  • People with knee osteoarthritis are usually guided by healthcare professionals to focus on doing thigh exercises to help improve knee arthritis.
    If exercise is supposed to help knee arthritis, why is there more pain during and after exercise?
    Are these really the right muscles to focus on?
    Could focusing on the wrong muscles possibly be the reason exercising is making your knee hurt more?
    In this video, Dr. David explains to you 7 reasons why stronger thigh muscles make knee arthritis worse.

    I had problems with my knee, I was starting to limp during my walks. Tried all the usual quad strengthening exercises but it didn't help. Then I found these videos and after watching all of them (check out the ones on how to walk, and how to walk stairs with knee pain!) I enrolled in Dr Davids 28days knee wellness course. Man, such great value for my money! There are only lessons for one week, you repeat them for the other 3 weeks. But don't worry, as your knees, muscles and posture change during this time, you will experience and understand the exercises anew each round, and they show how to make the exercises harder for when you've mustered the inital versions! I've finished the 28 days, but will continue following the videos for thr forseeable future. Im not getting payed for this glowing review, I just wish everyone such relief from pain as I have experienced it!

    Absolutely spot on! I've had bone on bone for some time now, and have problems walking any distance. The Orthopaedic Surgeon says I'm not in enough pain to warrant an operation, so I'm coping with the situation. Around 18 months ago, I began seeing a Physio from the Gym. Her method of treatment was to increase the weight on leg presses, leg extensions and leg curls, her reasoning behind this was that by doing these exercises with heavier weight I would make my quads and hamstrings loosen up and more flexible in order to work better with my glutes. I suffered this for several months, and found things were getting worse. I could hardly walk after a leg workout. Completely gave up doing my legs for a couple of months, and things began to feel a wee bit better. Started concentrating on the glute exercises I picked up from this channel. - but included backward walking, which I know you don't recommend., but it helps me activate my glutes. Now a leg workout day is nothing but glutes,
    You are right. There seems to be the belief out there - even among health care professionals, that leg extension and similar exercises are good for crook knees. This couldn't be further from the truth.

      Hey @conningdale8805, we’re so happy to hear that you’re getting better!!! Sorry you had to go through making it worse with those quadriceps dominated exercises :-(
      That’s great that you found that walking backwards helps you use you’re glutes better! As long as you’re using your glutes more and avoiding using your quads while being safe - proceed!!! That’s the magic formula that will help you heal!!! We hope you get to feeling 100% very fast!!!

    Thank you so very much! I've been going to the gym my entire life and I have extremely strong legs. However my gluts are so weak in comparison. I just started going through menopause and my knees are in pain so much. I did all your glute exercises 2 days ago and can hardly walk because I'm so sore - but I love it!! This makes so much sense and I know after I have stronger glutes and calves my knees will be much better 😊. God bless you for helping us.

    You're a why true life saver! I have been in the gym 4 days a week since March and I love Quads! So between Hack Squats, Leg Extension and Leg Press I have over worked my quads. (Not to mention, sumo squats I don't feel in my glutes anymore because of the reciprocal inhibition lol or whatever u called it.... I may not have the correct terminology but I do have the understanding. I took notes and used my massage gun that just came today. And when I got up out the bed to go to the restroom I literally felt better!!!
    I will watch this again and again. & be mindful in the gym to lay off of my quads for now. (Because the knee pain just came out of no where and I couldn't understand after losing 70lbs why would I have knee pain now .... but I totally get it ) so again Thank you!

    Your explanation and information are gold Dr. David! My PT had me doing quad sets as PT for burst complex bakers cyst w/2-4 osteo arthritis, meniscus tears and other ligament issues. He hadn’t seen my MRI yet. My hip and knee were really sore afterwards. PT did also talk to me about strengthening my glutes and cycling. He seems very skilled, contentious and knowledgeable. He did have 3 knee surgeries so may be drawing from his recovery🤷‍♀️ I’ll give him this link and discuss with him. Thanks again!💜

    You are absolutely right about this. I have been suffering from knee pain with patella femoral compartment for 6 years.I am doing most exercise focus on the quad and hamstring which put a lot of pressure on my knee and patella . Going up stair daily at home cause my knee pain daily . Thanks to your you teaching , I focus to exercises on my glutes muscle, lean how to walk with my glutes . Just 2 weeks, l practice waling using glutes muscles and to go up stair . My pain is better , almost reduce 60percent. God send you like angel to help me. I can not thanks enough for your you tube. I am swimming 1 hour x3 per week with breast strokevand back stroke, but I am stopping bc of my inner thigh and patella increase pain for 1 months after my knee hurt and swollen. I was dx with pain OA and mild arthritis . Dr David, do you think I can be back to swim ? Love watching your you tube and hope I can pause my arthritis . I am 64 retired RN. Thanks for your advice, encouraging . I appreciate your honesty. I can see a miracle lights shining at the end of tunnel .

