I can't get the anomaly of Tsubaki (stage 3): I guess other than time-out her first non-spell, there is a 2nd trigger, as seen just after you beat the fairy, before her 2nd non-spell. What are the requirements?

Don't worry about that - the orb there doesn't appear on Easy/Normal, as you can't face Minoriko's Spellcard on E/N. Only Hard/Lunatic can do it.

The actual requirement is to timeout first mid nonspell, and not get hit between midboss appearances
