  • In today's video, I'd like to talk to you about one of the most common causes of knee pain -- meniscus tears. I'm going to talk about what constitutes a meniscus tear, some little known facts about meniscus tears, how you know if a tear is causing your pain, and what are the treatment options for a symptomatic tear from different exercises to injection options to regenerative medicine to meniscus transplants and other surgeries.

    Thank you for this Honest report !
    I was told I needed a knee replacement,,,after a fall
    Bottom line I had a MRI sent it to a A Surgeon in Iran ,,
    Followed his directions ,,, did my exercises every day
    18 months later ,,,I was as good as New
    Dr . You are 100% correct
    My knee just needed time to heal ,,,,not Surgery

      Hii. Which exercises you did for mensicus tear? Can you just summerize in brief and recovery. I'm devastated and depressed after mensicus tear (I've grade 2) just not finding anything useful. Doctor says exercise will help but he only recommended one quad stretch

    I have a completely severed ACL and two torn meniscus. I'm a 68 year old Australian and was in Canada when I had a first day ski injury and felt a sudden acute pain which disappeared as quick as it happened. I got up and walked back up the hill and spent six weeks at the ski resort resting and then following youtube videos for rehab after the first 10 days. I hoped it wasn't an act. It wasn't good for skiing but wasn't unstable when I returned to Australia where I had an MRI and was sent to an orthopaedic specialist. I saw the doctor with the world record in ACL surgeries and he told me he treated symptoms not MRI results and since I had no instability I should continue with my rehab and I could ski but if it became unstable to come and see him. That was 8 years ago. I'm not 68 and my "bad" knee is less painful than my ininjured knee.

    Exercises done with PT expert absolutely improves...strengthening so what my knee was used to before I hope I continue...Ibuprofin etc. Kills my stomache so do not take. Lemon w olive oil, ice, exercise and sometimes heat.

    Yes, I heard all about the survey and studies and about older ppl having meniscus tears and NOT knowing it. But, no one has discussed if your are older and had a actual injury in which you twisted the knee and 10 min later could not walk & terrible pain. X-ray showed no fracture or arthritis, MRI shows complex medial meniscus tear, bone contusion & a Baker's Cyst. My DR says because of my age I may not get surgery, but, if younger I would have already received it. If I get mechanical symptoms I will have a arthroscopy, he says, and then states that I don't have arthritis. Why can't anyone discuss having an acute tear and be an older person?

      I tore mine about 2 years ago. If I don't walk much then it's ok. But I can't do a treadmill or elipical. I am at the point now. Where I am scheduling surgery

      @@daviddavis692 Hi David, I would consider it. 6 mos. after my knee injury I had a partial meniscectomy after all the other measures failed. I had loose bodies, also so it was a easy call for my surgeon The 1st Dr. I saw was a ortho, not, a surgeon. This practice makes you get a steroid shot first, did you get one? It didn't help me. My knee still hurts, but less than before. I'm still doing PT. The awful thing is I have injured my other knee! 5 wks. ago, I was in denial, but, will go to my surgeon on Tues. Good luck on your knee, a nightmare injury

      Absolutely! I'm 58 was fit no knee pain at all and smashed my knee during a slip n fall due to applesauce on a concrete floor. Doc waited 5 months to write an order for knee mri which had been terribly painful for months. In the original xray 2 days after injury it showed baker's cyst. I was told that is common etc. But I recently researched and those can also happen from an impact injury...I have several tears and also have had my foot give out...

      @@d5434 So glad you came back to me. I was finally sent to a surgeon and I did have a partial meniscectomy 6 mos. after injury A ortho did the work-up and a MRI and then sent me to the surgeon. Believe or not, in March I injured my OTHER knee The pain is worse, the surgeon is who I'm seeing about this .1st. visit he gave me a cortisone shot, 2 days later the worst pain in my life, I couldn't even walk . Next visit he says he has no idea what's wrong, all 2 x-rays are ok, no swelling ,etc. This time he gave me Methylpred a step down med. It worked a wk. now there nothing. For some reason he won't order a MRI, he says he think it's degenerative, which I have no arthritis, no space narrowing. I think I tore my meniscus again, UR right, I think I have a Baker's cyst again I also think a MCL tear, due to a new intense pain than I didn't have last knee. I am mad that in 10 mos. I injured BOTH knees. I've had it! So painful! I could scream!

    Thank you for this great video.. i loved it when you said.. the body is always trying to heal itself.. Ive been in pain for 2 months and finally seeing that i may be on the way to feeling better.. i m not going to rush into going back to my exercise routine - giving myself another 2 weeks. Getting my walking gait back to almost normal feels so freeing!