If you feel you are using your quads more than your glutes while swimming, then you will likely make your knee problem worse :-( Hope this helps!

    For my knee arthritis, my physiotherapist suggests leg extension, quad set, and other exercises to develop the quad and hamstrings.I will ask her to study your video 7 reasons why....I have been working a lot with two machines for the legs and probably that made my arthritis worse.
    Thanks to your video I finally discovered how to use my gluts for pedaling and walking. The problem is that my brain cannot keep the quad and hamstring at rest when the glutes are working.
    The doctor is in favor of PRP (plaquette-rich plasma) injections. Those are very expensive and it is not proven to work, What is your experience with PRP.

    Doc, thank you so much for this detailed explanation. I am a very analytical person and everything you say makes perfect sense. I’m trying to get my PT to understand and he keeps saying “In life you need to be able to flex and extend your knee.” And I keep thinking, well I CAN. But why do I need to put force on it? My question is, with respect to the function of the quad and the function of the glutes, in your opinion, do you foresee risk of any bio mechanical issues in relying on your glutes over your quads? I know it hasn’t been researched much but I’m asking for your opinion. Thank you again so much! Words can’t explain the joy I feel in having found a solution for my pain and mobility. I’m bone on bone in my patella due to multiple dislocations. But I see a path forward now thanks to you!

    I'm guessing the same thing applies to using resistance bands for knee flexion and extension - which are a key part of the help arthritis program I got from my physio. I did those conscientiously for the prescribed 6 weeks but no improvement in my ability to walk properly without pain. Recently, with the help of videos from this channel & a couple of others, I have realised that my glutes are in fact very weak and need work - and doing this is beginning to make a real difference.
    Thank you for this explanation, and the helpful exercises too!

    This video is filled with master wisdom It reminds me of Algebra at school I never knew that muscles can shorten during night time No wonder I feel if I cannot strech my legs with ease in the morning With effort I have to push my legs in a straight line to get up Should I get up at night and just walk to the kitchen or do some donkey kicks Will a warm water bag or heat on the muscles stop shortening of muscles My primary puzzle is? How can I avoid morning stiff leg muscles Should I do glute exercises before I get to bed or do some glute exercises after I went to the toilet at 2 o'clock in the morning I concede glute exercises is not a walk in the park

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  7 หลายเดือนก่อน

      Hey Anton! Great questions! First and foremost, glutes are most definitely a key to decreasing some of that stiffness. However, it doesn't even have to be specific exercises that you perform. Those exercises would be great, but it would be even better to include the glutes in something that you already do on a daily basis. That is why we want people to learn how to walk with their glutes. If you can begin to walk subconsciously with your glutes, then you will increase their activity throughout a normal day. This will inadvertently decrease the activity of some of these other really tight muscles. We have a great video that shows how to walk with your glutes if you would like to check it out. Hey Michael, eventually you'll be able to train your thighs! But at this moment, stick to improving your glute strength so you can safely do some quad strengthening in the future! We firmly believe most knee replacements can be avoided with the right treatment. Hello doctor .. I have a horizontal tear both in lateral and medial meniscus .. also have grade 3 chondromalacia patella - medial facet . Added to this I have knee osteoarthritis
    Can you share a video for the correct exercise to be done in this condition

    I wish I'd seen this and the other knee problem thigh, hamstring strengthening videos that you posted. I understand the premise behind what you are describing. I wonder what you do to slow sarcopenia, muscle wasting of the thigh. I have knee osteoarthritis with bone spurs but I don't want to lose the muscle mass of my thighs. Isn't it possible to work the glutes and thighs in some way that allows a person to spare muscle?

    I know you had some glute exercises I want to follow but what do you think about a stair stepper exercies for glutes or in general? I have two painful knees I'm working on but my weakest movement is stepping up on a curb or stairs.

