I know this is kind of late, but lookup Matt's utility. It fixed mine after months of not knowing how to fix it, and because there wasnt any battery left, I also needed a factory cable so that I could transfer data from my PC to the phone while also supplying power. Hope it helps.
Thank you for posting it. Great video, worked first time. After 3 months of frustration I installed HTC Sense on my M8 instead of clean android, which was on it. Custom ROM in the nick of time!!! Takes 25 min to back up files and boot new ROM but worth it!

Can you recommend which GApps version is good for people who don't really like much bloatware but still need some of the important ones (average bloatware maybe like google map etc.)?
Persie Wan It is possible to use CM without Gapps. You wont have the Google setup at the wizard. Although you can still install it after the setup wizard albeit you just need set it up manually. And just search for nano Gapps or micro Gapps if you only want like the Play Store and gmail for example.
The GAPPS is not completely necessary, and you can boot for the first time in your custom ROM without GAPPS. The only problem is that the Play Store and other Google Apps like Chrome will not be installed, but if you have the GAPPS zip file you can install it from TWRP recovery later. But you should just install it with your custom ROM. Unfortunately for copyright reasons, custom ROMs are not allowed to package GAPPS with them anymore, so this step is a must if you need those things.
+Igor Fernando If you are like me who rarely listens to people advices, just go on and start the system without it, it wont hurt your phone at all. After that you can decide whether you want the GApps or not ^^

I ma just posting this comment hoping it will help anyone having issues. I followed MAX up to installation of the ROM and my process failed. However I re-wiped the Dalvik Cache and the installed the gapps, then the kernal and the ROM last and then it installed perfectly, then I rebooted! Thanks MAX!!!! Trinidad loves you!

Ryan Jack It looks just like a basic TPU case for the Note 4. This is the one I got for my LG G3 (link is for the Note 4 version.) www.amazon.com/Cruzerlite-Bugdroid-Circuit-Compatible-versions/dp/B00N3L4K7C Pretty similar besides the Android design on it. But just search amazon for your phone and TPU case.

Thank you Max, I should have used this in the first place. The Clodcwork Mod Recovery screwed me over, it woudn't load the new roms right, and the backup didn't work either. At least TWRP loaded a new rom so I could use my phone again, although I can't get it back to factory easily like i had hoped for.
You should always also check the MD5 to ensure the file integrity is intact. Not simply that the files can be accessed, but that it is indeed the original file. Also, give it an antimalware/av scan.
Original file? I have not found a clear answer to this issue. You see, this is my thinking: suppose someone puts together a ROM and is defective by mistake. After ROM is packed, runs a utility to generate MD5 for the package. Then it posts the bad ROM for download along with its corresponding MD5. If we download, we can run a utility to generate an MD5 that would match the provided MD5, but it would still be a no good ROM... How to solve this dilemma? I don't know enough about the packaging procedures and packaging/compression tools to answer my question.

You can make a backup of your stock touchwiz (Google nandroid backup through twrp). If anything goes wrong then (bootloop etc) you should be back able to restore your phone via twrp back to the way it was.
For everybody who doesn't have a computer or usb make a back up file on dropbox and if your files get deleted just put the backup file that you made on dropbox back into your phone
Quick question which gaps should I use for the custom Sabermod CM12.. I know you said 5.0 standard when u made the video, since its been some months should I still use 5.0 standard or 5.1 instead? Just double checking? I'm new to all this. Thank you.
'Unable to mount storage' means you havent mounted the partition Its not a 'bug' and it wont 'work just fine'... In the main menu, you need to choose to mount the partitions you wish to wipe using the 'mount' button PRIOR to wiping.