    Excellent video, Dr. Cooper. With integrity and science, you described the nuances of meniscal injuries and why arthroscopic resections are not usually required. I've torn both menisci & rehabbed them successfully on my own. While initially painful and debilitating, each large, multidirectional, medial tear did not cause any issues after 4 months of at-home care + exercises.

      @shellybean423 (update May 2024) Hi! For my right knee multi-directional tear (June 2023), I stopped doing all exercises that aggravated my knee and iced it a lot. I kept the weight off it as much as I could. Tailgate swings throughout the day, even when I'm standing, helped a lot. They bring oxygen & nutrients to the knee.
      In January, I tore my left meniscus badly, a worse injury than the right meniscus. For the first 2 weeks, I elevated & iced my knee 2-3x daily, which relieved the intense pain & swelling.
      Weeks 3-5: Foam-rolling my quadriceps helped with the constant muscle tightness. Weeks 4-6: glute strengthening exercises like banded wide-stance walks for glute medius, frog pumps, banded hip abductions and adductions, and hip thrusts.
      By week 6, I could perform heavy leg presses and found they relieved my knee pain.
      Week 7: I could finally kneel and fully flex the knee, so I didn't have to do foam-rolling anymore, as I could perform the more effective runners' standing quad stretch.
      Week 8: resumed barbell front and back squatting with knee sleeves, and I have been continuing with the leg presses once weekly.
      Four months post-injury: I can consistently squat heavy loads (around 80% of what I was squatting pre-injury) with no knee pain or swelling. I always wear quality compression knee sleeves now. Consistent, challenging gym workouts are going well. Walking for two miles or longer still causes pain and swelling. This may be from osteoarthritis.

    Thank you Dr for taking the time to put this video together. I found the information to be very helpful and i can confirm what you have stated in the video, that i am oging through right now with my own doctor with a meniscal tear in the lateral and median. i have done an steriod injection and it helped a bit, but when i stand and put pressure on the knee it acks, i am dong PT for two weeks so far, i hope it heals on it is own. many thanks.

    Great video! Expertly knowledgeable about this topic and very forthcoming about the challenges of accurately diagnosing and treating knee issues. Love your caution against rushing to surgery if taking the proper strengthening and muscle balancing approach could allow natural healing of some injuries over time.     Very informative! I agree with you regarding unnecessary surgery. I worked in an operating room and watched many arthroscopic procedures. Seemed like instant gratification. Didnt consider long term effects of the procedure itself. I have just been diagnosed (MRI) with a complex medial meniscus tear, and have an appt with a specialist soon. I now have more insight of what to discuss, and feel confident i can avoid surgery at my age ; 76 even though im in excellent health.

    This report on meniscus tears added tremendous understanding and knowledge to diagnosing my issue with a bucket handle medial meniscus tear. The explanation of the location of the pain when having a meniscus tear and the simple way it was discussed provided me with a more accurate diagnosis. Besides having the meniscus tear there is a good chance that I have patella femoral pain syndrome and really need to start exercising the various muscles listed in this video. I’ve watched many videos on meniscus tears and this is the very Best one by far. Thank You.

    I just discovered your channel and have found your information about meniscus tears very informative. Thank you!! My knee shifted sideways (not a twist), with a loud crack while walking three years ago, and I had immediate pain that lasted about two weeks. Fluid accumulated over a couple of months, and I had an aspiration. An MRI showed a medial tear. I have no pain, but more fluid with another aspiration three months ago. Still no pain and never locked. It can feel unstable at times. I never did PT because I never had pain. I'm thinking that I should do PT to strengthen the muscles. I'm very grateful to have no pain. Besides some tightness, it's like nothing happened, and I can only hope that it doesn't progress. Thanks for the info.

    My daughter has an extensive estimated 4mm lateral meniscus tear they pushed surgery right away but I wanted to wait as shes 15 (&loves basketball!)
    I did lots of research most say Surgery is only option for recovery of sports...
    Long story short 6mos in with 2mos PT she is showing much progress leaning towards being healed within another 4-9mos.
    She couldnt use the knee for approx 3mis then the progress started as soon as we started PT she now can run and move almost well enough to play basketball but were taking it slow .. gonna focus on other muscle strength to assist and take pressure off the knee.

