    My doc. put me through every test under the sun and kept coming up with different reasons why my pain was so bad I couldn't stand for more than ten min. at a time. After ten years of guessing I finally started looking myself on the internet and within three day's deduced it was my SI joint. I put the wrong type of brace on but it was enough to pull my hips together enough so that I could see with no doubt the problem was the SI joint all along. I got a Serola Sacroiliac Belt and the pain is 90% gone! I know I still need to get it adjusted but the pain and witch hunt are OVER!!! It was videos like this one that led me to the light. Thank you nabil ebraheim!!!

    I suffer at times from referred sacroiliac pain in my buttock and groin. I was whipped from side to side in an auto accident that appears to have stretched the ligaments in the area and destabilized the S.I. joints. I have had some relief from chiropracty, yoga exercises for the back, and strengthening exercises such as sit-ups, weight training, etc. Thanks for the great explanatory video.

    I've spent 7 years in bed - had to travel in a bed in the back of the car because I could not sit up during a long drives to the doctors, and my husband had to frequently sleep on the couch because my lower back pain was so bad. I wore lidocaine patches every day RIGHT OVER the SI joints!!! And the doctors knew it!!! But said nothing... TONS OF DOCTORS later, at some of the best pain clinics in the world... and I found this video. Told my doctors, they injected, and my world changed instantly. BLESS YOU FOR POSTING THIS VIDEO!!!!!!

      Hi GetSwingIN, did you have your SI joint or back checked out by any Physiotherapist, Chiropractor, or Osteopathic doctor? Or were all the best doctors you saw medical doctors? Cause medical doctors specialty is diagnosis of various diseases not joints of the body at all! If its a very good medical doctor they can take an educated guess to what the cause of any joint pain is but can’t 100% diagnose as its simply not there specialty. Each health care practitioner has there own specialty and medical doctors specialty is diagnosis of diseases NOT joints!

    My understanding is (it wasn't stated in the video) that the SI joint "unlocks" when you sit. I sit on the couch with my back in the corner, my knees angled left and my torso squared forward or even turned a bit right to use the laptop or watch tv. Repeated, extended spinal torsion while sitting and the joint doesn't realign when I stand up again. Misalignment causes pain that can last for weeks. Attending to my postural habits has helped me greatly.

    @nabilebraheim continued...
    If you inject the joint and the patient gets better, then the patient most likely has an SI joint problem. That does not mean the patient is clear of other conditions that involve the hip and spine. Association of spine, hip and SI joint pain are very common. It is very common for MRI to show a disc bulge or hernation which may be normal as the patient gets older and confusion becomes worse.

  • @nabilebraheim
    @nabilebraheim  14 ปีที่แล้ว +1

    @ocubejan Yes, sacroiliac joint dysfunction can be a painful condition. It is best to be evaluated by a doctor if you suspect you have sacroiliac joint dysfunction.

  • @nabilebraheim
    @nabilebraheim  14 ปีที่แล้ว +1

    @nabilebraheim I'd like to respond to this question.
    SI joint pain will overlap with other causes of back pain. Typically in SI joint pain, the patient will have vague, non-specific pain but more to the side. The patient will point with the finger to the site of pain. Faber test or Patrick test is usually positive. The patient will have tenderness over the SI joint. Injection is the best way to diagnose SI joint dysfunction.

    This is by far the best video explain this issue. I've tried for years to find a good video to show my parents and others close to me who want to know what it is the cause of my pain (unfortunately I also have a disc bulge pinching a nerve and facet dyscopathia). Anyways, this clearly shows where one part of my pain comes from. The finger pointing example is really good as that's exactly one of two areas I point too when asked where my pain starrts.
    Thank you for this great public service.

    The number 1 thing that completely fixed my SI joint pain was doing the "inner and out leg" machines in the gym. You know the one where you are sitting with your legs are spread and you either are fighting resistance to bring them together, or fighting resistance to push them apart. Do 4x10 and up the weight as you can handle it each time you go back to the gym

    • @pthorne
      @pthorne 2 ปีที่แล้ว

      I've recently tweaked my back doing heavy squats and deadlifts, I was assuming that a lack of hip extension would be the problem. They have the machines you describe at my gym, how long has it taken you to feel better?

    • @koolman280
      @koolman280 2 ปีที่แล้ว +1

      @@pthorne So this was actually a long term issue for me. In maybe 2018 I was doing a set of deadlifts, felt something hurt, but was too egotistical and decided to finish my set. Excruciating pain for the next 2 weeks, had difficulty sleeping, gave up deadlifts for good. My back recovered over 6 months but a couple times a year I would turn wrong and the pain would flair up.
      I decided to try and give deadlifts a try again this year, started working my way up in weight until one day I hurt my back in basically the same day I did before. I gave it a full 10 days off of working out and looked up different solutions on YT and eventually narrowed down the problem to my SI joint. From then on I started working back DL and squat at low weight, and at the end of each session would do those inner and outer thigh machines. Its been about 2 months since starting those and I am back to above the weight I hit earlier this year with no signs of pain. No random shoots of pain down my back for twisting the wrong way either

    I have dealt with SI issues for years. I have found one thing that keeps it healthy, walking at least 7500, preferably 10,000 or more step per day. I have gone years without pain and then get sick or drive too much and have 3-4 days where I only get 2000 steps a day and my problems come back full force and I need to work it out over a coarse of weeks. If you can get it back in place I highly recommend you try the walking. It probably is just keeping muscles loose and could be replaced with the right kind on stretching, but the walking is so good for you for so many other reasons it is my preferred way to keep it in line.

