    For anyone concerned about this happening to them, this type of injury is EXTREMELY rare when anabolic steroids aren’t involved. Steroids do not equally strengthen muscles and tendons, so even though your muscles might be able to lift a certain amount of weight, your tendons can’t handle the tension.

    I had a full distal bicep tendon tear on my left arm about 3 years ago. The physical rehab wasn't as bad as the mental rehab. It took me about a year to get over the mental aspect of not thinking about it everyday. Compared to other people that have had the same injury as me, I consider myself lucky. Besides the occasional muscle spasm after my bicep workouts, the most frustrating thing is I probably have about 75-80% of my previous bicep strength. I have to scale back my bicep workouts to keep both arms symmetrical.

I knew right away what it was because Calum Von Moger had torn his bicep about a week earlier. The day before I tore it I had worked out my arms really hard at the gym and I think that played a role in my injury. I knew I hadn't picked up the TV the right way and instead of putting it down and grabbing it a better way, I tried to just "push" it up slightly with all the weight of the TV on my left arm. It was a dumb mistake and I paid for it.

    My personal and professional advice. Dont do preachers seated. Always do them standing so you can adjust your body and the angle with the dumbell as needed to keep things in a safe position. Also No need to do them heavy, 1or 2 sets close to failure with light or medium weight is plenty. Heavy curls can and should be done to strengthen tendons but not in a preacher. I much prefer regular alt dumb curls or hammer curls for heavy loading unless youre chasing strict curls for some reason.

    That tearing is tough to watch! I tore my biceps playing football way back in my college hay day. Got surgery the next morning, amazing physical therapy for the following 4 months, and was back at it. As a matter of fact, my bicep is bigger than it ever was, and just as strong. A quick turn-time on surgery, great therapy, and being young helped out tremendously. I know because I've since torn my triceps in the weight room then my quadriceps skateboarding, and neither ever healed to even 70% of what they once were. • @joshp5563
    @joshp5563 ปีที่แล้ว +10

    Doctor, I have seen quite a few distal biceps tendon ruptures including the infamous bilateral. nearly 9 out of 10 of those injuries were sustained performing the preacher curl. Based on clinical experience and with input from other Strength and Conditioning coaches there are a few reasons why this tends to happen.
    1. The preacher bench gives lifters a level of support (as you stated in this video) that makes them feel like they can lift heavier than they otherwise would. This causes many lifters to load their bars in a way that are not appropriate for linear progressive overload.
    2. As demonstrated by the lifter, when doing something like a ladder series to determine (potentially) 1 rep max lifters will often go for a standard (20kg/45#) olympic barbell instead of an easy curl bar because you can more readily add/subtract weight instead of having to switch out bars between lifts. Because the weight is further out on an Olympic bar relative to an easy curl bar this obviously extends the lever arm of the load adding additional stress to the structures responsible for moving the weight.
    3. The preacher bench encourages full extension of the elbow joint. Many lifters believe that without full extension of the elbow at the bottom of the curl movement you wouldn't be doing a "full" curl and are therefore reducing the effectiveness of the lift. This is understandable as full range of motion isn't well defined outside of professional lifting organizations such as IPF. With that in mind most default to Full Range of Motion means "I can't go any further". I see this most frequently with preacher curls, squats (of all types), lunges and bench press.
    Simple weight lifting instruction (and often ego checks) can prevent these injuries quite readily.

    It hurts insanely bad... I ruptured my left proximal bicep tendon in 2018. The sagging muscle would randomly spasm like a live-wire and thrash to the point I'd have exterior bruises appear on my skin that looked like I'd been hit in the bicep area with a baseball. It took the better part of a year before the random spasms subsided... The lasting impression my experience left on me is painful enough that I can barely even stomach watching someone else endure that (let alone THIS double biceps rupture!)... I'd seriously rather touch an electric fence.

    I have had this happen to both right and left distal biceps tendons (5 years apart). I had two different methods to reattach them. Double incision method on left arm and single incision on the right. The left tendon let go while doing single arm preacher curls with 30lbs. It happened on a Friday and I didn't opt for surgery right away because I was going to The Dominican Republic on the Sunday. I had the surgery when I returned (about 14 days after injury). The right arm went when I had my arm extended to lift a bench with a friend. As soon as I put load on it, it let go. I had been back to the gym for about 6 months at that time. I had surgery 4 days later this time. My advice is that the biggest difference maker in your recovery is how quickly you get surgery. My right arm is damn near perfect and my left biceps look different and I have nerve damage felt in my hand. Other than that, I feel very confident in my repairs. The Doctors assured me that they would be stronger than before. Both times I was off work for 4 months each. The first time I was using testosterone. The second time was damage done from using testosterone the first time. The first thing the doctor asked me when I went in was if I was on gear. When young, healthy people rupture biceps tendons it is almost always a result of steroid abuse. Anyway, I have been working out consistently now for years with no real issues from the biceps other than the annoying nerve stuff on the left and I guess a bit of loss of strength overall but you can still build muscle without having to go real heavy. My arms are looking pretty good if I do say so myself.