    This is good information and easily digestible. I'm 33 and have medial and lateral tears on mri as well a fissure and delamination on my kneecap. My right knee is the one they looked at but I've bilateral soreness, the right is just worse. I've been now refered to surgeon but wondering what else I should consider before I see the surgeon.     I had a MRI, it shows that I have osteoarthritis, small menicus tear, cartilage thinning, baker cyst, i have swollen knee, stiffness, can't fully bend. I was suggested to have knee fluid aspiration, but knee fluid contains hyaluronic acid. Should i have knee fluid aspiration? You mention about the hyaluronic acid injection? But my knee has exces fluid, isn't it a result of inflammation? What is the alternative treatment ? Thanks

    Thank you, excellent info. I have had an mri and the consultant says I have a year of the lateral meniscus and some signs of arthritis in the knee. I am considering whether to have the tear removed via arthroscopic surgery now or whether to leave it say 3-4 months to see if the symptoms largely resolve. No locking or catching of knee but clicking and aching . Do you know roughly how long it can take for a torn meniscus to resolve without surgery? I want to avoid partial meniscectomy if I can

      @TheKayakCrew My large, multidirectional medial tear resolved completely in 14 weeks with ice & elevation initially, then strengthening exercises and targeted stretches.

      @@a_woman_who_loves_to_lift That's great, glad it resolved for you. I tore my meniscus last February and finally had an operation in late November. The surgeon trimmed out some of the tear on the inside of the meniscus which was flipped up and sutured the outer rim which he said was a bit unstable. Was on crutches for 4 weeks due to the repair element of the surgery, then 4-6 weeks of gradually getting the leg working again and rebuilding the muscle. I'm walking almost normally now and started some light swimming but not yet back cycling (other than on machines). The clicking in I was having in my knee has gone but it's still probably too early to tell if the knee will be better longer term. I wanted to avoid the operation and had postponed it once already but bit the bullet in the end. Time will tell if it was the right decision I guess

It will definitely be interesting and instructional to find out how that knee fares in 5, 10, and 20 years, and in the cold, when it rains, and when you return to sport and other activities. I have a feeling that you won't regret it. With the clicking and catching pedunculated meniscal tissues excised, you won't have those forces grinding away at your chondral articular cartilage, so less osteoarthritis down the road, don't you think?

    Love all your videos.
    I've been having pain on my right knee for a couple of weeks.
    Seems it feels better now because I was concentrating on my knee so much I noticed that I cannot straighten my knee when I am standing however when I lay down it seems I can't straighten my leg what could be the problem. Thank you so much for your help.

    very interesting, informative, and very similar to what my physical therapist said! she's great and we're working together. but I keep on looking for what to do or not to do during the day, as I live my regular life. how should I sit by the computer? why does it feel fine while i'm sitting but is SO painful every time I rise? is there something i'm doing wrong?
    can I go back to my regular 3 times a week exercise class and just watch my knee, or am i really supposed to rest?
    I live on the forth floor with no elevator - is it bad for me to go out?
    sorry about all the questions but you seem very knowledgeable and I need to ask someone. thank you!!! keep well!

      ​@rivyon1 hello, maybe I can help. I've torn both menisci & rehabbed successfully on my own to full knee function. Just as Dr. Cooper described in this video, my large, multidirectional, medial tears did not cause any issues after 4 months of at-home care + exercises.
      Your post was from a year ago. How does your knee feel today?

I had a physical therapist come to my home and together we did some exercise and some very painful massages that I didn't continue for long because it hurt so much. but today both my knees are squeaking - they hurt mainly when I walk up or down the stairs. i'd love exercise to fix that. Do you know what I can do?

    Study cited showed knee osteoarthritis may result from surgery or injections and this what I brought up w my specialist once I finally got to see him! He said that since I did not have arthritis he wouldn' t prob suggest surgery right away on only one injection. I then asked him for PT (11 months in) my regular doctor had said no but he said yes. I have been seeing a physical therapist who is also a kinesiologist and slowly improving with many types of exercising and movements and strengthening...At the end of the day I can still flare up, and have terrible pain....After 1 yr 1 months still not back to normal...

    Just been diagnosed with Medial and Lateral tears, requiring debridement. Not looking forward to it at all, as I'm allergic to all pain and anti-inflammatory medications. Wondering how I'm going to deal with the pain, and 8 weeks of being on crutches and an inflexible leg brace. Apparently my leg will be fixed at 15°, and each week Physio will be adjusting a few degrees until 45°'s is reached. Seems like a lengthy & painful process 😕

    My 13-year-old volleyball player daughter has a torn meniscus in the white zone. One doctor thinks that it’s kind of shredded and he’s going to have to remove about 20%. I am not on board with us. He said she could go back to playing in five or six weeks. Volleyball is super intense on the knees. She is not swollen anymore and only has pain occasionally. We are trying to rehab. Can the white zone be healed?also considering cold laser therapy or ozone injections. She’s currently doing the knees over toes program.