    I had two spinal fusion surgeries. The second was because the first failed and the hardware became lose. I was just starting my health career when this happened so I was fresh to the idea of back surgery. Looking back now which was 7 years ago my pain management doctor and my surgeon NEVER even mentioned that my SI joints could be bad. Despite me telling them that a lot of my pain was not in the center of my spine but rather down my leg, in my butt, and off to the side of the lower back. Of course I did not know anything nor heard about SI joint surgery. I just knew about the joint from anatomy. Seven years later, 30+ injections, years of PT, 2 spine surgeries, "I" final spoke up and said Im going to get my SI looked at. In my area only 4 surgeons do SI surgery. And I live in a hospitals city of big time hospitals. So I went to one doctor and he suggesting doing an SI injection to see if that was my pain. I did it three weeks ago. They did a MAC so I was asleep. Don't remember anything. It was wonderful. The next day my pain went down by 50%. No joke. I knew that it was my SI joint. Today I saw the surgeon and he said I could wait it out with another injection that only puts a band aid on it. I said nope....I want surgery. In 7 years with several surgeries on my body, I said I WANT this surgery. I am scheduled in three weeks to have my left fused followed by my right when I am healed. My point behind all of this. If your having low back pain make sure you get your pain management to consider SI joints and your surgeon to consider it. Do not let them talk you into fusing your lumbar without considering this and having testing done like me. Looking back I really did not need a lumbar fusion. I have been through hell every since. ALWAYS get a second opinion if you have decent insurance. Medicine is a practice. Every doc has a different opinion. What works for one person may not work for you. Everybody is different. And push your doctor to review your MRI or CT scan. With all do respect to radiologist, they don't see inside the body. They are trained to look at imaging, period. I work with docs in the OR. I can tell you many times when a doc said, "that was not on the MRI." Radiologist miss things all the time. Not because they are not looking. They just don't apply what they know to what the body looks like cut open. The radiologist completely missed my unfused spinal fusion and lose hardware. As soon as my surgeon saw the MRI he knew it did not fuse and the screws were lose. Get your doctor to look at the MRI, not read the dictation from the radiologist. Some docs go right off the radiologist report and don't look at the MRI.

    Why are Orthopedists and other MDs not assessing the ankles and feet as a possible contributing (or major root cause) of SI dysfunction? The feet play a major role in the chain. Knees, hips, and spine depend on the proper functioning of the feet/arches- during standing, walking, and running.

    @nabilebraheim A positive straight straight leg raising is a very strong indication that the nerve is irritated. A contralateral straight leg raising is also very specific. Most of the pain of the low back gets better with time. About 85=95 percent improve with conservative treatment within 6 weeks. If the patient doesn't get better, dig deeper.

    Thank you for the very informative and helpful video. For more than a year I have in vain been seeing different experts who could not figure out what was wrong with me. I had all of the symtoms mentioned in the video, back pain, my piriformes muscle was tight all the time, and pain in the back of my right leg. A few months ago an osteopat found out that my hip was to high in the right side and he put it back at its right place.

    Wow! it's like all those boxes got ticked!!!
    I even thought I have one leg longer than the other.
    It took six doctors to finally get a CT scan to show SI joint degeneration. Now I'm red flagged for, "doctor shopping"!
    Thanks for the video. I was even getting suicidal thoughts because the pain killers weren't doing anything; now I can sit in a chair after taking a few pills. You guys putting these videos up are doing a great community service!!!

    You left out 3 major causes for SI joint instability: 1. An externally rotated femur head usually on the left. 2. A trigger point lateral to L3 on the side of SI joint pain. 3. The average car seat is lower in the back than in the front. This causes the lumbar musculature to go into spasm due to the lack of support.

      The car seat example hits the nail on the head for me. Also most sofa & lounge chair furniture have the same issue - even my 'so called' zero gravity chair & inflatable river tube floats in the pool puts pressure on the wrong angle & in minutes leads to pain in the butt. When you mention rotated - my original injury from 15 yrs ago was a rotation while shoveling snow- probably L5 S1. I did chiro and it did go away but would return when aggravated - but my more recent flare up that has now lasted almost 2 months in my 64th year - foolishly overdid lifting - but then didn't allow it to rest enough before taking on other jobs that made the pain worse - likely a combination of both L5 S1 (as reported on my recent MRI) - causing sciatica pain - and then a sacroiliac flare up. I believe my sciatiaca exercises may have aggravated the sacroiliac as the pain would relocate depending on certain movements. The only saving grace is I can find a comfortable position on my side to sleep - with no weight on my spine - but I pay for it in the morning when waking up.

    @sajjadaxe1 The sacroiliac joint can be very tricky. It's a common source for low back pain, however, it is often misdiagnosed.
