Thank you for addressing the topic of OA progression. Many of us don't just care much about the treatment of pain, we want a treatment that will SLOW the loss of articular cartilage. We already have dozens of treatments to reduce pain, and we have almost NOTHING to modify the natural history of the osteoarthritis! This should be an embarrassment for the field, especially because we have so many excellent treatments to modify the disease trajectory of other important diseases (e.g. Rheumatoid arthritis, CVD)
Yea that’s why OA is so frustrating for patients and doctors. What works for disease modification in other arthritis does not work in osteoarthritis. This is also why PRP is so promising

(Near-complete oblique radial tear of the medial meniscus at the posterior horn, 21 mm from its posterior root insertion.) It cannot be repaired per my knee surgeon. I have had one PRP injection from a 60cc blood draw. I likely will do a series after hearing your presentation. My physician injected my left knee from the lateral side. He did not use ultrasound guided injection but a landmark. I certainly will be asking questions on my next visit.
Both my husband and myself got the PRP injections by Dr. Peng. My husband got 2 shots one for his left knee, one for his right Gluteal Tendinopathy. Both worked really well. So I just got my PRP treatment for my right shoulder tear due to martial art practice. I am eagerly waiting for the PRP to work.

I was told the same thing. Proactively I called my insurance provider. They cover this procedure and have since 2010. My provider is Aetna Medicare Advantage. Out of pocket cost at my doctor’s office is $503 for everything. I’ve had one injection in my left knee for a totally blown medial meniscus. Results have been fantastic. I’m taking another one this Wednesday. Each of us is different but I recommend the procedure to ward off old Arthur just around the corner.

Any thoughts on this? - Draw 4x10mL of blood. Spin at 250g for 10 minutes. Result is about 4x3.5mL of plasma. Aspirate to 2x7.0mL tubes. - Spin the plasma at 1500g for 10 minutes. Collect the top 5.0mL from each tube and discard (this is the PPP). What remains is 2x2.0mL PRP (a single 4.0mL dose). This is 10% of the draw and should contain the highest concentration possible.
I actually have a torn meniscus and arthritis in my left knee. I used to wake up in pain during the night, but after having a couple of cortisone injections, the pain seems to have subsided. However, now, my knee is swollen and cracks when getting up out of bed in the morning. The sports medicine doctor I spoke with suggested gel injections and drainage of the fluid, since this would be covered by insurance. If this doesn't work, than PRP may be an option for me. He recommended against surgery, since it could make the arthritis worse.

Please help! Hi Dr. Peng, I'm scheduled to have PRP in my knees in 5 days. It's my 2nd in the right knee and first for my left. Could you give me your protocol or instructions on what supplements/foods to avoid and what to continue with? My Physiatrist doesn't give any suggestions. And there is a lot of conflicting info online! Some say no caffeine before or after 😱 and some say avoid Omega 3 for 2 weeks before. Obviously, I will avoid NSAIDs. Also what about diet? I've done PRP for thinning hair 4 times (it works!) and the dermatologist just says to "eat clean". I have no food allergies and I try to eat organic when possible. Extended fasting (beyond about 3 days) instigates autophagy, which essentially turbo charges our body's production of HGH (the major element of our body's ability to self repair tissue). It sounds to me like PRP is badically, potentially, concebtrating our already existing HGH levels, and THAT may be what is happening to produce these positice results at the cellular level. Maybe a study of extended fast patients vs. non-fasting? It's all about what's in that platelet concentration on a molecular level. I'm goingvforward w/ a local provider who uses all of your recommendations, and I intend to fast for at least 5 days prior. What triggered this thinking is the simple fact that every time my knee flares up and I've fasted for 3-5 days, it has ALWAYS resolved. This has never been the case w/ oral meds and rest alone. Anecdotal, to be sure, but I've done it a half dozen times now w/ the same outcome. I'm very disappointed in PRPs for my shoulder arthritis. I switched back to HA and saw bigger benefits. Of course the PRP I received in the first place involved 20cc blood draw and not 60cc as mentioned in the video.
I got 1 PRP injection in my shoulder that was confirmed via MRI to have a partial thickness supraspinatus tear. My main problem was the inability to sleep due to pain, as well as inability to workout. I’m probably 1.5 years from injection and had great results, still no problems with sleeping and overall pain much much better. I had it done at a very good place, Stanford grad who is very big in the PRP research community, so that probably also helped results. Let me know if you have other questions

Hey! I had prp in my right shoulder for a partial tear and had great success. One question I always had was the pain of the injection - the injection itself was very painful, but the pain post injection was insane haha, like I couldn’t weightlift for several months and really do anything involving my right shoulder for several months, different than what I’d seen from others. Once the pain went away, everything was good, but I just thought my pain lasted waaaaay longer. What could be a reason for this?

I always just thought it was crazy because I would see people getting injections and being able to return to things like weightlifting after only a few weeks.
Nobody actually knows why or the extent which PRP can ‘regrow’ cartilage. And really larger studies need to confirm the findings. Low platelets does not cause nor is it associated with osteoarthritis (as far as I’m aware)

This great info. PRP injection are out of pocket. 3 Injections sounds like a great idea but we talking about $3600 which is unfortunately prohibitive.
My mom got her PRP in Belgrade, €20 per injection.
She got 6 injections in 6 months, it was amazing improvement.

Yes PRP has been studied for hip osteoarthritis and it can help reduce pain and symptoms. However the effects seem to be less than for knees.

Works great for shoulder OA but needs high dose. Works VERY well for partial or high grade partial rotator cuff tears and rotator cuff tendinopathy

Yea PRP can cause a fairly significant post inflammatory reaction and response. Doesn’t happen to everyone but if it does, it’s fairly painful

Why do platelets heal the cartilage? Does a person with low amount of platelets on CBC test tend to have osteoarthritis?

In theory, but the small studies looking at them show they don’t outperform other stem cells nor do they outperform corticosteroid injections. No point doing umbilical cells costing $$$ when PRP is just as effective and costs much less.

I had a ultrasound guided 4.5cc prp injection for chondral fissures and got literally no benefit. Sports medicine doctor says no benefit in doing repeat injections. Not sure how to find another provider for this.
4.5cc is meaningless without knowing total platelet count. Repeat injections of high to ultra high dose is key for chondral defects and fissures.
