Holy crap, I was nearly in tears watching this! For those who have never been in Fire or EMS, you have no way to knowing how freaking realistic this is. I deal with this nurse every day at work...
I’m a nurse and my husband is a paramedic. I think this is funny, especially when I watch it with him. Yes, he has told me some similar stories. I wish there were more of these to watch. The nursing home nurse one is especially on the nose, he said.
I work as a medic in Chicago, we got called for SOB and tachyardia. I asked the nurse if the pt is usually on O2, she said yes PRN. I said, don't you think it would have been a good idea to have done that when this started. The O2 tank was in the same room as the pt but they didn't even hook it up. She was SOB since 8am, they called us at noon. Blows my mind!
Nurse: "I'm not sure, this is not normally my shift and I just got off vacation and I don't know this patients history". Wow! Right on the head, I hear this ALL the time.
Amazing. So true. What really gets me ... we (all of us watching this) ALL deal with this across the country ... this same, exact, word for word situation. How does that happen?!
As a Legal Nurse Consultant who once did patient care, my last SNF job was house supervisor in Las Vegas, NV, and RN's sat on as while LPNs did most of work. All they did was push pills. The CNA's of today do more hands on patient care than most nurses do. In a nursing home it's easier to break open the crash cart or to give critical meds without an order. Many nurses do it. Why? Because if you waited around for the house physician to call back, your patient will be dead by then.
LMFAO, the only inaccuracy with this video is that I have yet to find a nursing home nurse who actually speaks this much english. Awesome movie!!! bravo
i worked in a nursing home as an aid for 4 yrs before going into ems. i always got mad at the nurses when i knew more of what to do than they did, and usually ended up giving reports to the emts, now that i am a paramedic things have not changed, i don't know how many times i've ran this exact call
As a Firefighter/EMT I can honestly say that this is a typical turnover at many of the nursing homes that we go to on the medic. I was waiting for the "I don't usually work on this floor" answer from the nurse.
A nurse has said all of this to me and more. The last cardiac arrest I had at a nursing home had the staff running around like mad,trying to ventilate the patient with a BVM WITH NO MASK ATTACHED. At least that time they were trying to do something. I've been to them before where they had done absolutley nothing for a full arrest patient. This video is funny because its everything you ever wanted to say to an incompetent nursing home employee. Not all of them are like this,but way too many are.

An EMT-Basic works under the protocol of the Hospital and can do things like 12-lead acquisition and phlebotomy (some even include peripheral IV initiation).
An RN is normally more heavily trained, however most RNs that work in Nursing homes do so because a Hospital will not hire them. You'll see SOME good RNs that work in Nursing homes, but it's rare. An Paramedic on the other hand is also heavily trained health care professional and is has a larger scope of practice on standing orders then that of an RN, things such an Intubation, interpretation of cardiac rhythms and treatment modality's based on that interpretation are standing orders for a medic
went to the nursing home in our town. called by the pt. daughter from a CELL PHONE because the CNA's and the head nurse didn't think it was a true emergency. Guy had difficulty breathing since 4 am, we were called at 2300! respirations went from 16 a minute to 40 a minute in one hour and they didn't do jack. get on scene guy had CHF, fluid in 90% of his lungs. couldn't push Lasix because his BP was 80/34, and still they felt it wasn't a "true emergency"

In todays world Firefighters run EMS, not every Department, but most Fire Dept. run with EMS, and have thier own medics and EMT's, the world of Firefighting has expanded, because we work hand and hand together,its not uncommonn, for Departments to take this mission.
I'm currently doing my 24-hour rideouts for my Paramedic program...and (unfortunately) Nursing Homes are 75% of the call volume at this clinical site....and I dont know how it is across the USA, but I have yet to find an actual RN in ANY of the facilities we've gone to...EVER!!

