  • Dr. Ebraheim’s educational animated video explains about The sacroiliac joint dysfunction as a source of low back pain. Due to the proximity of the sacroiliac joint to the hip and the spine, pain of SI joint origin may be attributed to a hip or spine condition rather than the sacroiliac joint itself. SI joint pain is an unappreciated problem causing low back pain . Clinical and radiological evaluation is not usually reliable. The only reliable method for diagnosing SI joint pain is by injection.
    Symptoms of sacroiliac joint dysfunction include:
    •Lower back, buttock, back of thigh and knee pain.
    •Occasional groin pain
    •Difficulty and discomfort while sitting.
    •Patient frequently changes position to become comfortable.
    Tests used to determine the presence of SI joint pain:
    •Finger test
    •Faber test
    •Straight leg test
    Differential diagnosis of SI joint pain
    1-Trochanteric bursitis
    2-Piriformis syndrome
    3-Myofascial pain
    4-Lumbosacral disc herniation and bulge
    5-Lumbosacral facet syndrome
    6-Lumbar radiculopathy
    7-Cluneal nerve entrapment.
    Cause of SI joint pain
    1-Leg length discrepancy
    2-Mechanical dysfunction
    3-SI joint infection
    4-Ankylosing spondylitis
    5-Crystal arthropathy
    6-Pyogenic arthropathy
    7-Post spinal fusion
    8-Stress fractures of the sacrum.
    Treatment of SI joint pain
    •Anti-inflammatory medication
    •Physical activity, manipulation, massage, yoga, exercise and acupuncture.
    •SI joint injections; steroids, platelet rich plasma, prolotherapy.
    •Radiofrequency ablation.
    •Stabilization or fusion of the SI joint

    Thanks for your video. My sister's been having SI joint area pain for some years. She hikes a lot but sitting & driving for long time gets painful. Early this week, I got out of bed & felt SI joint area pain. It's the most pain in that area I've ever felt. It's like a bull trampled on lower back, butt, & hip area. I'm still in pain. A work client looked at me & instantly lifted her shirt to reveal lower back surgical incision from 2 weeks ago. I know so many young people who have low back pain.

    After many years of struggling with SI Joint dyfunction and several other joint andd muscle problems. I am completly cured by going on a gluten free diet. I wish someone had told me to try it years ago. My pain was completly gone within a week, and my muscles are starting to heal and I can now do the exercises I need to strengthen my entire body. I urge anyone with si joint dysfuntcion to look into gluten and other food allergies as your cause of pain. It has saved me from what I thought was going to be a lifetime of disability!

    For years I have been trying to explain to doctors the back pain I have experienced. They all assumed I was after drugs. I have found some success with ab exercises, keeping a strong core. I happened on this by accident and it took about a year of pilates, yoga, etc to finally get in a position where my back doesn't "go out".

    been suffering from this for 4 year my just put me on painkillers after many tests and mri xrays he still no wiser gone show him this .going out my mine this is the only video that is on the number for me that shows ever thing on a day to day for me ty

    After crashing hard at a water ski tournament I went 15 years with intermittent acute pain in the lower back. My doctor said chiropracters were quacks, but ended up seeing one. During the first adjustment it took awhile for him to wrestle me around enough to loosen me up, but when it finally popped it sounded it like a firecracker going off in my lower back. Instant relief. After several more adjustements, I learned how to self adjust it, and have been pain free since.

    Dear Dr. Ebraheim, Thank you so much for this video. I "popped" the SI joint in my back a week ago and wow, have I been in for a rude awakening. Your video was so concise and so user friendly, even I can now understand what my doctor didn't take the time to explain. I know it is a waiting game, but I pray this pain goes away in a reasonable amount of time - I am a "go-getter" type person and this limited mobility is already like a death sentence to me. :(

      Thanks so much for sharing. It just happened to me a week ago and I still have pain and difficulty in holding some positions like sitting. My doctor already diagnosed it as an SI joint related problem and asked me to do physio and strengthening exercises with painkillers for the first couple days. I was wondering what was the treatment you followed and how is it's impact in the long term? Thanks.

    I would say yes. I'm definitely pain free. When I wake up in the morning and if I lay on my right side (where the pain was), if I flex and extend my hips a bit I can feel some crackling and popping, but it's very unclear whether or not that is the SI joint or just some other ligaments in that general region. I should also add that I am in the gym almost every day and spend at least half of my workout on core exercises using TRX, Swiss balls, med balls, ViPR and take Pilates class.

    injections are not the only means to diagnoses. Their are a myriad or low back/si testing as well as METs that can confirm/ improve/ or relieve that pain.

    Just got over 11 days of pain. First 3 days I couldn't move. Steam room and going into a swimming pool and walking helps along with a heat pad and ice intervals

    This is great information. Thanks for posting. I have one question.... I have had lower back pain episodes that usually go away with lying flat and and naproxen and some ice. Today is different.... I am in the same amount of pain.... sitting at my desk is out of the question.... and my left hip appears to be higher than my right hip...my left hip is sticking out. Is this because I have one leg shorter and it is now exacerbated or have I managed to injure or pull a muscle so badly it is causing my hip to poke out? I have made an appointment for the Dr.... but since it is 3 days from now can you offer any exercises to relieve this? Sorry... that might be more than one question.

    along with the symptoms that you mention in your video , is it possible an SI joint problem could cause an IT band related symptoms aswell ? Can the SI joint problems agitate the TFL muscle and consequently pull on the IT band ?