    I am here in Miami and in excruciating pain.. it is mostly the inside knee…
    I have had several bad falls, but after the bruising went down I felt ok….
    I am going to physical therapy and my doctor wants me to stay off my feet… I thought jumping in my pool and doing stretches, jumping jacks and laps would be ok, but he said no. I have a saltwater heated pool and I can raise the temperature so it becomes very warm.
    Can u please give me your opinion ? He has not recommended any stretches or exercises… also he gave me an injection without the ultrasound machine…
    Also, do u know of a doctor I could see here in Miami or South Florida for a second opinion? I would like to visit u , but I can barely walk… my cousin lives in New Jersey and I could stay with her if I could just get the pain to a bearable level…

    Hi There, I have had this pain and swelling for 9 months. First PA told me arthritis, DR said no arthritis, deep tissue damage. MRI said quadricep tendinopathy and some other thing but meniscus in tact. Changed Ortho Dr. he said after review of the MRI and based on where pain is it's more than likely Meniscus tear and wants me to do 4 more weeks of PT then perhaps arthroscopic. What are the pros and cons of the surgery. So over this pain and swelling!

    I had knee pain for a while that became unbearable almost a year ago. I did 3 rounds of PT and wore a brace. I was very active and modified my exercises. Last July (2023) I could barely walk so the ortho gave me a cortisone shot that worked immediately and then just as quickly went back to pain/swelling. MRI determined a meniscus tear. More PT. Another cortisone shot. Nothing. A round of gel shots in December (2023). Nothing. Continued PT exercises at home, and added a stationary bike. Another cortisone shot (did absolutely nothing). Another MRI revealed I needed a partial knee replacement, which I had two weeks ago. Other than the pain from surgery, I already feel better. I tried everything and followed my doctor's instructions. Sometimes, surgery is the best (only) option and sooner rather than later.

    I wish there would’ve been ultrasound back in the early 90s, blade injections did not do any benefit to my knees, surgeon said third times a charm which it was not ended up having surgery on both took years of rehab to get better now at 60 still able to do decent squats And still compete in Olympic lifting

    Have a meniscus tear and arthritis in my knee. Came on quickly about 6 months ago.....although I've played tennis for 30 yrs (except not now since my tear). I'm doing PT consistently but recovery has been very, very slow

    I had a sudden and excruciating pain in my calf muscle while jogging one day, and I bore with it for two years, thinking that it would settle down slowly. I've consulted many orthos and physios including chiropractors but the pain persisted. Then one gp suggested that it could be meniscus and advised mri. The mri revealed meniscus tear(grade 2) and went for arthroscopy. It's been four months and it still feels painful. I'm 74 and I'm still wondering whether it's torn meniscus or torn calf. I do walking and cycling

    I have a medial miniscus tear diagnosed with an MRI but I have fluid on the outside of my knee. Could that be related to the tear or could it be something else since it’s on the other side of my knee which doesn’t hurt but is uncomfortable.
    Do I need to have it drained or is there a better way?

      I have the same problem and my Family doctor advised me to do a minor surgery which I am hesitant to do !!!

      Massage the leg muscles above the knee. Or wherever you feel pain or tightened hard muscles. This is often in the muscles above.
      The pain of fluid build up is a nasty pain.
      You may find, as you get the muscles softened and the circulation working again and the fluid will go away.
      This happened to a friend of mine, and within a couple of weeks of massage her knee area healed.

      Look at other options . Pay attention to tightening glut exersises. On you tube is a site where you can do a tibial bone shift. ElPaso Manual Physio.

      I had a tear three years ago. Fluid accumulated after a couple of months and my knee became very tight and difficult to bend. I had a knee aspiration and had immediate relief. Three months ago my knee was tightening up again, so I had another aspiration. So far, so good. I should add that I have no pain.

    Well just great I had three meniscus tears that I was told I needed surgery for. 2 on left and one right. the last one on the right was a huge one.they took a lot out. Now I tore it again and it's caught and locked.now they tell me I need a knee replacement. No cartilage left and arthritis .to know this could have all been avoided. And not once did they ever offer an injection under guides imagery . I do not want any replacement but I'm dragging my leg for 2 months since this tear I was an avid hiker.now nothing.

      Had surgery on my meniscus 20 years ago now I have arthritis in my knee but no pain.
      Knee is going inward. What can I do to align it?

    But it is depends on what meniscus tear, right? You will need surgery for bucket handle tear?

    I am frustrated. My meniscus has a tear but actually from the mri it looks mostly gone. I have done pt and try to do the physical therapy. I love to walk and was before it was injured. Now I barely can walk. It has been 8 months.

    I was given 10 mg prednisone for my swollen knee caused by a suspected quadriceps tendonopathy - waiting for the MRI appointment- but I decided not to take prednisone because I’m diabetic and I have a history of low potassium that kept me at the hospital for 3 days (weak legs ) - I’m only elevating my leg and icing the knee to reduce the swelling - lucky for me the pain in the knee is now minimal (like a 1 to 2 out of 10 ) and walking with crutches 🩼
    I hate MRI appointments taking a few business days to arrange! I’m hoping for no tendon tear from the MRI results