OMG this is our companies calls to nursing homes in our area!!! They are so incompetent. Everyone on our rig would love to say these things. I know one of the guys on my cew must have made this!
bromes32, you're right, there are nurses that don't need Dr's orders for everything. But many evasive procedures do need to be run by/ ordered by a Dr. An RN might have a higher level of care inside the doors of the hospital/clinic/ nursing home they work @ but out in the streets a Paramedic has the higher level of care. How many RN's know how to: read a 12 lead, intubate, do a surgical airway, cardio vert, D-fib, start a Lido, NTG, or Dopamine drip w/o Dr's orders. Yup get your facts straight.

Well you got me there, Volunteer academy is only about 320 hours to be certified, but the hours and training are only limited to a persons will to go to school, they offer IFSAC ff 1 and 2 and all the others, but your right about the academy, all though I have over 4,000 state hours, most of which I receieved through Volunteer work, and yes I work as a paid FF to pay the bills, sorry to assume, its just some people paint Volly's in a bad light.
Well, I work on a paid Dept, and am a voulnteer, The state requierment is the same paid or voulenteer, but the point is they are Voulenteer, they dont want to be paid they just enjoy what they do, as with any real firefighter, its not about the money, it about helping people.

This job is about helping people, yeah some people put stickers all over thier cars and brag and what not,but overall people just want to help, and are trained to help, otherwise no one would show up, its called pride in what we do, I can get a job right now in computers making alot more money, but loving what you do, theres no price, its not about money.and by the way I have a real job too.
Reminds me of how we feel at the psych er when someone like this poor pt comes in. They're altered , but they are an alzheimers pt. And now the nursing home won't take them back!
To CyanoticFuture: I wouldn't say an RN is "more trained" than a medic, but their training goes far more in-depth into pathophysiology, disease process, etc, etc. Plus, the training is VERY different. EMS does not operate the same way as a hospital...just as an ER does not operate like an ICU. My wife is an NP and ATLS Instructor (she teaches ER physicians how to NOT kill their trauma patients). This video, although funny, does NOT accurately represent "Nursing". A Nurses Aide, yes.

Master's in Emergency Health Services and 8 years of EMT experience here. This is completely true of the majority of 'nurses' at nursing homes. Note that I say 'nurses' at nursing homes, because there is a difference between them and real nurses. Real nurses tend to be competent and have degrees from industrialized countries. 'Nurses' may or may not be actual nurses, with degrees from countries where it's a miracle they have running water.
@zuku, I'd love for you come to our area to work. In 10 years, I've used on-line medical control ONCE - and that was for a med (Geodon) we're required to call for. We work trauma's, codes, STEMI's, and every other call you can think of without having someone "remind us of who is really in control of our rigs." Can you local EMS do trach's and crich's without consulting med control? We can. Believe it or not, a well managed EMS system doesn't need to rely on an ER to run their calls for them.

We are more diversely trained so we can do more than just put the wet stuff on the red stuff as some call it. It's kind of nice to have people like us around when the emt's get sent on sevral runs for bs calls like "my toe hurts, or I have the flu, come take me to the hospital so we can waste the taxpayers money instead of driving ourselves....

Sounds like this firefighter needs to work in a city where all he has to do is fight fires and not go on rescue calls, better yet a city that only has emergencies during the day.
This is a sad example of the kind of people we now have in the fire service. Please do not associate this kind of behavior or mentality from this sorry excuse for a firefighter with those of us that are professionals and take our job and responsibilities seriously. FYI: I have been an LPN for 13 yrs and of those 13 yrs I worked in the same LTC facility. When I need to call 911 to transport a pt in some kind of life threatening emergency situation. Yes, having the paperwork ready would be nice, and I usually do and we all know its all about the paperwork.... Whatever! I have my CNA waiting at the door for your gripping ass because I'm in the room with the pt trying to keep them stable with the equipment I have available. As for transferring.......
This is so insulting. I have NEVER handed off a patient like this nor have I ever handed off a patient like this. I have heard that EMS didnt have any respect for nurses but this is horrible. I look at EMS TECHS in a very new light