    Damn! I've gone through a distal bicep rupture myself... just one though. Although I work out regularly, I'm not a steroid user at all (which is why most of these injuries happen). Instead I ruptured mine during a mountain bike crash while grabbing a log - to keep myself from going over an embankment. Luckily I had it fixed within 36hrs. Recovery took 14-16 weeks...ish.
    But because I was dedicated with my rehab - I managed to return to full range of motion (many don't) and full strength (again, many don't). But I attribute that to quick reattachment... plus slow, painful static wrist stretching every day. I also continued my workout regime (modified of course) starting a week after surgery. Not fun, but at least not life altering.

    Had a partial tendon tare in my right bicep down near the attachment point in the forearm from a accident. Man they are the worst and my doc said as much you'd prefer a full tare over a partial because bloodflow to tendons and ligaments is limited. 9 month recovery and I'm still dealing with the muscles surrounding the tendon in protection mode. Lots of rehab and slow slow build back up. Took me a solid year to get that confidence back. I was lucky and it was only a small tare. A garage door shifted while I was lifting it during a storm and popped it.
    It was so annoyingly painful, just a constant ache etc. Again this is more related to partials. I always thought a partial was the lesser with pain and discomfort but no. Because it didn't break fully it's inflamed like mad for a long long time. I couldnt rehab it until the swelling went down.
    I finally got the ok from my rehab to go hard in the gym again from my physio and rehab but to be very careful. I avoid preacher curl because it's putting so much pressure on your joint. I only use light weight for preacher these days if I even do it.
    My rehab doc said it would have been better with a full tare and surgery because they anchor the tendon back down stronger then before. Where as a partial for a lot of the rehab has a weak point. I needed constant scans to make sure it had healed over before we stepped it up. Almost a year recovery just under.

      I think if you go faster on progressive overload the stretch and pump will help strengthen your tendons, the way he does it going slow like that really fatigues the muscle and puts his tendons at risk, especially at the bottom. Also the load is too heavy even though he has strong biceps it gives false confidence once the muscles fatigue

    • @KobeWanKenobi
      @KobeWanKenobi ปีที่แล้ว +13

      Pick a weight you can do 10-15 reps with and you should be safe. Don’t go for a one rep max on an isolation excercise like this one cause then you’re likely to get injured like the guy in the video.

    • @KobeWanKenobi
      @KobeWanKenobi ปีที่แล้ว +4

      In addition to my previous comment: if you want to strengthen your tendon you should pick light weights and really focus on the ecentric part of the movement. I’m talking about a 6 second ecentric each rep with light weight (just an example). This way you’re slowly strengthening your tendons.

    • @Chadius_Thundercock
      @Chadius_Thundercock ปีที่แล้ว +1

      The rep you can’t complete, don’t force it. Put the weight down
      When you have mechanical failure, stop the set

    Ugh. I ruptured my right distal tendon in a ski accident, crashing into a tree. I thought it was just a bad bruise and waited far too long to note that it was a rupture. When I finally did have surgery, the surgeon had to take a piece from my leg because the tendon had retracted too far. I went through the surgery and consequent physical therapy but then took a fall and re-ruptured it. So I just live with that now. There are two attachments so I can still use the muscle. Pronation is difficult and incomplete and I have the reverse Popeye look but, generally, I haven’t lost much use.

    Tore my distal biceps tendon 15 years ago during a deadlift. Supinated arm popped off the bone. As you say I almost waited too long to go to the hospital. Even a couple days later they were barely able to reattach it. Spent months in a sling immobilized after surgery. And many months getting my arm to fully straighten and to a point it could bare weight.. Lesson learned. Make sure you keep your arm locked straight during deadlifts. Oh, and no steroids involved. And not as EXTREMELY rare as you think when it involves mixed grip deadlifts

      Lots of evidence that eccentric movements actually being the main driver of tendon growth.
      So just lower it down slowly with 1 hand but don't attempt to pick it back up without using 2 hands.
      I routinely do this with loads above my concentric 1 rep max, so it's definitely heavy and haven't had an issue.

    I ruptured both. One 20 years ago preacher curl, and the other trying to push and lift a car out of a ditch 10 years ago. First surgery 20 years ago was poor. Fixed but lost supination and can’t extend my arm all the way anymore. Right arm 10 years ago was a better doc and 100% fixed. Left arm is never going to be right. I’m a guitar player also and the loss of supination sucks! Difference is the first surgery was semi immobilized for 3 months. Second doc said no cast and start flexing it and using it immediately but no heavy lifting of any kind. Was a big difference and I recovered almost right away without immobilizing it.

    I ruptured my right lower biceps tendon while moving furniture, when my arm was in a nearly extended position. It was not very painful, even as it occurred, just kind of a mental shock. My surgery was 100% successful. Eventually I started free curls, starting light, and after I had lost some weight, I switched to pullups.