    Do you recommend anyone in the Orlando area for treatment? My ortho did several x-rays and MRIs, found nothing, and told me that I'm imagining my pain because there is nothing wrong. He was hung up on it being sciatica though.

    I have had this condition for years and have been diagnosed by a sports physician but no doctor, physiotherapist, or chiropractor has been able to help me. I am in constant pain from my right hip & piraformsis to my right shoulder. Over the years my back has developed scylosis in my lumbar spine. Where can I get help. I live in Canada.

    Why is the SI joint the problem? Maybe its a symptom? What about the affect of scar tissue in sacrotuberous ligament pulling on the SI or tight hamstrings or scar tissue on piriformis? What about lifestyle changes and more exercise to strengthen core? What about the role of stress and emotion on pain? What about massage to release tension?....
    Critical comments are endless. This seems a very simplistic view of the body

    I have osteoporosis with a T score of -2.6 in the spine/pelvis, i have been receiving periodic chiropractic treatment for 22 years which helps keep me cycling , my reason to live ! recently i have noticed a deterioration to my level of fitness do to acute pain in lower back. If i am forced to stop cycling i will be taking a one way ticket to Switzerland where i can be put out of my misery in a dignified manner, hopefully not for awhile yet but without a cure for osteoporosis this seems inevitabl

    Unfortunately, massage was not mentioned as a viable alternative, in conjunction with Physiotherapy and Chiropractic manipulation. I certainly would take full advantage of all manipulation therapies before I resorted to sticking some needle and injection in my back.
    I just wish that doctors would get on board and see that massage, yes massage, too can have life changing results.

    1. Take dolo-neurobion (Complex B+Diclofenac).
    2. Make these exercises: Top 3 Exercises for SI and Pelvic Instability
    3. DIET and swimming EVERYDAY to get your weight in the correct level
    4. Don´t sit corved, put the back straight. Use the back of your seat to support your chest.
    5. Insist with exercise, if you don´t get thinner the weight will collapse your SI joint
    6. Massage that back muscle with Voltaren

    your dance career definitely has something to do with the pain (if you have a leg length discrepancy, that makes it worse). I was a long-distance speed walker for thirty years and did it when my husband died ..helped depression, wait etc. but I carried it to extremes and did eight to ten miles a day and nothing but ice and snow kept me home. To my horror, I was diagnosed with degenerative discs and s1 joint syndrome and I was told point blank it was from overuse. who knew?

    I had lumbar spinal fusion surgery 9 months ago I have some pain in my hip joints and my back started popping at the slightest movement recently I called my surgeon who said it was normal but I really don't think that it's normal for my spine to pop with an electric shock pain if I breath in deeply what can I do? the doctors here aren't being very helpful I'm starting to regret having the surgery that I thought was going to improve my life style

    I had X-Ray last week in which it came THORACOLUMBAR SPINE : There is a mild thoracic scoliosis convex towards the right & a slight lumbar scoliosis convex to the left.
    There are bilateral defects in the pars interarticularis at L5 with a grade one spondylolisthesis of L5 on S1. The disc spaces are well maintained. The facet joints and sacroiliac joints are unremarkable. No fracture.
    Any anyone explain me how harmful is this to my body & what more could i face due to this ?

    For months now I can't even walk without pain in my lower right back into buttock. Chiropractor was pretty useless. See orthopedic Dr in couple weeks. I'm 32 people asking why I'm limping, it's awful, can't even play with my daughter.

    My X-rays are normal, MRI reports is also normal but still i m having this lower back pain. I have consulted with 3-4 doctors, they all say that the reports are normal and no need to worry. Please help...

      Vijay Khampa Ask if you can get into a pain management. I have had 3 back surgeries and am in constant pain, and at 32 now cannot work.If you tell them how much its affecting your life you might get some one who understand. I hope u find hope. Im doing another procedure soon:(

      They did that to me too. When i finally found a doctor who cares he said i have arthritic changes in my SI joint, foraminal stenosis and a few squished disks. I'm still in pain. It keeps getting worse. I'm not even 32 and today is one of the days i can't walk because the pain is so bad. I finally have a diagnose now but the problem is not being solved at all. I get some pain meds but not enough. So i can't function. I hate america with their stupid opioid bullshit that makes it impossible for people to get the proper amount and strength of medication. I suffer a lot every day because of this. I can't take care of my toddler sometimes so my mom has to stay home from work to help me. This is so wrong.

    Where is the mention of hypertonic or SPASTIC MUSCLES? Thomas Griner should be googled. His work read and understood. Neurosoma is the website where all his work can be read. Most doctors spend less than four hours studying muscle physiology. They know virtually nothing about muscle spasms, why they occur and how they can be treated. If they don't understand how something works they will not be able to fix it. This is the case with most MDs.
