tired, lower back hurts less, and I can walk/ stand longer than ever before.
SJ injection keeps area lubed and reduced swelling. Need it once a year.
If only my neck was as simple.
Took 5 years to go from paralyzed to walking unaided most days, and I thank my shoes!

That’s quite a journey. Birkenstock sandals saved me . I call them my prosthetics and wear them round the house as a barefoot option.

@@polyannamoonbeam They are my lifesaver too.
Can't wear any other shoes now or crippling neuropathy returns before I can get out of the house.
They are shoes a podiatrist can prescribe, which gets tax taken off.

Can you swim at all..? I go to a pool ( warm not cold) and swim on my back arms by my side. It helps. You tuck your chin down slightly and it is supposed to help straighten out the minor issues so the bigger ones don’t hurt so much. There are 2or 3 other people doing the same so it must work at least some way ?

Thank you so much! This video answered my questions about all the pain in my body. The wear on the outside of the shoe was my biggest puzzle. Check into correct toes to help with pronation and collapsing arches. I tried arch supports but finding more improvement with correct toes approach and the shoes with wider toe boxes.

Correct Toes are amazing products and all the work that is done at Northwest Foot and Ankle (www.nwfootankle.com). I love their products, have had good success using them and yes having a shoe with width (especially across the transverse metatarsal arch) is so important. Thanks for your comment!

Dr., very informative video. I suffer from a Leg Length discrepancy. My right leg is 1 inch shorter than my left. I'm 70 YO. In 1966, playing HS Football, I fractured the right femur, I was 15 years old. After the 6 weeks of traction in the hospital, and 3 months in a full body cast, when the doctor cut off the cast, he was bewlidered at how much my right leg was shorter than the left. My parents and really shame on the Ortho Doctor at that time, never presctibed that I shopuld wear an adjusted shoe for the rest of my life. That Trauma and body affliction has wreaked HAVOC. I now suffer with SI joint pain and inflammation, Lumbar and Cervical stenosis at various levels, Facet Joint arthritis, DDD (too bad I didn't drink water ... so important for Spinal Disc Health), arthritis in both Hips, knee pain and foot pain , almost exactly as you describe here. Flat footed too since I can remember. I differ with you in one major regard: I beleive from my experience, that the SI Joint Dysfunction is the major contributor to the calf and foot pains, instead of your info here of the feet affecting the SI joints... Nonetheless, this is extremely informative. My Rheumatologist after examining me for RA (negative for that), and then AS (negative), indicated that the SI Joints are the Motherlode of Spinal health... Everything is affected by those 2 joints. The LLD for which I really did mnothing about fo decades (finally making real shoe adjustments only 12 years ago), has distorted my spine with 25 degree Dextroscoliosis too. In my case, the maladies have traversed all the way up to the cervical spine with disc herniations, DDD, Military Neck causing a series of afflictions: Chronic Sinusitis, TMJ issues, Temporal pain and tension, IBS, and on and off chest pain , rib cage stiffness,.. I have started to wear Orthodics prescribed by a Podiatrist only a year ago. No real help yet. My currehnt Chiro says those Hard Orhodics are not good, she wants to order new ones pursuant to her analysis. But your info is far more extensive with very finite adjustments and inserts , especially for the one leg. I will subsribe to your Channel and watch all your other Videos. THX for doing this. Knowledge is POWER

Wow! You have been through a lot. Yes, an anatomical leg length difference like you acquired from your injury when you were 15 is really significant and has ramifications throughout the body as you detailed. I am glad that you haven't given up, that you keep searching for knowledge and that you are working with medical professionals. Thank you for your comment and feedback on my video. I hope that you will continue to find answers and keep feeling better and better. Take care!

I’m also interested in how the Dr feels about the reverse situation you’re describing.
I had a traumatic fall/fracture to the coccyx with ongoing SI joint pain that is progressing to the hip, knee and now foot.
Any clarification on this would be most appreciated.

Very interesting video about how the feet affect the rest of the body, but I have high arches and would like to know how that affects my SI joint? Do you have a video that covers that?

Hello! Wrote you about 10 days ago when I saw the video on exercises for the SI joint. You suggested the Serola belt which I ordered and wearing fir the first time today. I am still working in getting it the right tightness. When I sit down it is too tight so I have to find the happy medium.
This video really spoke to me. My left ankle hurts and sore most of the time. My massage therapist says 8 am walking wrong but I don’t know how to change it. How would I look for a podiatrist that would use the scanner to decide what orthodontist I need. I wear orthotics but not sure they are correct for me. I live in Georgetown, TX which is about an hour away from Austin. Since I am 74, I am picky how far I go into Austin but would willing to go a few times. Thanks fir your videos and help. Wish I could come to you.

Omg. Thank you so much. I'm 52 and have had chronic back pain since I was 22. Im now on s8 opiates for my s4 s5 vertabrae and s1 joint. I've always known I had flat feet but didn't relate this pain to my feet. I scored 100% on the sore points you explained here. I'm also wondering if having collapsed arches is why my knee nearly dislocates, from 10yrs of age and to this day? I'm ğoing to the link to try the supports and I might be able to get off my pain meds. Thank you for this video. New sub👍

You gave me lots of insight into my problem even though it wasn't directly mentioned. I got a hip replacement 2 years ago and that leg is now about an inch longer. I'm getting funny pains all over like you said up and down my back. My doctor is sending me to a sports injury specialist. Hope we can get to the bottom of it quickly. Thanks for the insight, maybe I won't sound as dumb as I look when I go see him now. lol

@@PositiveMotionChiro Actually, I didn't want to sound like I was gushing but when I went to give your video a thumbs up I noticed by the black thumb I had already been here. I got that home-hit feeling you talked about by putting something stretchy around my hips. In fact, it was SO home-hit I KNEW just like you said, I KNEW!!!
I couldn't get the SI belt ordered from Amazon fast enough and when I saw the mechanics of it, it made perfect sense. It tells the SI joint "you can come this far, BUT NO FURTHER! Then the elastic straps act like your butt muscles to hold the SI joint even tighter for the therapy training.
This was all some time ago but when I went to my therapist he said that was not my problem and he got me veered off in another direction. So all this time has been wasted, kinda.
When I saw your video again I went and got that SI belt out again and when I put it on, WOW!!! That feeling of relief ONLY THAT belt can provide!!! Now I wear it all day every day for about three days and I'm now on my feet hours longer per day because of it.
I'm watching all your videos on the SI joint like the "I know, it looks funny video" which I must have seen and forgotten before because when I'm at the kitchen sink it hurts the worse but when I push my hips forwards it gives instant relief as if I sat down!!!
This injury happened some 30 years ago when I fell off the back of a sofa right on my butt, I hope the tendons and ligaments haven't set into a permanent deformation! The fall was hard enough that I chipped the end of my tailbone! It wiggled back and forth for years making sitting very painful but it finally when away. I just figured autophagy or something ate it up.
I'll do my walking and easy stretching for SI joints (one knee pelvic tilt) every day and hopefully, it will eventually heal itself in time. But what the poopy, at least I know how to minimize the pain greatly, and all because of you! Thank you so much and I really don't mind gushing, you've changed my life!!! I can finally get out of this stupid chair!!!
