No it doesn't have to be neutral. I did mine while it was turned off and in park. You could put it in neutral to get better access to that pin but you really don't.
You mean the fabric cover portion? You just pull it down. Hold it from the top so you don't tear the fabric. It's pretty tight so it takes a bit of force.

You can get one of those automatic to manual shift knob adapters. You’ll have a variety of shift knobs and you’d have to press down to shift
I'm not aware of any no. Maybe there's a way to modify the shift mechanism?
