The toughest leaps are access with CMHT staff notions of treatment and primary care GP’s pulling all meds hanging someone out to dry. Another is price. In a cost of living crisis you *have* to undercut the illicit market by price significantly. Industry and commerce operate on both sides of the market, costs *can* come down and if these aspects were at least acknowledged your patient ‘market’ will grow. Ethics, essentially. Stigma is long gone for many but the system is labyrinthine - clinic choice, product range - and while a drug worker can access such protocols, people are wary of current cost/benefit analysis.

At some point there will be strikes and the Woke brigade they employ will always have their "feelings" hurt by something, someone or a pronoun.