    • @pjj.5649
      @pjj.5649 ปีที่แล้ว

      I too had knee pain when climbing stairs, stepping up on the curb and going from sit to standing.
      I watched Dr. David's video on glute exercises, and walking and the quick relief is amazing. Most of all, I want to do the exercises and the dread of doing the quad exercises (which my body was telling me to stop - which I irgnored) is gone.
      The glutes are the key. And you can do them anywhere, all the time. Knee pain has deminished by upwards of 80+% for me. I have no hesitation with stepping up on a curb and I am in NY which is a walking town where the blocks are short with plenty of curbs and stairs.
      Give it a try. All the best to you.💯👍

    I've done an extreme amount of bike riding over the years and now I'm wondering if I've over strengthened my quads and maybe making my knee pain worse? I hope that's not it because I love riding my bike. Maybe trying to concentrate on using my glutes more instead of my thighs if that's possible?

    thanks for the info. You are right, it is often said to strengthen the quadricips. I did this and as a result, the kneecap began to hurt even more. Question: why when I go down the stairs I feel discomfort in the kneecap, and when I go up there is no pain, for that when I go up I feel the buttocks well. Thank you

    I have post polio so my muscles don’t strengthen. After 2 years of daily biking my muscles are the same as they were 2 years ago. I have had prolotherapy and that seems to make the knee tighter. My knee have slipped off so are loose and sit on the outer edge of the grove. I have small weak side muscles and my quads do all the work for me to walk. Sorry my pain is behind the patella only. Any suggestions?

    I have complete no understanding but what can I do? Till last week my knee was ok and I started playing basketball again. Hey Babarw2006, Using a bike will be helpful if you can get your glutes to work during the exercise. If you feel you are using your quads more than your glutes while using the bike, then you will likely make your knee problem worse.😃 Dr. David has a video that discusses how to properly use a stationary bike to help heal knee arthritis. After twenty years of running, I am now 81, my knees are arthritic of course. They never hurt till last month when I torqued my knee and saw stars. So I’ve been going to Physical Therapy for a month now and this therapist I had was trying to strengthen my glutes and not giving me quads or hamstring exercises. The knee got 95% better to the point I was walking 3 miles once a week plus pool exercises but still going to therapy. However, the office changed my therapist, and this one yesterday put me in the quad and hamstring machine and then do squats plus sitting bike. And last night I had pain in my knees. so again I looked for your videos because you’ve been able to solve my muscle imbalance problems and listened to this video and today when I went to Physical Therapy I told him that I only wanted glute exercises because my knees hurt when I do these other ones.. This is something Physical Therapists. should know. It should be publicized. I am so grateful to all your videos and I will continue doing glute exercises and I’ve learned now to walk using my glutes like your other video! .I can’t thank you enough for all you’ve helped me.

    So what do athletes do who need very strong quads and hamstrings for peak performance?

    • @epmanualphysicaltherapy
      @epmanualphysicaltherapy  ปีที่แล้ว +1

      Hey Gothops, that is a good question! 😀 It's important that the muscles are balanced. Often times when someone is experiencing knee pain they may have quads or hamstrings that are more developed than other muscles in the area. This is what causes the imbalance which leads to the pain. It's common for some health care providers to tell someone that in order to fix the pain in the knee that they need to strengthen the quads or hamstrings when really the person should be strengthening the glutes to ease the pressure on the knee. 🧡

    • @gothops2632
      @gothops2632 ปีที่แล้ว

      @El Paso Manual Physical Therapy Ok, thanks. So, for athletes who need strong quads and glutes, it's important that they not only strengthen these muscles but also the glutes and probably hip adductors and abductors too, right?
      Btw, have you done a video on how to reduce the incidence of hamstring strains in athletes who are involved in sprinting? It would be good to get your opinion on this as I think a lot of other physical therapists have gotten it wrong. A lot of athletes strain their hamstrings over and over again. I suspect that perhaps they are focusing too much on getting the hamstrings strong, which, as you point out in this video, decreases resting muscle length.

    • @epmanualphysicaltherapy

      Hey @enza3770, skinny jeans can definitely can put a lot of pressure on your knee cap. If you feel that your knees are worse on days that you wear skinny jeans then, yes, absolutely you might want to give up skinny jeans. It might help to see how you feel on days that you don't wear them.
